I am a female patient with an extensive history of abuse, and I only recently became fully aware of…read morethe extent of the trauma I endured due to dissociation. I initially sought treatment from Dr. David Shaw in my 20's for what I believed to be depression. After 10 years of prescribing me drugs of little help (without ever asking if I had a history of trauma), Dr. Shaw began injecting me with Ketamine (a dissociative anesthetic) starting with 80mg and then quickly moving to 120mg per injection of Ketamine weekly. I weigh 120 lbs and every injection caused complete loss of consciousness and amnesia for the duration of the session afterwards. After asking him to reduce the dosage, he eventually injected me with 100mg of Ketamine on one routine visit and then proceeded to allow me to leave his office (unconscious) 18 minutes after injecting me. I have ZERO recollection of this. I "came to" IN JAIL, after the drug he injected had worn off, and after being charged with a DUI - all due to him injecting me with Ketamine and neglecting to supervise my recovery. He does not supervise patients that are under the effects of this dissociative drug and has no one in the office to do so. This is extremely dangerous, and proved to be so in my case. After this incident, I researched Ketamine administration and most clinical standards dictate that a patient is not allowed to drive for the entire DAY of Ketamine administration. The patient should be monitored for the duration of the drug administration and both be driven to and back home by a responsible adult. Dr. Shaw allowed patients themselves to drive both to and home from his office at their SEVERELY IMPAIRED and UNCONSCIOUS discretion after injecting them with Ketamine, dismissing them, and moving on to the next patient. I am not sure how I made it home all of the other times he dismissed me and let me wait alone while I decided (again, impaired) when I could drive the 45 minutes home. I have dramatically regressed since this incident - my PTSD and anxiety are unmanageable. The trauma of being arrested and put through the court system for over two years while I waited for the blood test to come back, the stigma and shame caused by his unsupervised Ketamine administration and resulting arrest, night in jail, and prolonged court procedures have been utterly debilitating, detrimental and have severely harmed my quality of life. The harm caused by Dr. Shaw's negligence still has a devastating impact. Prior to this event, I was transitioning careers, had just completed a rigorous continued education course and earned a certification, was interviewing, and fully capable of functioning at a high level. Since that day, I have been unable to function normally and have undergone intensive therapy to address the trauma caused by this incident. I am now thankfully in treatment with two very compassionate and safe female therapists and beginning to work on my ability to trust healthcare providers. Another incident of note: I also "came to" during one of his ketamine injection sessions (prior to the incident I described above - I have not seen him since) on his couch with a large bleeding gash on my forehead, and upon touching my head and seeing blood, I asked Dr. Shaw what happened. He then proceeded to tell me I "fell over" during the session and "hit my head on his rock crystal," and grabbed a paper towel and sprayed it with an alcohol based antiseptic and started blotting at my head without my consent. Looking back, this was a huge red flag. I had a large laceration and egg sized bump on my head and have no recollection of how it got there, other than him telling me I fell on his rock crystal. And then sprayed my head (yes, he also sprayed my head) with antiseptic, blotted it with a paper towel, and let me go. My hair was covered with blood near the gash and I had a very painful shower and a painful two weeks healing after that. AVOID THIS DOCTOR. I do not want any other vulnerable person to be harmed by his negligence.

Dr. Shaw immediately went to work on my meds, cutting out more than half of them and helping me feel so much better. Though we are still at work on the right balance, I never doubt his ability to help me. The best thing about Dr. Shaw is that he ACTUALLY CARES. Most psychiatrists (or any MD) see a walking ball of symptoms. Dr. Shaw sees you. He wants to know who you are so he can identify your symptoms. She stated that they "were not going in that direction" of taking on new patients... odd way of saying "no" but that's fine. I then asked if they have any referrals to offer she laughed and said they didn't do that... then went on to explain that I would have to meet with the doctor to get a referral. It's not odd for a doctor to refer other professionals in their field; I've received referrals from other doctors before. Such a terrible experience for anyone suffering from anxiety or any other mental illness. Marla should consider another profession if she can't show consideration for the type of clientele that would call a psychiatrist's office.

Dr. Orenstein has been my doctor for the better part of a year and has been extremely helpful with…read moremy anxiety issues as well as my ADHD. After my first meeting I was still undecided and continued looking for doctors but made another appointment anyways and during that second meeting the basis of an extremely dynamic and trusting relationship formed. Dr. Orenstein primarily practices psychopharmacotherapy which means that he treats psychological disorders with medication in conjunction with talk therapy; however, he has admitted that this approach does not work for everyone/every condition and also offers alternative suggestions. The extent of his knowledge about psychopharmacology is extreme and he ensures that his patients are well aware of possible side effects and follows the guidelines of informed consent. One such example of an alternative therapy was suggesting exposure therapy for my driving anxiety. He suggested I try going on long driving sessions and document any changes in my mental state as time progressed. Within 2 months of following this advice (I was limited to driving on the weekends) I was able to overcome the majority of my driving anxiety and actually procure a driver's license. Additionally Dr. Orenstein follows the American Psychiatric Association's (APA) Guidelines for Ethics and Practice, erring on the side of caution. The office does this--in part--through their office policy that does not allow patients to be seen that are/have been romantically involved with a current patient or family members of current patients unless they are seeking therapy jointly (in the same session). Marla handles his scheduling and the administrative duties of the practice and does a wonderful job of accommodating busy schedules. If you have a restrictive schedule they will do their best to work around your availability and will even go as far as asking more flexible patients if they would consider moving their appointment (I have offered to change my appointment on several occasions). Overall, Dr. Orenstein is an extremely competent and effective psychiatrist who is very patient oriented.

Today I am still a satisfied client. Recent checkup up was optimal. I developed severe anxiety…read moreduring the pandemic. Saw multiple psychiatrists testing meds on me was hospitalized. When I got out I had no energy. Dr Chapek and The Tukwilla Amen clinic did 2 brain scans and supported me in blood testing for deficiencies. Dr Chapek helped me get off all the meds and switched me to amino acids, increased my vitamin D which lifted my depression. Adjusted a hormone imbalance which regained my lack of energy. First of all, let me just say that just because the Amen clinic was not a good solution for us, it…read moremay very well be the solution some people are looking for. The office staff, Ruchika, was AMAZING and on the ball when helping clients. We saw Dr Mona Karimpour who is knowledgable and initially got back to our questions within 24 hours. Getting appointments became trying. The average wait time was 3 months, and we could not even expect our insurance to cover the hefty bills. We were also given access to a naturopathic doctor, but sadly, even though I knew Dr Chapek did his absolute best, the labs he ordered did not address my son's extreme fatigue. He stated it was mast cells and prescribed us Quercetin to help with this. My son took it religiously for three months, as prescribed, but he was still wiped out. Dr Karimpour seemed to think, based on the SPECT scans, that the fatigue was caused by PTSD and recommended EMDR therapy, which sadly only marginally worked. Fast forward three months, a parent (a parent, mind you, NOT a doctor) suggested I screen my son for PANS/PANDAS, an autoimmune disease with severe neuropsychiatric implications. We contacted a doctor knowledgeable in the field who ran tests that indicated my son's cortisol level was only half of where it should be (explaining the extreme fatigue), and also an antibody panel that indicated he had a subclinical presentation of STREP. A few days on antibiotics, and my son is already showing better energy level and focus. Granted, this condition is still heavily researched and still, there are doctors who refuse to acknowledge the link between infections and neuropsychiatric conditions. Having spent a hefty 6000 dollars for brains scans that only scratched the surface of the problem was certainyl not worth it. What's worse though, is the ARROGANCE of doctors who yell at parents about being "board-certified" instead of listening to parent concerns and encouaring open communication. Would I recommend the Amen clinic? If you are only facing a simple condition, sure why not, but for a complex condition we are faced with...not a chance.
