This property was sold in early 2025, and the name changed to Cascades. The previous ownership (affiliated with Abbey Delray South) provided a stimulating calendar of events, but most importantly they had a trained chef in the kitchen who prepared well-cooked meals using mostly fresh ingredients. Menu options were varied and nutritious. Since becoming Cascades, the quality of the meals has taken a nose dive. Fewer choices, smaller portions, vegetables often bathed in strange yellow oil, and regular use of processed meat products masquerading as real food. Management says they're taking resident's concerns into consideration and that things will improve, but it's only gotten worse month by month. It's been almost 3 weeks since the kitchen was shut down due to a cockroach infestation. There's no excuse as to why this problem has not been resolved at this point. In the meantime, residents have to deal with extremely limited menu choices and small protein servings. A recent brunch offered nothing but hard-boiled eggs, orange juice, and unthawed frozen waffles. A couple of weeks ago residents were served takeout pizza. For the New Year's Eve dinner main dish, residents were given a choice between a pressed turkey sandwich or a pressed ham sandwich. Each building has only one elevator, and in recent months it's sometimes taken up to a week to repair a broken elevator. This has left some residents on upper floors with no options to leave their apartments for days at a time. Long-term residents have been moving as a result of all this. Many residents don't have the means or the support to move to a different residence. Think twice before moving your loved one into this facility. read more

Horrible!!! BED SORES and lied about it!!! They said the sores were due to my Father was not getting enough protien, but it was from lying in his own filth. They refused antibiotics because they said it might cause diarrihea. Met with the Admin twice, still nothing was done. Asked to speak to a Dr and was told I would be contacted, but was never contacted. Dr would sneak in and out. I had to move him to Betesda Hospital to get treatment for the two terrible bed sores. They are really bad down to the bone, but the lieing about them prevented me from getting competent care. After a week in the hospital, he was moved to Isles of Boynton where they are healing. If I was litigous I'd call a lawyer. read more

My grandpa passed away this past week. He was 99 and as he told me often "getting old is not for the sissies," I do think his last 12 years were as good as possible thanks to Abbey Delray. My grandmother passed 13 years ago and spent the last of her life here at Abbey Delray. She had been in another facility before this one and Abbey Delray seemed like the Ritz in comparison to the Holiday Inn conditions of the first one. So when my grandpa decided he was ready to move into a facility he chose this. He spent the first 10 years in independent living and kept quite busy with bridge, poker, daily swims (even at the age of 96!), socializing and more. It seemed like summer camp for the elderly! He made some great friends thru his years here, although it was sad to see many of them pass away thru the years. During the last 2 years he needed more assistance and so we moved him into the assisted living building. While he had a lot of complaints about it, I think he was mostly just complaining about the aches and pains of growing older. They took great care of him and in the last few weeks it was amazing to see how much the nurses had come to care for him as a person (even coming in to check on him on their days off). A special shoutout to Zanelle and Rachelle who went above and beyond to make him comfortable. Yesterday was the funeral and then we rented the art room in the assisted living building so that his friends could stop by to celebrate his life with us. It was amazing hearing about all the activities and friendships they too had made at Abbey Delray. Beverly, the activities director, helped us set everything up and she too was another impressive member of the staff here. After watching my grandpa struggle with the challenges of being 99 I 100% agree that "getting old is not for the sissies," but nice to know that there are places like Abbey Delray that make it more manageable and fun. read more

Lack of proper staff - patients have to sit in wet diapers for extended periods of time and wind up with bed sores and UTI's. Read all the other reviews and it seems to be a bigger problem . I don't believe that patients get a weekly shower - more staff is needed to take care of the sick and/or elderly. read more

As the Abbey, the staffing, food, and services were bad. As the Cascades, they are awful. Food is horrible, entertainment down to almost nothing-they don't have trips anywhere. Kitchen has been closed for 2 weeks. Staff always quitting. Renters leaving. People who paid for their units (it used to be all residents had to buy in) feel stuck. Assisted living is a disaster. I would avoid at all costs. read more

DO NOT LEAVE YOUR LOVED ONE HERE! My father had to stay here on two separate occasions. They were horrible - - actually the PT was excellent but it was only 1 hour/day and the rest of the day was spent in bed. Nobody helps the patient - - nobody brought him fresh water - - when he called to go to the bathroom, one of the staff told him to go in the bed and they'd clean it later - - - UMMMMMMMM - - I called the manager - had a nice conversation - then he was let go the next day. I called the front desk - nobody answers - - they actually had my dad in a room KNOWING that his call button didn't work! DO NOT LEAVE YOUR LOVED ONES HERE. read more

we didn't have hot water for 6 days. the doors to our building are not functioning. as a result my neighbor fell while trying to negotiate her walker and the heavy door. no sign these will get fixed. our third floor ceiling is falling down and i suspect mold. another building has not had elevator working this whole week and some people are shut in. the maintenance staff is unable to complete or start many projects because they only order from one place, who doesn't have the parts. things are deteriorating daily. read more

I wish there were minus reviews. This facility now called the Cascades needs to be shut down- I come from a medical world so know what to watch for. We moved our mother into Abbey Delray North in late Aug.2024 until the day she died. The place is pretty both inside and out and thats where it ends. The food was inedible- my 90 yr old mother was receiving food with spicy seasoning or BBQ type so she just did not eat. most of the time the trays came cold. This was addressed multi times with the nutritionist(who was very nice) Trying to reach any nurses - impossible, phones just ring and ring even in the wee hours of the morning. PT was wonderful, social worker at our start was wonderful then she quit. The new social worker did nothing. Her phone was constantly on full so you could not leave a message. The director of nursing was the same until we sent the state in and then she picked up or returned calls We informed everyone at her admission that she had chronic bladder issues and it fell on deaf ears. Lying in wet diapers waiting over an hour or two for help changing caused her to have repeated UTI that went untreated and 3X it pushed her mental status to dementia like symptoms. She was calling us all hours of the night hullucinating yet our calls to the late night staff to check on her were unanswered. After multiple times and us documenting everything, we needed to put a complaint in with the state. They put on a good show when they arrived, even to how they claimed they changed patient diapers. I settled that issue by sending her cases of pull up diapers/pull ups so she was able to clean herself. If they were short staffed then what needed to happen was them hiring from outside agencies, but i assume its just easier to leave the elderly as they did. My mother was not an easy patient by any means - my sister lives in CT and I am almost 4 hours away. I was traveling 1 to 2X every month thru Sept 2024 when my own health issues precluded my traveling. In the late winter of 2024 a Dermatologist- Dr Friedman made rounds and did a skin check on mom. She took off several areas in question. I was never contacted once with either this visit nor what those results were. Mom started to complain in Feb 2025 that her leg was bothering her. The nurse said it was nothing until some sort of mass started to protrude thru her leg where that doc took off what i was then told showed squamous cell carcinoma. Once again I was told from nursing that the provider gave them an ointment to apply, wound care nurse Anna complained to me that it was to be applied twice a day, but then it was ?once daily. I was told at that time it was the size of a dime. I was then sent a picture of the actual wound, it was 3X the size of a quarter and weepy looking indicating infection. Their dermatologist never came. This was now very painful. I made arrangements to get my mother to her private Dermatatologist because I sent them the picture - he biopsied it and got her in immediately to the surgeon the following week. The ointment the NH used per her doctor was nt the right medication. This Dr. Friedman finally showed up and asked my mother what was going on weeks later- Mom told her she could leave her own Dermatologist was treating her. The PCP saw her for pre-op clearance after 3 calls to his office as this was now urgent. On Thurs she went to her cardiologist for same. Her BP dropped drastically and he sent her to the ER...Thank God.... The surgeon in the ER took one look at this mass, as well as a 2nd one now on the other leg and did the surgery immediately the following morning. I was now headed to her. The biopsy came in with sepsis and mercer. I had to be gowned and gloved and after 3 days she was seriously declining and hospice was called in. She passed away on Easter. All the providers that touched her had the same comments about Abbey Delray and their care and each stated due to her decline she would not be going back there You can look at this beautiful facility where you are WARMLY welcomed - and I understand that most nursing homes are the same or worse but my 90 year old mother had sepsis and mercer running thru her.. This was my first interaction with a nursing home for a family member. While I tried to move her closer to me I saw horrendous places and two nice ones and the comments were all the same. Unless you the family are living close by and constantly checking on your loved one and show your presences it gets bad quickly. Its a shame that due to staffing issues the patients suffer and these are all vulnerable patients. I was so happy with the hospice staff and Delray Medical Center. The care, shared feelings, and gentleness they showed my mother made her transition a quiet one. read more

I want to share my experience with Abbey Delray North's skilled nursing facility, where my father has been staying for five weeks. From the moment he arrived, it was clear that the facility struggles with serious staffing, communication, and equipment issues. When my father was admitted, he was simply placed in a room by a transporter with no information, no orientation. I had to track down the administrator myself just to get a welcome booklet. We quickly realized that the facility is severely understaffed and poorly equipped. The furniture in the patient rooms is old and worn out. Some rooms have donated smart TVs, while others have older models, creating a hodgepodge of mismatched furnishings. Each room has two beds and two TVs, but the beds lack speakers like those in hospitals, so every room becomes a battle of dueling, blaring televisions. The entire facility is loud: doors are left open, people are talking in the hallways, TVs blare from every room, and demented or delirious patients scream from their beds. There is simply no way to get recuperative sleep in that environment. Another major issue was medication timing. My father's morning pills were usually given to him around noon, and his evening pills were administered around 6 p.m. Twice-a-day medication should be spaced 12 hours apart, not just six. This kind of scheduling error is concerning and reflects poor coordination in patient care. The most helpful staff members were the front desk employees, Veronica and Esther. They were the only people I could reach by phone, sometimes, once I flew home. My father did receive physical and occupational therapy 30 minutes each, five times a week, but the lack of communication meant I never knew the schedule. The most troubling issue was the severe shortage of aides. After my father's first week, I had to return home to another state. I mentioned this to his wonderful nurse, Isabelle, and her response was: "Oh no. Who will take care of your father?". I told her we were considering hiring a private aide, and she said, "That is a very good idea." Even the staff knows there aren't enough aides. Because of this shortage, my father, who is not incontinent, was put in diapers not for medical reasons, but because there simply weren't enough aides to help him to the bathroom. When my dad got fed up waiting for help, he would get up on his own, climb into his wheelchair, and walk himself to the toilet. He fell twice trying to do this. No patient should be forced to put themselves at risk just to maintain their dignity. On one occasion, my father had an accident, and I was desperately trying to find an aide to help me. After searching with no success, I had to clean his poop off the floor myself. When an aide finally arrived to assist him, she helped my father but didn't have time to clean the rest of the mess. Since the bathroom was shared with his roommate, I ended up cleaning the toilet seat and surrounding area myself. No family member should ever be put in that position in a skilled nursing facility. To make matters worse, in the five weeks my father was there, he never had a proper shower. I noticed a binder at the nurses station labeled "Shower" and asked about it. I was told he could sign up for one. However, no one ever informed us of this process, and he was only sponge-bathed the entire time. Basic hygiene should not require a sign-up system that families have to figure out on their own. Food is another issue. If you don't want to drink water, they have small cans of soda in the kitchen area but if you aren't mobile, you can't get a drink. The food is delivered cold if you aren't able to make it to the dinning area. Laundry service was another disappointment. I was told his clothes would be cleaned twice a week, but in the first week, no one even picked them up. Instead, he received a bag of freshly laundered clothes belonging to another patient. I brought it to Veronica, who took care of returning it to the rightful owner. On more than one occasion, as I walked through the halls, patients sitting in wheelchairs stopped me to ask for help just to get back to their rooms. This was heartbreaking and further proof of the extreme understaffing. No patient should be left stranded in a hallway, waiting on a staff member who may never come. Overall, Abbey Delray North's skilled nursing facility is plagued by staffing shortages, poor communication, outdated furnishings, and inadequate patient care. While a few individuals like Veronica, Esther, and Isabelle were kind and helpful, the overall experience was frustrating and disappointing. read more

This community was sold at the beginning of the year to a new owner, Citadel Care Centers out of New York who brought in a new management company, Onyx based in Miami. Unfortunately, to date, the transition has been a disaster. Many of the services have been reduced or eliminated. In addition, the restaurant has reduced portion sizes and the higher quality meal choices have been taken off of the menu. MANY of the residents are upset and frustrated with the lack of communication and information about the changes. The quality of life for the elderly residents is diminished and due to their age, moving is not an option as it would be too disruptive and unsettling. They are stuck in a bad situation and are being taken advantage of due to their age. I would strongly urge anyone that is considering Cascades as their next move to wait to see if the new ownership improves the quality of life for these seniors or if they continue to abuse the situation. read more

This community is now Cascades. There is a brand new management company that just acquired the community. So far it seems the staff are more attentive, and the leadership team is making changes to make significant improvements both to the rehab and to the independent living and assisted living. This place will be back to matching the beauty of the community with these folks onboard. read more

I have lived at Abbey Del Rey North for 2 and a 1/2 years. I recently had a knee in shoulder replacement. I have my life back!

My 86 year old mother has been here for 7 days . It is the worst rehab facility I've ever had the unpleasure of dealing with . The staff is nice enough but the place is in total disarray . The air conditioner is broken in the room and the thermostat fell off the wall when I try to adjust it . She's left hours on end without someone checking in on her and I am moving her to Assisted Living where she can get the care that anyone deserves . The reviews of this place are Justified because I'm not the only one who thinks it should be shut down completely and renovated . The social worker is a total waste of time and does not know how to return calls . The entire experience was subpar and I highly recommend you look elsewhere if your Elder parent needs rehab . I wish I can rate it a zero but it won't let me . Heed my words this place is a sub par and be shut until overhauled. read more

My father was in the Rehab Center for two weeks. I thought that the PTs and the OTs were terrific. The nurses were caring but overworked and burned out. The CNAs were kind but also overworked (and undertrained.) If you are there for physical therapy and you are mobile and can self-advocate, you should be fine there. If you are confined to a bed, expect to beg for every single thing you need, like having your diaper changed. Expect that you will never, ever, ever, be turned in your bed and will run the risk of developing bedsores. Expect to see a doctor once a week, on a schedule that is convenient for her. And as a prior reviewer said, expect to be completely neglected on the weekends. I tremendously regret having chosen this place for my father, and I believe in my heart that their lack of urgency contributed to his death there. read more

My mother-in-law was in the health center. The nurses try very hard to help the patients. However, they cannot get to all the patients. If you are patient at Abbey Delray on the weekend, you are left alone. There is no help. You will receive no PT or OT. If you are lucky you might get your meds by 9:00 p.m. Patients are yelling to the aids, "Help me," "Help" I walked the halls, and it looked like the shining. The only positive are the aides. They are kind, and try very hard to help. read more

My dad was at their rehabilitation center and the help is really bad. While I was there someone from another room rang for help and the busier rang in the halls for 45 mins while I was there. In addition the food was horrible. read more
