First of all, while the nurses may be medical professionals to some degree, in no way should their opinions be utilized for life changing decisions. We had some incompetent named Paul going to see our aunt in her home, where she wanted to die. While we understand that her home was not an ideal place for anyone to live, it was her home for nearly 60 years and where she wished to be. Paul informed us that she had likely less than four months to live and would need to come live with us. This was 1.5 years ago, and his diagnosis couldn't have been more incorrect or detrimental to the lives of our entire household. For nearly two years we've had to alter everything to accommodate her. I would strongly suggest Seasons advise their staff to not influence important family decisions. They don't care, as they are in the "ease the pain and move on" business. It's like a conveyor belt. Henry Ford would be proud. I know this review won't cause Seasons to make any changes, but I'm hoping someone will read this and understand that the nurses' guidance could potentially be detrimental to a family.

My mom past almost 6 years to the day and I still think about all of them with warmth in my heart. The attention my mom got was top notch. They titrated meds so mom didn't have pain but still was able to function almost to the end. When one medication stop working tegu switched it to another by admitting her into hospice facility. I will never forget nurses there and how compassionate and understanding they were. The violinist came and played music in my moms room and that was just so wonderful. After switching meds she was able to function and enjoy pain free days and our company longer. The person who was coming to bathe her hugged and kissed her as if it was her own mother,per my mom's admission. Humanity of staff and compassionate care she had was unmatched . I heard this company was bought by another company, I surely hope that they would continue their wonderful care of terminally I'll patients as they did when my mom was transitioning. P.S. if you think that all hospices like this don't be mistaken: first hospice we got into, I would not name it, almost killed my mom by overdosing her on morphine in the first week , while I was at work. Thank God I came on time to find her almost unconscious. The nurse gave her huge dose of it and quickly left, leaving her to deal with it. That what prompted me to switched to Seasons and it was the best decision I ever made!

My mom fought pancreatic cancer for just days short of a year. She left the hospital to come home to hospice. We didn't know what to expect but they far exceeded any expectations we had. Every single person we interacted with was knowledgeable, professional and above all compassionate. I don't know how they do what they do. They became a part of our family. This all happened in the midst of the corona virus quarantine and they balanced protecting themselves and us yet still giving all the care needed and never making us feel like we were a burden. Our main nurse was Tami and the CNA was Iris. They both took amazing care of her and trained us to do what was needed when they were not with us. My mom instantly bonded with both. They were gentle and caring. I don't know what else I can say but we made the right choice with Seasons Hospice. I didn't see any reviews at the time I was looking and I hope this helps someone who might be on the fence.

    My wife has Inflammatory Breast Cancer (IBC) - Stage IV, Metastatic, Aggressive. Like 90% of IBC…read morepatients, the IBC was already Stage IV when it was diagnosed. We opted for the hospice option because it was already spreading around her body. Life Choice Hospice has been there for us with a caring and professional intake session the morning after we contacted them through their website. That evening, Life Choice had delivered the equipment that my wife would need and started transitioning her to her new medications. We have a core care team of three. The names I have been using for them on our CaringBridge page are: The Amazing Nurse Julie (the primary caregiver who expertly changes the tumor's dressing and provides on-point answers, advice and support for both of us three times a week), The Fabulous Jessica (social worker whose visits, calm assurances, and insightful suggestions are truly priceless), and Stephanie the Magnificent (our Certified Nursing Assistant - because like a magician, she can turn my wife's soiled bed linens into clean ones really fast without my wife ever having to leave the comfort of the bed). When my wife woke up in pain at 2 am, we called the Life Choice Hospice 24-hour line, expecting to get an AI or an answering service. A Nurse Practitioner (NP) answered, (yes, a real person), and had the on-call person here in less time than we usually had to wait in an Emergency Room. The on-call (I forget if they were LPN or NP) was professional, caring, understanding, competent, and thorough who took care of her and did not leave until she and I both understood what was going on, why, what they were going to do about it, and what I had to do and watch for as they change her medication dosage and frequency to adapt to her increasing pain levels. We have needed the Life Choice 24-hour services at other odd hours between 11 pm and 4 am, and, while the NP on the phone and the LPN or NP that showed up were different, the pattern of professionalism, care, concern, and thoroughness was the same. When she noticed that I was getting overwhelmed with caring for my wife, The Fabulous Jessica suggested a "respite" meaning that my wife could visit a 24/7 care facility for 5 days to give us both a break. I did not know such a thing existed or could be so beneficial and timely. All three of my wife's core care team continued to care for her while she was in the 24/7 care facility. Life Choice had already contracted with a service award-winning facility, arranged transportation, got my wife settled in there, provided continuity of care by keeping their previous schedule, and coordinated the care my wife received from the 24/7 facility when Life Choice was not there. They accepted both my insurance from work and Jean's Medicare Part A.

We were assigned Emily as our case manager and she was great to work with. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. A nurse was then scheduled to conduct an assessment on Monday afternoon to determine whether independent or assisted living would be most appropriate. While I could not be present in person due to work obligations, I called in during the assessment to ensure I could provide support. My father answered the nurse's questions to the best of his ability, and there were no indications of issues at that time. Following the assessment, Lisa informed me that an internal review would be conducted. Today, after sending a brief follow-up text to Lisa, I received a call from your Assistant Director. During this call, I was shocked by the tone and manner in which I was addressed. I was told, in a very agitated and dismissive way, that my father would be better suited for another facility, with no further explanation. When I pressed politely for clarification, I was refused any details, spoken to in a disrespectful and aggressive manner, and ultimately hung up on. This behavior is unacceptable from any staff member, let alone someone in a leadership position. My questions were reasonable: I simply wanted to understand why there had been a sudden change after positive earlier interactions. Instead, I was treated with hostility and given no professional courtesy. I am deeply concerned that the treatment my father and I received may have been influenced by bias. As an African American family, it is difficult not to draw this conclusion when no substantive explanation was provided for such abrupt and disrespectful behavior. While I understand if their team ultimately determines that Brookdale Oak Park is not the right fit for my father, there is no justification for unprofessional and dismissive treatment. Families seeking care for their loved ones deserve transparent communication, respect, and courtesy.

My mom was averse to anything that had the word senior in it already. A serious health event accelerated my plans and I had no choice, but to force the decision and move her to Brookdale. Much of the up front decision-making was done by me. Of the options in the area, I preferred the layout, amenities and that there were both independent and assisted living options. I had tons of questions and they answered all of them, they also visited my mom in the hospital to talk about Brookdale and introduce themselves. My mom didn't like being there, to say she was downright disagreeable her first few weeks is being generous. Over time she slowly adjusted. When Covid hit, I was terrified and relieved all at once. Terrified because senior living communities were getting hit hard, relieved because I knew there would people to help her no matter what. They had strict protocols and regular updates, which I'm sure every family member greatly appreciated. When we had issues getting PPE for her medical condition, they were able to help us out. They even had staff on hand that could facilitate virtual meetings. Not seeing her for months at a time was hard, but they did their best. Over time, my mom came to adore members of the staff and they were saying great things about her (even though I knew she could be quite difficult). I'm not going to dress it up, senior living isn't cheap and assisted living is stupid expensive. Seriously, like make you nauseous every time you get the bill expensive. That said, the peace of mind knowing that she was safe and looked after was priceless. Even as my mom's health declined and she moved into hospice (they do that too), the staff was kind, compassionate, and informative.

    I've lived near this place for almost two decades. It should be shut down. In addition, I have videos of their workers smoking weed behind my garage. I am so tired of this disgusting building that smells like human piss and feces 24/7. Fun fact - about a year after this place opened a bunch of people died due to inadequate fire suppression. Owners should have seen that as a hint and shut the facility down at that time...unfortunately it's been 50 years since the fire but very little seems to have changed. This facility is a blight on our community and should be shut down and demolished...it is irredeemable.

We have a new kitchen manager, and the food has improved greatly, so I would like to commend Lynette Gonzalez for her outstanding service to City View. We also have a new assistant administrator named Martin Lee. he is an outstanding member of Cityview, observes the entire facility, and has an open-door policy to get issues resolved right away for the comfort of Cityview residents. I am going to suggest showcasing outstanding employees by having an employee of the month photographed on the wall behind the front desk as you enter the facility as well as a resident of the month. I have been here for 2 1/2 years, so I am quite familiar with the functions of Cityview.

Nurses bullied us into letting them take my grandfather for "a few days" because they wanted to manage his symptoms better. He was taken last night and we got a call this morning that he died. They didn't feed him. They didn't give him his medications. They didn't check his oxygen levels because they "don't usually do that" but they PROMISED THEY WOULD BECAUSE IT WOUKD MAKE US FEEL BETTER FOR HIS OXYGEN TO BE STABILIZED. They pumped him up with morphine when we specifically said we only give it to him once a day before sleep. They took him away from us to leave him in a room by himself and die.
