I spoke to the Administrator about when I saw at least six staff members parking in front rather than around the back so there was no room for visitors, He was almost rude and pretty much stated it wasn't his problem. Food was less than stellar. They were supposed to take orders so you didn't have to eat the meal of the day, sometimes that happened and sometimes not. Way over salted but when my husband mentioned that he was told they didn't salt anything. Might want change who the food is sourced from. Beds were so uncomfortable according to my husband. Activities were not something most would want to participate in, It was better than some but there is still room for improvement and if the higher staff members don't see a problem then nothing changes

Upon arrival, she was served a meal consisting of a hoagie roll with two very thin slices of turkey, a piece of Kraft cheese cut in half, cold fries, and fruit that was unrecognizable. This raised immediate concerns about basic care and nutrition. The goal of short term rehabilitation is to help patients regain strength, improve their health, and return home to their normal activities. Unfortunately, our experience was the opposite. My mother became weaker with each passing day. Given her condition, it should have been immediately clear that she required assistance with basic tasks. However, it took until the following day for staff to recognize that she was unable to feed herself. When assistance was requested for toileting, a CNA stated in front of me that my mother could use her diaper and would be cleaned later. This appeared to be standard practice, as she was frequently left in bed until the afternoon and not regularly assisted to the restroom. As a result, her hygiene declined significantly. A physical therapist later expressed concern about her condition, noting that at 3:30 PM she was still in bed and had not been properly cared for. Communication throughout her stay was extremely poor. There were constant staff changes, and no clear continuity of care. Nursing and physical therapy appeared to operate in separate systems with little coordination. Despite multiple requests, we were never able to speak with a physician at any point during her stay. During this time, my mother developed pneumonia. We have since moved her to another facility. Within the first 30 minutes of her arrival at Orchard Park, she received more care and attention than she did during the entire two weeks at Avamere. I fear for patients who do not have an advocate, as they may be left with little to no care. Overall, the level of care, communication, and coordination fell well below expectations.

Most of the staff were very accommodating. However, because the doctor and nurse practitioner were unwilling to adjust his medication when he needed it, my husband has been hospitalized for a week. He developed huge water blisters and sores on his legs as a result of their negligence. At one time, he was left on the toilet for so long and the nurse did not respond to the call light, so he had to get up himself. There was constant urine on the bathroom floor because the equipment was old and inadequate. Many times there was noone in the halls or at the nurses station through the day. Wait times for a response to a call button many times exceeded an hour. It is only pure luck that he did not fall again undertheir poor care. The facility is old and outdated. The phone didn't work properly due to being old and dropped so many times. I had to bring in many things from home because they provided a bed and old nightstand period. We chose this place because we were told it was a 5 star facility with a doctor and nurse practitioner 7days a week. There was no doctor or nurse practitioner on site on the weekends. Be very careful if you place a loved one here. The staff is friendly and seem caring, but when you need real medical help, the doctor and nurse practitioner make the decisions. In my husband's case, their negligence caused him a week in the hospital.

My daughter was a resident due to a brain injury, and the care she received was absolutely unacceptable. From the start, I was routinely gaslighted by staff when I voiced concerns about her health. She was hospitalized multiple times for completely preventable issues--like dehydration--because they weren't even giving her water until after her first hospital visit. She developed blisters on both feet, I found that her bed didn't even have a sheet--nothing to protect her from the rubber mattress. The area around her feeding tube was infected. Over time, I noticed swelling and hardness in her abdomen. When I brought this up to the nurse, I was dismissed and told it was nothing. I insisted on a hospital transfer, and thankfully I did. At the hospital, she was diagnosed with a distended colon and had dangerous amounts of air and gas in her abdomen from her feeding tube. The doctor said she was close to needing a colostomy bag. The rooms were filthy--floors were never clean, and mattresses were only wiped down when I did it myself. I had requested that she be placed in a wheelchair for at least two hours a day as part of her rehab plan. That only happened when I was present. Otherwise, she was just left lying in bed with no meaningful rehabilitation. She was rarely bathed or cleaned, and every time I visited (twice a week), I had to clean her myself. She often had congestion, despite the facility having an on-site respiratory team. It was clear that very little care or attention was being given. The staff was not only dismissive but repeatedly made me feel like I didn't know what I was talking about when I raised legitimate concerns. Even the doctor was essentially non-communicative until I was ready to remove my daughter from the facility. After the final hospital visit, I transferred her to another facility where she finally began receiving the care she needed. Park Rose Nursing Home is not a place for healing or recovery. If your loved one is vulnerable or unable to speak for themselves, I urge you to look elsewhere. This facility needs a complete overhaul--from staffing and training to hygiene and basic human decency.

No accountability or promise to do better, just deflect and defensiveness. Please take your family anywhere else. Now that my mother is out of this places, they can't find her stuff. Not once were we asked to pick anything up, they just don't have it. It's seriously disappointing. While waiting, staff is making jokes about ignoring the call light for room 208, while it beeped and beeped for well over 20 minutes. Like seriously do better.

The food was almost inedible most days, so he requested PB&J sandwiches for lunch. He RARELY got them unless I brought them when I visited. The staff turnover is high, and the facility is sub-par to put it mildly. When you inquire about care, it takes most times WEEKS for an answer if you get one at all. I requested podiatrist care for my nephew's feet because he was a diabetic and was told he was "at the top of the list." In the 13 months he was there, he NEVER received foot care as promised. I must have left messages just for this one care item at least 10 times!!! I rarely would get a call back even after leaving numerous voice messages. When talking with the current administrator Monique about the lack of response, she said she wasn't aware of it. I had to tell her that I finally got to talk with her after leaving urgent voice messages to her 3 days in a row, and finally got to talk with her when I called the front desk on the 4th day and was again told she was in a meeting and to leave a message. I told her that she only took my call because I threatened to call DSHS. My nephew had many health issues and has since passed away. At the hospital, the nurse said that his Foley catheter that they had to take out due to a severe UTI was inserted improperly. It was unfortunate that my nephew had to spend the last year of his life at this facility. I hope the owners and administrators are taken to task for their facilities' shortcomings. It seems that a healthcare facility should always have decaff for patients.
