This is a long overdue review for both my Father-in-law and Mother-in-law, and I truly couldn't be more grateful for the care they've received. From the moment they arrived, the care and professionalism stood out. The admission process was smooth and welcoming thanks to Carol, the Admission Director--she's truly wonderful, kind, and very helpful every step of the way. Jason, the Assistant Director of Nursing, is amazing--attentive, responsive, and genuinely cares about his patients. There was a moment that really stood out to our family--he made the critical call to send my father-in-law to the ER after suspecting a GI bleed related to his G-tube. That decision made a huge difference, and we're incredibly thankful for his clinical judgment and quick action. The entire team deserves recognition--from the Social Workers, Dietician, Nurses and CNAs to PTs and OTs. Everyone is compassionate, patient, and hardworking. You can tell they take pride in what they do and consistently go above and beyond to provide quality care. Honestly, the entire staff deserves some kind of recognition for the level of care they provide. The facility is clean, organized, and well-managed, and communication has been consistent and reassuring throughout. Not only did they provide for all of her medical needs, they laughed with her, listened to her, and truly cared for her. In her final days, the entire staff showed amazing compassion for her. My mom had been at this facility for a month and was discharged yesterday. And it has been amazing! I highly recommend this place! Once I got a tour of the place, I had high hopes for this place. And they delivered! Shoutout to Yasmeen for giving me the tour. She welcomed me with open arms. The lead of the station that my mom was fantastic! Huge thanks to Lanie and her team! The nurses in the station took really good care of my mother. Johnathan and Jacklin were the best nurses! The CNAs were also great! Karla was amazing CNA! Also their physical and occupational team has been exceptional! Shoutout to Gloria and Jackie! My mother had lost 40 pounds since being at this facility! This place was recommended by another facility that did not have availability for my relative. Took a tour and things looked and seemed okay. My relative spent one week at Redwoods after being released & transferred there from a hospital. I went to the Redwoods everyday except one day while my relative was in their care. While getting my relative situated in their bed, I noticed on the other side of the bed the floor was sticky. Looked under the bed and saw a pill near one the wheels. Looked up the numbers and discovered it was an oxycodone pill. I threw it in the trash as it was not proscribed to my relative and was apparently left there from another patient. Later, I went inside the bathroom and noticed a toothbrush and toothpaste apparently was left there by a prior resident/patient. Left it there thinking maybe it belonged to someone who may be coming back. After 5 days of the items being in the exact location untouched, I threw it away myself. My relative eventually got a roommate after having a couple days in the room by themself. Obviously, the room was not fully cleaned prior to my relatives arrival. Now the care and intake. The facility temporarily "lost" my relatives paperwork that had been filled out upon arrival that day. It was found awhile later. Was told it wasn't found because their name was missing from the first page. In my visits there, most of the staff were helpful & kind. However, had to always be vigilant with asking for help and new staff always had to be brought up-to-speed upon every shift change. This you would think be done automatically when there is a shift change and have a new patient. There seems to be a lot of bureaucracy within the facility & treatment and how to go about things there. Also it is obvious that there is not enough staff to sufficiently accommodate the patient to staff ratio. My relative was having a very hard time breathing toward the end of their stay. Was on oxygen tubes, but still wasn't getting enough air and wanted something for anxiety. After two days with with no relief, length of time for the doctor to proscribe something, and getting to nursing staff to please take their vitals and listen to their lungs, they finally agreed my relative needed to go to the hospital asap. Seems if one doesn't advocate for the their loved ones in these types of environments or places, visit often to see how things are done, or find someone who will act with a sense of urgency, they can easily be overlooked. When I told them of the concerns and what I encountered & that they won't be back, all they said was "I'm sorry, I wish I knew." Seriously?! read more

No consistency lack of training or bad management not sure which one yet. Maintenance and cleaning are important. Being stuck in a care home sucks so at least the care home can do is be as vigilant as possible to keep things visually clean to the eye. read more

The Redwoods: Let me start with the admissions process: My experience was wonderful. The same day I was notified a female hospice bed was available for my mother at The Redwoods the admissions nurse, doctor, Redwoods Hospice, all came to my mother's home. The process was expeditious, comfortable and caring. That same evening, I received a phone call from the Marketing Director, Carol asking more personal information about my mother wanting to get to know her personally as well as medically. This phone call really made me feel like they really cared about my mom. When my mother and I arrived at the facility walking through the doors my first thought was "wow" this place looks and smells clean. The front desk receptionist was friendly. Visitors sign in on computer and a printer prints out a label with your name. The facility was organized, and the staff was friendly and very helpful filling out the initial paperwork for my mother. My mother has Dementia and Parkinsons and has now been living at The Redwoods for 4 weeks. The CNA's, Nurses, and Wound Care personnel treat my mom gently physically, and when talking with her. My mom feels safe and comfortable. My mom is at the stage of not wanting to eat solid food, so the staff is quick to make sure she has yogurt, jello, and Ensure when needed. Redwoods has been on too of everything w/ safety during the pandemic. The staff has been taking precautions as needed to keep any covid cases away from the patients. They provide physical therapy etc as needed, as well as any food requirements and medication. it has been a bummer to not be able to visit my MIL, but we've been able to FT her as much as possible and drop off her snacks and food that she loves to eat. Over the last year, the staff has been notifying us of anything that affects my MIL immediately. They are very friendly and work we'll with all of us. Prior to the pandemic they had events for the holidays where the families were also invited to attend. Daily events on the calendar such as church, movie days, bingo, etc to keep the residents involved. read more

My auntie stayed at Redwoods for more than 3 months and we visit her almost every week. I find their administrator to be very nice when he came in and talked to us. Their nursing staff are very caring and promptly answers the call bell. Their rehab staff are very awesome too. They helped my aunt a lot who was there for rehab due to a broken hip from her fall. Their social workers are super nice as they always help in my aunt's transportation, and provided us wirh the resources that she would need when she goes home. I would certainly recommend this facility to our friends and relatives! read more

The building is unassuming outside but it really looks so nice and clean inside. The staff are welcoming and always cheerful and greets everyone. My father had been to several facilities but it was at Redwoods that he felt he was really cared for. The nursing staff are very efficient, the rehab staff know their job well, and the food is really great! Their social workers are also very nice and very helpful. They helped us in applying for IHSS, among other things. They helped us with transportation too. They made our lives easier while our father stayed there. read more

This is my 3rd go around in dealing with rehab/nursing facilities in San Jose. My aunt was living in one from 2009-2012 & was one of her primary caregivers. My Dad did short stints in 2 different facilities in 2017 before his 2018 passing. Now my brother is in this facility for neuropathy & neck surgery rehabilitation. All I can say is that I'm suffering from PTSD as a result of having to deal with these facilities again. The lobby of this facility has been remodeled & appears presentable. They have a good visitor check in procedure & takes necessary COVID precautions. Peeling back the onion, the rooms are shabby with old broken down dressers & the bathroom & doors appear unclean & barely functional. The same protocol goes on in this facility just like the others where the nurses sit behind the desk & completely ignore you if you ask a question or inquiry. They rely solely on the few OVERWORKED CNA's who can barely manage the patient needs & certainly NOT in a timely manner. My brother has said to us that he doesn't expect to be given preferential treatment & run to his beck & call but waiting up to 1 hour for help to the bathroom especially in the middle of the night is unacceptable! My brother was here before his surgery & had a diabetic diet. He left for 1 week to have the surgery & was put on a regular diet upon his return despite mentioning his first day back that he can't eat the regular diet. It was a week later that I called & confirmed he was still NOT on a diabetic diet & requested the change. Tonight when his meal was delivered, 2 CNA's & nurse couldn't tell me if his meal was as diabetic as they delivered Lay's potatoes chips & saltine crackers with his soup & sandwich. They said the food preparers know what meals to make but yet they had no record. My brother takes a once per week Oxempic insulin shot. The nursing staff initially asked me to provide his personal medication from home because they had trouble getting it. After his surgery he hadn't received his weekly shot despite asking for it. I called the nurse again who confirmed they had the medication on site & had not given him the shot. My brother was moved once to another room next to a big window without a blind. I requested a blind be out up since the room gets drafty at night by the window. It took over a week to get a new blind installed that didn't even cover the entire window. The worst part of this experience was with one of his roommates. This man had obvious mental health concerns & yelled obscenities at the top of his voice all day & night. F*^K was his favorite obscenity & even called my brother & other black CNA's the N-word. He always has his curtain pulled back with no pants or diaper on completely exposed & yelling. Tonight he got up from his bed & walked to the nurses station again with no bottoms on & exposed. NO ONE tried to get him back in his room. When I called it to the nurses attention who was at her medicine cart in the hallway, her response was "oh yeah, he just did that" & continued doing what she was doing. I don't even know what that means! Someone finally got him back to his bed & another visitor was horrified that nothing was being done about this patient & voiced his concern to me. Needless to say I spoke to the facility Social Worker & requested my brother's room be changed tonight so he could finally get some rest after his surgery. I only hope & pray my brother's rehab goes well & discharged to return home soon. Wish I could give a better review but when you see things right in front of you that are not acceptable, what might be going on when not there. Heartbreaking & frightening! Just don't understand why all have the same flawed procedures & protocols that seem to make the patient worse instead of getting better. read more

My family member has been here for a little while. Admissions process was so easy, Carol and her team were great from start to finish. The nurses and cna have been great with him. However, can't give 5 stars because our family's experience with a discharge that isn't supposed to happen has been a nightmare. The social worker Yomi I can only describe as bully. And her "director" isn't much better. They think they are in charge of the whole place. My family member supposed to stay there but social workers trying to kick us out. I wish I would has have saved all the voicemails. So rude and threatening. We told them we want information if what is going to happen so we can think about and know it all, that's not too much to ask. But they go behind our backs and set up a discharge and WE DON'T KNOW WHERE THEY ARE SENDING OR FAMILY MEMBER! Redwoods needs to consider having new people because if not they are going to mess up the reputation. I will only recommend if they have new social worker read more

My dad had a stroke and was bedbound, and couldn't even move his eyes. When he was admitted rehabilitation for him was quote thorough, and he's now capable of opening his eyes, talking, and moving an arm. The rehabilitation center here is wonderful and the staff here was nothing but helpful when it came to attending to my dad. read more

Placed my senior parent here for care and here are a few things I would say after more than a few months: 1) staff has good intentions and 2) they run on a very tight budget. What this means is 1) over worked staff and 2) delays in everything from hygienic care to getting meds. Note that day shifts are filled nicely meaning night shift are on a skeleton crew. You ask for something nicely and they ignore you; so you have to yell and be unfriendly to get what you want/need. b. The staff is poorly trained.Didn't even know how to help me out of bed. 2. Physical Therapy is practically non-existent. 3. They are not truthful to their patients. a. Told me that my insurance would not pay for my transportation home because I was leaving against their advice. The only advice that they gave me was to stay.at the nursing home. I never did hear about a healing or therapy plan . b. Told me that they would arrange for home treatment like physical therapy, a walker, prescribed medicine. I had to go back to hospital to get it. c. Had to cal the insurance myself in order to get transportation home. The food that is supposed to be served warm is served cold and isn't prepared well. My mom had a serious stroke in January 2019 and then moved into this nursing home. The experience for her and me was very bad. There was a mentally sick male patient who always stormed into her room. I was advised to report it to the Department of Public Health and a few months later he was transferred. But the staff scrutinized us including constantly searched and confiscated personal supplies like shower/shampoo. The director of nursing even relentlessly pursuaded my mom to have high blood dosage blood thinner to dissolve her lung clots but the hospital medical team had already advised against it to prevent triggering another stroke. Then she tried relentlessly to have my mom sign up for hospice that I refused (my mom is still recovering well as of August 2020. Since I arranged my mom to go outside to see her Stanford doctors and accupuncturists, the staff there imposed on moving her to another nursing home the same day they found an alternative. I was not even allowed to visit the second nursing home. It's an extremely horrible experience with this nursing home. read more
