We were new to memory care because my dad was mom's caretaker until he passed away suddenly, so we had no idea was was good or bad or right or wrong with memory care. Let me just say...facility on Precinct Line Road in Colleyville, Bad! Silverado Southlake, Good! We moved mom to Southlake in January 2018. As with most Alzheimer's patients, mom has declined in the last year. I've had at least quarterly care plan meetings with the staff and they are always tweaking her care. The staff always calls me to remind me its time and schedule the meeting. I usually drop in unannounced and always find the staff engaged. The residents seem to have full days and the staff tries to engage the residents with activities. Mom doesn't know me any more, but just last week, on a Saturday, they had a fellow with a guitar singing old songs. Those residents, including my mom, where sitting in a circle singing along to those tunes. I sat down with her and watched the response. She knew those songs but not me :-) I have found the staff to be very good, when compared with my previous experience. Is it perfect? No. It wouldn't be perfect if you had your loved one in your home. Is the care exceptional? Yes! There has been some turnover of staff, but that seems to be the norm in the industry. It is a tough business. In my view, the residents receive excellent care. The communications are excellent. If something happens, like a fall, which will happen, they are on the phone as soon as it is discovered, even if its in the middle of the night. Frankly, after getting involved in a family council with the other facility, I have come to learn that some people complain no matter what. Most of the complainers aren't interested in problem solving. Most of the folks that complained the most were on their fifth or sixth facility. At some point, you gotta ask yourself...what is the constant denominator? In summary, I love Silverado Southlake and the staff. I could not bring my mother to my home and care for her better. She is always clean, and mostly happy, considering she has a 60 second memory loop. The staff seem to really care about taking care of these vulnerable people and in my view, provide the best care available.

There were some wonderful staff at Sage Oak (Amanda, Mitchell, Diedre, Tay, Ashley, Dorcus, Felecia, Soozie and more) but the constant turnover was a real problem when my brother was a resident there. Some of the attitudes of some the caregivers were truly despicable and that is what made the experience at Sage Oak a difficult one. Staff not properly trained resulted in patient accidents. Staff laziness, gossip circles, complete insubordination, and cell phone obsession took place while residents' needs were often times ignored. Many in the management branch of this company changed positions frequently. During my brother's 19 month residency, there were 5 different directors of nursing. There were executive director changes. (at least 4 that I can remember.) The one person in upper management who always addressed family concerns was Skylar, the last executive director we experienced. He truly walks a dedicated path in the field of elder care and is a huge credit to this company. The owner Loe is rarely visible and manages to live in a different reality from the difficult issues that Sage Oak staff, residents and their families often endure.

In just one week, it is clear that their success stems from kindness, compassion, specialized dementia training, and genuine accountability. My dad has frontal lobe dementia, which makes communication a significant challenge. While any move has a learning curve, the staff has come alongside us every step of the way. My dad is smiling, happy, and honestly seems more content here than he was at home. Communication is phenomenal. We receive regular updates, and the care is incredibly personalized. A perfect example: my brother visited unannounced and found staff sitting with my dad helping him do 'work.' When he needed to use the restroom, the staff immediately jumped in to assist so my brother could focus on just being a son. This allows us to enjoy our visits rather than acting as caregivers. Contrast this with our previous facility, where my dad had two ER visits for falls and a police call for aggression within his first week. At Sage Oaks, he has had zero falls and zero aggressive episodes that couldn't be redirected by their skilled team. When he had a 'bad' day, they didn't call us to come 'fix it'; they called to ask questions about him as a person so they could tailor their approach. If you are looking for a true home away from home where you can entrust your loved one's care. We were worried about the distance from our home to Sage Oaks and with the care he is receiving the distance has been no problem. The entire team has been a godsend during this challenging time, and we cannot recommend them highly enough. We want to specifically recognize two exceptional individuals: Dora and Sunny. Their dedication to my 88-year-old mother's recovery has been remarkable. They approach her care with a perfect blend of professional expertise and genuine compassion, consistently going above and beyond to help her regain her strength and independence. Dora and Sunny's consistent encouragement and specialized support have been crucial in getting her back on her feet and well on her way to normal. The rehabilitation process can be daunting, but thanks to the tireless efforts of the care/rehab staff, my mother feels motivated and cared for every step of the way. If you can visit, you will find it to be beautifully appointed without being fussy. My mother went from a townhouse to a one bedroom apartment and it was perfect for her. The weak link to the whole operation is the dining room, and my experience is that most assisted living properties struggle in this area. The dining room is not carpeted and so quite noisy for the residents. Food service can be uneven and the food was often not very hot! But I found that if my mother asked to have something fixed, the servers were always kind and accommodating! My mother passed away one year ago. I was with her along with hospice, which they allowed to take care of her in her apartment. I found the staff of The Oaks to be so accommodating and concerned. I wish my mother could have enjoyed living there for many more years!

My dealings with Sandy, Nita, Rachel and Nico were outstanding! They always made me feel like they'd known me forever! Nico is the hardest working maintenance man I've ever seen and while it was my Mom's decision not to go there it had nothing to do with the facility or staff. This decision had to be made by her and at the end of the day she chose a location with a larger footprint for the room. I will say however that if things don't work out I won't hesitate to move her to FM. I appreciate all the time they spent with me and highly recommend them.

His room and bathroom were rarely cleaned, and laundry was frequently mishandled with belongings often going missing. There were also multiple instances where he was not checked on for extended periods, including after falls and in the mornings after not being checked on a single time during the night. Staff turnover was constant, and caregivers were often disengaged, distracted, and rude. Despite repeatedly raising urgent concerns, nothing improved. Hospice involvement was necessary to ensure he received basic care and supervision. I had a 24/7 camera in his room so you would think they'd have been more diligent knowing I could see and record all that was going on. I can only imagine the neglect for those who have no camera and no advocate. This facility did not provide the safe, consistent care that memory care residents require.
