I placed him there because I had the unique opportunity of working next door at Springwood Retirement - independant living. Working next door gave me the ability to check on my dad daily. At first, things were fine. As the months went on, I started to notice the high turnover with caregivers and the QMAPS. I also began to notice that my dad's foot rests for his wheelchair were not being used on a regular basis. My dad became wheelchair bound due to having diabetic neuropathy - he was not able to feel his feet therefore, he needed the foot rests because he was unable to hold his feet up off the ground while being pushed in his chair. The fact that the footrests were not used caused him to drag his feet and his right ankle was repeatedly hit with the front right wheelchair wheel. I found a wound one morning, even though he was being dressed by the staff every day; they either didn't see it or saw it and didn't care and guess what, it became infected. The bad part about that wound was, my dad could not feel it either due to the neuropathy. Also, one morning I noticed his left elbow was extremely swollen and it had a scrape on it. When I asked about the injury, nobody knew what had happened. Additionally, there was one morning I went in to visit and noticed his carpet had a huge stain on it, to the right of his recliner. Upon looking at it further, I noticed that he had deficated on the carpet during a fall the day before; the fall occurred around 3:30pm the day prior and I was in to visit him around 9:00am the morning after the fall so, that mess had sat on the carpet for 18 hours. I ended up cleaning it up before I went to work. I then notified the director about the carpet situation and they had the carpet shampooed. My dad paid a lot of money for his care so I expected things to be better which is the reason for the 2 stars - things could have been better. My dad was also on insulin - 28 units daily, If his blood sugar was 150 or less, he was not to get insulin that day - 150 or higher, he would get his insulin shot. One evening I was visiting my dad, the QMAP came in to test his glucose, it was 150 therefore he should have not recieved a shot that day, he got the shot anyway and by the time I asked what his blood sugar was, he had given himself the shot - he was not himself for 2 days after receiving that shot. When I brought this issue up to managment, I was told, they would counsel the QMAP - the damage had been done at this point. There were also numerous times I found his medication in his recliner or under his recliner - medication management is one of the things that residents pay for, yet it was not being monitored correctly. Let me add, upon any concerns or compliants, the standard reponse is - "we will counsel that staff member" Now the fair part.....He did have a couple of good caretakers, he did enjoy the food and the company of the other residents. My dad was stubborn and would not push his button when he needed to get up to use the restroom but he also had advanced dementia and he was a level 4 which meant he was to be checked on hourly, I do not think that was happening and I say that because, I would be with him for a couple of hours at times and vary rarely would anyone come to check on him. I chose not to move my dad because he was so confused at times and I was worried that a move would cause him to spiral. When choosing assisted living for a parent, its hard. You never know what goes on when you are not there. Which is why I feel obligated to write this review and be as thorough as possible.

I strongly caution families against placing a loved one at Mountain Vista Memory Care. We had…read morenothing but trouble throughout my dad's time there, but the following are a few of our most serious concerns. During his stay, my dad was given a high dosage of Haldol that left him unable to get out of bed, incontinent, and unable to swallow properly. He lost more than 40 pounds, yet our family was never notified about the severity of his decline. We watched him deteriorate to the point that he appeared comatose and was unable to eat. Our experience with the nursing leadership and director Jason left us with no confidence in their judgment, honesty, or willingness to take responsibility. A memory care facility is entrusted with people who often cannot advocate for themselves. Families should be immediately informed about major weight loss, difficulty swallowing, drastic changes in mobility, and the effects of powerful medications, not left to discover the condition of their loved one after the damage has been done. We also received deeply troubling reports from people inside the facility about how our dad was being treated. I will not identify those individuals publicly, but what they shared reinforced what our family was witnessing firsthand. Instead of working with our family to remove our father from their care, Mountain Vista transferred him to the emergency room by ambulance and within 24 hours had filed paperwork stating that it could no longer meet his care needs and refused to allow him to return. We later learned that staff had reportedly been told ahead of time that he would not be coming back. Based on that sequence of events, the hospital told us it was not a legitimate medical transfer but rather a planned way to remove him from the facility. Based on what happened to my dad, I would never trust this facility with another vulnerable family member. Families should remember that the initial tour is a sales process and may not reflect the communication, accountability, or quality of care they will experience after move-in.

Hospice review We…read moreare so pleased that we chose the Collier hospice for my mother's end of life care. She had a wonderful nurse, Ann, while she was at home. She visited weekly and helped my mother adjust to the end of life. As her care became more involved Ann was there each step of the way supporting my mother and our family. I have never med someone who was so kind and understanding and competent. Finally My mother had to spend her final days in-patient at their facility. She raved over how quiet and beautiful it was. Again the staff was so very kind and supportive of both my mother and our family. The Chaplin was very helpful with family dynamics. Even the chief was wonderful, providing her with warm chocolate chip cookies which gave her much enjoyment in her final days. What a gift it was to have this organization available to care for my mother and our family at this hard time. As a retired nurse I am picky and could not fina any fault, just very competent care! I can not recommend this hospice more. 100/100.

The reason I am giving Rowan Community a 5-star review is because they care. The staff care about…read morethe well-being of their patients and several of the staff members are long time employees. We have 2 children at the facility that are suffering from a terrible neurological disease. They will never recover, and Rowan Community is their home. The staff treat them as such and take care of them like they are their own. You can't put a price tag or a star rating on that. If something were to happen to my wife or myself, we know that they would be taking care of. With that said, it is an older building and has a lot of the issues that come with an older building. They unfortunately are dealing with staff shortages much like every other business these days, but they are doing the best they can. Everyone pitches in to help. It is all hands-on deck when it comes to meal service, and they do the best they can. We are very involved with the care of our kids and you will find my wife there at least 3 times a week. Unfortunately, several of the residents have been forgotten by their family members and they expect the staff to just "handle" their needs. If you are looking for a state-of-the-art facility, Rowan probably isn't that. If you are looking for people who care, Rowan is the place.

My father was brought to Rowan after a back surgery for rehab, because it was affiliated with the VA. They seemed to be highly rated but this is the biggest nightmare of a care facility I've ever experienced in my life. This is the hell you send your loved ones to when they are dying! Before I even got there, my dad was texting us non-stop to come save him. He did not get breakfast because his dinner the night before was practically inedible. He begged us to bring him outside food. Upon walking in you are immediately hit with a smell of a mixture of morgue and mold-riddled cafe. Then you see the paint, decor, and bedding is all from the 1970s and 1980s. The beds spring boards and the mattresses are foam, not memory foam, regular camping mat foam. The beds at the hospital were more comfortable. The staff seemed mostly nice, but the Director of Nursing seemed to find us audacious for saying that the beds were not adequate. Dad was complaining of pain three times more in these beds than he did at the hospital before his surgery. I asked her how old the beds were, and she answered with "They are perfectly functional! What's wrong with them?!" That's not what I asked. The more I sat with my dad, the more triggered I got by the sounds and smells. It sounds like an insane asylum. Doors being slammed every 30 seconds, equipment knocking and slamming, staff yelling at each other across the halls, patients melting down, etc. There is no AC in the individual rooms. The PT center can only handle one patient at a time, and with as many patients as they had, I'm not sure how often they could have gotten around to my dad's care. As I walked to different places in the facility I was hit with what smells like feces and rotten menstrual pads. Then I had to get on the floor because my dad dropped his hearing aid battery and the floor was sticky. There were no chairs for visitors or patients to sit on. We sat on the unoccupied bed in the room and our backs started hurting as well.
