My mother has been at Autumn Leaves for 13 years and loves it. Although it has changed hands several times during those years, it has continued to improve. The current owners are responsive and many of the staff have been there for years and remember when Mom first moved there. She started there with a period on the skilled nursing floor, went back home for less than a year and then moved permanently into assisted living. Management and staff have developed a culture of working together to provide a safe and nurturing environment. Glenn in facilities has always responded quickly whenever the inevitable issues arise. My elderly mother has lived at Autumn Leaves for well over a decade, starting with the nursing care floor after she had a serious seizure. When she recovered, she moved into a nice-sized single room with a kitchenette and enough space for some of her favorite furniture, including a table to work puzzles. She even had a patio. Years later she moved to another area where she receives more care. The management has changed a few times over the years, but they have always worked with the family to ensure mom had proper care. Many of the residents and staff have become like a second family to her. I was sorry that most people who wrote reviews did not have the good experience that my family has had. No place is going to be the same as being in your own home. But overall, we are grateful to have mom in Autumn Leaves and she is grateful to be there. This place is a death trap. Black mold was discovered in my grandmothers room and throughout the building in her wing. She lost so much weight while she was there, I was scared she was going to die from malnutrition. A TON of horrible negligent things occurred to my grandmother while she was under their care. DO NOT SEND ANYONE YOU CARE ABOUT TO AUTUMN LEAVES!!! This place is deplorable, outdated, smells like urine and is being held together with duct tape and plastic. Woefully under staffed, the administration is a joke special shout out to Matthew in administration and the terrible Rolando Castillo (Hospital Liaison) did not lift a finger to make things right by getting my 70 year old husband out of there. He had knee replacement surgery at city hospital...so-called social social worker Erin Snyder. (Shout out to her incompetence!) The place is literally falling apart, I do not know who in the world they have passed code enforcement. I had to keep asking the nurses for them to check my husband's vital signs, medications, ice for his cooling machine, towels, to take out the trash etc 1) old dated furniture and bowing elevator entrance . 2) Staff is either sleeping or on their phone 3) It took me 5 minutes to wake up the staff on the day we arrived. 4) Wires exposed, plastic where walls should be 5) Husband was there for Physical Therapy ( in 4 days, he only received 30 minutes of PT) 6) Food was horrible. Food shipped in ice chest from a central kitchen. 7) Commons area had several broken chairs that were missing wheels, which were strewn all over ( fell out of one of the chairs) 8) They do not offer PT or OT on the weekends. 9) Staff does not care about anyone there. 10) Air fresheners all over the buildings, to off set the overwhelming stench of urine. 11) Staff was supposed to change the dressing on his knee daily. This did not happen. My husband now has an infection thanks to the staff at Autumn Leaves. STAFF NEEDS MORE TRAINING!!! 12) I suspect they have mold and asbestos. 13) HELP me shut this place down 14) The 1st room we were given did not have a working toilet. 15) Room did not have a trash can until I asked for it. At no time was the room cleaned or vacuumed. Simply unsanitary. I have pictures to prove all of this..not sure how to upload them. I'm making this review in hopes someone who is thinking about this place doesn't take their loved one there. I was simply doing a delivery there for a lady that was unable to get food herself. I was met by RUDE and terrible staff speaking negatively about this women. The women I was delivering for said to leave the food with the receptionist. The "receptionist" was more concerned about why she said to leave it with her vs attempting to help me find this women. She was blocking the door and and visibly annoyed that those were the only directions I was given. About 15 minutes of arguing I found the lady after giving her the name. She said "oh she's not coming down here she's bed ridden. We aren't taking it up." She was unable to leave her bed and there would have been no way for her to get it. The place smelled of urine and unattended seniors. As I was attempting to leave I was unable to leave without a code. Clearly I am lost and have no idea what I'm doing and SEVERAL staff passed me and ignored that I couldn't get out. Most of them didn't even care that I was in there unattended. I was horrified by how they treated me I cannot IMAGINE the way they are treating these people. I'm hoping this post can be helpful to advocate for the poor people living here. read more

I will not give this place a rating of even a 1. Because we buried my 90 year old Father, Billy Preston, who had lived there only since February of this year 2017. I blame Autumn Leaves for my Dads death and please please DO NOT LET YOUR ELDERLY RELATIVE LIVE AT THIS HORRIBLE PLACE! I mean truthfully what I am writing here as my Dad was in and out of the Hospitals, and he came here after he had had a house fire. My Dad ate a "Laundry Pod" which he found on his kitchen cabinet and not even 4 days later my Dad was DEAD!!! Call me at 214-662-6819 if you are even thinking of placing an elderly parent in this place!! I will tell you everything that has happened to my Dad while he lived at this Facility only 4 months! I am trying to deal with my 90 year old Fathers Death, but it is very very hard. And as of July 7th 2017, Autumn Leaves now has been fined as This Facility did not turn in my 90 year old Fathers Death there at their facility to the Department of Aging & Disabilities. So, their ratings just went down. The Facility told the DADS State investigator that they did NOT Know they had to report that! Please, they have been in business for how long??? This is a place we looked at before Dad became so ill that there wasn't an option for him to live alone. I think some reviews may be confused with another Autumn Leaves. Dad stayed in the Rehab center (yes there are no phone but that was great for Dad not to be disturbed and concentrate on his exercise). Besides, I brought him a cell phone. Dad is living in the EC1 section 1. He gets his meals, meds, housekeeping and laundry done. He also gets a small kitchen (no stove but a microwave) and a small patio. All the nurses and other staff members are absolutely wonderful - to him and to me. Dad's had 2 trips to the hospital since he's been here and it is wonderful to know that the Autumn Leaves staff watches over him - they also confer with his dr. He says the food is ok but then he is used to going out 5-6 times a week. And he is a picky eater (and he isn't hungry anymore and I don't think food tastes the same to him). Barbeq's is nearby so we visit there. This is a nice comfortable place for my dad. read more

This review deals more with the home office rather than the facility. My mother received good care while at the Greenville location. HOWEVER, getting your refund from the main office is next to impossible. I have never spoken with so many incompetent people. And when I was give the name of the CEO, Mitch Warren, and his number I tried to call to get the matter reserved. The lady who answered was rude, would not commit to him returning my call and then said she didn't have time for me because she was moving furniture. Really! They have held onto the money for 3 months now. If they are doing this to everyone then they are earning interest on large amounts of money that is not theirs. My father went here after being discharged from the hospital for rehabilitation. We lived out of the state, so the only contact we had was from him and the staff. It is really disgusting the care he received here and by looking at the ratings, we should have known better. He was there for about two weeks and slowly his health deteriorated to the point that when they discharged him, we took him immediately to the emergency room in our home state. He was dehydrated, low on oxygen, and his pneumonia returned. He was hallucinating to the point that he would not stop talking for a good 24 hours. It doesn't take a person with a medical background to realize that he was not in good standing. During the time he was there, it was nearly impossible to get a hold of the nurses. We kept getting their voicemail. They refused to let us talk to the doctors and told us we could only speak to the nurses. It was a horrid situation and I strongly urge those to not consider this place. My father ended up worse than when he entered this POS. This place needs to get investigated, I cannot see how a licensed doctor could have looked at him and thought he didn't need additional medical attention. What a shame! read more

Well my sweet friend Evelyn passed away the 19th. I'm so sad that her final day's were at this horrible facility. They are so understaffed and probably not paid well. They didn't even have a top sheet available so Evelyn had the rough backing bedspread on her tender skin. One of the aides came in to leave her breakfast tray. She put eggs in Evelyn's mouth eventhough she COULD NOT swallow. They didn't even have mouth sponge swabs to keep her lips moistened. I had to find a medical supply store nearby. I still question care given to any resident here that has no one looking out for them. How our elderly how treated here is beyond sad. But obviously it's about how much profit the Corporation makes, right? Shame on anyone who lets this deplorable behavior continue. Evelyn deserved way better than this! read more

My aunt arrived in April 2018. Since I live out of state I see her once a month. Today's visit was especially difficult as she cried because she missed "the earth's air and the sun on her skin". I was shocked to learn that they don't normally take out their residents for fresh air. They have a nice garden area where it would take all of 10 minutes to make a circle. It's so disappointing that they don't focus on the whole wellbeing and just on the medical part of it. Any additional ailment? Get another prescription. They had her in a run down, dirty, stinking room I had to access through the filthy service elevator. We lasted there a couple of hours before I had her transferred. What a disgrace to our elderly, I really can't believe they have not been shut down. The owner needs to be incarcerated. Whatever you do find another facility! read more

A friend of mine went into this facility in November for six months and lost 35lbs., had glasses stolen, and lacked edible food. There was tremendous staff shortages and turnover mostly due to Covid, but the attention by staff was dismal. When the dining room could be opened they continued to feed everyone in styrofoam clamshells in their room most likely because they didn't have the staff to attend to an open dining room. Often times a sandwich was the choice of the day and we never saw either a balanced or nutritious meal there. We had to supplement the meal service, but our friend still continued to lose weight until we moved her six months later to a much better facility. There was also a lack of communication between the staff and the family, and a lack of security. Several times we walked into the property without seeing anyone in sight or checking our credentials. Unfortunately, this property is old and has carpeting in every room. While the size of the rooms is good, the carpeting was dirty and caused callouses from trying to roll a wheelchair across the room. The doors can just barely accommodate a wheelchair and the rehab facility is in another building across the parking lot. There is also not a good community areas for socialization other than around the small dining rooms or front desk area. Another issue is their lack of consistent HVAC temperature since they run their entire property on a chiller system which is either turned onto heat or air conditioning. When the weather changes there is no option to cool the room when the outdoor temperatures were at 85+ degrees. I'm sure the same in the fall when temperatures turn cold. I would never recommend this facility to anyone. read more

My daughter is at Autumm Leaves for a short stay for physical therapy. 2016. This is day 5 and it has been horrible. They say it's a "5 Star" facility. I have some experience with this in that my father is in an assisted living home and between my father and daughter being in and out of hospitals, rehabs, and skilled nursing facilities, This is far from 5 star. Just a few of my complaints. The room and bathroom has not been cleaned yet. The 3rd day I came to visit I was looking for her dirty clothes as she is re learning to use the bathroom ...still wearing depends. They said that they were laundering them even though I said I would do it. 5th day out of clothes and I smelt a horrible smell in an empty closet... There were her soiled clothes..sitting there for 2 days! The next thing...the nurse put her on the toilet and left her. I'm in the room straightening things up and hear her calling my name because she has peed all over the floor. The nurse failed to sit her on the seat correctly and over the seat. Then I said we need to get her soiled clothes off and stand her under the shower. No hot water!!!!. I cleaned her up as best I could and got her in bed. Her bottom sheet was completely off the edges. On top of that when I left at 2 or 3 pm, I ask the Charge Nurse to make sure her bathroom was cleaned and sanitized... That I was leaving to do her laundry and would be back. Got back around 6 pm, bathroom had not been cleaned!!! Physical therapy is good when she gets it...but that is a guessing game as to how much and when. Not all, but most of the staff could care less about these patients. The kitchen areas are filthy...old food sitting around, trash etc... Can't get out soon enough! read more

The management of this facility does not always have nurturing or compassionate relations towards its residents. They talk down to the older folk and are arbitrary and rude at best other times. There are some good people working there, but the management can be very nasty. I had to move my mother out in tears as she was losing all her friends after 3 years calling this place her home. I would not recommend putting your loved one in Autumn Leaves on Emeral Isle. read more

My Father-in law (Otto Mothrenweiser) was a resident at Autumn Leaves in Rockwall for about 2 years. I could not have asked for better care. They were wonderful , all the staff and nurses just loved him. They really made him feel special and were there for the family when we needed then. DO NOT EVER TAKE ANYONE TO THIS FACILITY - THEIR STAFF ABUSES THE PATIENTS: My Mother has Alzheimer's and Dementia, I took her to Autumn Leaves in Dallas, TX for a 3 day Respite Care Stay and have for over a year now, they had a new nurse Belva who was rude and uncaring when I dropped her off, they had her in a room with a patient that was on oxygen, cords going across the floor, as I'm trying to explain to my Mother how to step over the cords the nurse continued to just stand there and watch, did not offer to help or move the cords, I expressed to her my concern of how unsafe this was for my Mother as well as the other patient in the room. I almost didn't leave her because of the way she made me feel but I told myself you always get nervous about leaving her, it will be ok. Well I was wrong! I picked her up, she was walking crooked, cut on her hand and severe bruising on her arms and hands and hip. They have no explanation and nothing noted in her charts that would have caused this to happen to her which to me proves ABUSE even more. I know nothing was noted in her charts because I call administration/head nurse and she told me nothing was noted. I contacted both our hospice social worker and head RN as soon as I got her home to report what had happened. They came out and bandaged up her hand, took their own pictures and did x-ray's. They also know my Mother is never in this type of condition! I have made a complaint with the facility (not that will do anything) and I've contacted the Dept. of Aging and Disabilities (DADS) to report the abuse. The TX Department of Aging and Disabilities (DADS) went out and did an investigation, of course everyone that works there said nothing happened to my Mom and they found nothing noted in her chart that would explain the severe bruising and abuse she received when she was there for only 3 days, well that's because they are going to cover for each other and cover each others tracks especially since I contacted the facility and they knew they were wrong and caused these injuries to my Mother. Since it's my word against the staffs nothing will be done to the facility or the employees and they will continue to be able to ABUSE people and get away with it because they have all the rights, not the patients. read more

Just awful! Checked my mother in after cancer surgery. The place is shabby, dated and in a general state of disrepair. The night we checked in, they forgot to order her pain meds (she was four days post surgery). She was in pain for 9 hours while waiting for her medication. The food was practically inedible. Not even sure it meets the minimum requirements. Whatever my mother ordered for her meals, she ALWAYS got something different. She lost 15 pounds in two weeks. The water was turned off to her room for a couple of days. It was freezing in her room, and we were told that they could not turn on the heat. No phones in the room. The staff was nice enough, but overworked because of severe understaffing problem. Several of them complained to us about it There was a general atmosphere of gloom. I actually felt sorry for most of the staff because they were pulled in so many directions at once and seemed so unhappy to be there. My mom would have to wait a very long time for assistance because there were never enough nurses on duty, and often had to wet herself because she needed help getting to the toilet. Physical therapy was a joke. Cookie-cutter, one-size-fits-all approach, and outdated equipment. One therapist in particular was very rude to both my mother and me, berating my mother because she could not do more. If you love your family member, DO NOT SEND THEM TO THIS PLACE! read more
