A nurse was then scheduled to conduct an assessment on Monday afternoon to determine whether independent or assisted living would be most appropriate. While I could not be present in person due to work obligations, I called in during the assessment to ensure I could provide support. My father answered the nurse's questions to the best of his ability, and there were no indications of issues at that time. Following the assessment, Lisa informed me that an internal review would be conducted. Today, after sending a brief follow-up text to Lisa, I received a call from your Assistant Director. During this call, I was shocked by the tone and manner in which I was addressed. I was told, in a very agitated and dismissive way, that my father would be better suited for another facility, with no further explanation. When I pressed politely for clarification, I was refused any details, spoken to in a disrespectful and aggressive manner, and ultimately hung up on. This behavior is unacceptable from any staff member, let alone someone in a leadership position. My questions were reasonable: I simply wanted to understand why there had been a sudden change after positive earlier interactions. Instead, I was treated with hostility and given no professional courtesy. I am deeply concerned that the treatment my father and I received may have been influenced by bias. As an African American family, it is difficult not to draw this conclusion when no substantive explanation was provided for such abrupt and disrespectful behavior. While I understand if their team ultimately determines that Brookdale Oak Park is not the right fit for my father, there is no justification for unprofessional and dismissive treatment. Families seeking care for their loved ones deserve transparent communication, respect, and courtesy.

My mom was averse to anything that had the word senior in it already. A serious health event accelerated my plans and I had no choice, but to force the decision and move her to Brookdale. Much of the up front decision-making was done by me. Of the options in the area, I preferred the layout, amenities and that there were both independent and assisted living options. I had tons of questions and they answered all of them, they also visited my mom in the hospital to talk about Brookdale and introduce themselves. My mom didn't like being there, to say she was downright disagreeable her first few weeks is being generous. Over time she slowly adjusted. When Covid hit, I was terrified and relieved all at once. Terrified because senior living communities were getting hit hard, relieved because I knew there would people to help her no matter what. They had strict protocols and regular updates, which I'm sure every family member greatly appreciated. When we had issues getting PPE for her medical condition, they were able to help us out. They even had staff on hand that could facilitate virtual meetings. Not seeing her for months at a time was hard, but they did their best. Over time, my mom came to adore members of the staff and they were saying great things about her (even though I knew she could be quite difficult). I'm not going to dress it up, senior living isn't cheap and assisted living is stupid expensive. Seriously, like make you nauseous every time you get the bill expensive. That said, the peace of mind knowing that she was safe and looked after was priceless. Even as my mom's health declined and she moved into hospice (they do that too), the staff was kind, compassionate, and informative.

My mother stayed there from November 2021 until February 2023. We were promised the world when given a tour - the best food, best care, night motion detectors in case they get up at night, call button watches in case they need help, the works! Everything ended up being a catastrophic failure which was a combination of true neglect, staff turnover, overpromising, and lack of problem solving from upper management. Staff would take hours to answer a call alert. That means if your loved one falls off the toilet and presses their watch or the wall string, they could bleed out and never be helped. The excuse was that assisted living alerts were clogging the phones of memory care staff phones, but they never tried to figure it out, or we were told their phones were dead. I constantly had to go get staff if my Mom needed to be changed, or just end up doing it myself if I couldnt find anyone. If your loved ones medications change or they have a lot of meds, make sure to be on top of the staff. There were countless issues with getting the proper meds were I had to go home and grab backup meds when they werent able to fill critical meds properly. Neglect and lies from the nursing staff. i was called one day to be told my mom had fallen but they went through protocol and she was fine. Much to my horror, the ring camera I had in her room showed the fall, two caregivers helping her up and tossing her back on the bed. The nurse never came to do an exam. My mom clearly was shaken up from the fall as she was barely responsive and all the nurse ended up doing was trying to shove pills down my moms throat as she could barely hold herself up. I am crying as I write this thinking about how neglectful this was. I showed the video to the executive director and it was just a shrug. They terminated the Nurse but nothing else was done or changed with protocol. The food portions were abysmal. if you didnt make it to the diner, which my mom didnt because the stroke she suffered caused her to be fearful of being out in the main room, you would be left with the smallest portions. And despite the fact it was known my Mom had trouble feeding herself, they would just leave the food with her and I would come and just see a cold dinner and her not having eaten. The turnover in staff using outside agencies meant that either staff didnt care about their job or they didnt know the personalities and needs of the residents. There were times staff would sit my mom on the toilet and then say they would be right back and leave here sitting on the toilet for 30 minutes - as a fall risk that was absolutely unacceptable. I could go on and on but it was just absolutely awful. There was no way I could sleep at night or take a day off with so much going wrong all the time. There were attempts to create updated plans for her but they were never followed through due to staff turnover or just lack of follow through. Dont let the nice facade of the building and main lobby fool you when it comes to their memory care. Additionally, multiple management members cursed me out when all I was trying to do was ensure my Mom was getting the care promised.

Management allows clients to cancel services permanently with no other offers. Management is focused on making money and not much else. Caregivers suffer with no pay. This place needs a new HR team.

She once told me that " my husband's goals are her goals". Melinda walks the walk as a Caregiver because even the medical staff and one of my husband's doctors have commented on how good she is. Recently a doctor told us that my husband is improving in his condition. We attribute this comment in part to the QUALITY Caregiving support from Melinda and Aris at Home. The supervisor /managerial office staff are exceptional in that they will often come out to the home to work a shift if the Caregiver is not available or had to be absent from a shift, just so that my husband can receive CONTINUITY of CARE! This is so unique because some office staff also hold certification to work as caregivers in the home. The office staff are always available 24/7 if you have a concern. My mother lives here in independent housing. She made an informed choice to live here. There are…read morepluses for us : close proximity to amenities and to my home. The Wye Valley building seems comfortable enough but there are some glaring issues. 1) The meals are not very healthy or satisfying. For the price that is paid, I would expect fresher food, more variety, more flavor (I have eaten there myself a few times.) More importantly, the meals don't seem to follow appropriate dietary guidelines for seniors with health issues, i.e. most seniors. There was a brief period when the staff had a dietician that was making a real effort to make sure the food was tasty, well presented and appropriately nutritional for the individuals. But now, the food is not very good. My mother tells me about other residents complaining about the food, but are afraid to speak up. 2) The second elevator has been inoperable for months, because of needed repairs. However, and this is on Brookfield, an inspection has not yet been completed, it seems like it's been out of service most of the year. 3) Although the building is for independent living, seniors still need support and stimulation. There is a small game room and a room with an exercise bike. The staff I have encountered do well with the resources they have, but it seems like more interactive activities could be added - things that are a little more engaging and lively - that would be appropriate for an independent living situation.

This place seems ripe for at least one lawsuit, and I have NO idea why Loyola / Gottlieb Hospital sent my father there for short term rehab.

Slow response to having no cable, no call butt0n - they gave me a bell that could not be heard and closed. the room door! Not fully ADA compliant for wheelchair use - dangerous. Many independent living residents have been here over 20 years -they are happy.

When my mom moved in, she required a great deal of patience and support in her new environment. She received that support from the talented team and management. She was welcomed and offered as much or as little support and engagement as would make her feel comfortable. I do not have a concern that hasn't been addressed by this wonderful team of caretakers who have come alongside me in navigating how best to serve my mom at this time of her life. The facilities are clean and well cared for. The Staff has looked out for my mom and for me. They have been such a blessing. I am grateful for everyone at Plum Creek!
