My mother was a recent patient at Vista Manor for two times, one month each for two knee replacement surgeries. The facility and support teams were incredible! There were bilingual, multi-identity medical staff that made sure that all or any patients were cared for. As the caregiver and son, the center kept in consistent communication with me about my mother's health and care plans. The center hosts inclusive and accessible events for patients, I appreciate how the center prioritizes cultural identity in staff and patients. The visiting hours were easy to come in-and-out, and enough space to have patients relax outside or in common spaces. I am very grateful to the entire Vista Manor care team for taking great care of my dad. Director Jerome and his team (Social Services - Nadine, Nurse Supervisor - Diane, Speech Therapist - Navya, and all of the staff have taken wonderful care of my dad. My dad has multiple complicated conditions: catheter, feeding tube, and is pretty much completely on bed with no independent mobility. I can see it is very tough to take care of him. Although the staff is friendly, the bathrooms are dirty, they over medicate patients (they combined a pain med (norco) with another pain med (tramadol) causing me severe breathing problems (I have COPD). Beyond that, they would come into my room with gloves on (they are required to change gloves when handling each patient) and handled me, the toilet, the bedside commode, and the other patients in my room. I repeatedly reported this but nothing changed. The only ones to follow protocol were the nurses and physical therapists. It's just gross and against the healthcare code. I had to make a formal complaint with the board I won't even mention the slop they call food. read more

NEVER EVER COME HERE Worst nursing home. extremely old facilities that are not taken care of. Place reeks of smell and is so dirty, my sick family member did not even want to stay or touch anything. Nurses were extremely slow and low energy. when pressing the call button, they take so long that the patients can not even get attention in timely manner. multiple times i've seen my family member urgently need to take medicine or go to the bathroom, and nurses show up 10, 20 to 30 minutes later. We spoke to the team there multiple times, like activities person Luis, but we saw absolutely no improvements, if not even worse, during our stay. such a ridiculous facility. they did not even have the medication for my family member. we only came here because it looked like it had good reviews, but this place is not trusting at all. i would never trust my family member to their care again. bathrooms are ridiculously dirty. if you leave your family here, you might as well stay there. i don't trust them at all. staff walk in with gloves, and don't change between patient rooms. food is mush and when we request changes, we had to ask multiple times, for them to MAYBE fix it for you. i've reported them to the board. place is horrendous. needs to be closed down read more

My father just spent 3 weeks at Vista Manor and I can't say enough about the staff. They were ALL so kind and helpful, always. We have a large family and often there were many of us visiting at once, which the staff never attempted to regulate. They even allowed a large group of us to attend my father's PT session; the PTA was kind and patient with us all. Charlene, the Patient Care Coordinator and Shelly the Social Services Director were both especially amazing, bending over backwards to help us coordinate services and equipment purchases prior to our father's departure. The staff regulars who attended to our father always wore smiles and even laughed at our father's jokes! Our only complaints would be the quality of meals (stay away from the dried out, overcooked meats!) and the air conditioning glitches during 105 degree weather. read more

My mom had tumor removal and spine surgery two weeks ago. After a week staying at the hospital, she got transported to Vista Manor on last Monday. My mom speaks very little English. Their staffs are very kind, they found mandarin-speaking phone translator for her to better communicating. My mom can't move much at the moment, but all the nurses, OT, PT are very friendly and patient with her. I flew out to visit her last week and I'm so glad my mom gets treated well there. If I could give this place negative stars, I would. Since I can't, I'll just list some complaints from my grandma's experience in the final two months of her life. Hopefully that will help you make a decision on placing your loved ones here or not. On 3/21 Monday, grandma's ride never arrived to pick her up from the dialysis clinic back to Vista Manor. They left her waiting out there for hours after her treatment and eventually my family came out there to pick her up. I regret not taking my grandma away from Vista Manor at that point since I thought it was a one time incident. They picked her up late and dropped her off late few more times. On 3/24 Thursday, grandma was left in a wheelchair unattended and fell, which led to a bump on her head. The care conference said CNA checks diapers every 2 hours but have yet to see that. My family members and I took turns here for 4 hours at a time and don't see anyone until we press the button. We occasionally see other elders yelling in pain and were neglected. On 3/26-27: my grandma caught VRE, a common virus in these environments, which includes fever, low hb, then sent to ER on that Sunday evening. On 4/18: NP checked and said oxygen wasn't needed. Daily nurses didn't know her hands were colder and needed warming up before getting an accurate oxygen level and unnecessarily continued on the oxygen machine. I also had to inform them that they should not be using petroleum jelly near the oxygen machine as the manual had warned. On 4/19: grandma was left with a diaper of poop. How often do they check the patients here? Especially those who aren't as mobile and incontinent? On 4/20: grandma had a mysterious arm sling, and left unattended on a wheel chair. Not supposed to be by herself on a chair or she may fall again. Nurses were clueless as to why she had that arm sling, it was not recorded anywhere and her arm seemed okay, maybe just a little tight and needed some stretching if anything. Early lunch was scheduled for dialysis days on MWF = 1130am, but didn't come until noon, which means she barely had time to eat as she ate very slowly due to no teeth, weak swallowing, dry cracked mouth/lip sores. Grandma was also missing her shoes, dentures, clothes, dialysis duffel bag, Blistex ever since she returned from the hospital. Keep an eye out for the worst CNA, Boy (big Filipino male); he was very grumpy every shift and not very gentle. On 5/4: grandma's vitals ok but didn't eat/drink, and was just too fatigued and had to skip dialysis; my uncle asked for grandma to have IV or some sort of action at 6pm if she's not eating/drinking at all for almost two days at that point. He had to explain the need. On 5/5/22: at 4pm DON Jerome agreed to have transportation pick up my grandma to Stanford in 60-80mins. EMT arrived at 6:25pm, assessed her and refused to take her anywhere, but Regional Medical Center. We signed AMA 6:56pm to take her ourselves. They lent us the oxygen tank and asked for a credit card. Nurses were very lost and confused. I really question their competency. Also, we needed to remind them of phosphate binder powder at meals because they often forget it most of the time. We found out from dialysis clinic blood lab results that her levels were very high. The CNAs here also use rough paper towels and wet it with water to wipe patients. I brought in actual hygiene wipes for them to use on grandma. They're supposed to re-position patients every 2-3 hours to avoid bed sores, which I don't think the nurses were consistent on, since Grandma got painful bed sores. I have to remind them when I'm there. Admissions/front desk staff seem very air headed and may need some basic communication courses. It's very hard to communicate with them as there are "umm" and awkward silences in every sentence... There were more similar instances, but I think you get the drift from here. It's a sad regret that I let the hospital trick our family into transferring my grandma here for some rehab and supposedly to go home stronger. Instead, she came there and almost two months later she passed away. Beware. read more

Normally what do you expect when you walk into a nursing home? The small of urine and an unpleasant feeling but this place is not like that.. their is a reception desk for guests to check-in and the staff here (nurses and aides) are quite friendly but I don't think they are thorough because the other day my Grandma had a stomach-ache and since my Aunt was there, she walked up to the counter and told the staff about my Grandma's condition only hours later when I asked them about it, they said nobody reported anything so that was quite a shocker to me because the people that are in here.. their condition can change in a heartbeat so I'm concerned w/ that. Oh and they are not quick to respond but I'm not sure how the ratios are so I can't say anything but it is worth mentioning for improvements, if management happens to look at my review. My 86 y.o. mother fractured her right hip on 9/26/22. (Left hip was fractured 7 years prior.) After 1 week at RMC, Mom was transferred to Vista Manor, (room 10A) for Physical & Occupational Therapy. I have nothing but positive things to say, & a long list of compliments for all the professional, caring, competent staff that cared for my Mom. With my high standards, I have to say Vista Manor exceeded my expectations. From day one, greeted by Isabel & Lav, all reception staff are friendly & made you feel welcomed. At intake, Karen, RN was so responsive, I felt confident Mom would be well taken care of, & I was able to go home, worry free & decompress that night. I applaud VM's Covid-19 safety protocols to keep staff, residents, & visitors safe. I noticed how clean the facility & environment were. I was pleasantly surprised there is no dreaded urine stench that I've encountered at 5 other SNF as a visitor. VM spared no expense when it comes to changing bed linens daily & frequently as needed. The awesome CNA's assisted my Mom & kept her clean, dry, fed, hydrated & comfortable. Staff is consistently cleaning the rooms, bathrooms, floors, trash, (e.g., Adult Diapers, supplies replenished daily) like clockwork. Although my Mom did not participate, there were lots of activities to choose from. Mom's dietary needs were met, & I was impressed that Mom's breakfast was served earlier one morning before being transported to & from RMC for suture removal that went smoothly. I appreciated attending the organized care conference with Renee, SW & team. The highly skilled PT/ OT team made a difference as Mom improved each day & regained her mobility, increased stamina, & discharged after 19 days. The only drawback is the shortage of parking. It's a safe neighborhood, so just allow time to park on the street. Our hearts are so thankful to Jerome Alacre, Director of Nursing for his compassionate care & understanding in accommodating my Dad to stay with my Mom due to her dementia. (Shout out to Mai) My eyes well with tears at the thought of how much Jerome cares about his residents' health, well-being & safety. That speaks volumes about the leadership, & attention to trusted care given at VM. We can't thank you enough Jerome, you & your staff are a blessing to those you serve in the community & you are all storing many treasures in heaven! After a short hospital stay I needed to be transferred to a Skilled Nursing Facility for a couple of weeks. The hospital gave me a list of SNFs to choose from, and even though I'm not on medicare I used that site to see which SNFs were rated the best. Vista Manor had highest ratings. The facility is old and a bit tattered with small rooms, however the staff made up for it. (I told myself: "I like camping, so I can deal with this tiny bare bones shared room"). The staff were exceptional: attentive, friendly, and super helpful. The OT & PT staff were phenomenal and made a huge difference in my recovery. The food was delicious, with a 4 week rotating menu. I firmly believe that it's better to stay at a shabby place with outstanding staff than at a fancier place where staff ignores you (I'd heard horror stories of other places, these were first hand experiences from friends, and when I checked ratings the other places were rated poorly). I was transferred here from Regional Hospital 3 weeks ago to do rehab and I'm still currently a patient here. When my Case Manager at Regional told me that their rehab department recommended I go to a SNF, I said no. I opted for Home Health but was told by my sister who's an RN that they can't do much and explained to me that she felt this was a good idea. That I would get the help I needed and rehab would be good for me. So I thought about it and changed my mind. Upon admission, I was instantly welcomed with warm open arms, I didn't feel like I was a new patient. I felt like I was family already. Once settled in my room, Babylyn came by to introduce herself as my CNA for the evening and told me she was going to check my blood pressure. Ever since then, I've settled in well. It was a rough first week for me. I was scared, nervous and excited all at the same time. I met the DON, Jerome on my first day and he was so nice. My cousin was in town from San Diego for a week visiting family and there was a strict visiting policy. Jerome was able to help and approved visits even though I technically wasn't supposed to have visits the first 48 hours. My cousin was with me upon admission and was able to visit with me for 15 minutes since I was a new admission. Everyone here is super nice and helpful All my nurses and CNAs are great. But my favorite CNA is Babylyn. HANDS DOWN! I love her so much! There's something about her...you can definitely feel how loving, genuine, compassionate and caring she is to her patients. She constantly checks on me and makes sure I'm okay. I feed off of energy and know when someone genuinely cares about me and my best interest. The other day, Babylyn checked my blood pressure and it was low. She asked me if I felt okay and I told her yes I and that I felt nothing. I was sitting in my wheelchair and she suggested I lay down and elevate my feet to get my BP up. She said if I stayed in my wheelchair, it would drop lower. So I listened to her. I really enjoy working with her. She is such a sweetheart! It truly takes a special person to be a CNA. I'm not a CNA but I do know it's not an easy job. I am so grateful for her and for all my Nurses. Thank you for all that guys do! I feel like coming here was truly the best decision and it saved my life. read more

This review is for the rehab team at Vista Manor especially Claudette and Lena. My mom broke her knee and femur in March and came here for rehab after surgery. My mom wasn't easy to deal with. She was old and set in her ways and she really didn't want to stay there for rehab. However Lena and Claudette were patient with her and taught her all she needed to safely go home. They also worked with me to show me how to help my mom in and out of the car and how to take her home. Best physical and occupational therapists I've ever encountered! The facility itself is a bit dated as with most rehab facilities in the area. The nurses and staff tried their best to accommodate my mom. It's very difficult to find parking here. Their parking lot is super small and all the surrounding parking is for county workers. Don't even think about parking in one of those spots really quickly as there is a security guard in the lot that patrols around. Street parking is ok but there is a walk. read more

My father just spent a month at Vista Manor after breaking both feet. No one in our area could handle his surgery complexity, so he was transferred to San Jose. After his first surgery they figured it would be easier for him to stay near by (we live 2 hours away) while awaiting his last surgery. Jerome and the entire staff have been nothing short of amazing. The only reason my sister and I felt comfortable visiting every other day was because we knew he was in great hands. Everyone from nurses, CNA's, social services, physical therapy and even janitorial and reception staff have all been beyond kind, caring and compassionate. After my fathers second surgery he will be transferred closer to home, however if we could move this entire facility closer to home to continue care for him, we would do it in a heartbeat! read more

My 80-year old dad was here after a 5-week hospital stay and was extremely weak and malnourished coming into Vista Manor to the point he could not sit up on his own. After 5 weeks at Vista Manor, he was able to walk the halls of the manor and was able to walk up 8 steps which was required to get into our house. The staff there was extremely diligent in not only physical and occupational therapy, but in wound care and bathing. The staff were very responsive to our concerns and gave us a solid amount of training for us to be comfortable taking care of him at home. If anything were to happen again, I would definitely consider Vista Manor again. read more

My dad had hip replacement surgery in early March, so we chose Vista Manor for his rehab needs. Two weeks into rehab, he unfortunately had massive bleeding which resulted in a 10 day ICU stay/hosiptalization, so he returned to Vista Manor at the end of March. This is the first time a family member needed rehab, so we were very stressed out, and had no idea what to expect. A special thanks to Vista's Business Office Manager Lorena, she helped to answered all of my questions and concerns when I called in initially to get information. Throughout my dad's stay, she was very helpful and always tried to help us when we reached out to her. The entire nursing team was very caring and professional. They took great care of my dad, informed us of changes in his medications or condition, and helped to arrange all necessary doctor appointments/transportation. What touched me the most is the fact that the Nursing Director (Jerome) is easily accesible, and he always took the time to answer our questions. If we had a request for/ or a question about my dad's medical needs, he ensured that they were answered. The physical therapy team led by Cherisha always worked with my dad patiently, and kept us updated on his progress. Due to his medical conditions, my dad's ability to do physical therapy was limited during the first few weeks of April. Cherisha never gave up, and trid to find ways to motivate and help him during each session. We are thankful for the care my dad has received from the entire Vista Manor team, and would highly recommend this facility to anyone looking for rehab services for a loved one. read more

My mom was transferred here to be on hospice. I had a bad feeling about this place the minute we walked in! It was loud and everyone was walking around with no masks handing out cake. Yet with people coming in you have to fill out 3 damn papers just to get in. I get the COVID rules and protocols but damn! My mom's first night they had no med list for my mom so she was given nothing for the first few hours she was there. The hospice nurse was more on it then these women here. The only time my mom got any attention from them was when the brought her meds! No one ever just came into just check on her. When my mom took her last breath, after me sitting with her for 5 mins after I HAD TO GO FIND THE NURSE AND TELL THEM MY MOM PASSED. If my mom was there alone who knows how long it would have taken them to find my mom gone! Thankfully I was there with her! Mind you my mom was on hospice and didn't last long there. I feel like these people could have been way more compassionate towards my mom. One nurse kept laughing AND screaming loud like if she was at a damn party! NO CONSIDERATION FOR THE PEOPLE AROUND HER. I had to shut the door! When my mom passed not ONE of her nurses came in to give condolences. The only lady that did was the one who brought my mom her food. This is my 2nd time coming to this facility. Currently, my mom is in a short term 24/7 health monitoring after being discharged from Regional Hospital. The admission personnel remembers me and thanked for choosing their program again. Customer service is on point, kudos to Arissa/Christina/Rayleen who made process easy to understand. Also a plus, they do this care conference in person, which is a round table from beginning to end an overview of standard of care. Nurses call for updates and with every changes in-house care. Not expecting to even rate a skilled nursing facility at this age LOL, but when I came to visit my girlfriends father I was surprised. From the moment we walked in to leaving .. I expect any nursing facility to be held at this standard. Jerome A. who is the director of nursing was super professional and explained the visiting guidelines/protocols. He answered all my questions and concerns about COVID and my girlfriend's question regarding her father. At first I didn't even know Jerome was the director; as he made everyone feel welcomed and comfortable. He did not have a complex that you would think "directors" would project, he was so friendly, interactive and knowledgeable. His leadership, compassion and engagement for his team and patients is admirable. My GF was even surprised how her father was taken care of and how much of the CNAs especially Teodola and Angie adore him and vice versa. Not to say even the nurses and Jerome my dad knew! The nurses were super friendly and are busy! We did not feel rushed nor pressured, everything was seamless once walked in. I'm so happy I chose this place for them because they loved how clean it was and didn't have a distinct odor like usual nursing homes! I visited often and I was always greeted kindly by the staff! Majority of the staff are Filipino and I noticed that they naturally treat everyone as family which was a plus for me, personally. A nursing home is the most sensitive place to be in, but with the staff, they made us (visitors) so comfortable like it was our 2nd home and treated their patients well. Vista Manor Nursing Center is right behid Alexian Brother Hospital. aka Regional Medical Center. They have a small parking lot so I wasn't so sure about parking since it is clearly marked for Permit Only. I was afraid of my vehicle getting towed away. STreet parking was pretty full for the reason that it is situated by Residential Town homes. But I squeezed in a parking. This facility is quite clean, nice furniture and painting on the wall. The decor is very impressive. First thing I noticed on the entry door is a sign stating "If you are sick with the FLU do not enter for visiting" I find that very caring for their patients. They have a waiting area and extremely pleasant receptionist that will look up the patient and tell you what room they are in, and she will direct you to their area. They will also have you sign in on a registry book. This shows who visits each patient and their relationship. Of course this is a rehabilitation center so they have a dinning area where you can find several patients moving around in their wheel chairs. I have been in quite a few nursing homes visiting serveral members in my family. I found this one to be very pleasant and inviting, homey with a touch of class. I was visiting my 86 year hold aunt that had fallen and fracured her hip. She had surgery and they put a pin in her hip. She was here to rehabilitate. Visiting was pleasant, my aunt Sally was very happy with the care that she was provided. Her room was shared. My aunt happen to buz them while I was there and they actually came to her room within minutes to see what she needed. And if she was okay. My mother was a resident of Vista Manor (VM) for 7 years. My research, screening, and interviewing led me to VM and I never regretted that decision. Admissions and the business office were patient with me during the onboarding process. Tia and the social services staff kept me apprised of my mother's progress through quarterly status updates and Q&A sessions with the entire care team. Jerome and his nursing staff are professional, dedicated, and caring. I could always count on them to keep me readily informed and consulted on my mother's care on a daily / as-needed basis. They were always open to suggestions and patient with my concerns and questions. Cristina and Vesta kept my mother busy and active with regular planned activities & many scheduled celebrations. Vesta adopted my mom and looked in on her every day to ensure that she was never lonely and always felt at "home". The CNA's have a difficult and thankless job but were always attentive and responsive. Carol and Colette in physical / occupational therapy worked hard to keep my mom strong for the longest time. Kristina modified my mom's dietary plan proactively to sustain her. Everyone there, including housekeeping/laundry, maintenance, reception is focused on helping you and your loved ones get through whatever difficult situation brings you to their front door.
