Got the exam and took the form they provided. The license branch wouldn't take the form because it was an outdated form. I contacted all about eyes and the manager told me she had just downloaded it from the states site(Lie). I went to the states site that day and it was not the old form. I sent them the correct form and the dr filled it out and they sent it to me in the mail. Fast forward to March 2026 and I get a letter from the state saying that the form was not dated within the 6 month period of my exam date. I got the exam two weeks prior to my license renewal. After checking it out, the form was dated from my previous exam in march of 2025. When I asked to get it corrected, they said they couldn't because if they did they would have to charge me for the additional exam. I told them I would be glad to pay if they would just put the correct exam date on the form. They had the doctor put a line with an arrow to the bottom of the page with a note that said re-tested in September of 2025. The state still wouldn't accept the form. I asked again and was denied! I talked to the area manager in Bloomington and they still refused to fix the form. Now I am facing the fact that I may lose my license because this company will not do the right thing! Their medical director said that I needed to have my eye health checked before they would consider changing the form. I informed them that I had seen a retinal specialist and a cornea specialist in the last few months and my eye health was good! Avoid this place at all costs!

My visit was on January 30,2026, and although Betsy was out to lunch when I arrived, I didn't mind waiting. As soon as she returned, she greeted me right away. Betsy was the most helpful-polite, smiling, and very easygoing. She took the time to thoroughly explain my insurance, referral, and out-of-pocket costs, answering all my questions with patience and clarity. I found her extremely knowledgeable and delightful to talk with. She even encouraged me to come see her with any future concerns. One thing I have found disappointing is their attitude toward patients who choose to use their prescription to order contacts or glasses elsewhere. It feels as though if you do not purchase products directly through their office, you are treated differently because they are losing out on those sales. Most recently, when I scheduled my annual eye exam, I was told there were no contact lens samples available to help me get by until my appointment because I had previously ordered my contacts on my own. After being a loyal patient for more than two decades, I found this very frustrating and disappointing. I understand businesses need to make money, but patients should not feel penalized for exercising their right to purchase contacts and glasses where they choose. Customer service and patient care should come first, regardless of where prescriptions are filled. Because of this experience, I have decided to take my business elsewhere.

We needed to see a neural ophthalmologist and the doctor she referred us to was not qualified for this exam. And we found out she knew the doctor was not qualified. This was not only cruel, it borders on malpractice. We found a neural ophthalmologist through our Primary Care Provider only 30 minutes away. The care we received from the Gailey doctor not only wasted our time and money, but was extremely cruel to my wife.
