This is where they send people to die. It is terrible facility it is scared, old and understaffed. My mother was staying at this place and apparently was taken to an outside hospital when she was confused and unaware of where she was without even contacting me and when I went to grab her belongings they had no knowledge of where anything was and told me to check with local ambulance companies. She was taken to the hospital on a Saturday and I arrived on Monday from out of town to find all this out. That whole time not a single word from them about what was happening. Horrible experience. Recently a family member was here. He received the appropriate care for his needs I visited often and experienced a caring, dedicated staff that met the needs of all the patients that I interacted with. I found those patients to be friendly and engaged with the activities selected for them. The facility was always clean and they kept me informed of his progress and any concerns. Amazing! The entire staff was very good. Each department knew there job. They tentatively addressed all my concerns from the first day of my brother's arrival to the last day of his discharge. Everyone was very respectful and courtesy. My family and I never felt unwelcomed or seen any hostility. Everyone at this facility was pleasant. The maintenance and housekeeping was right on it. A very clean facility, plus the cleaniness of their patients rooms a priority. The quality of care was excellent from the D.O.N., CNA's, and Activity Coordinator. Valley Skilled Nursing Center is a place that provided exceptional care to me. The Staff from the Administrator to Housekeeping are very nice, caring and helpful no matter what you need. I have been here twice and I wouldn't go anywhere else. The Therapy Department is excellent and they met my needs according to doctor's orders and I was able to leave walking on my own with my walker. My dog was allowed to come and visit me and that made me happy and I think it helped me to heal faster. They offer a variety of activities and programs that keeps the residents engaged and active and it's fun and you get a chance to meet new people that can become friends if you give them a chance!! The food is good and they met my likes and dislikes and if they didn't they made it right. read more

The first time I walked into this facility 18 years ago was an awful experience and the way the patients were treated was the worst. Yesterday I walked into this same facility and it was the best experience the patients were happy the staff was engaging and this facility was welcoming. It felt WELCOMING as a 54 year old facing future care this is the facility I would want to be in. I spoke to Ammon the director of Valley Skilled Nursing, Ammon really cares about his staff, facility , and the patients. Which in return the staff really cares about the patients. read more

The staff in this place are amazing. They are so friendly and caring. The love and care given to my Aunt had a lasting impact on our family. The Activities Director, Chris, and Assistant Cecilia, are the best I've seen in a rehab facility. This facility deserves a Community award for best skilled-care Excellence in Modesto. read more

This place should have their license revoked! I had to call the police and fire department to go down and assist my friend. They didn't answer the phone, and the voicemail was full. Doors were locked when the police arrived. The workers let the police in, and didn't even go with them to see what my friend needed. She was in there for rehab on her broken shoulder. Two days later, she passed... of a broken shoulder?! Do not go there! read more

Would give zero starts if I could. This rehabilitation is a dangerous place to leave in care of a relative. My grandpa was there for only 2.5 days & I know if I didn't come to visit that evening, he would have died that night. He got pneumonia there & had sugar level over 800. 2 days of being unconscious, saying he was confused but not understanding he had dementia. They took his clothing we brought from home & did not return them. He was left without blankets. Please, if you are considering this facility, think twice. They are remodeling the outside but do not fooled by the outside of a building. I know most nursing & rehabilitations are not great due to being short staffed & staff who are not trained well to care for patients but I refused to left him return there after returning to hospital. This is a terrible place ,I would not take any one here . A lot of people in the hall ways sleeping ? When someone is ready to leave if this person needs help finding a place to go ,they send them to a place that is not for them to be .....They just sent him not even checking ....it was not a good place to be .They don't care about people just $$$$$ read more

I was at Valley Rehab Center of Modesto for my rehabilitation after my stroke. I had the best care provided by the nurses who all spent their time making sure I was getting better everyday. The rehabilitation team was excellent and provided me with therapy everyday. read more

I just want to say this place sure saved my moms life. She fell & fractured her hip & wrist after a dialysis treatment & hospital transferred her to Vintage fair nursing rehab. This place was awful. There was never no one to talk to. They put her on diapers & in 2 weeks she was there she lived off 5 diapers. Every time we asked for more they said they would take care of it & didn't so we ended bringing ours & they got mad we brought our own diapers. Long story short we transferred her to Valley rehab. All it took was one week for my mom to start walkin & do a 110% improvement. Since the other facility wouldn't let her get up & the ladies at physical therapy would talk amongs each other how they were to tired to do therapy. Peoples experiences are different but this facility alone with staff went beyond especially even took mom 2 dialysis once when the transportation cancelled in their own vehicle. My dad was here in February because he needed short term rehab. We have been to 3 or 4 different facilities in the last few years and this is by far the best rehab we have ever taken him to. The nursing staff is so caring and so attentive to my questions and concerns. The administrator seemed to be very involved with the staff and I had never even seen the administrative staff at any other facility beside Valley Skilled Nursing. I would recommend this place to anyone needing skilled nursing. read more

THIS IS NOT A REHABILITATION CENTER. Don't let anyone tell you it is for that. Sure they have physical therapist to work with you, but it is a NURSING HOME. Patients sitting in wheelchairs, screaming and hollering for help. Very loud. Even at night when trying to sleep the staff is loud. The staff tells back at the screaming patients. Horrible horrible place if you are of sound mind. My 64 years old mother was sent here for "rehab" after breaking her leg in 3 places. She is here waiting for surgery. We were advised by Doctors Hospital Manteca that this was NOT a nursing home. They lied. If you call for help, you wait for over an hour before anyone comes in. Do not go here! Brenda D., one of the CNA's here is great though. She has gone above and beyond to help, even when she is not assigned to the patient in a certain room. read more

I have a friend in there right now she is 89 years old and a very dear lady. She was transferred to this Skilled Nursing Facility from the Doctor's Hospital in Modesto last week for rehab and she contacts me several times during the day and night with one complaint after another: She had turned over on her bed and was falling out because no one had put the guard rail up and she was yelling for help several times before someone came into her room to assist her! Another time she turned on her call light because her diaper was wet and she was freezing cold and again yelling for help! Today she turned on her call light because she had to go poop and no one came so she had to set in her pooped diaper an hour crying before a kind young man came into the room and gently helped her out of bed and took her into the bathroom telling her "It is awful what they do here sometimes" then he went to the nursing station to complain for my friend and 2 CNA's were sent to her room they gave my friend some wet wipes to clean herself and a fresh diaper. When I spoke with her last she needed a shower but keeps getting promised and nothing happens. It is absolutely heartbreaking!!!!! read more
