I was in the hospital every day feeding my uncle every day making sure he was thriving and getting back to health. Once they kicked me out they placed him here in November. He fell out of bed, his health declined, he slowly starved, got pumped with pain meds, lost weight and was dead on February 10, 2026. My uncle was isolated, I was told we could not visit because APS said so. I was threatened that If I or anyone comes to visit we will be arrested. I don't even live in the state. He was isolated until the day he died. He was given a court-appointed guardian through APS (adult protective services. He visited one time for about 15 minutes on December 26, 2026. They did nothing in his best interest. the judge did not allow anyone to visit him. My uncle died alone. they removed everyone who loved him due to a vindictive, distant, greedy family member, who was allowed to further isolate him from his loved ones. To top it off, after we were told that the state would take over, we were never notified of my uncle's death. I just happened to call the guardian of the court on February 23, 2026 to find out why the distant relative was going in and out of my uncles house. I was informed that my uncle had been dead for almost 2 weeks. He died at Palos hospital. No one even had the heart to notify his family. There is a different kind of evil in this world. I don't know who's worse Palos Hospital or this place. It should be a special kind of hell for people who treat the elderly like this.

First of all, the location could not be better with ample parking spaces and brand new therapy room. But more importantly the staff is amazing. I have worked with both Melissa and Kasia who are extremely very knowledgeable and friendly . They had me out of the bulky boot before target date.     I was in back pain for over three months. The pain was so bad that I couldn't walk even a few…read moresteps, sit for more than 15-minutes or sleep more than 2 to 3 hours a night. I even had a difficult time dress myself or even putting on my shoes. Yes, all simple tasks were nearly impossible! I went to urgent care, the chiropractor, a spine specialist, and even tried PT at Shirley Ryan Ability Lab with little to no relief. I heard about aqua therapy and ask my doctor to investigate it with me. I found Rush PT at Morraine Valley. Religiously I went 3-days a week. It was AMAZING. Things I could NOT do on land I could accomplish in the water. Eventually, we moved our therapy program to land with great success. Sarah my therapist recommended dried needling to help relieve my pain. I was reluctant at first but she assured me it was safe and effective. SURPRISE.... I was able to get off all my pain meds. (I was on 3 different types 3 times a day). It was like the spasms and pain stopped misbehaving. My pain was gone! My movement restored! Today, I am following the exercise program that Sarah has laid out for me and try to swim at least 2-days a week. I am back to my old self, med-free, pain-free, and happy to be moving again! Also, a big thanks to Sara in billing who provided up to date detailed bills and working with my crazy schedule. Thank you to Sarah, Melissa and Sara for taking such good care of me!

    #19/2024 After having a major leg bypass and a two toe…read moreamputation at the University of Chicago and Hyde Park Illinois main campus I was not equipped to head home just yet because my leg was not cooperating for the stairs that we're going to be needed. I chose Elevate Care Center and Palos heights because it wasn't too far from home and the reviews on Google were good the one review on Yelp I just kind of laughed at. Elevate care centers are basically a group of rehab and nursing homes managed by a corporation who are buying older facilities and remodeling them and doing a whole lot more they're not brand new built from the ground places. This location in Palos heights Illinois in the Chicago suburbs is basically once known as The park Villa nursing home Center. They added on a few years back to the rehab part you can see the difference as you come through between the old building and the new building. Is it still a nursing home for the most part with long-term care and long-term rehab but they have branched out into short-term rehab. The rehab rooms are quite nice they have very good air conditioning units I can say there were some very very hot days and did a killer job cooling the room.. they also have refrigerators in the room. The only thing they don't have is a microwave but there is a microwave you can use by the dining area community dining areas they call it. Through the most part the staff is quite cordial. Although they're a little limited sometimes on the night shift it is possible you will push the button to get a CNA and have to wait a little while depending on what area of the building they're in. It happens in the post covid world. The nurses are fairly thorough they get your medicines dispensed in a timely manner each throughout the day and night if need. You're assigned to Doctor who basically just oversees your pills and looks over things and reports you don't really consider him a main doctor but he just kind of the doctor that's there at the facility. The TVs are really nice only problem is they're not high-def here in 2024 LOL. They're a little staticky but it is what it is. There's a decent choice of channels for the most part though. There are no showers in the rooms except for a couple of the older units you have basically community showers you're allowed to showers a week. But the bathrooms are very nice good sink good water pressure. The cleaning crew comes in once a day or every other day and gets the floor and supplies garbage cans ETC. if you happen to need your sheets change she just asked one of the nurses during the day and they will. The wound care team is very good also they basically changed my dressings as needed about every 3 days which was fine they did a very good job two days during the week except the weekend. Clean the wounds and the stitches and everything so I had no trouble as far as I went. The food's good it's not great all the time you get a menu and it pretty much tells you what you're going to get. The nice thing is though there's food delivery nearby you can call out and have it delivered I did a few times Pizza, Jimmy John's, and dad brought me a Italian beef one day. My sister went to the store for me I gave her a list so I was able to have Doritos and yogurt cottage cheese diet Dr pepper and stuff like that if I needed not having to waste a bunch of money on the vending machines. As I said it's a decent place it's not perfect they have their hiccups here and there but for the most part they do what they can and as for short-term rehab I was only in there a little over a week I didn't have many problems. They keep up on your appointments as long as you let them know when they are they weren't able to arrange a medical car for me on the day I had to go back to Hyde Park Illinois but the head of nursing Don director of nursing did give me an Uber, and because I had some trouble here and there they had somebody escort me and go with me on that day too so that was very professional. But I will definitely say is the PT and the OT team are very good and thorough they will get you bring you and get you moving which is a good thing and that's what I needed was leg strengthening and they worked with me so when I went home I was able to climb the stairs. I have eight stairs leading upstairs and I have seven stairs leading downstairs from the hallway. So all in all it's been pretty good and because of the fact that my left leg is not in the greatest of shape I may have to repeat the process again I would probably pick here yet again. If I needed rehab. So basically it's really three and a half stars but when I look at everything even with the little hiccups here and there I bumped it up to four stars because I didn't like seeing the one star and the reason as to why this guy was complaining

My great aunt who never complains, immedicately asked for us to get her out of there after tranistioning to the long term care side. She was there for 4 very long months. The administrator never responded to phone calls and admitted that the best way to engage with him was on-site. The care team sent her to Northwestern Palos Hospital without contacting her family. Worst care experience witnessed. Do not send your loved ones here.

    I am extremely disappointed with my experience at Symetria. Before I ever started treatment, I was…read moretold over the phone that with my insurance my costs should be around $60 per month. Instead, I later found out they were billing my insurance approximately $750 PER DAY (and yes, they will force you to go in everyday for your meds in the beginning) for visits that consisted of little more than coming in and receiving my Suboxone at a window. According to my Blue Cross Blue Shield account, the first four days alone were already nearly $3,000 in charges, and there are still additional days marked as "in process," and Lord only knows what those days are going to be costing. My guess is 750 dollars again. Sickening. For context, with my prescription plan, my Suboxone would typically be a $10 copay for a 30-day supply if prescribed through a regular doctor and filled at a pharmacy. Instead, I was placed into a system where I was required to come in repeatedly just to pick up medication at a window, while each visit was billed at hundreds of dollars. I was initially told I would only need to come in for the first seven days. When those seven days were coming to an end they informed me I would have to continue coming in three days a week for another three weeks just to receive my medication. Each of these visits was being billed at around $750, despite the fact that I was simply showing up to a window to get medication that could otherwise be managed through a standard pharmacy prescription. Talk about scam! During my entire time there, I saw a doctor over Zoom ONE TIME and met with a drug and alcohol counselor ONE TIME for intake and a treatment plan. Blood was drawn, and I was given lab results that were never adequately reviewed or explained to me. Additionally, most of the blood work results I received were marked "test not performed -- specimen past stability," meaning they were not actually completed or were unusable, and I was never given a clear explanation as to why or what that meant for my care. What I find most disturbing is the amount being charged to insurance compared to the actual care provided. The vast majority of my visits involved no physician interaction, no counseling, and no meaningful treatment beyond walking up to a window for medication. Yet, hundreds and hundreds of dollars per visit were being billed. It felt less like patient-centered care and more like a system designed to maximize billing. Patients seeking help for addiction are vulnerable and deserve transparent pricing and ethical treatment. My experience left me feeling misled about costs and pressured into unnecessary in-person visits simply to access medication. I cannot recommend Symetria based on my experience.

Positives * The facility was clean and had a pleasant atmosphere. * Many staff members, including Roy, Artis, Kayla, Yasmin, Toni, Courtney, and Latoya, showed kindness and attentiveness in their care despite being very busy. * My mother made progress with physical and occupational therapy. Negatives 1. Overnight Care Issues: The overnight care was insufficient. My mother, who has Parkinson's disease and dementia, was left unattended for hours on multiple occasions. I arrived one morning to find her soaking wet and hanging off the bed. On another night, no one checked on her from 10:30 PM until I rang the call bell at 4:00 AM. 2. Policy Inconsistencies: Despite being told that overnight stays were not allowed, I was informed by staff that other families had stayed overnight either on in room sofa or by using an air mattresses. After purchasing a mattress and staying with my mother for seven consecutive nights, I was told this was against guidelines. This explanation felt inconsistent, as I had been doing so for seven days, in addition to other rooms having sofas in the same area where I placed my mattress (contradicting the reason that where the air mattress was placed was against guidelines). 3. Communication Failures: * A sign I created to help staff understand my mother's condition was removed, with staff claiming it was a HIPAA violation. However, I could not find this restriction in any patient rights documentation or the HIPAA guidelines. (they can't share the info but we can share the info) * The frequent turnover of agency staff meant that new caregivers were often unfamiliar with my mother's condition, causing frustration for both her and our family. 4. Incident Transparency: My mother experienced two falls during her stay. Despite me being on-site for both incidents (not in room with her), I was never shown or asked to review the incident reports. When I requested them, I was told fall information could not be shared, and I needed to resubmit my request (to receive the basic record information) with specific verbiage that wasn't initially communicated to me. 5. Unsafe Practices: In one fall incident, my mother was left unattended (Parkinson's and Demetia) in the bathroom without gripping socks or shoes, leading to a fall. She was found on the floor after knocking on the door. 6. Lack of Response to Feedback: After discharging my mother early due to these ongoing issues, I sent detailed written feedback to the facility's leadership, including specific information requests. I received one acknowledgment from someone outside the scope of responsibility, who stated that the email would be escalated to the appropriate person. However, I have not received any direct response or resolution from the facility's director (who was included on the initial email) or leadership. This lack of accountability further underscores my concerns about the quality of care and management at this facility. While some staff members showed genuine care and kindness, the lack of consistent care, transparency, and communication overshadowed the positives. I ultimately decided to discharge my mother early due to these ongoing issues. I cannot recommend Avantara Chicago Ridge based on our experience. Families seeking care for their loved ones deserve better.
