I visited the Luverne, Alabama location. My two in-network vision insurance policies were confirmed…read moretwice by EyeCare Associates and by each insurance company. As I was leaving Kay Mount Sport, the office manager, demanded $55 cash. She repeatedly stated that she had never heard of my secondary in-network insurance company. She was forceful, loud, and unprofessional. Four patients in the waiting room dropped their heads in embarrassment. [She also stated my contact lenses would be $544 +/- out-of-pocket for the year. Also, she stated that they did have a $250 mail-in-rebate I could use; showing a 6"x10" advertisement flyer. Or at a six months or three months prescription, I would pay a doctor fee plus the prescription. Fishy. Later, the prescription for the contact lenses was filled at $31 for three months with the in-network insurance she denied having ever heard of. The secondary in-network insurance also covers two follow-up visits if medically necessary. Not just for prescription pick-up. By Federal law, I'm owed one year of the prescription.] I insisted that I didn't owe the $55 for the visit. Kay Mount Sport told employee Adrian Odom to look at my secondary in-network insurance card. Adrian was also dramatic, loud, and acted confused by my secondary in-network insurance card. I paid. I had to leave to pick up my son from school. Their actions were deliberate and done with the act to defraud me. As a provider, they obtained my property through false representation. It was theft by deception and insurance fraud. EyeCare Associates billing department emailed, correcting them the next day after my bank's fraud department became involved. Looking back, I should have filed a police report. I did get my money back immediately after they taunted me that morning for hours on how I wasn't getting it back when trying to resolve it directly with them. Ladies, isn't it wild? You don't get to steal and break Federal and State laws. On my visit, as a new contact lenses wearer they led me to believe I would pay $600 out-of-pocket. In reality, the visit was $0 and the contact lenses are $124 for the year. Attached is the secondary in-network insurance company that office manager Kay Mount Sport and employee Adrian Odom claimed to have never heard of showing their employer as a contracted provider.

Been a patient of this facility for several years, can I say since my children were in grade…read moreschool. They are now grown (ages 28-30). Always love the atmosphere and the care Dr. Lamb ministered toward her patients. She always gave you the sense of you being important not just for the funds, but for the medical care of your vision... Monica and Kate are also amazing, I got the chance to meet Bella today who was exceptionally nice... I always find the most awesome frames, each pair that I've gotten are unique and I've never seen another pair like them on anyone else, which is great cause I love setting my own trend... I will be seeing a new physician today, I pray I can give 2 thumbs up...

I went in to see Dr. Eades. I had two appointments here. The first was a consultation to discuss…read morewhat needed done, and the second was for the procedure itself. For my first appointment, I went in and spoke with them and they looked at my tooth and some images and we agreed on a root canal with local anesthetic. When I checked out with the front desk after my consult, they said they weren't sure if my insurance would cover the cost and they would let me know. They said that out of pocket, it would cost around $700ish. Later that afternoon, I received a call telling me that my insurance went through. So, for my second appointment, I checked in at the front desk and was asked to pay for my procedure in advance - I was asked to pay around $150, and I assumed that was the remaining cost after my insurance. Weird that I was asked to pay BEFORE the procedure, but whatever. During the procedure, I was injected with quite a bit of anesthetic under my top lip. Then, without speaking to me or asking if I was fully numb, the dentist came in and began drilling on my tooth. After I screamed from the pain, he injected MORE anesthetic in my lip. He continued drilling and I screamed again because I could still feel the drill. At that point I asked if he could numb the backside of my tooth/roof of my mouth. He obliged and without waiting, continued drilling. I STILL felt it. He then decided to inject anesthetic directly into the nerve itself which was excruciatingly painful, but I was willing if it meant I wasn't going to keep feeling him drilling into my tooth. He immediately continued drilling after injecting the nerve, and he was able to work for maybe a minute before he reached a spot where I could feel again. At this point, he was scraping the pulp/nerve out and I could feel EVERY part of the procedure. At no point did the dentist ask if I wanted to continue, or make the decision to halt the procedure because of how severe the pain was. I think my favorite part was when the dentist turned on a torch and heated his metal instrument and stuck it inside the socket, and not only did I get to smell my own flesh burn, but I got to FEEL it too. So, I left what was easily the most traumatic dentist appointment I've ever had. I'm now one month post-op and am absolutely petrified of seeing another dentist and I have lasting nerve damage in my lip from the sheer volume of anesthetic they injected into the same spot (with no increased effect). I can't smile or kiss my boyfriend now without pain and being reminded of this horrific experience. Oh, and I was mailed a bill for $300+ when nobody communicated with me at ALL regarding any costs aside from my first estimate when I was quoted an out-of-pocket cost of ~$700, and my second visit after I was told my insurance would be accepted and I was asked to pay ~$150. Suffice to say, I don't feel responsible for this bill. Not after that experience and definitely not after the lack of communication and clarity around the costs associated with the procedure.
