There were some wonderful staff at Sage Oak (Amanda, Mitchell, Diedre, Tay, Ashley, Dorcus, Felecia, Soozie and more) but the constant turnover was a real problem when my brother was a resident there. Some of the attitudes of some the caregivers were truly despicable and that is what made the experience at Sage Oak a difficult one. Staff not properly trained resulted in patient accidents. Staff laziness, gossip circles, complete insubordination, and cell phone obsession took place while residents' needs were often times ignored. Many in the management branch of this company changed positions frequently. During my brother's 19 month residency, there were 5 different directors of nursing. There were executive director changes. (at least 4 that I can remember.) The one person in upper management who always addressed family concerns was Skylar, the last executive director we experienced. He truly walks a dedicated path in the field of elder care and is a huge credit to this company. The owner Loe is rarely visible and manages to live in a different reality from the difficult issues that Sage Oak staff, residents and their families often endure.

My dad has frontal lobe dementia, which makes communication a significant challenge. While any move has a learning curve, the staff has come alongside us every step of the way. My dad is smiling, happy, and honestly seems more content here than he was at home. Communication is phenomenal. We receive regular updates, and the care is incredibly personalized. A perfect example: my brother visited unannounced and found staff sitting with my dad helping him do 'work.' When he needed to use the restroom, the staff immediately jumped in to assist so my brother could focus on just being a son. This allows us to enjoy our visits rather than acting as caregivers. Contrast this with our previous facility, where my dad had two ER visits for falls and a police call for aggression within his first week. At Sage Oaks, he has had zero falls and zero aggressive episodes that couldn't be redirected by their skilled team. When he had a 'bad' day, they didn't call us to come 'fix it'; they called to ask questions about him as a person so they could tailor their approach. If you are looking for a true home away from home where you can entrust your loved one's care. We were worried about the distance from our home to Sage Oaks and with the care he is receiving the distance has been no problem. This lack of consideration for residents on set incomes feels insensitive and places unnecessary stress on families trying to plan and budget. I hope Discovery Village will reconsider how amenity-based charges are applied and work more compassionately with long-term residents and their families to find fair solutions.

The entire team has been a godsend during this challenging time, and we cannot recommend them highly enough. We want to specifically recognize two exceptional individuals: Dora and Sunny. Their dedication to my 88-year-old mother's recovery has been remarkable. They approach her care with a perfect blend of professional expertise and genuine compassion, consistently going above and beyond to help her regain her strength and independence. Dora and Sunny's consistent encouragement and specialized support have been crucial in getting her back on her feet and well on her way to normal. The rehabilitation process can be daunting, but thanks to the tireless efforts of the care/rehab staff, my mother feels motivated and cared for every step of the way. The entire facility provides a warm, supportive, and safe environment for its residents. We are incredibly grateful for the peace of mind knowing she is in such capable hands. The weak link to the whole operation is the dining room, and my experience is that most assisted living properties struggle in this area. The dining room is not carpeted and so quite noisy for the residents. Food service can be uneven and the food was often not very hot! But I found that if my mother asked to have something fixed, the servers were always kind and accommodating! My mother passed away one year ago. I was with her along with hospice, which they allowed to take care of her in her apartment. I found the staff of The Oaks to be so accommodating and concerned. I wish my mother could have enjoyed living there for many more years!

We were able to visit the day we called. Ask for Carrie! She spent over 2 hours with us. She's not about a hard sell, absolutely no pressure -- she really wants your family to be happy there. She truly understands what you are going through. This location was built in the late 1990s and I believe they are in the final stages of remodeling it. The drive up still feels outdated, BUT don't let that fool you. The apartments we saw were newly painted, updated bathroom, updated kitchenette and new hard wood floors. The main building is peaceful, spacious, and full of energy. A grand piano sits in the entry. There are all types of seating areas, for example there was a breakfast area, outside patio, small cozy chairs nestled around a tv (the news was on) and a full service dining room. Many residents stopped to say hello and expressed how much they enjoy living there (that was a huge plus for us!). TIP: They are pet friendly! In fact, they just built a beautiful new enclosed court yard. Bright green grass. Sitting areas too! So what does it cost? That's what you really want to know. When we visited they were running a special: 1 bedroom/ 1 bath, 3 meals a day, all utilities plus internet is included, all activities (no hidden fees), access to a washer and dryer (free), once a week light housekeeping/ trash pick up, 24/7 nurse on hand for evaluations, transportation services within a reasonable radius, concierge desk for miscellaneous requests and 24 hour security. We were quoted 1k. They also had larger apartments 2 bedroom/ 2 bath that were $1,700 a month. This community is called "Independent Living Plus" meaning you come and go independently but it includes amenities you get from an assisted living facilities. It's a pretty darn good deal!

The first day he was transported to this facility someone from the staff stole his fanny pack with his wallet, cell phone, etc. When I get there the first day after work the night shift people blamed it on the day shift people. I go back to the day shift people and they play dumb that nobody knows anything. I filed a complaint with them but the fanny pack still doesn't turn up so I had to file a police report for theft. Once I filed the police report we had to get him out of there. Miraculously when my father was moved to another facility I get a call that they found the fanny pack. At that point I'm thinking everything is fine now. Well, the next day my Dad's bank account gets suspended for fraudulent charges. Someone literally took a joyride on the town. They had a fancy meal and an Uber ride before the card got shut down. I'm still looking for the perpetrator. Oh, and conveniently they don't have cameras there. Do your loved ones a favor and just STAY AWAY!!!!!

As of late 2025, my rating of this nursing home bottomed out. I'm sorry to say the place is unacceptable. My brother WAS well and comfortable, but he has deteriorated after being abused and physically assaulted by a mentally ill roommate. The Facility lied to us about it. At 6 ft 2 in, my brother now weighs less than 130 pounds, and looks like he was just released from a Nazi concentration camp. The Director of Nursing looks me in the eye and declares he's fine.

The facility is clean, the food is satisfactory and adequate. Our family is grateful that our brother has a place there. He nearly lost his life due to careless medication management in a private assisted living facility in Hunt County where he stayed for 17 bleak months. That ended with an ambulance ride to the ER and 2 weeks in ICU due to bad medication management. In that process, our brother's cognitive faculties were so damaged that he qualified for Medicaid long term care. I'm sad about his medical status but the Meadows takes good care of him. I regret that Medicaid reimbursement rates are so stingy that life is not very diverting and interesting for Medicaid-dependent nursing home residents. Of course I have fears about the general trend in Medicaid reimbursement. But the Meadows does very well with the resources it has to work with. I'll pay special tribute to Mr.Bill, the loving heart and soul of the place! He's not in management nor is he a licensed professional therapist or anything, but he is all over that facility doing his best to ensure that spirits are high. He has been doing this for decades and I believe he truly is the house Angel.

Medication, durable medical equipment, clothing, jewelry from my deceased mother, my computer, food, furniture, everything, in the trash and all because Maddie, the housekeeping manager, acted on her own and against company policy after being informed that we still had time and we were not finished moving. I cannot believe someone could be so cruel working at a place with such a vulnerable population. I am hurt beyond words. Neeli Smith, the director, is very sweet and she apologized for Maddie's actions and couldn't understand why she would do such a thing when the policy is clear and it was definitely violated. The company offered $1000 to replace my computer but nothing else and I refused their offer. When I refused their next offer of $3000, the asset manager asked how I was going to pay past due rent. I thought that was incredibly unprofessional and shows me that the safety of residents isn't their concern. I have tried to work this out but have been left with no other choice but contact an attorney. It's a sad situation. I am left to suffer over one employee's very bad decision and she goes on to work at the Emerson.
