I came there after spending 3 weeks in ICU, I couldn't walk or even roll over, I was that incapacitated. Their PT department is the best. Between them all the got me up and moving again once I got out of my own head (I was having a "poor me" situation) and Kim was the best. She taught me to trust my body again to do the things it used to and it worked. I don't know if I ever could do enough to repay them for my life back. I'm still not 100% but close and never would have got here except for them and my own determination to get well enough to go home to my husband and 4 legged kids. Thank you for everything, I have nothing but positive things to and recommend. At some point you have to be responsible for yourself and put the hard work in.

He's solely dependent on assistance at all times! **He was a patient for 11 days and was bathed ONCE!! (Shower days were Tuesday & Friday's) and was given one on **They FALSIFIED paperwork stating that someone bathed him in actuality his Physical Therapist was the one who gave him a bath. **No one came to look at his bandages to see if they were soiled, leaking or falling off for 11 days! **He's steady pressing the button for assistance to use the bathroom and no one comes!. *He's asking for help and he gets audacious response of, "that's not my job!" (By the CNA) *He's pulling the cord for assistance from an empty bed *He's wearing depends (that the family has brought him) because he doesn't trust the CNA's will come in a timely manner to assist him to the bathroom. *He'd asked the CNA to bring him a bucket with warm water & soap to wash up and the CNA's response is, "It's too late, you'll disturb your neighbors!" *** The physician assistant, NICK COLLINS IS RUDE, he absolutely has NO BEDSIDE MANNERS!! He's talks at you and not to you, he talks down to you, makes empty threats, he assumes EVERYONE is on Medical, the tone he speaks to you is very undermining. Needless to say, I'm VERY displeased Very about the series of events that has happened since my Uncle has been a patient here. They have an ant & cockroach infestation, ants that crawl on the patients and inside of the trach tubes, and then the interdisciplinary team discusses whether it's legal or not that they have to tell the patient's family (they chose not to) they being Jamie, who was director of nursing, Arnold who works with compliance and Malou who was Charge Nurse. They cover up abuse from employees, and go over any problematic patients in their monthly QAPI meetings. If a patient is found to be problematic, the hospital will transfer them anywhere else even if it's over 50 miles from the family. The employees are not allowed to break HIPAA by law, but they regularly look up the patients on the Internet and access their charts, even though they are not under their specific care. They found out that a patient had sued an airline and the second that the family made a complaint, they moved the patient to a different facility. They always leave their computer monitors on which is also a HIPAA violation because they are supposed to lock their screen so that nobody can see patient charts or information and they regularly ignore that rule. They had state come in and audit them for their bathing and showering, I overheard a patient's telling an employee that they had not been bathed in over a week and I could smell the foul odor coming out of the room. They had been dinged by state for not giving baths or showers properly or on time or consistently to their patients, and their solution to that was for management to tell the employees and CNA's to chart that they did it even if they had not so that they don't get in trouble from state again. If they were to audit the bathing and showering again, they would see that multiple CNA's are signing off as bathing the same patient in one day which is not accurate or true. These employees at the top management are corrupt, they overcharge Medicare for patients care that they are not providing, the case managers work closely with management to move any problematic patient, and they have no accountability for patient care. They break HIPPA laws by leaving patient information accessible, and by looking them up in charts or on the internet when they have no legal right to. IF YOU DO SEND YOUR FAMILY HERE, MAKE SURE TO BE PRESENT IN PERSON 4x A WEEK TO MAKE SURE YOUR FAMILY IS BEING CARED FOR, THE STAFF ABUSE PATIENTS WHO DO NOT HAVE FAMILY ADVOCATES!!!!!!!

It creates a safe and welcoming environment for both patients and staff. It is a facility where professionalism, teamwork, and compassion are clearly prioritized. It is a place where I would be honored in pursuing employment after completing my clinicals. During my clinical experience, I felt encouraged to learn and was consistently supported by staff who were willing to share their knowledge while maintaining a strong focus on proper patient centered care. Having a grandfather with Parkinson's disease deepened my understanding of the importance of patience, dignity, and compassionate care; values that Creekview consistently demonstrates. I am truly appreciative of the professionalism and empathy shown by the entire team, and I would feel very confident entrusting my own family's care to this facility. Luckily, I was able to get him transferred to Creekview, where he thrived. The physical, occupational, and speech therapists were outstanding. They all really made the effort to make sure that he would get better and be able to come home. (Shout-outs to Govin and Maya.) The facility is gorgeous and welcoming and the rooms very pleasant; not institional at all. The staff is very caring. They paid attention to my husband's dietary needs, yet the food was pretty good for a nursing facility.

I made my appointment a week in advance, and when I got there, they confirmed my appointment and which booster I wanted, and everything went super smoothly. After your shot, you went into a room for 15 minutes to make sure you don't have any adverse reactions to the vaccine. Everyone sits at their own table with a timer at every table. You sit down, they start the timer, and when it rings 15 minutes later, if you feel fine, you're free to go. Easy peasy!!!
