This place needs to be shut down I'm here right now...there is literally maybe 2 staff nurses…read morethat actually do there job..most of the nurses will clean up pee from one room and then come to my room and try to touch my picc line that goes to my heart, any nurse with common sense should always change there gloves and flush my lines properly and change my dressing every 7 days but they didn't do Any of those things. They administered the wrong doses and iv meds for the first week..how do you give someone the wrong medicine, anyways.I'm not a doctor but I knew more about certain things then the staff..I'm not sure where the found there staff but I doubt any of them have any kind of certification in anything medical related..I'm here now for another week..chances of survival not As high as they should be..lol the have Flys, knats, and bedbugs, spoiled food ,I've had better food in jail..and Nicole, wow the people that run this place are cold hearted ,uneducated and are only here to get a pay check..the few good reviews they do have are not people that were recently here there fake reviews, I have been here 5 weeks and those people have not even been here for treatment..that's sad if u have to put fake good reviews just to get your 3 stars when u just had 1 the other day....this place needs to be shut down ,sterilized and hire staff members that actually have some kind of knowledge in the medical field...and 70 percent of the floor staff don't even speak english...my roommate next to me hasn't showed in 5 weeks that I been here ,you would think they would make people shower...all statements I have made on this so called facility are all true, if u don't believe me come see for your self, but I recommend bringing a can of raid and bed bug spray and were a mask cause it smells like bodily fluids down every hallway...

The staff--from Abby to Grace and the entire team in every cottage--deserve recognition for their attentiveness, compassion, and support toward both residents and family members. The cottages are consistently clean and well maintained. Meals are nutritious, balanced, and served on time every day. Even when I was away for the summer, I felt confident knowing my mother was in good hands. The staff communicate regularly about residents' health and behavior, sharing both positive updates and any concerns promptly. My only suggestion for improvement would be to ensure better management of personal clothing items, as they were occasionally misplaced or taken by other residents. I recommend the facility to family members looking for a place for their parents or family Louise T

The State has closed two cottages due to the infestation and poor conditions (full of roaches). The director, Dawn Grady, is a horrible human being who only cares about filling the place and residents are dying due to neglect. I regret sending my mother to Pacifica Bellair for the 8 months that she resided there, ending up with a huge hematoma that bled for two weeks. One of the residents, a little old lady that lived in the same cottage than my mother, was sent to the hospital with maggots in her legs and private parts. She died in the hospital. This place SHOULD BE CLOSED and Dawn Grady should not be working with our fragile loved ones.

Since moving my mother into The Oaks, I have personally witnessed seven separate occasions where…read moreher prescribed medications were left in her apartment and not administered--an alarming violation of proper medication management protocols. I brought this issue to Pamela's attention each time, yet nothing changed. As a result, my mother repeatedly missed critical doses of medication due to the negligence of the staff. Even more concerning, there have been two separate incidents where her emergency pendant went missing, only to be found hidden in areas of her apartment she physically could not access on her own. Instead of taking accountability, The Oaks blamed us for the missing pendants--when in fact, it's evident that an employee removed and concealed them. This is a direct threat to her safety and raises serious questions about staff conduct and integrity. In mid-February, my mother suffered a serious fall in her apartment. I was never notified by staff--it wasn't until the next morning, when I spoke to her on the phone, that I learned of the incident. Despite the injury requiring 16 stitches and later becoming infected, no one encouraged her to seek immediate medical attention, nor was I called. I was told afterward that the wound care team would "follow the case," which was completely insufficient for such a significant injury. The Oaks scheduled a doctor's appointment for my mother. The driver, Bill, dropped her off at the address written on her paperwork--only it was the wrong location and in the wrong city. I happened to show up to her appointment and saw her sitting alone in a building lobby with no way to contact anyone. When I spoke with Pamela, she claimed she had no way of knowing the doctor's office had moved. However, I confirmed directly with the office that they relocated over three months ago, and The Oaks had just scheduled the appointment the day prior. My mother was left alone, at the wrong address, due to a completely avoidable and careless oversight. These are not minor slip-ups--they are repeated failures to meet even the most basic standards of care, safety, and communication. Due to these ongoing issues, we are moved my mother out of The Oaks.

My mother was a resident here for 4 months following a surgical procedure at a local hospital…read more Initial intake process was good, the facility is clean and staff seemed relatively happy with their jobs. The facility is in the process of adding more Assisted Living space and updating the main living spaces residents share in the central building. Having been a Dietary Director for many years for hospitals and assisted living facilities, I knew what to look for when choosing a space for my mother to heal. The first month was rocky but understandably so as mom transitioned. As the months progressed, mom (a retired Director of Nursing) had concerns around sanitation after witnessing aides that have long acrylic nails assisting other residents with feeding or med pass and not washing their hands or sanitizing with foam between people. I was at the facility yesterday (6/24) and witnessed this happening and was also concerned. If you're not washing your hands after you just had your fingers near someone mouth or assisting them with their bathroom needs, and then coming to my mom's room for med pass, what sickness are you passing to my mother who is still healing from surgery? Then today we hit a new low. This time with the billing office. I received an email on June 25th stating my mother's account was billed for July charges. This was concerning as my mother moved out on the 24th. Of June. Yesterday. As I was getting ready to respond to the original email I received another email stating that I should "DISREGARD THE PREVIOUS EMAIL" -all caps. The billing office Director informed me that she had taken almost $3000 of my mother's money in error and that they would cut a check and mail it to her and it would take 30 days. Excuse me? I was flabbergasted. They took her money in error, from a woman who's on a fixed income, who no longer lived there, and expected her to wait 30 days to get her money back!? My 82 year old mother was just discharged and needed to spend money to restock her fridge and get all of the things she needed to start living independently again but now she had $2689.59 less to do it with. Let that amount sink in. Does your 82 year old mom have an extra $3k lying around? When I reached out to the business office I was told that it was per their policy and that they can't just refund her back her money. That a check would be issued and to expect it in 30 days. My question is this: why did they get to be the cause of the problem and also get to be the ones who get to decide when and how the money they took INCORRECTLY comes back to my mother? My mother didn't accidentally charge 2600$ in the dining room and now we are asking for a refund. YOU didn't pay attention to what you were doing and took my mother's money in error. We trusted you with my mother's financial information and you "accidentally" took money that you were not owed and want to borrow it for 30 days?! And we're supposed to be ok with that? After pushing back on the 30 day wait time I asked to have the check cut and available at the facility for me to pick up immediately. And what a surprise I can come pick it up in 4 days. Why didn't you offer that as an option from the beginning!? Why tell me 30 days and have me tell my family that the place we trusted with our mother has taken her money and we can't get it back for 30 days when that was not the only option!? Had you presented that from the beginning this would have been a very different review. We had hoped that The Preserve was going to be a place mom could go back to in the future if needed. She will not be back. It may be pretty on the outside and the paints being refreshed on the inside but there is something fundamentally wrong with their hiring process or policies and procedures when this is how mistakes are handled. I sincerely hope someone on their Board of Directors or the new owners that have just taken over see this review and makes some changes so another resident doesn't have this happen to them. I wanted to be able to recommend The Preserve to others. After today's debacle and mishandling that is no longer an option. Families beware and stay vigilant when it comes to your loved one's bills and invoices. You shouldn't have to worry about things like this happening.

Jeannie walked me through the process, while I was panicking. It was not an easy transition, which is apparently quite common. The location is good, the apartments are two-room, so you feel as if you are living in a real apartment, not a dorm room. The location is excellent, with a lovely outdoor area for sun and fresh air, that overlooks a lake. Now, some months later, we are so glad she is at the Gables. Care givers are lovely and friendly, Mom has made friend with other residents. She's exercising and playing games. She's gone on some of the outings, as well. Dee and Michael are outstanding activities leaders. Lance, Kim, Lisa and the rest of the PT team is fantastic. No, it is not perfect. They have "adequate" staff, which means if your parent needs 2 person transfer, they wait longer. Adequate is not always enough. But, overall, Mom is happy, we're generally happy with her care, the food is not awful (as it was at rehab) and Sheila and the staff genuinely seem to care about residents.
