    I came to The Refuge specifically for trauma treatment. I was referred and admitted for CPTSD, body…read moredysmorphic disorder, agoraphobia, and multiple panic and anxiety disorders. Body dysmorphic disorder is an important part of this story because it has nothing to do with food or eating, and you would think staff at a trauma-informed facility would know that. The most consistently positive part of my stay was the other clients in the trauma community. They were kind, supportive, and genuinely looked out for one another. The food was good, and the cabins were fine overall, though there was some mold. Group therapy ran all day, which is typical for a trauma program, but a lot of the groups were very poorly facilitated. Many times, groups did not even have someone assigned to run them. Yes, this took place over the Thanksgiving holiday, but as a treatment center, you know you still have clients who need care. Staffing and group coverage should have been addressed and organized well in advance of the holiday. Where things went seriously wrong was with placement and leadership. After arriving and doing well in the trauma community, I was suddenly told I was being placed in Oak House, the eating disorder unit, even though I do not have an eating disorder. No one gave me a reason. I was abruptly removed from the trauma program and placed in a highly restrictive unit with daily weigh-ins, locked bathrooms, monitored showers, plated meals, and constant escorts -- none of which matched my diagnoses. I reported to my therapist that I was concerned about my safety while in Oak House. She told me she escalated my concerns to leadership, but no corrective action was taken. In a subsequent meeting with the clinical director, Shelly, I again asked why I had been placed in the eating disorder unit. Instead of answering, she deflected my questions. When I tried to explain why I did not belong there -- that I do not have an eating disorder and that body dysmorphic disorder has nothing to do with food -- she told me my questions were a "trauma response." As a psychology major with a 4.0 GPA, I know what a trauma response is. I told her directly that this was not one. This was one adult asking another adult for an explanation about a clear misplacement, and she was not able or willing to give me an answer. I was ultimately given an ultimatum: return to Oak House or leave the facility that night, even though I was over a thousand miles from home. I was also told I could not use a phone to call my husband unless I agreed to return to Oak House first. I went back only long enough to use the landline to arrange a ride home. Thankfully, I had a friend nearby who dropped everything to come get me. I am thankful for the other clients. But the placement decisions, lack of transparency, poor planning, and leadership response were confusing, dismissive, and not trauma-informed. People seeking trauma treatment deserve clear communication, respect, and care that actually aligns with their diagnoses

The good stuff - a handful of very kind workers who are trying their best. The bad - this place is worse than jail in my opinion. Jail at least has programs and keeps people safe. Overworked clinical care for managing meds means patients don't get what they need. Only two groups a week / should be 3/5 times a day. Visitors are bringing in vapes and drugs, at least one patient was in a leadership role and was fermenting fruit for alcohol! Terrible terrible place - since they have contracts with the state they don't care about really treating people and anyone they deem 'difficult' they find a way to kick them out leading to probation violations and arrests. Avoid at all costs!

I went and visited my son during their days where they allowed visitation. I first noticed that nobody ever searched me or my belongings. I would pack a bag with snacks and drinks for the visit and I walked right through. It would be very easy to bring drugs in there and it wouldn't be caught. The day my son was sentenced to Phoenix House, I was told by a probation officer in the courtroom that this facility is full of drugs and she was right.The Director of this facility is Mr. Ivan White. I had spoken to Mr. White and thought the man seemed to really care about his clients and their wellbeing. I was proven wrong! My son was discharged for "breaking the house rules". That's what was stated on the arrest record. It said nothing about drugs being involved and they never found drugs on him. I reached out to Mr. White via email and asked him if my son could be given another chance to prove himself since they had no proof of him being in possession of drugs. I was told by him that he needed to check with his "clinical team". After a few emails to him he finally responded with a "NO" answer that they wouldn't take him back at this time. After that no answer, Mr. White changed his demeanor with me. I was told that I had 90 days to pick up his belongings and that they needed to be picked up during normal business hours and I needed to make arrangements with staff there. I stated in a few emails that I could not be there during their normal business hours as I work full time. After several times emailing him and being ignored, I sent his counselor, Harry, and email hoping to get a response. Harry forwarded the email to Mr. White who thanked Harry and forwarded the email to another woman who was told to reach out to me. This woman never reached out. At this point, after a while of not hearing from anyone and not being able to make arrangements with anyone, I decided to call Mr. White directly. Once he answered the phone and I told him who I was, he started talking fast as if to get me off the phone. I told him that I would be there before noon on Saturday. He quickly told me that I could pick up his belongings 24/7. Not what I was originally told. I also asked him to check my son's belongings for his iPad, headphones and speaker because he's very concerned about his wallet with ID and his electronics. Again, Mr. White spoke very fast and said "let me go check on that for you real quick and I'll call you right back". He never called me back!! We made the hour and a half drive to Phoenix House that Saturday. We were there for close to two hours. Staff were trying to find his belongings. They didn't find anything of my son's. We were told there were two bags in a room with about "100" bags that weren't labeled. I asked if I could look in the bags to see if one of them happened to be his and I was told no because it's an invasion of privacy. Really??? We were extremely upset. A man by the name of Nick took us to his office and said that he would be a contact person for us and he gave us his card. He told us that he would request to see the cameras and would do everything he could for us. After a few text messages back and forth and being told that he's put in the request to see the cameras, I haven't heard from him and it's been a couple of weeks. My son called and spoke to one of his friends and this friend stated that all of his belongings were thrown away. I do believe that as absolutely nothing as been found. I also want to state that this belongings were locked in his locker so I believe that this was staff negligence. The staff told me that Saturday that my son signed a form stating that the facility was not responsible for lost or stolen items. I get that, but that's while my son is there! As soon as my son was escorted out of the building, his belongings were now the RESPONSIBILITY OF THE PHOENIX HOUSE! My son lost everything he had, which wasn't much to begin with!

The entire facility smelled of urine, feces, feer and death! Upon reaching the room tgey attempted to assign to me , there was a large mess on the floor , in the entrance to my room ! One of the staff said. I told them to clean that up earlier, now what does that look like ! Being on a gurney , I couldn't see so I said , tell me what does it look like ? No one answered. My friend later told me there was what appeared to be blood and urine on the floor. The room was about 80 degrees and looked like it was furnished in 1975. Also in my room was what looked like someone in a bed with several blankets that may or may not have been alive. I saw no movement or anything spoken from this victim of avante. Needless to say , my visit was extremely short . On the way out the staff actually tried to bribe me with a hamburger to stay ! I would not bring my goldfish to this facility. I was very disturbed with this facility.

Occupational, Speech and Physical - I'd give these people 5 stars. Yet, as an overall rehab facility this place is a nightmare of miscommunication and ineptness. I could not even begin to count the amount of times they could not find the special meds my mom needed to take each day, the amount of times it took forever for a nurse or staff member to attend to her when needed, the staff doctors who had no clue what her specialist and prime physician at the Mayo Clinic ordered for her treatment, the times they screwed up her special dietary meals, the amount of times we had to rush over there to get things straightened out, it goes on and on. Also, be aware if your loved one is here on a weekend as it seems they are extra slow, and the food is cold garbage which I wouldn't even feed a stray dog. Yes, it was that bad and many times my mom hardly ate. As she was there due to a rare heart and lung condition keeping up her strength was imperative. We had many arguments with the weekend staff heads while here as we had it and were fed up with the excuses. It just seemed like it was a bother to ask for things that were more than normal. Weekdays were a bit better but not as bad as the weekend shift it seemed. Think twice about sending a family member or loved one to this facility. Think three times and do some research as there are other rehab facilities in Ocala and the surrounding area to consider. If my mom has a relapse and needs rehab again we would never send her here again. I'm glad she is out of this crap show of a place now doing better and has home care.

I could not endorse a medical and management staff enough. I never had to wait for medical care when requested. They always had a good disposition and made me feel like I was always safe and had the best medical care possible.

When I left, l had more pain very much similar to the original had from my accident. I was encouraged by a friend to not give as maybe another therapist would help me. I got the therapist by the name of Ken. He read the notes and asked how the pain is now from 1-10, told him 8. We got started with some of the stuff I worked on at the beginning. He explained each workout clearly and would ask if I understood. He would also show me how to do with a reminder that going that we don't want to overdo it. I had to ask the gal my therapist's name. I described him as the guy that has flat like when in the military...he oh that's Ken, headed home painless, I figured, well let's see how I feel tomorrow. Had a great night sleep with very, very little pain. I contributed that I am not to sleep on my right shoulder. After 74 years, it's a little hard to break the habit.
