I would not leave my Dog here. If you loved one ends up here and can't speak for themselves they will receive the barest minimum of care. One diaper change per shift. If they can't feed themselves expect a rapid weight loss. Mentally abusive staff calling patients lazy. My Aunt was there to receive sub Acute rehab so that she could come home. They didn't do anything with her never even sat her up in a chair. In the week she was there they did not let me speak to her. I brought her comfort items from home they never opened the bag. They put her in a room with a woman who had pneumonia I requested that my Aunt wear a mask and they never followed through with any family requests. read more

my aunt has been in there for a long while. she thankfully is being discharged tomorrow. unfortunately during her stay the staff has treated her beyond poorly. then to top off that instead of the social worker doing her job and help her (such as setting up the ambulance ride home) she tells her things such as "I told you to find a ride, I'll be here at 4 and you better have one" normally I wouldn't want to bel someone that wanted to help people would be so ugly but my mother was on the phone for that particular conversation. don't let the staged photos fool you! if there was a rating below one star this place would get it! reading the reviews makes me so damn angry! 1. my grandmother got covid, pneumonia and a uti, she was in good health when she went in, was only there for rehab, yet she left this hell hole sick , went to the hospital and tested positive! (the dr's said if she was not vaccinated for both covid/flu and pneumonia, it would've been a diffrent outcome) 2. they went a whole day without feeding her, left the breakfast tray in her room all damn day, she never ate it! they never gave her lunch or dinner! 3. the mats they had on the floor next to her bed had old food and ants were underneath them! this is just a few things of what was done, i have so much more to say about the lying ignorant, unprofessional and disrespectful staff, i will be writing letters to all the people and places that will investigate this cesspool and copying my lawyer!! thank god she is no longer in this place!! read more

Avantara is understaffed. The nurses and doctors for the most are unqualified to work in a regular hospital. The rehabilitation is inadequate and is dependent on insurance payments. Pre-COVID was bad enough but now with no in person visits the place has hit rock bottom. My Love one has deteriorated quickly suffering from dehydration bad hygiene and bad diet. I pray GOD will intervene and change the hearts of the people who run these places so that focus is on humans and not money! I would have given no stars if possible. read more

Avantara of Evergreen Park is horrible. The place is extremely understaffed causing patients to lay in their own waste for hours. The equipment is old and doesn't work half the time and water must cost a lot since it takes an act of god to get any. Also they're always turning off call lights, but almost never assisting the patient with their issue. The State is here all time investigate this place due to complaints submitted by patients or their relatives. I most note that the Therapy Department and their staff are great and truly care about the patients. They're not owned by Avantara. They're a 3rd party vendor who is contacted by Avantara. They have to fight every day with the facility to get their patients up and ready for therapy. The facility thinks its more important for the nursing staff to pass out food trays and later to pick them up instead of getting the patients ready for therapy in the morning. read more

This place should be called experience your very own nightmare at Avantara!! Let me begin by saying that the CNA's on 300, which is called a private, recovery rehabilitation suite are excellent. They round regularly, attend to your needs and if your food is undesirable, they actually tell the chef and he personally ensures that you receive an acceptable meal. Now, I received a fake physical therapy assessment, that only consisted of asking me a few questions. No measurements were taken, the lights were dimmed in the room and the supposed assessor only peeked briefly from a distance at my wound. How can you ascertain where someone needs to advance to, if you don't know where they began? Anyhow, lets move forward, many of the nurses were kind and well versed, but a few were not versed on the indications of certain meds and a couple of times I had to tell them that they weren't administering the correct med for my specific ailment. How many times could this be happening to others that don't have a clue? The social worker Peter and the Administrator Sue are two of the most condescending, false sense of caring people that I've ever met. They both should be ashamed of themselves. I don't know how they sleep at night.The aesthetics and fruit infused water do not wow me! Take care of your patients, show genuine concern, at least act like you care. You're getting paid six figures and you've lost basic compassion?? Retire! I asked repeatedly when I would discharged and was told by several different people that they didn't know. I had to call Blue Cross and Blue Shield myself to get any information and when changes occurred, I was never updated and was made to feel as if I was troubling them to ask a question. They were pissed and it showed. At the last minute, the day of, I was told I was going to be discharged that day. I had nowhere to go and did not have things situated to have someone care for me at home. I ended up going home to a unsafe environment with several stairs and could potentially fall down the stairs and break my neck. Actually, I think that they'd probably be glad if I did. Please, when you walk through the door, do not be impressed by the appearance; it is not what it appears to be. It is a horror story and a nightmare. I feel so sorry for the patients there that are demented or simply do not know how to advocate for themselves. Oh, they had a booklet and asked me to sign every page and they'd explain later. Nope, are you nuts! Although I asked many questions about my discharge in advance, it made no difference. I was basically told that if I did not leave the facility I would have to pay for my continued stay although there was an upcoming review. If my questions regarding discharge had been answered previously, I could have made other provisions. No provision was made for me at all they listened to me condescendingly and then simply walked away leaving me there with tears rolling down my face. Do not send your loved ones there!!! The rehab that I received there was top of the line, but it is not worth going there for the treatment and the mental anguish that I'm going through now after having been there. I'm traumatized and awe stricken. You and your love one's don't have to experience this. DO NOT GO THERE. Bake your own cookies and make your own fruit infused water and pass up this true to life EPIC horror story!! Shut this place down until you can find a competent, caring Administrator, Social Workers, DON and ADON! read more

My grandmother was placed in this facility in July 2019. The first time I visited her, she was discombobulated, and her room reeked of urine. After complaining, Grandma was fine the next time I was her. Her room didn't smell of urine and she was herself. When Covid happened, the facility implemented the FaceTime feature. The last time I was able to FaceTime my grandmother was Mother's Day of 2020 and that was after I was on hold for two hours and cried to the staff because I was never separated from my grandmother. After that, they stopped answering the phones so to FaceTime her was impossible. September 2020, my grandmother was released to hospice. When we got her, she wasn't clean, she had bedsores so deep that you could create a pond, and her hair was matted to her head. Anyone who knew my grandmother knew that she was a clean woman who took pride in her appearance. My grandmother passed away three weeks after we got her. She was severely neglected by the nursing home and had she not went there, she may be alive to celebrate her 83rd birthday which is coming up. I've truly lost hope on nursing homes and I hope none of you reading this put your family in this facility. read more

They should have minus stars for this place. My brother went into Avantara in Evergreen Park, IL to recover after a surgery on March 2, 2020. He was a quadriplegic and required constant care. My son and I kept a 24 hour vigil on him until we were forced out on March 12 ,2020 due the pandemic lockdown. We had seen first hand how the staff fails to respond to patients in a timely manner. My brother could not feed or get drinks for himself. The special bed he was on would cause him to overheat and we had to open the door to his room and open a window to cool his room down. I talked to CINDY on March 13, 2020 and pleaded the case for us to stay with him for the one more week he had to be there. CINDY, in a very hubris tone, assured me the would do everything necessary without our help. I called my brother on March 15th, after 17 minutes of being put on hold and transferred 5 times, I got through to someone to tell them to have Scott call back. He called back and said he is doing alright and that this quarantine is dumb. I asked if he wanted to leave and he said I only have one more week here. On March 17th I got a call he was in the emergency room. When I got there he was unconscious. They said he had Aspiration pneumonia, probably from food or drink getting into his lungs. He never recovered and died on the 23rd one week short of his 55th birthday. I get angry thinking how much he suffered the 5 days we were not there. I hope CINDY and the other lazy asses there suffer at some point in their lives as much as they made my brother suffer. Also I went to pick up his belongings and they could not find them. I was waiting in the lobby when I saw a bag on the floor next to a garbage can. I noticed his electric toothbrush poking out of the bag. I could not believe my eyes. I said "I found his stuff" and walked out because I felt I might blow up and break something. A few days later I got a call asking how my brother was and how his stay at Avantara was. I gave that lady an ear full. and told her he was dead and that you killed him and hung up. read more

The VILLIA, wow it needs to change its name to YOUR WORST DAYS OF HELL TILL GOD COMEShalf of this place is pretty, you get around the corner , all the way down to five hundred area and it's HELL!!!!!! The care on the five hundred unit is a disgrace!!!!!!!! In the summer the water was cold everyday, until the weather changed in the fall. It's horrible. A CNA LET MY DAD FALL ON THE FLOOR, and she left the rails down. Several of times, I had to go in and wash my dad face and hands, four times feces was on his hands while he ate dinner. There are about five good aids, there, and about five good nurses. You can't put that staff in a hospital to work , they won't last , because they will have to really do work. No staff anywhere to be found. One nurse to 40 plus patients. Filthy, dark rooms. Food not fit for a dog. There is nothing good to say about this place. A nightmare, I had my family member removed after 2 days. if you want your family member to lay in bed and rot, this is the place for you. my rehab therapy stay (11/08/-11/19/18) (i was in the 300 section rm 302 a very clean & beautiful mini suite) from day one i had excellent care ! i was there for a total knee replacement & i needed a lot of care with the pain level i was feeling ! the food was good & i had great choices ! all of the employees who cared for me was amazing (from my therapists, nurses, management, cna's, housekeeping, activity, front desk, dietary, office personnel, dr. & any employee who care for me during my stay i would like to thank you all ! your kindness while caring for me will never be forgotten ! i was a first time patient ! yes, i do recommend the villa at evergreen for your rehab treatment ! god & mrs. e clark love you all ! I didn't read the reviews before helping my dad find placement for rehab. It would have definitely not been the choice. I actually toured the place and made my decision. I was able to see where he was going to be before he admitted. Not sure if were are reviewing the same place because therapy has been terrific and the nursing team has been nothing but helpful to my dad and I, answering all questions I had . Secretary set up appointments and transportation. Concierge got us the goodies for his fridge, he's able to choose his meals daily, The only issue we've encountered was the TV Channels, 4 have no sound. Other than that everything has been fine. The facilty is beautiful and convienent for my family and I. Our church family is amazed as well. Were definitely happy with our decision. Thanks Villa! T Gilliand read more

My Grandmother was in The Villa for about 6 weeks. She was rehabbing her hip. I can't complain about the staff, because family took turns spending the night in her room. If you simply "like" your family member or friend, DO NOT send them to Avantara of Evergreen Park. I beg of you. Please do not do it. They are unresponsive, unhelpful, and neglectful. Do not trust anything that Jarrell, Denise, Jennifer, Rainey, or Dr. Hussein say. The staff reflects leadership. Many of the "policies" in place are justified by the ongoing pandemic. These policies are nonsensical and not in place at other facilities. This place highlights state of the art equipment, yet patients are not allowed to leave their rooms (even if they are fully vaccinated). It is useless to promote state of the art equipment if patients are not allowed to utilize them. Patients are left in their room in their soiled undergarments, without a shower/wash up, and left to stay in bed all day. A patient cannot improve and get proper rehabilitation services in bed. Speaking of what was promoted as intensive rehab services, one 30-minute physical therapy, one 30-minute occupational therapy, and one 30-minute speech therapy session daily is not intensive! Do better, Avantara! This is even more alarming as guests are not permitted to enter, in the name of COVID-19. Not the case at better facilities. It is well documented that patients experience a better and faster recovery with the support of loved ones. They are unable to achieve this degree of success and improvement at Avantara. Again, I challenge you, Avantar, to do better. For those considering this place for your loved one, please, please, please, reconsider. They will get better care and rehabilitation services at home. read more

This nursing and rehab center is a joke. Mgirlfriend been there for almost 2 months now and i can tell you they need to get their crap together. She says the wound care team is great at what they do and they always make sure she is taken care of with respect. Most of the cna's are nice and caring as well as most of the nurses. Some of them need to get their act together. Her first night there her bandage was coming off so she called the nurse to help her and her response was thats what we have wound care for. Wound care is not there are night they work during the day. They need to have a talk with their staff about that. The kitchen is a total mess. They hardly get her food right. The dietician gives her a menu to mark what she can eat out of whats on there and they barely follow it. She has spent over $420 on her own food between foodstamps and doordash just so she has something to eat. She shouldn't have to spend her own money. On top of that the kitchen is rude as hell. She gets they are busy but they need to at least listen to what shes asking for instead of rushing her off the phone or hanging up on her. That is uncalled for. If you don't have patience for people maybe you shouldn't work in the healthcare field. They really need to hire cooks that know what they are doing. She used to work as a cna and as a cook in a nursing home so she knows how it is but she always tried to send out secent food that people could eat and if they didn't want it she would get them something right away and not make them wait. Seriously this place needs to have a meeting on human decency. read more

This is the First review of anything that I've ever took the time to write...this place will fool you with exterior looks but they have mastered the skill of deception!! Please make sure you document your love ones daily by video, pictures and ensure you sign in and out! Trust me unfortunately you might need the proof once you file a claim about how they treated your love one even tried to cover up injuries!! My Dear Dad health plunged quickly in their that ultimately ended in his death!! I took him out but by the time I took him to the hospital sooooo much damage had been done to him with terrible bedsores he was took we to fight the infections. Terrible, heartless place that should be shut down! They are skilled in covering up their mistakes so watched them and document every visit with video and Audio is my advice!! She will not work with rectifying the situation but instead ignores phone calls and emails. Has not shown where these funds were applied. From what I understand the rooms are clean, as the facility is new, and my mother thinks the food is good. So the above review is not accurate in those ways, per my family's experience. But, I must agree one of the most appalling claims, they are severely understaffed. It takes several calls and an average of 20 mins before I can get a nurse to answer my call at the nurses' station to transfer the call to my mother's room. It is an extremely frustrating experience. After a near death diagnosis with sepsis and dehydration, we placed her in another facility where she could get proper care. Patient care is definitely not their top priority. read more

This is my first review I've ever written, I am hoping to keep anyone from bringing their loved ones to the Villa. My mother was so worried about having a nice private room for after surgery therapy, that she didn't listen to the negative feedback regarding the incompetent care. I give them one half star for having decent private rooms. The care was so terrible, I became scared for my moms life at one point. After my moms surgery, she did not receive one pill of blood thinner for over 4 days. This is even after we begged and pleaded with them to give the blood thinner, it is crucial after surgery. The surgeon put a call in to set them straight and so he didn't get sued, and even still they did not give her the blood thinner. This can cause strokes and blood clots. She developed a blood pooling on her leg they had to then Doppler for blood clots. During her days there they also did not give her-her regular daily meds she takes on a normal basis. She spent days crying for her medicine. The food was so inedible you could not cut through it with a knife. I've had bad hospital food, I know it tastes bad, but their food was unchewable and foul, and tasted compromised. The food was the least of my worries as we could bring her food in from the store. After a week of crying to get out of there, her wounds and blood pooling was only checked once by the onsite doctor. The nurses did not help to put her ice machine on her leg, left it in the box that was prescribed by the surgeon. No matter how many times you yell or complain, the workers all did not care. Total shit hole, hell hole. How any of the nurses ever received a certification scares me!! My grandmother stayed in a nursing home before. It smelled like urine and it has shared rooms, but the workers were friendly and did a nice job with her care. I can't believe the Villa is open for business, it is something so horrible I've never dealt with anything like it before. Please please do not bring your family there for care, it needs to be shut down. read more

This is a first class facility and is very clean and well attended by the staff,,,the staff are all very nice and attend to the patients very courteously. It was very quiet in my room with the door closed. The food was ok but not always my favorite but acceptable. The whole building looks clean and modern and looks more like a hotel than a rehab or nursing center. All my visitors were impressed with this place and compared it favorably above the previous place I had stayed at after my back surgery in March of this year. read more

It's sad that a few bad apples has completely soured my opinion of this facility... My mom was there for a couple of weeks for rehab... The Physical Therapist staff was very patient with my Mom but several of the C.N.A's need to find another profession... They were rude, did not accommodate my mom's request, unplugged her call button at night, threw her soiled clothes on top of her clean clothes, had another's patient's clothes mixed in with hers, had bugs in her closet when I removed her clothe and last but certainly not least, they removed a Sh*tty diaper and left it and the wash cloth used in the chair where family sits...We were a family that was there at different times daily and all of this still occurred..This place is nothing like what the brochure claims...Thank God my mom was able to tell us what was happening when we were not there at night... I was in the villa in April 2014 while there I developed an infection that required surgery. I am still at home recovering. While at the villa in April my incision was never cleaned. A new bandage was replaced every 3 days. I was not given antibiotics to ward off infection during my stay. The nurses were sometimes rude and non responsive. My bedding was not changed daily. My medications were not given in a timely manner and I experienced painful moments. During PT and OT I was in a gym like facility with at least 15 other patients and did not receive any one on one service. The food was awful very unappetizing I lost weight while at the villa. read more

I've read all the reviews with my mother but still decided to choose this place due to the surgeon recommending rehab and particularly The Villa.I told him about the reviews and his response was none of his patients has had any issues. She's at the Villa...I must say so far so good. She admitted on the 8th of April, and was over whelmed with welcoming from staff. I admit my antennas stay up due to reviewes I've read. The staff so far seem very attentive. A young lady actually went out of her way to get my mom fresh fruit from the store the same day she admitted, due to her having a taste for fruit but the fruit mom wanted wasn't on the menu. I requested to stay the first night due to my own anxiety and I was denied due to facility "policy". Thankfully my mom said staff was actually attentive that night. She did mention that staff are quite noisy during the night with their own conversations but that also meant that staff was around so she didn't bother to complain. Medicine did take longer then expected. We found out the pharmacy is not inside the facility, which I believe is why it was delayed. Pharmacy definitely needs to be looked into. Thank God she wasn't in excruciating pain from the ride to facility.. The next morning mom finally got her meds and she wasn't that thrilled with the breakfast but that was to be expected. She was offered an alternate but declined it. Therapy evaluated her and they seem to be very caring and excited about getting her up and about. I didn't see any big shots besides the lady that got the fruit for her she popped in a few times to check on her. Today is day 3 and I pray she continues to have a positive rehab experience. She did say again, the food is not so good but we can live with that. We dont expect royalty but we do expect to be treated fairly and for staff to remain humble and attentive. I will add more if things change and when she discharges sucessfully I'll update the Star rating I gave. Right now it's a 4 due to the food and pharmacy. read more
