INADEQUATE!!! Where do I begin?? My uncle had surgery on his femur bone and sent to this DUMP for rehabilitation!!! He's solely dependent on assistance at all times! **He was a patient for 11 days and was bathed ONCE!! (Shower days were Tuesday & Friday's) and was given one on **They FALSIFIED paperwork stating that someone bathed him in actuality his Physical Therapist was the one who gave him a bath. **No one came to look at his bandages to see if they were soiled, leaking or falling off for 11 days! **He's steady pressing the button for assistance to use the bathroom and no one comes!. *He's asking for help and he gets audacious response of, "that's not my job!" (By the CNA) *He's pulling the cord for assistance from an empty bed *He's wearing depends (that the family has brought him) because he doesn't trust the CNA's will come in a timely manner to assist him to the bathroom. *He'd asked the CNA to bring him a bucket with warm water & soap to wash up and the CNA's response is, "It's too late, you'll disturb your neighbors!" *** The physician assistant, NICK COLLINS IS RUDE, he absolutely has NO BEDSIDE MANNERS!! He's talks at you and not to you, he talks down to you, makes empty threats, he assumes EVERYONE is on Medical, the tone he speaks to you is very undermining. Needless to say, I'm VERY displeased Very about the series of events that has happened since my Uncle has been a patient here. My uncle broke his femur bone, so needless to say he's very dependent on staffing until he's completely rehabilitated. He's been a patient for 16 days (as of today) read more

After having been here for six months, the only redeeming service they offer is excellent physical therapy. The food was represented to us by a pretty young sales person as "chef inspired". The food was decent the first few weeks, even showing some creativity. Over the last four weeks it has rapidly declined, in pallatability, quantity and nutritional content. The meals are lacking in protein and fresh fruit and vegetables. The meals depend on a lot of frozen prepared items, like frozen breaded fish, and primarily canned vegetables and fruits. Sandwiches made with packaged lunch meats and salty cheeses are substituted if you decline a meal. You're absolutely required to take vitamin supplements. Meals are high in sugar, salt, carbohydrates with little regard for personal needs or preferences. They promote themselves as giving excellent wound care, but are reluctant to purchase wound care items that might seem too pricey. I've been trying for two weeks to get a medicated numbing spray to ease in the painful procedure I'm put through daily. The facility had a recent change in Administrators, and while the previous administrator was largely ineffective in making a lot of changes, under the current administrator of about a month, there seem to be fewer CNAs on duty, more CNAs and nurses working double shifts and not up to their usual efficiency, and poorer quality meals. The current administrator hasn't even made the time to introduce himself to the residents leaving one to wonder if he's leveraging patient care against budget constraints. Or maybe he's just not interested. Don't leave your loved one here unless you're prepared to advocate and participate in their care. read more

This place is horrible! Very noisy and very unprofessional. Cna's and some nurses are so busy with their rude big mouths laughing constantly with patients for long periods of time, that they dont give a dam about other patients needs. I waited one and a half hours for water and a cup. Food is discusting and causes stomach issues. Plus they dont give much food. Have someone bring you food. Sleep? Whats that? Do they care about you? Not really. There are alot more bad issues about that place. Too many to discuss here. Good luck folks! read more

I recommend this place because of the staff, like Alexa, Dieula, Kitty, Erica, Eify, Patricia, Diana Rose, Marcela, Diana Johnson,Mariah, Julius, Benjamin, Eric and Jesus! To name a few, I have been here four years this October and most of theses people are here making my life better and a little brighter . I can't mention everyone one but there are alot more that help me recover and rehabilitate like the excellent therapists in the rehab dept. My favorite people there are Preanka, Teena, Richard, Bobby, Judy and Jasmine thanks for your patience and your successes, this is why I am here! my friend was there because she is on medi cal. The food was bad. Three patients in one room sharing one bathroom that the people don't even clean. Staff don't listen when you call them. Sad to say that equality care for all people doesn't exist there. So please don't put your loved ones in that place. look for other options. read more

If you happen upon this place RUN FOR THE HILLS!! If you care for your loved one, DO NOT SEND THEM HERE. Management is heartless, rude and does not provide the correct guidance. Staff is disorganized and it is obvious if you are there any longer than 5 mins. They do not truly care as all they want to do is shove my elderly grandparent out the door. They refused to help with getting authorization for my grandma to stay and she was not even close to meeting her goals. When I called to check with the insurance, I was informed that East Bay Post Acute only sent their charges and NO supporting documentation which caused authorization to be denied. This is their fault and when speaking to the manager, she told my family they can submit on their own and REFUSED to help. Oh, and they threaten to charge $1,000 PER DAY as a scare tactic to patients who they feel are there too long. Avoid this facility at all costs! read more

East bay post acute health center is a wonderful place to go for rehabilitation for physical ailments and recoveries from different medical treatments. The staff is spot on, and I personally got a wonderful treatment from all levels and came out of there after five months feeling really good and ready for my knee surgery , I recommend it for anyone who needs the services they go over the top to make sure that you're treated well and taken care of until the day you check out thank you everybody there, especially Borlu Rita, Emily, Angie And Christine these were my day-to-day caregivers the whole time I was there and they were the absolute best, so is the rest of the staff, so many were so good and it would take too long to name them all. his is probably the place not to send your loved ones if you love them. Communication there is just confused chaos information. That is with management there nurses were nice. Only thing that is obvious was this . For a 2 day stay ok anything more, don't do it in the end my mom left with urinary track infection and in pain still released her. They kept reminding us if she was not picked up at certain that we would be charged ( sit down for this one)$1000.00 a day . The workers were nice to my mom food sucked. Problem is the Management they are the ones that make it terrible. There staff does alot picking up for management . Definetly a 2 star because of management and confused communication.. Again the nursing staff were nice.. it is with upper management department that really make it terrible also updating this facility would be nice also ! I have only been pleasantly surprised a few times with the quality of care received by a family member. One of those times has been at East Bay Post Acute. I can not begin to express my gratitude and appreciation with the staff and facility at East Bay Post Acute. Nurse Antonio was so wonderful and professional in his care. He is beyond caring and respectful and has an inmemse amount of integrity, which is a testament and reflection of the facilty as well. I'd like to start by saying this is my first and only experience with a place like this. This is just my personal view. This place was awful. When my grandma was admitted Thursday night they incorrectly listed her as covid-19 positive and had her quarantined to a room by herself. After we brought it to the attention of the staff that she wasn't positive and had been vaccinated months ago they gave her a roommate. My grandma was given thick liquids to drink and didn't know why they would t let her drink her Normal juice and coffee. Turns out they were giving her her roommates special diet and got their charts mixed up. My grandma had fallen and skinned her face and wanted to take a shower to get the blood and debris off. The nurse kept telling her they only give patients 1 shower a week and it wasn't her turn yet. This went on for over a week before I was able to talk with someone and have them explain why she had t been provided with a shower. Turns out there water had been off and there was no running water in the facility! They were lying to us! Not to worry though it would be on tomorrow. The next day I asked another nurse i the water working. She didn't know she had no idea the water wasn't working. "Can I speak to someone in charge?" "I am the one in charge." Every time I'd ask someone they either didn't know or it should be on tomorrow. My grandma didn't get her shower for more than 9 DAYS of being in their care. Meanwhile, I had tried to get her transferred to another facility because this one was unacceptable. They never picked up the phone to speak with the other therapy center. If you call the phone rings and rings. I hope no one ever has to go here. The photos they have posted themselves on Yelp are misleading. read more

The facility is clean which is big plus. Nursing care and physical therapy are provided on site. Compassionate employees are genuinely concerned about your safety, recovery and progress. The majority are happy, helpful people. One employee Cheryl (CNA), shines like a ray of light. She goes the extra mile and does it with a smile, working tirelessly to make sure everyone and everything is taken care of. The food is not five star but it is good and nutritious. I will start by saying that I gave four out of five stars due to the telephone situation at East Bay, and I will explain this further in my review. But before I do that, I want to first express my sincere thanks to Anita and Flo, and on my mother's behalf, Lorilei, all of whom work at East Bay Post Acute. Anita, the Director at East Bay, was very quick in responding to concerns I had expressed through an email, and she was kind and understanding when I called her cell phone late one evening during an incident when my mother (there for broken hip) was having a panic attack. I am especially grateful to Flo, one of the nurses that attended to my mother. She was one of several nurses that endured my mother's panic induced outbursts yelling at everyone. Flo was exceptionally understanding and kind to me when I apologized to her for my mother's behavior, which really impressed me. After the panic attacks subsided, Flo was very sweet in trying to help me see my mom when I was there outside of visitation times, and although I got caught and had to leave, I appreciate the fact that she cared to try to make my mom happy by seeing me. Lorilei is a PT nurses that my mother heavily relied upon, so I want to also thank her for all she did for my mother. Now the one issue I have about East Bay Post Acute is that they do not have telephones in the patients' rooms. You must call the main number and ask them to bring the phone to the patient. You can have a personal cell phone, but if your loved one does not have a cell phone, you must call the main number. I provided my mother with a cell phone, but she didn't know how to use a smart phone, and I couldn't go into her room to explain it. I had many reasons throughout her stay to have to call the main number at East Bay, and most times I called, noone answered the phone. This became distressing considering I couldn't reach my mom and I was not allowed to visit her without scheduling an appointment. My inability to visit my mother or get an answer when calling left me feeling uneasy and upset much of the time, and I feel strongly that this is unacceptable. Due to the lack of telephone access, I would only recommend this facility to those that can provide their loved ones with a cell phone. Be aware that visitation is by appointment only, I assume due to COVID, so I do not recommend it to anyone without a personal phone as you will be cut off from contact with your loved one for much of the time unless they have a cell phone. Overall, I think this place is clean, and not a bad place. There are a few people working there make this place better by their presence/service, and East Bay Post Acute is lucky to have them. read more

I usually don't write reviews or read into them because I like to give everything a fair chance, however, after our visit here, I am. Driving up to the residence was kind of weird, there is VERY limited parking, even with the limited parking there is ONE spot to get someone in or out who needs a walker or wheel chair accessible. My grandfather is in a wheel chair so I have to drop him off on the side- go to the back to park and go back to push him inside through an exit door. My grandfather was placed here after an amputation in a hospital. Prior to his arrival, a case manager called the facility to go over his diagnosis and his necessities to see if they Had the ability to care for him in which they agreed to everything and said they were skilled in his type of needs. DAY 1, arriving into the room was not too bad, seemed to be clean-ish, the drawer for his clothing was broke but we made due. When I was setting up his belongings the inside of the drawers were clean but behind the dresser and bed had debris, however, I understand not every spot can be gotten to if a patient is in the bed and was unaware if someone had just left so didn't think much of it. Some rooms have 2 beds while some others have 3. The hallways have lights but still seem dim & dark during the day. It seems as if half of the facility is A convalescent home and the other half is rehab related. They have 2 areas where the patients can socialize with games. My grandfather arrived with a wound vac for his foot which we were told by the hospital a firm 2 HOURS it is allowed to be unplugged nothing longer than that.. we reiterated that with the nurse upon arrival whom in which stated it needed to get it cleared by their doctor( which was weird since his DR proscribed it, he already had the initial tubes it just needed a new machine AND again they were already made aware he was coming in at a certain time with orders faxed over previously)..the wound vac cords were NOT attached to another machine for over 8 hours because we continued to wait for "clearance" which wasn't even the worst part- even once the lines were attached, the machine was never powered ON for 14 HOURS!!!! After our first Dr apt after being here not going so well, due to the wound not healing as expected I decided to read up on the importance of the wound vac. After reading numerous studies, the wound vac and the proper operation plays a HUGE part of the healing process and could absolutely affect the way a wound heals or doesn't. And FORGET hitting your call button, even if/when someone responds- it's not your nurse and the response is "I'll have to get your nurse for that". Even just while visiting I became immune to the noise of all the call buttons. Our family was receiving text messages because no one responded to the call button and my grandpa or his roommate needed to use the restroom, so we had to call the facility to make them aware. In the early hours of the morning someone tried to come into his room from the back sliding door, thankfully it was locked. While at home my grandpa only takes 3 pills at night, here they were giving him 7 in the evening. They were dressing his wound differently then he was shown initially, When he would ask questions about why things were being done or why he was taking certain medications some nurses would become argumentative and rude. He was on IV antibiotics, which were given late more than once. Not all of the nursing staff is negligent, but definitely over worked. We wanted to pull my grandfather out of this home after a week but he and his roommate were advocates for one another and became very close. He felt without staying there to witness the treatments each of them were receiving it would get worse for who ever stayed, so he decided to stay in hope speaking up for one another would help things change. The both of them also tried to speak to the administration about all of these issues but nothing ever got done. When a member of the family tried to speak with administration, the response we received was "we are use to speaking with unhappy patients." I understand you can't please everyone, but it is NOT Normal to being used to speaking to unhappy patients and their families. The kitchen staff however, is very friendly and knows most patients by name and acknowledges them if they see them in the hallways. Which you can tell the patients like that. All in all, we decide to pull him out of this facility after 2 short weeks in hopes for him to heal and get the proper treatment. The staff at the East bay facility packed his pills he was taking as well as a small container of the fluid needed for his wound care-which is THREE times a day, but failed to include the new FULL bottle they had used that morning. So we had a very limited stock at the new facility due to the fact it was a prescription that needed to be specially ordered. I'm glad he's out of this facility and if you have a loved one in this home get them out. read more

My auntie went to a nursing home close by and her room was so small, her roommate was too loud, and she got the flu. After going back to Eden she was sent to East Bay. The team was so nice to my Auntie. She got the rehab she needed; she returned home last night. So thankful to Shayne. He listened to my concerns and was very helpful. read more

1 1/2 week ago a family member that is staying at this facility his patient rights were violated. The family member reached out to me very distraught. I drove to the facility within 30 minutes. The lady in charge, Anita was at the front desk when i went to sign in, I introduced myself and she immediately knew why i was there. I asked to speak to her but she informed me that shes busy and that i need to come back. Anita told me where my family member was and I went to meet with him. After 30 minutes i came back to Anita's office and she had the door open. Anita glanced at me for a brief second, and went back to doing her paperwork, and says to me in an anoying tone " i guess you can come in now". I walked into her office, and even though she knew the reason i was there, and what took place i gave her my family's member version of the story. I explained to her that im too in the medical field and how the situation should have been handled and her staff needs to be retrained. Meanwhile, while im telling my story to her, she rolled her eyes at me twice and her face expressions told me she just does not care! I informed her that she is being very rude, and i will be filing a complaint against the facility with the Medical Board. Anita advised me thats fine! I told her if the facility retaliates on my family member that this will not be the end of it! Anita, then asks me "are you threating me"? Clearly that was not a threat! I was nice enough to let her know the consequences. Right after that incident Anita and the staff "forgot to put in there paperwork to the Transportation Company" or didnt turn the paperwork on time for the Transportation Company for my family member to get rides to his appointments 3 times in a row within 10 days. Since then i have filed a complaint against the facility and it gives me great pleasure to tell my story on yelp. PLEASE Do not put your loved ones here! I will say there is a few good people there, but Anita is not one of them. If she cant be sympathetic against her patients or does not know how to handle or trear upset family members, then she clearly needs to be retrained or move on to another career. read more

DO NOT SEND YOUR LOVEONES HERE!!! this is the worst rehabilitation facility I have ever known!! the staff is not friendly, empathetic nor sympathetic. They do not communicate with relatives. No one ever calls you back, and you can never get in touch with anyone to get answers. We have been trying to get in touch with the facility director, the social worker, etc. but no one have returned our call. You cannot freely visit your love ones, because the visits are monitored. Even a window visit has to be scheduled. On rare occasions when they do answer the phone to speak with your relative, they have to have your relative call you back. Sometimes they never give them the phone. I have tried calling this facility for two hours today and no one answered!! My sister was there for a few weeks before they gave her antibiotics. It seems like the patients are in some type of prison, rather than a caring facility. We will be getting her out of there ASAP! Do not send your love one here!! In addition, a friend of our family was in here, and her family had a horrible experience here as well. By the way, it is after 10:30pm, i just tried calling the facility again, still no one is answering! read more

My aunt recently stayed here after what the doctor explained was a complicated uti. We were hesitant about her going into a nursing home because she's never been before, but we didn't have an option because she needed IV antibiotics. I have to say, it wasn't anything like we had worried it would be. The staff were all so kind to her and made her feel like she was part of their family. I wish I could give them more than 5 stars because not only did they put my aunt at ease, they did the same for us by their gracious care and knowledge of work. Now she's back at home and doing well. I can't say enough of the way East Bay cared for my mother. I work at a Planetree Hospital that specialize in patient center care and in many ways the facility and the care takers performed the same care as we do and can't thank them enough. I know in my heart mom loved the people that cared for her which made her happy for the time she was there and more importantly at the end of her life. The staff did everything to help me with the situation of a caretaker that abused her at her home and stopped him from entering the facility so security was not a problem for persons concerned about security. Mom was placed at East bay by the hospital and they couldn't have picked a better place. I love you all for the care!! read more

If i could give this place no stars i would. They don't contribute and help each other out at this facility as a whole. Every time i walk into this place i get ignored and I'm not meant to be there past 5 minutes but somehow its always just a problem to get ONE favor. They always have attitudes & no strive to help anyone but themselves, so therefore my review is a complete zero for all of the staff that work there. There is only a selective TWO staff members that actually care to help & properly do their job. Yes i do understand that there is also other patients who have needs but taking one second out of your time to help or say a word so that way i can receive a bit of help wont hurt. What you DO get is no communication, filthy rooms with peeling paint, falling apart furniture, dirty vents, dried fluids on walls, no phones for patients etc. I would also give no stars if could! If you happen to end up here, or know anyone here, please check on them daily! make sure they are getting what supposed to and being actually checked on - not just medicatating and dropping food off (which also leaves a lot to be desired). read more

I have never stayed in a center like this before because I've always been pretty healthy. I just had knee replacement surgery a few weeks ago and spent time at East Bay Post-Acute Center and I went in not knowing what to expect. I can only say that EVERY SINGLE PERSON at this place was kind, caring, and good to me. It didn't matter how many times I was in pain at night and needed help, someone was always at my side within a minute or two. There was positivity in this staff that spoke to me of their desire to help promote my recovery and that they cared about my well-being. The Occupational Therapy was great. So was the physical therapy. In particular I want to thank Tan for his relentless efforts in getting me to do the necessary exercises and then for checking in on me to see if it was helping. There is another fellow I want to thank and I can NOT, unfortunately, think of his name. He was in OT. He was really good. Raj was great also. Also -- one of the nurses -- her name was Jeannette -- spent time with me, encouraged me, talked to me, joked with me, and just really made me feel that I was going to overcome this and be back on my feet soon. I think about about Lisa -- the girl who often times gave me my meds, and how we referred to getting my meds as our daily "happy hour" & she'd say, "Ready for your margarita?" She made me laugh. The people at this place were so personable and helpful. I can not say enough positive! The facility, although perhaps older, was clean and well-kept. Orderly. The food was good and there was always plenty of it. I will have to have my other knee replaced in 2018 and knowing that after the surgery I'll be in the capable hands of this staff makes me feel a lot more at ease.
