With each visit, Suzanne demonstrated outstanding listening skills, and happily answered all my questions, with clarity, making sure I understood her responses. Our sessions went beyond the traditional PT kind of exercises one expects. She explained why the given exercise was important to speed my recovery, and she modified and added to some of the exercises the hospital had sent me home to do. Her extensive knowledge base was also evident when I asked her questions such as, "How would I know that I am about to dislocate my new hip joint, when bending incorrectly?" Her explanation was clear, and assuring. It was a pleasure working with Suzanne every visit. I would recommend Suzanne strongly for anyone's home PT needs.

Their competence, knowledge, compassion and great listening skills is why I believe UC Davis has partnered with them as their affiliate Home Care Agency. Communication skills, patience and experience with homebound patients is different from health care providers in an outpatient or hospital setting. They definitely have that from years of experience in home care. Thank you to this team and the owner of American River Health Care for his insight and providing extra much needed services for free that are not provided by insurance. You have set the bar high! P.S. It's important for NEW home care patients to understand that home care professionals are unable to stick to a specific appointment time frame due to travel time and the importance of judging when the patient is physically and mentally ready before leaving for their next patient.

My mom stayed here most nights and I bought her a reclining foldable chair and they did not complain. Facility is also super clean and does not have a funny smell. I work EMS so I've been to a few SNF and this one seems to be the best. Wooden floors = no lingering smell = easy to clean. Bathroom is clean. The staff at first took a couple of days to adjust to my dad's medications, they forgot to give him his eye drops etc but after a while they were on it. They also gave me a sandwich and beverages which was very much appreciated as I visited my dad for 4-6 hours. There were also times where I stayed past the suggested visiting hours and never got a hard time from them about it. My only complaint would be the parking. It is tiny and when it's full it could be hard to find parking on the street. But honestly most residents here never really got visited as often as I did with my dad so it wasn't a huge issue. I'd like to thank the Filipino staff that took care of my dad as well as the rest of the staff including the physical therapists. The staff took excellent care of her--carefully monitoring her condition, administering medications, and supporting her recovery. Notably, she did not experience any falls during her stay, which was a significant relief given that she had fallen on her very first day at a previous skilled nursing facility. The reason I am giving four stars instead of five is due to a lack of communication after admission. Neither my husband nor I were contacted regarding her condition, diagnosis, care plan, goals, or discharge planning. It wasn't until I visited the facility in person four days later and requested to speak with someone that we received this information. My mother-in-law did mention that the care environment felt very constant, saying, "They never leave you alone--up at 5:30 for medications, bright lights, no privacy." However, I reassured her that the staff was being attentive and doing their job to support her recovery. By the fourth day, her infection had cleared, and she had regained independence in getting to and from the bathroom without incontinence issues. According to the discharge planner, she was also able to walk 175 feet with a walker under supervision. Special recognition goes to Sondra, the discharge planner, who was incredibly efficient. She coordinated the physician's discharge orders, medication reconciliation, and completed the necessary 602 forms for her return to assisted living--all by 2:55 PM on the same day I met her. We arranged private medical transportation, and my mother-in-law was comfortably settled back into her own bed by 6:15 PM that evening. Overall, we are grateful for the quality of care she received, with the main area for improvement being communication with family members.

    My father was brought there from the hospital around 4pm one day after having a stroke. They did…read morenot tend to his pain so after many requests for a warm washcloth my cousin had to track one down to help ease the pain. They told my mother they would find rails for the bed and it ends up they never did. He would constantly try to get out of bed to use the bathroom even though he had a catheter. He was extremely confused and needed monitoring. Well at 11 pm that evening I got a call that my dad had pulled his catheter out and they were taking him to ER. I waited for my father at the hospital and once I saw him his hands were covered in dry blood and said he needed help and no one would come. He couldn't remember anything else but I assume the process of ripping the catheter out and tearing the lining of his body sent I'm into a state of shock. When I called them once I was at the hospital the man gave the phone to what I guess would be his care taker and she seemed panicked pretty much saying she didn't know what happened. That was the beginning of the downfall. Sepsis. Infection. This place is awful. My father is now dead. No stars but it wouldn't post unless I put one.

The staff has been awesome! I still have zero complaints. I want to give thanks to the new administrator, Toni Jones for making some positive changes. One of the changes made was the parking situation. I work around the corner, and I see my mom every day on my break. In the past I would drive around trying to find a place to park and it would take up some of the time that I could be spending with my mom. Not anymore! The change that was made is the staff park elsewhere. Now visitors can find parking. Parking is no longer an issue. On 5/24, Toni had arranged a "pink and purple candlelight dinner" for the residents and their family members. It was amazing! The food was so good I had thought it was catered in. The dinner was made from their main kitchen. A lot of smiles that evening. What a thoughtful/meaningful event for the residence and their family. Attached are some pictures.

Right when it seemed impossible, a few in home support nurses recommended Gramercy Court, saying it is the home to their happiest & best cared for patients. By the grace of god, as soon as I inquired, a room became available for my mother. In disbelief we moved her into a lovely, private room with her own bathroom. She instantly made new friends with both residents & staff. She'd text me all day about the excellent food & would video call me every night smiling, in tears over how much she felt cared for. Admin Toni Jones is warm, present, supportive and professional at every turn, and has truly created a home like environment at Gramercy Court with the highest standard of care. A big thank you to her staff, especially head nurse Joanne, who love & advocate for residents like they are family. Gramercy Court has been a safe haven for my mother & one of the biggest blessings of our lives! My family and I highly recommend.

Even though it's obvious that they are busy; they're never too busy to stop and talk to the patients or their families. Having worked in the medical field I know how important it is to find this characteristic in a facility! I also know that it's very hard to find!!!!! So I appreciate the fact that these qualities are presented so freely every single day. This is the quality that the entire facility gives and if there is a problem of any kind it is taken care of ASAP with care and concern and respect!!!! If that ever get the food to eatable status I myself would freely give this facility and every staff member a (10) ten from the bottom to the top of the employee pool. The shower was broken for the first week I was there and has no grab bars. The CNA's were caring. Our women's room shared a bathroom with a men's bedroom, and I had to clean it every time I wanted to use it. They lost my personal medicine. They didn't give me a release date until I threatened to call the ombudsman. The director of nursing did not return my family's calls. The dietitian worked with me because the food made me vomit. Physical therapy was pretty good. The activities director was great.

    Impressed. We all…read moreknow the stereotypical negative reputations that go along with care at Skilled Nursing Facilities, but this place actually cares! My husband was transferred here from a rehab facility to receive rehabilitation for a below knee amputation. He was at deaths door, practically, when he was admitted. Incredibly weak, out of it, at times lethargic from the care he had just escaped from. At American River Center, it's lively, clean, the staff is exceptional, and above all the physical therapy has been the best my husband has received the whole year of this newly experienced amputation ordeal. Who ever oversees the management of this facility, my hats off to you! It smells GOOD in here, they take care with organizing the menu, they provide weekly newsletters with the menu to their patients, and you can make requests to the menu. They allow family or visitors to come and go. The staff is very friendly and very attentive. The facility is always being maintained whether it's inside or out. I really just don't have enough nice things to say about it here. By the time my husband was discharged he was able to walk with assistance of a rolling walker. Compared to when he arrived needing 3 people to help him out of the car, there was literally no need for help upon discharge. Physical therapy helped him reach his goals. I am so grateful as his caregiver! Above all, I felt comfortable knowing he was in good hands and never worried about his cared. This is saying alot for SNF's!!! I'd give more stars if I could, but 5 it is!! Thank you American River Center!

    It's certainly better than the three others my husband was unfortunate enough to have to endure in…read morethe past six years, but there were problems with a bed that injured him needlessly, and the food on his diet was very lacking. There is a clear disconnect between the kitchen and the other staff at the moment but the rehab. staff is mostly very good and as they don't have long term care they do seem make a genuine effort in that area at least. I'd recommend using it if the need arises, and keeping a close eye on it while you do.
