I walked into this facility and know what met me, almost all the staff were smiling and busy with taking care of their residents, no smells or coverup smells , a nice wide open lobby with residents smiling and interacting with one another and the staff in a wonderful manner, as I took a tour around the smiles abounded and hardworking staff were busy attending to their residents needs, this a a large facility and i was amazed no call lights were going off, no one was screaming or calling for help and the facility has a tracking system on each station that displays the times a residents call light had been on and off the 2 nursing stations I walked past no one was calling for more than 5 minutes. I grew up around nursing homes and my family owned and operated 2 of them back on the east coast, in my years of work in healthcare I have been in numerous facilities and I would say this is in my top five especially for its location, its close to everything. I highly recommend coming in for your own tour and to see what is what. read more

My grandmother was miserable here. On our visits, she'd press the assistance button and it would take no less than 20 mins for someone to come help her. We purchased noise cancelling headphones, expensive ones, that went missing within the first week... even after labeling them. The room had 3 patients, who blasted their tv's at full volume. Understandable, since how are they supposed to watch their shows if the person just 3 feet away is also blasting their tv? But with no volume or sound limitations/hours, my grandmother could not get any sleep. We requested a room change that even after confirming, was never achieved. read more

I have stayed here twice and each experience was great. The staff treat you like family, and they are dedicated to your recovery. The nurses are exceptional, paying very close attention to your medications and needs. Even the receptionist is so helpful and lovely. The true gem Is the THERAPY DEPARTMENT! After not being able to walk for over a year, they literally got me back on my feet. I can't say enough great things about the THERAPY staff. Tessa, Ashley, Shoob, Valerie, Chris, Tariq, Mo and the whole crew are incredibly talented. So grateful for my care at Capital Post Acute. read more

I was having so much anxiety sending my Mom here after a horrific experience at another facility. The whole family was traumatized. She's been here for a few days now and she is being extremely well taken care of. She's getting the therapy she needs, the staff are all so kind to her, and very attentive. She's also likes the food. This place is not to be if you are here for short or long term. The staff is very slow in assisting my mom to the bathroom, there are times she just got up and went by herself which she's not suppose to due to fall risk. There got to a point where I told the nurse in charge that she has to wait 30 mins for someone to just help assist her to the bathroom. There got to a point where my mom ate and drank less due to this situation and end up in the ER for 3 days because she was on a antibiotic and didn't consume enough water which made her kidney fail. She never went back to this facility after, we looked for another facility. read more

Beautiful high quality skilled nursing and Therapy Care. They have great team members all the way from the front desk receptionist Pushpa who always smiles and greets everyone who walks in and out, to the nurses and Physical Therapy staff. Please do not send your loved ones here. I absolutely agree that this place is out for the money, not for the care of its patients. I read a few and whoever has taken charge has done nothing to improve patient care AND family visitations. They are trying to kick out without due process, threaten to send hours away, and constantly belittling. I worry for my family member who is currently there. Instead of improving, his health has decreased. The type of care does not meet my standards or patient needs. I'd give zero stars if possible. They do not deserve the 3.9 rating they have. If you love your family, send them elsewhere. read more

I do not recommend this facility for specialized care, especially for patients recovering from stroke or living with dementia. A loved one of mine stayed here for about a week, and the experience was deeply disappointing and horrifying. The lobby looks clean, but the patient rooms were extremely outdated and in need of a remodel. It seemed like some work was underway, as the first room my loved one stayed in was awful, but the room he was moved to was somewhat better. The rooms were 3 to a room.... and uncomfortably hot, as if the air conditioning wasn't working. There was a strong smell of cigarette smoke throughout, which is unacceptable. The facility is open with no visible security, and it seemed like anyone could walk in and out freely, which is very concerning for patient safety. The staff did not seem trained to care for patients with dementia or behavioral issues and often made situations worse instead of better. Food quality was poor, and on day one I overheard a staff member say there wasn't much food because "whoever was in charge hadn't gone grocery shopping." My stomach literally dropped and left me wanting to leave with my loved that same day.. The only positive was CNA Monica, who went above and beyond and made us feel our loved one was in good hands. we definitely noticed a positive difference in our loved one( when she was there), and her dedication provided a rare sense of comfort and trust during an otherwise troubling stay. This facility may be suitable for someone who does not require much supervision or care and is not at risk of falling. Also, for patients who like to smoke.. but it was not the right fit for our family. I would not recommend this place. When initially selecting this facility, I was influenced by the numerous five-star reviews, which struck me as somewhat surprising/odd. I hope my review provides helpful insight for others as they make their decisions. It is my sincere hope that this location will address the concerns for future residents and their families.. so they can have a better experience. read more

Shoutout to the therapy crew. I was involved in an accident where I needed care. Facility staff was well attentive friendly and supportive. Just wish there was more coverage if someone called out. But the therapist did an excellent job. They helped me build my strength and confidence in becoming independent while in my healing phase. Still not fully recovered but gained knowledge and confidence on caring on my own and was able to leave and apply those skills in my own home and be with my family. read more

I was in this place from Sutter hospital in Sacramento and was taken over to Capital post acute nursing facility. Trevor who runs the place was very nice treated me good . But the staff was awesome and the nurses was kind and very polite catered to my needs and made me feel at home until I got well enough to go home . This place is the best when it comes to caring for their clients . Couldn't of came to a better place . I give this place 5 stars . read more

This facility provides excellent skilled nurses and and physical therapist for short term and long term support.

My mother in law receives attentive care that is genuine. From activities directors, Physical therapists, nutritionists, front staff etc. Someone always pokes head into her room to make sure of her comfort and needs. Clean and well staffed. this is the worst place anybody could ever send their loved ... It like and if you pay most can't talk or walk to even speak up for themselves. I have personally witnessed the staff be rude to some of the patients telling them you're in my way and they're literally in a wheelchair... My loved one sat and feces for hours. This place needs to be investigated and either these are all fake reviews or those people did not visit their loved ones on a regular basis even a blind man could see that this is the worst place ever and completely unsanitary. read more

If I could give this place zero stars I would. (This place having a four star rating is absolutely comical) Please do not send your loved ones here. My fiance who was fully able to walk, talk and do most things for himself was placed in this SNF because he needed IV antibiotics. There's 30 patients to 1 nurse. No proper training has been given, to the RNs or the CNA's. He had a picc line, which goes straight to your heart, one of the nurses left the access cap off which left room for bacteria to enter, when he was already fighting 2 infections, another RN handed him a 30 gallon trash bag and told him to breathe into it and that he should be fine, when he was having a hard time breathing (hyperventilating for over an hour), and then walked out, no one came back to check on him for 4 hours. He was not given breakfast or lunch for 3 days straight. The things I witnessed them do and say to other patients who were blind, unable to walk, or unable to advocate for themselves is absolutely volatile. I heard a CNA call one of the patients in the same room as my fiance a dog and tell him to shut up, and lay down and don't move and aggressively pull him back up on the bed by his legs when he just had hip surgery and was blind and had a brain injury so his memory and perception was very bad. They yell at patients, don't care about the pain they're going through and have a total disregard for safety. There were feces on the curtains and walls, the whole place reeked of urine and fish. There were people smoking m*th and cra*ck outside in the cigarette smoking area. The director of nursing Nadia was extremely rude and degrading to my fiance and I, when we brought our concerns to her and she invalidated every single one, and got very defensive. When we brought something up more than once she would say " since we already talked about it we don't need to circle back around and bring it up again" when nothing was done to resolve it in the first place. There's so much more I could include but I would be typing forever. Again I'm going to say PLEASE DO NOT BRING YOUR LOVED ONES HERE!!!! I do wanna say Serena the community resource director was the only person at that facility who actually cared and tried to resolve anything at all, so big props to her she is the only thing this nursing faculty has going for them. read more

My dearest friend had stage 4 cancer. And he died in this nursing home. A few days before he died he was sitting in his bed he couldn't breathe. There was pee under the bed and all over the bed. There was milk in glasses and it was unkept. Me and another friend complained. And when the nurse came in he quietly said only one word hospital. He never went to that hospital and died a few days later. I believe he was over drugged between oxycodone and morphine. I understand about making someone comfortable but not to the point they cannot talk or they pee all over themselves. A few days later he passed I was never called being the emergency contact. And when his sister went to pick up his stuff. His wallet was missing among a backpack I had brought over that was Lakers and among other things that I had just bought for him. Everything is gone including the money in his bank account now. This is the most horrible place to go. They said they comforted him in the end. But there is no camera in those back rooms how would they have known if he couldn't breathe. In fact when my friend checked on him. She saw that he wasn't able to breathe very good. And she had to go to the nurses station and address that. I feel he died alone. And I feel that this company this place should be shut down due to the fact that things are being stolen from people that have no control and no say-so in the end. It's a horrible horrible place. I wouldn't wish this on my worst enemy. His family is collected his few things. Which added up to nothing. And everything else is gone. When you pass a family should be able to come and get the things that they had. Sometimes that's the only comfort they have. They're the last gifts that are given to someone who's dying. I'm shocked at this whole thing. It is unprofessional in every way. If I could I wouldn't even get it if it is sorry at this point. I never understood why nobody ate in the dining rooms. And they took everything to their rooms to eat. Socialization especially at your last stages of life is which you need. At this facility it is not run like this. It's too sad to watch so many lay in bed and have no active part of life. What's the point of wanting to live when you're staring at a wall or staring at the ceiling and sleep all day long. My brother has been staying at Capital Post Acute for several months now and all I have are praises to the entire staff. Since day one everyone has shown kindness and sincere caring not only for the patients and residents but also to the family members. Thanks to the amazing leadership of Eric Lawrence, Administrator, and Nadia Vermeulen, DON, my brother is in very safe hands. I am a living witness to the very admirable traits that all Interdisciplinary Team display from the nursing staff and to all ancillary team members who diligently collaborate to deliver safe and excellent care. As proof, I went up to the office to vent out my concerns on the first week--Eric and Nadia intently listened to my case. Eric then blurted out, "Let me make some phone calls and see what I can do to help." I am so reassured that my brother is in very good care. Thank you all for all that you do! Think twice before sending a loved one here. My 93 year old mother was sent to Capital for rehab after fracturing her hip. What was supposed to be a two week stay turned into a month long stay with my mothers condition deteriorating in the process. While scheduled for Physical therapy each day she failed to make improvement and I suspected that the hip had been dislocated due to the therapy given however they did not and did not order another xray instead they wanted us to send her to an assisted living facility they partner with, I contacted her doctor and scheduled an appointment . My fears were confirmed the hip was dislocated and she had been walking on it for weeks , and the staff did not notice the problem. My mother needed surgical intervention to repair the damage caused. The Social Service director in particular (KKD) was a bully- manipulative , dismissive- rude and unsympathetic and was more concerned with accounts receivable than actual patient care. I do not recommend this facility as a result read more

My father was recently released from this facility. It was an amazing experience. He had gone to another facility before in Galt and that was a complete nightmare. So we didn't have high hopes for this one. Very happy we were wrong. Physical therapy was great, he's more mobile than he ever was before. The food was actually very impressive. Very clean.
