They were absolutely awful. They refused to clean his bedding because he could not get up out of bed. It was excruciatingly painful to put weight on his FRESHLY RODDED LEG. They left him in soiled bedding covered in pus and blood that oozed out of his surgical wound. At some point he told one of the employees that they could be kinder to him because he has cancer. She replied that she knows other people who have cancer and that those people "didn't lay down to die." HE JUST HAD MAJOR ORTHOPEDIC SURGERY, YOU PSYCHOPATH! Other things: The physical therapist had no idea how to work with his condition and help him regain mobility. They wheeled him to physical therapy in a wheelchair that was missing the footrest on his bad leg, so that was excruciating for him, then they wanted him to do an exercise involving pulling weights toward him that were mounted on some kind of a wall or machine (not 100% certain) but it was a rowing motion. It caused the chair to move forward and he had to use his bad, freshly operated on leg to brace himself. Also, there are no phones in the rooms so if your loved one does not have a cell phone, as mine didn't, it will be insanely difficult for them to get in touch with you. To make matters even worse, they would not help him to get up to use the bathroom and they recommended an adult diaper... and they did not like to change people. The entire place reeked of urine. There are better options. Don't send anyone here, especially if they have issues with mobility.

His sheets were filled with wet urine & it appeared as if he had not been bathed or fed in a very long time. They never put his hearing aids in & he struggles to hear what is happening around him. This is a deplorable place & I would never leave my loved one here!

Having had a not so good experience at a Renaissance with a relative, I decided to choose Manor Care for my post-hospital care. I am very glad I chose Manor Care as my experience there was very good. From the nurses to the Physical/Occupational Therapy Staff everyone was attentive to my needs. Quite honestly as I was there over Christmas, I thought the staffing would not be as attentive, but I was wrong. There was more than a skeleton staff there the whole weekend and everyone was more than happy to help out. Even the food at this location was excellent. Sure there were some drawbacks but the staff there made sure I was comfortable and they helped me get better while I was there just over a week. If I ever had the need for skilled nursing again or I knew someone else who needed it, I would not hesitate to recommend Manor Care as their service is excellent and am grateful that this place helped me on the road to recovery.

    My mother has multiple comorbities: DJD & OA of the left hip with pain, generalized weakness,…read moredecreased activity tolerance, generalized OA, RA, hyperlipidemia, CAD, ESRD stage 3, DMII, HTN, COPD, abnormalities of gait, impaired balance, hx of falls, CVA, ADLs/IADLs dysfunction, generalized weakness, incontinence of B&B, difficulty with transfers & mobility. Active Home Health Care accepted physician's orders to provide skilled nursing, occupational & physical therapy services for my mother on May 3, 2021. Janice Gladney, the client's daughter, called the agency and spoke with Ruthie who confirmed their company was in receipt of the physician's orders from Dr. Gruss from West Suburban Hospital in Oak Park, Illinois. On Saturday, May 15, 2021, the nurse arrived with her equipment in a black garbage bag, per my Mom. [What about the nurse's bag with a clean/dirty side of the nurse's bag or COVID-19 precautions?] The occupational and physical therapists never showed up, which is client abandonment. I am an OT. industry standard practice of 48-72 hours is generally the policy to contact a client and initiate services. I called Active Home Health and spoke to Ruthie about canceling nursing services on May 20, 2021 secondary to the lack of their availability; i.e., the nurse was only available on Saturdays or Sundays. What happened to the other 5 days of the week? Why accept the physician's order to provide the needed care upon discharge when Active Home Health Care knew they could not provide the services? Previously, my Mom was being seen by Active Home Health Care physical therapist who was initially providing PT twice weekly and then her frequency was decreased to once weekly all which happened within a week. When asked why her services were decreased, my Mom stated the physical therapist told her Active Home Health Care did not have the staff to provide coverage for the initial frequency. When I spoke with Susie on May 27, 2021 regarding the aforementioned problem, Susie was very unprofessional and told me to go elsewhere, go to another home health agency. Basically, a month of much needed services were not provided. I asked why did her agency take the physician orders to provide services on May 3, 2021 with the nurse's SOC initiated on Saturday on May 15, 2021. What happen to the standard of 48-72 SOC? The RN told my mom she would only be available on Saturday at 9 AM. Susie stated on 5/27/21, they had skilled nursing services only on Saturday and Sunday for my Mom's area - Austin area of Chicago. Again, why accept my mother's case if the ordered services were not available? The occupational and physical therapist never completed their assessment. On May 27, 2021, my mother saw an orthopedic specialist, Dr. Jeffery Meisles of Elmhurst, IL. He is in the process of evaluating my mother's options for a left hip replacement. As Dr. Meisles looked for the physical therapist's notes, he do not fine any notes. He asked how long had my mother been discharged from West Suburban Rehab? My sister replied, May 3, 2021. Needless to stay, Dr. Meisles' facial expression said it all, "what the hell." I advise anyone who is scheduled to receive services from Active Home Health Care to run in the opposite direction. I will do everything in my power to have their accreditation and license removed. Do not use this agency if you want your love one to have consistent, timely care and physician ordered services.     #19/2024 After having a major leg bypass and a two toe…read moreamputation at the University of Chicago and Hyde Park Illinois main campus I was not equipped to head home just yet because my leg was not cooperating for the stairs that we're going to be needed. I chose Elevate Care Center and Palos heights because it wasn't too far from home and the reviews on Google were good the one review on Yelp I just kind of laughed at. Elevate care centers are basically a group of rehab and nursing homes managed by a corporation who are buying older facilities and remodeling them and doing a whole lot more they're not brand new built from the ground places. This location in Palos heights Illinois in the Chicago suburbs is basically once known as The park Villa nursing home Center. They added on a few years back to the rehab part you can see the difference as you come through between the old building and the new building. Is it still a nursing home for the most part with long-term care and long-term rehab but they have branched out into short-term rehab. The rehab rooms are quite nice they have very good air conditioning units I can say there were some very very hot days and did a killer job cooling the room.. they also have refrigerators in the room. The only thing they don't have is a microwave but there is a microwave you can use by the dining area community dining areas they call it. Through the most part the staff is quite cordial. Although they're a little limited sometimes on the night shift it is possible you will push the button to get a CNA and have to wait a little while depending on what area of the building they're in. It happens in the post covid world. The nurses are fairly thorough they get your medicines dispensed in a timely manner each throughout the day and night if need. You're assigned to Doctor who basically just oversees your pills and looks over things and reports you don't really consider him a main doctor but he just kind of the doctor that's there at the facility. The TVs are really nice only problem is they're not high-def here in 2024 LOL. They're a little staticky but it is what it is. There's a decent choice of channels for the most part though. There are no showers in the rooms except for a couple of the older units you have basically community showers you're allowed to showers a week. But the bathrooms are very nice good sink good water pressure. The cleaning crew comes in once a day or every other day and gets the floor and supplies garbage cans ETC. if you happen to need your sheets change she just asked one of the nurses during the day and they will. The wound care team is very good also they basically changed my dressings as needed about every 3 days which was fine they did a very good job two days during the week except the weekend. Clean the wounds and the stitches and everything so I had no trouble as far as I went. The food's good it's not great all the time you get a menu and it pretty much tells you what you're going to get. The nice thing is though there's food delivery nearby you can call out and have it delivered I did a few times Pizza, Jimmy John's, and dad brought me a Italian beef one day. My sister went to the store for me I gave her a list so I was able to have Doritos and yogurt cottage cheese diet Dr pepper and stuff like that if I needed not having to waste a bunch of money on the vending machines. As I said it's a decent place it's not perfect they have their hiccups here and there but for the most part they do what they can and as for short-term rehab I was only in there a little over a week I didn't have many problems. They keep up on your appointments as long as you let them know when they are they weren't able to arrange a medical car for me on the day I had to go back to Hyde Park Illinois but the head of nursing Don director of nursing did give me an Uber, and because I had some trouble here and there they had somebody escort me and go with me on that day too so that was very professional. But I will definitely say is the PT and the OT team are very good and thorough they will get you bring you and get you moving which is a good thing and that's what I needed was leg strengthening and they worked with me so when I went home I was able to climb the stairs. I have eight stairs leading upstairs and I have seven stairs leading downstairs from the hallway. So all in all it's been pretty good and because of the fact that my left leg is not in the greatest of shape I may have to repeat the process again I would probably pick here yet again. If I needed rehab. So basically it's really three and a half stars but when I look at everything even with the little hiccups here and there I bumped it up to four stars because I didn't like seeing the one star and the reason as to why this guy was complaining

The administrator never responded to phone calls and admitted that the best way to engage with him was on-site. The care team sent her to Northwestern Palos Hospital without contacting her family. Worst care experience witnessed. Do not send your loved ones here.

    I have been there twice after accidents and surgery. The staff is severly understaffed. However…read moremost of the staff are good and do care, they just can't possibly meet the needs of the patients without additional staff. A couple of staff members stand out as exemplary, but then there are others that don't care at all. Have you EVER heard of CNA's delivering food. That should not be their job - they should have food service workers do that. And they talke about the aqua therapy and pool. I was one of the very few people to use it. I was told it gets used about 2 or 3 times a year. So, they should stop promoting that feature. May main complaint is the FOOD. It would have to work hard to get up to the level of terrible. A health care facility where you cannot get a piece of fruit, if your life depended on it. One time my stomach was queasy and a nurses aid looked all over the building for a couple of crackers. They should be at each station. If my husband did not bring me food every day i would have starved. One time i had a little of the fish???..shortly thereafter i became violently ill - and so did about 50 others. They told everyone it was the stomach flu. I think not - more like food poisoning. They have to get their kitchen and staffing levels fixed. Until they do, i would also say - stay away.

    My uncle was thrown in this place after Palos Northwestern Medicine Hospital allowed a distant…read morefamily member to have me removed from my uncle's care. My uncle's health was improving. I was in the hospital every day feeding my uncle every day making sure he was thriving and getting back to health. Once they kicked me out they placed him here in November. He fell out of bed, his health declined, he slowly starved, got pumped with pain meds, lost weight and was dead on February 10, 2026. My uncle was isolated, I was told we could not visit because APS said so. I was threatened that If I or anyone comes to visit we will be arrested. I don't even live in the state. He was isolated until the day he died. He was given a court-appointed guardian through APS (adult protective services. He visited one time for about 15 minutes on December 26, 2026. They did nothing in his best interest. the judge did not allow anyone to visit him. My uncle died alone. they removed everyone who loved him due to a vindictive, distant, greedy family member, who was allowed to further isolate him from his loved ones. To top it off, after we were told that the state would take over, we were never notified of my uncle's death. I just happened to call the guardian of the court on February 23, 2026 to find out why the distant relative was going in and out of my uncles house. I was informed that my uncle had been dead for almost 2 weeks. He died at Palos hospital. No one even had the heart to notify his family. There is a different kind of evil in this world. I don't know who's worse Palos Hospital or this place. It should be a special kind of hell for people who treat the elderly like this.

Individual employees are, for the most part, committed to their jobs. For that, as the family of a resident, we are grateful. However, SV seriously lacks cohesion as clients are promised and redirected frequently. There's no single chain of command you can follow when meds aren't dispersed (on time or at all), the doctor doesn't show, appointments gets rescheduled, allergies are ignored, etc. It's understandably difficult to run a facility of this size but when you commit, you need to commit wholeheartedly to the needs of a clientele who will not or cannot speak for themselves. It's been our experience that SV is a place where employees redirect, rather than act, when an issue occurs. Often, because they don't know how to make things right. Remedy: Hire a client care coordinator or team who is available, responsible, and has the authority to act when something goes wrong or doesn't happen. Document it and work to make sure it doesn't reoccur. This way, you truly partner with those of us who don't know who to contact, or constantly have to address the same issue. If this person or team exists, let's hear about who he/she/they are. SV has wonderful employees in place to serve the clientel. Management now needs to make themselves known and take further steps to lead the masses. Thanks.
