Wonderful people, warm, attentive, lots of good will and connection with the staff and even other patients. I cannot believe how much my mom benefited from the treatment and socialization she has gotten. Place is clean, well staffed and staff are professional. This place doesn't even deserve 1 star in all honesty. They truly don't give a rats ass about their residents. It started off okay, about 2.5 to 3.0 stars and from there it went down hill FAST! I'm a type 1 diabetic and I would have to go without eating anything sometimes because all they would serve me was foods drowning in carbs and sugars. I begged and pleaded for a while then after that I had to start yelling and screaming and be the squeaky wheel to get anything done. Their social workers suck. If you don't keep on them they'll blow you off. As a matter of fact, Social Security told me that they did my forms wrong and I have to start all over again. So now I have to wait 200+days for my SSDI. What I was admitted for which was an infection in my leg was never taken care of and I was discharged in the same condition that I started. Most of the CNAs and Nurses are terrible. They barely speak English and they don't understand you most of the time. One time they pricked my pinky to test my blood sugar and it bled so bad it ran all the way down to my elbow. When I asked the nurse for a band aid she told me that she didn't have one! When I asked how in the HELL does a nurse in a Skilled Nursing Facility doesn't have a band aid she told me that they were locked up and only the MOD had the key and she was not going to be in until 8am. This happened at 6am. When I asked how come there isn't a first aid kit available she shrugged her shoulders and gave me a look of cluelessness. Totally deer in the headlights. They have an Asian resident that goes up and down the hall screaming and does nothing about it so don't plan on any peace and quiet. My roommate fell out of his bed once and was screaming bloody murder begging for help and I had to crawl out of my bed into my wheelchair and go scream at the nursing station to get him help. Their physicians suck. Lazy and dumb. I had to go to the emergency room twice in three months because they are that bad. It's pretty bad when you ride in the ambulance and the EMTs apologize for the shitty help your getting. The Emergency room says they're known as Shitty Creek Post Acute not City Creek Post Acute. I haven't even begun to scratch the surface of the nightmares I have had to endure. Now that I have been discharged 8 days early, they won't take my calls and I need them to transfer my prescriptions to my pharmacy and they said that they would pay the pro rated amount which I'm positive that is why they are dodging my calls. I'm out of certain medicine now and have no way of taking my insulin. I'm going to call my regular physician and see if he can get my prescriptions refilled for me and sent to my pharmacy. So much for getting the appropriate and professional help. The place needs a full audit and CHHSA needs to be notified and also the Ombudsman. This is no laughing matter. I am so thankful for City Creek. The facility, the staff, and the care is all wonderful. It was a tough decision for us to move my mom from her home to a care facility after she became immobile. We we very concerned whether she would adjust to the care facility environment. The staff has been very culturally sensitive and I am thankful for all the Punjabi speaking staff. You all are wonderful. Thank you all for making her feel at home. The facility is neat and clean. The reception staff always make me feel welcomed. I would mention some by name but I am afraid to leave anyone out. Can't say enough great things about this facility. It appeared as though they weren't able to care for me. Stayed a couple of days and sent back to the hospital. Came back and stayed around 5 hrs and sent back to the hospital again. Was told fever, Lethargic and possible infection. Arrived to hospital they found nothing. Went to retrieve my belongings and was sent someone else's things a few of my own. read more

The care quality is laughable. They rely on 18-22 cna/tech types to perform most functions & placate patients rather than provide a minimum standard of medical care. They leave their front line nurses with zero decision making authority so any real interventions that could or would ease suffering. It's currently 4:18 & I was woken up to a broken machine making high level of noise & staff didn't care. Their solution to the problem? 5 mil of melatonin & an apology. Tomorrow I can take a nap. That resident definitely lives there and everyone knows it, and knows who he is (D) It's absolutely disgusting that this not only be posted but also denied. I know better can be done, first is identifying the problem and address it, but to deny wholeheartedly?!! All looks good on outside but kitchen is absolutely horrible and unsanitary. Worst facility I've ever worked at and I pray for your loved ones. I came in to ask about my in-service hours and I had been contacting the administrator and the DSD but yet again this faculty is refusing to hand me my in-service hours. I have worked here for an entire year and in that whole year, I've been emotionally and physically abused by this facility and its staff/patients. One of the patients here scratched me causing a scar on my forearm and the facility told me not to report it. It's been so many staff hurt by the patients and it gets thrown under the rug. Who would want to work in these types of conditions? Ask yourself that. They said it was "just an accident" but it was very intentional and other employees have been hurt by "THE DONA". A name that causes fear of bodily damage. I have scratch marks to show for it. Another time I was being bullied by my co-workers about being the YOUNGEST CNA in the building. It was clear they were jealous of me and would do things like take all the Lenin and one had even admitted to removing chairs. Which I had no recollection of. The nurse was sitting in my face while the employee JUST admitted this to her and she did absolutely NOTHING about it and even refused to take the needed steps to stop the situation. On top of that, I also had to report her for another situation where she told me that it was insubordination. I didn't want to take on the assignment because I told her it was unsafe for me due to all the extra patients that night. I know when to admit the workload is insane. However, I was still forced to take on the assignment in fear that I would be fired. The faculty yet again did absolutely nothing about it. City Creek post-acute is unsafe for both staff and patients alike. There's so much abuse that goes on here unreported and they will come against you if you speak of it. Although I've told every time. And reported the nurse. I've been ignored too many times by this faculty and staff and don't believe they will give me my in-service hours. The DON has stood by and let all of this happen along with the administration here. Mr. Quinn. Grace the scheduling lady has been a dictator in this facility for over 2 years. She walks around and constantly tries to disrespect or get over on the other employees. My mother in law recently spent her hospice time at CityCreek Post Acute and during this very sad time our family and mother in law were taken care of so well by the nursing and administration staffs. They were extremely attentive and caring people. I did not expect this level of care we were so pleased. If you have diabetes, do not stay here. They are horrible at helping regulate blood sugar and one time my mother had high blood sugar and her nurse said "What do you want me to do about it?" I don't know if she was given insulin but it's best to find another facility. It takes forever for them to answer call buttons and requests go unanswered for days. She's recovering nicely from her stroke, but that has nothing to do with the lazy nurses. read more

After my spinal surgeries, I was sent to City Creek post acute facility in Sacramento. I received much better care And a previous skilled nursing facility in Modesto. For the most part, the staff was excellent. I definitely questioned everything. I was my own advocate for my care which I appreciate it. I immediately wanted to introduce myself to the administrator. There was any issues that I could potentially have. I would take those concerns directly to him. The meeting with him was brief And somewhat rushed. Most of the nurses and certain CNA's were excellent. Pinky and Jessica thank you for being such Great caregivers. You made my stay really enjoyable. Rajwant I will miss you. You are the best CNA. I would've given City Creek a five star, but one CNA, who I unfortunately was stuck with, named Gracie was very disrespectful and needs a lot of work with her personal Skills. Come to find out she's only 18 years old. Which answered a lot of questions. I had. I recommend that she continue receiving monitored training. She was the worst Individual I encountered there. In my opinion, an 18-year-old-year-old does not have the compassion to treat people who are sick with dignity and with respect respect that they deserve. City Creek is a wonderful company to work with! They where Previously owned by a company who owned Saint Francis. Since the new ownership staff is very helpful and sensitive and quick to respond to your needs. I highly recommend city creek for anyone who is needing a loved one to recover on their skilled nursing side or is looking for a long-term placement in the assisted-living side I would not let the past ruin the future of city creek I would give them a try over and over again. read more

my personal experience is they are overall managed poorly, inconsistent with test result and patient progress... reporting results changes depending who u talk with, they put patient at risk, make their own decision (contrary to the doctor) on when to administer pain meds, promise to call back and don't, was asked to call back because they might not remember. I'm not sure what constitutes elder abuse... i would think this is probably a version of it. my opinion they do as they please and have no accountability. need to be reported to someone. i read more

Gross was my first impression, very unprofessional, chaotic, it smelled from the second we walked in. Patients wandering, no one at the front reception verifying information or temperatures that you have to give yourself. It seemed like a bunch of unsupervised low level, un-trained employees that were left in charge. I recently started a position there and was only there for a week before being let go for a "bad fit". I was told this after I had asked to speak with the ADON about the way the "treatment nurse" had been acting towards me all week long. I'm a very experienced wound certified nurse and had been extremely excited to start there and be part of a team providing high quality care to patients all day. I very quickly realized that what was actually expected of me was to fall in line and under no circumstances was I to in any way shape or form contradict the queen bee. I'm very sorry to say that I'm not built that way and when I see something I do try to take the opportunity to help share my knowledge with others. I have a deep passion for wound care and I have a deep passion for learning and sharing my experiences and knowledge with others so that we can all be a stronger team together. What I got was being continually contradicted, shut down, ignored, and treated as if I didn't know what I was talking about. I was never taught the charting, only ridden hard about the treatments themselves. When I finally approached my ADON about the issue at the end of the week, I was made to believe that all would be resolved and everything was going to be fine. I then had a clearing of the air with my co-worker that seemed very positive and productive. I was then called into the administrators office and told that there had been "multiple complaints about the way I had approached people" and then they told me about an event that just absolutely never had occurred that was supposedly so concerning they had to let me go yet this had supposedly happened on Tuesday and they didn't bother addressing it until Friday AFTER I made a complaint. I had seen other reviews from former employees who talked about favoritism and all that and really didn't buy into it. I've been a nurse for 20 years and this is the first time I've ever been released from a job. I actually left a job making more money to come here, and they came up in their pay rate quite a bit to acquire me because of my credentials. Maybe for the sake of their patients they should really stop and realize that sometimes, the "culture" you're trying to protect is toxic and not what is best for your patients. They fired an extremely dedicated, compassionate, caring, kindhearted, and lastly a talented nurse who could have contributed a lot to their team if they could have allowed me to and not been only interested in credentials but not my brain. read more

So many wonderful people have been attentive to him and have been reassuring to us as this was our family's first time working with a Rehab Facility Skilled Nursing. I visited my friend of 50 years who was moved here to recoup from surgery. I was so pleasantly surprised at the cleanliness and the staff. Years ago I had to place my mom in a facility like this, I went to 6 different places, not one was as wonderful as City Creek. Girlfriend received great care, they listened to her and called doctor immediately whenever it was needed. Only issue was only 1 tv in room worked and doors are locked, no visits after 5pm. I was not asked to leave and stayed until 7:30 pm. After discharge, they called my girlfriend to check on her a few times...THEY CARE! read more

My mother was admitted after her knee surgery. Unfortunately it was just before Christmas and she was so sad about being away from family. To our delight the staff was amazing. The front desk, the nurses the physical therapist. She was treated so well and the nurses genuinely cared about their patients. I was so worried about her at first. You never really know how the facility will be and what kind of situation you are putting your loved one into. Our experience was wonderful and there was a months worth of follow calls checking on my moms recovery. I was very impressed and we made sure the facility management knew how grateful we were. The food was typical but hey she got hot meals. The beds have very thin mattresses on them and linens were worn. But that was doable. My mother's safety and care was the most important quality. AVOID AT ALL COST!!! Not sure where to start, facility was under construction and was totally inappropriate to admit anyone to recover from hospitalization...embarrassing for Kaiser to even have this place in there system. Rooms were cold, dark, and dirty. Dad had 3 falls and ultimately had to be re-admitted to kaiser. Food looked like something I would not feed my dog. Attendants seemed uninterested in caring for anyone. Bed and linens were awful. your loved ones deserve better. If you are offered this facility, reject/appeal...do anything you can to avoid this place. read more

City Creek staff are nothing but amazing. Since new ownership my father has been extremely happy there and staff are very pleasant. They return my calls and are always happy to help with the little things. Even been great at helping us find a way to pay for long term care in their assisted living facility. Went in for a quick tour today with Frances and she was amazing. I honestly can not believe how good and clean this place looks. The staff was very friendly and from what I observed they were kind to the residents. Dining room is a good size for residents like my mom that use wheelchair. My only complaint was parking was a challenge but it was probably due to the time of day. read more

If zero stars was an option that would better describe this facility. My brother was sent to this facility with clear direction in regards to his needed care. The staff was incapable of meeting his needs, even with something as simple as the food he requires with a jaw broken in multiple places. Strict no straw order, straw in his drink when I arrived. His stay consisted of one night and that was too long! By the time I arrived the next morning he was in excruciating pain, a level of pain I hadn't seen him in through the entire duration of time since his motorcycle accident. I had to call 911 to have him removed from their facility and transported to a nearby hospital to provide the care and medication he requires and was clearly not receiving. When he requested the medication in the form he was taking it previously at the hospital he was threatened by staff that if he complained about his pain medication it would be taken away from him. (Medicine not being given as directed by doctor, instead they ground up something prior to coming in the room and put it in applesauce) there was an elderly man in the same room that was making similar complaints to the nurse of pain, he was ignored. I provided multiple examples of issues with the care or lack of provided to my brother and they all went unaddressed. There are a total of four employees I spoke with and expressed my concerns and complaints to, when I asked the Administrator if as mandated reporters any of the employees my brother and myself had spoke with had reported the information they were provided, his response was no. There is something terrible happening at this facility. I would not recommend sending ANYONE to this facility for any type of care needed! read more

When we called to speak about transferring my dad to their facility we spoke with the manager Claudia. She was very pleasant to speak with. When we drove up to the facility we were not impressed with the area or the look of the building. Many staff were walking in and out of the facility without wearing masks. Visitors are stopped prior to coming in and must put a mask on prior to entering. There were patients wandering in the hallways or just facing walls. There was a patient just waiting for the door to open so she can attempt to leave. An employee quietly turned her around facing the opposite way of the exit. Front staff at 4pm was nice. It was after my Dad was brought to the facility that caused even more concern. My dad called us to let us know he was there, in the back ground we could hear people yelling. We could hear a nurse or staff member yelling to turn down their tv and I'm only assuming a patient yelling back at her. My dad was very uncomfortable. He did not have his cell phone and was calling from his room to see if we brought it. We had his bag delivered earlier so I called the facility to speak to a nurse or staff member. Holly answered the phone. I asked her if she knew where my Dad's bag was. She seemed irritated but I just ignored it at first. She asked where I left it. I let her know we gave it to the staff at the front desk around 4pm. She asked who and I did not know. She then said, did you just leave it at the door or did you give it to someone? I repeated that I gave it to staff and that I was just talking to my Dad and he is uncomfortable because people are yelling in the hall. She quickly became defensive and said that my Dad was far away and he couldn't hear anyone. I said why are you talking to me like this. Her tone was very rude and inappropriate. She said MAAM, I am just trying to help you. I let her know that I was on the phone with my Dad and even I could hear the yelling, Holly responded with, Well what were they saying then? I told her that if this is how she speaks to someone on the phone, I'm concerned with how she speaks with the patients and asked her to give the phone to anyone else because I did not want to speak with her anymore. She said fine, and as she handed the phone to Naomi, she said, you take the call, I can't deal with her. I let Naomi know that I felt Holly was extremely frustrated and should not be speaking like that at all and I just needed to locate my Dad's bag and at this point did not feel that my Dad was in a Safe place. Naomi offered to help me and I said thank you. She located my Dads bag and gave him his cell phone. I let her know that Holly should not be working there if she is that angry or frustrated with simple questions and to also let Holly know that I heard what she said and would be calling back to file a complaint. I thanked Naomi and hung up. My Dad called from his cell and the nurse did not know we were on the phone and she told my Dad that if he didn't sign the papers to be admitted then he would not get fed or receive his pain medications. My Dad said he would not be signing. My Mom then called the Manager Claudia who understood that we were not going to leave my Dad there and we had him transferred back to the hospital. Claudia and Naomi were great at customer service. I don't care if someone is having a bad day, you do not take it out on customers or patients. I personally feel Holly should not be working in the customer healthcare industry and the manager or owners of this facility need to understand that Holly, who is rude and inappropriate was representing them. Due to the conversation I had with Holly, there was no way we would leave my Dad there. I am thankful my Dad can speak and understand what is going on. Sadly, there are many patients there that could not speak up and if my review helps even one family, then it is worth it. read more

My mother in law is currently in this facility. Our family is beyond disappointed with the lack of communication and dishonesty that has left us in a state of extreme financial hardship and no closer to regaining her health. With very little opportunities for PT, the insurance stopped covering her stay at the facility who contacted us after 5 pm the night before coverage was to end to get verbal confirmation that we acknowledge and approve. According to the information submitted by the facility my mother in law can use the restroom, take a shower, and perform daily tasks with limited assistance.... this was the basis for the denial..... and she has not walked since entering the facility, is on a catheter, and has not performed one of these tasks independently. So based on this lie, she has been charged 300 dollars a day since July 4th and is no longer being offered PT to improve her condition. She is in need of 24 hour care and has been pending medi-cal since early June. The facility has been non responsive to calls (still waiting on a return call from last Thursday) and unhelpful in identifying alternatives...in the meantime they collect 300 dollars a day from a woman who is on a fixed income. They do not operate a human centered facility for the purpose of improving the outcomes of their clients....this is a shady business. read more

Where should I start? I kept calling to be kept in the loop for the care they were giving my relative. I never received a phone call back since she was transfered there. I had been calling every few months and was always talking with someone new and was told they would call when they were doing a review on her health. Never received a call or update. I was her payee and I asked about her bill but the told me it was taken care of by her insurance. Next thing I know is they had her sign paperwork to transfered her payee to someone else. Because I was unable to visit her at City Creek they started to take advantage of her. They wrote me a check from the place she was transferred from to deposit back into her savings but the check bounce. I definitely would not recommend this place. Oh yes, she had gotten COVID-19 twice! How? I thought the place was locked down and masks were in place. DO NOT go there. read more
