This all happened on 1st shift. My brother waited over an hour with his call light on to go the bathroom. When they finally got to him, they forgot about him in the bathroom for over 30 minutes. I have worked in the medical environment for over 20 years, I felt this was very inappropriate and unprofessional. His call light would be ignored, while the nurses were sitting at the nurse's station. I hate to wonder what they would do if their family was treated this way. 2nd shift is wonderful and caring and seemed like they were there for the patient's best care. read more

This place is like a puppy mill of rehab centers. My expectations of a rehab facility start low and it only got worse in this facility. Mom contracted norovirus and ended up at this place because they would take her. The smells are putrid so make sure you mask up because this is a petri dish of a place. If I could body wash myself in sanitizer I would before and after. The administrators should be forced to eat at least one meal a week of what the kitchen is serving. I have never seen uncooked, gray food in my life. Everybody who works there says that the food is disgusting and trust me I would not eat it. I had to bring in food for Mom that was palatable so she could try to regain her strength. She waits over an hour to get just a bed pan and many times they just don't show up for calls. It is disgusting that a place treats the elderly worse than our animals. read more

The most outdated, filthy care center I've EVER SEEN!! Cabinets in the room broken, don't open & are 30yrs old! Quarter size holes chipped off on walls! Looks like it hasn't been painted EVER!! Cobwebs, dead bugs & filth in window sills! CNA RUDE AS HELL! Ripped call button out of pop's hand & threw it at the wall when he called for help & no one answered several times!! The commode wasn't set up properly & it caused him to be soiled!! Shower area looked dirty!! Big stains on carpeting in day room!! Smells like urine walking in the hallway!! Upon arrival there is a very strong order of urine. I understand this is a home with many elderly patients with incontinence issues, but the smell was overwhelming and as if adult depends have been collecting someplace at the entrance, or people have been left soiled. There is no one greeting you when you walk in, which means there is no one monitoring who is coming in or going out which makes me worry about the safety of my loved one here, and the safety of others loved ones. The workers are sitting around having personal conversations or on their phones, I even witness a worker commenting to another worker after leaving s patients room with the hand gesture of that patient talking too much. I felt this was very inappropriate and unprofessional. During my first visit I needed to find someone to help my grandmother to the restroom, I walked to the nurses station and asked if there was anyone who could assist and I was told "her care working is around here somewhere" and had to wait for that person to show up in order for someone to help my grandmother needing to relieve herself. I walked with her back to her room and was told "I was waiting for her to wake up because she just kept saying help me help me, so I wanted her to sleep" this was alarming to me and something that should never been overlooked, or joked about. During my 2 hour visits with my grandmother who is here after a stint in the ER, not one single person checked in with her or introduced themselves as their caretaker, and as I left I noted many workers sitting around, on their cell phones or scrolling on the internet. Not one single person acknowledged me.... Furthermore, upon my very first visit, there was a pack of open adult depends diapers sitting next to a chair in a common area, which I thought very odd and unprofessional to have them sitting out in the open. The cleanliness of this place is lacking, greatly. In my grandmothers room there was a dirty latex glove sitting in the corner of her room, I have no clue how long it was there, or why it was there as there isn't a garbage in that spot. The garbage pail itself isn't lined with a bag, instead the bag is just sitting on the bottom of the pail. I often help my grandmother when I'm there with her feet and legs and she often complains of being uncomfortable, which I can understand as the beds are less than desirable. Upon lifting up the blanket to the sheet, there was a very large harden stain by her feet. My grandmother does not have any issues that would cause this, so this makes sick to my stomach thinking who was using this prior and why it wasn't properly cleaned. Thankfully she is only in her for 20 days, but in those 20 days she was to be rehabilitated for when she goes home so she is more independent. There has been zero progress and I fear for her return home as I do not think she has been worked with enough or at all, to be able to return home. I wouldn't recommend this home to anyone for their loved ones. If you truly want the best care for the people you love... Look elsewhere. read more

My mother calls me every day crying about how much she hates this place. She sends me pictures of her disgusting meals. Save your money, please send your loved ones elsewhere. I live out of state and thought this would be nice, it's not. I just want someone to treat my mother with the dignity we all except. read more

Hard pass. Some of the nursing staff are very good- however- my mother did not get her meds on time and when she asked she was told that she was crabby. She was not changed in a timely manner and had to sit in this for over four hours despite notifying the staff multiple times. We selected this facility because they had wound care-but her bandage was not changed daily on the doctors orders- in fact she went 5 days with out it being changed. Her hand was swollen and painful for unknown reasons and the doctor was not available to look at it. They dropped her as she was positioned wrong from the hoyer lift on to her wheelchair and she slid down to the ground and then asked her to stand up! Obviously she could not stand as she was lifted to her chair. How could they not know that after she was there for two weeks! Then when she was discharged unepectedly they gave us less than 48 hours to figure out a plan to get her home with out any equiptment ordered. The only thing she came home with besides a bad experience was a bed sore. Finally I requested records and when they did not come, I called the corporate office and was told that they did not recieve it, because they do not check their faxes.
