DO NOT TAKE ANYONE YOU CARE ABOUT HERE. My family member was there for rehabilitation after a fall. Otherwise relatively healthy.. UNTIL SHE WAS SENT THERE. This place leaves patients in soiled clothing- no showers for 6 days! No help when button pushed for bathroom assistance: then when diaper asked to be cleaned as it had been soiled for hours- was told "it's not dirty"... NOW WITH BED SORES - then when asked to help take a shower: 6 days later, the aide or nurse was so rough that there is bruising and bleeding from skin tears! Had be sent back to hospital from this! PLACE NEEDS STATE CALLED ON THEM! read more

Not a place for anyone who cares about their loved ones. My mom's clothes started to go missing the first week she was there. Now I have to go twice a week to bring her new store bought clothes. This is terrible, I do her laundry, they never make it to the laundry basket. New clothes gone, I don't even know if she wore them. I've gone to visit and witnessed her in someone else's clothes. [[HIGHLIGHT]]I have talked to every person in the facility for six weeks and no one will give me answers or make good on it.[[ENDHIGHLIGHT]] She never gets a straw with her daily water, it comes with a closed lid. They comb her hair as to her looking like a spider monkey. [[HIGHLIGHT]]The social worker is absolutely rude![[ENDHIGHLIGHT]] [[HIGHLIGHT]]The assistant Terry is great. Thank God for her.[[ENDHIGHLIGHT]] They hold meetings twice a day, for what? Nothing gets resolved. Shame on them! I am so much in the hole for money with this clothes situation. They need to come up with a plan. Maybe wash their clothes in a net bag, so nothing goes array. But I'm thinking maybe there are some sticky fingers in there, bec. I do my mom's laundry. I hate to think that way but seriously where are they???!!!!!!! Doctors were practically nonexistent--impossible to reach, and when they did appear, they spent mere minutes with patients without offering any real guidance or support. The aides were just as disappointing--slow to respond, inattentive, and often seemed disinterested in doing their jobs. Basic needs like hygiene, meals, and medication management were neglected far too often. It felt like the patients were left to fend for themselves. This facility runs more like a holding area than a place for recovery. Boulevard Rehab in Boynton beach was the most horrific facility I've come across. Husband had hip replacement done and was sent there for inpt rehab. Was put in a bed and laid there for 2 hrs before anyone came to see him. No intercoms. Press a button and light outside door is suppose to grab someone's attn. He couldn't get out of bed and needed to use bathroom. No portable container given. No one showed for 45 minutes. Needless to say he made a mess of bed and CNA was upset. Really. I had to clean and change him. Forget about getting any meds. No hospital band. No scanning anything to verify drugs given. Had to take him out after 2 days. CNA tried to move him from chair and he fell onto bed hurting hip and back. Signed papers to get him out and they wouldn't give us copies of paperwork. Wouldn't give to me to take a picture. [[HIGHLIGHT]]Name tags all on backwards. Ask their name several times and they mumble or can't understand them.[[ENDHIGHLIGHT]] Will be reporting them everywhere I can. read more

Rehab Care. If I could give a negative star I would have. This is not a place for a patient that has any dementia and needs rehab. The aides are unreal and will argue with you when you ask them why something is or why it smells so bad in the room. If they need feeding, good luck. They'll dress your loved one in dirty clothes because they are too lazy to look in a drawer. Communication between the staff and the family is NON existent and if you do ask questions as to why you haven't heard about a treatment plan etc for your loved one they will get aggressive no matter how nice you try to ask. You're wrong and they are right. Trying to get them out of this facility and back to somewhere else takes an act of Congress and days. If you value YOUR sanity and the care of your loved one look somewhere else. read more

If you are looking to kill your loved one, this is the place for you. My MIL was admitted 10 days ago, and not one Dr. in sight--why? They only have one Dr, but i'm pretty sure the Dr is a ghost. The Nurses are beyond incompentent, they have absolutely no idea what they are doing, it's a disgrace. The facility has no idea how to obtain medical records, which is a primary component of being admitted to a rehab facility. The administrator is a joke! Sure, they are warm and fuzzy prior to receiving the patient, but once the patient arrives, it's like being placed in a prision environment. Thankfully, we had family there to oversee the incompentence first hand. We hired a private transport company to get my MIL the hell out of this "facility". Don't send a loved one here unless you want to have high blood pressure and watch your loved one deteriorate. read more

This place is beyond horrible. Complete apathy from the aides but maybe 2. It's dirty, the patients are neglected to the point where it's inhumane. We had to schedule a flow of family and friends to check on my friend's mother while she was there. She was over medicated on pain killers and then when meds were needed, they were not available. Most of the aides are lazy and have more concern about snacking or being on their cell phone. No Drs and nurses to be found and it's a miracle my friend's mother made it out alive. I could go on and on, but other reviews cover similar details of how horrific this place is. It is shocking a facility like this is allowed to be open. This place needs to be shut down, and I can't believe insurance companies would pay for this! read more

Great friendly and happy staff. I have seen some very happy guests here as well. I am yet to hear a complaint about their services. This place is one of the few places that smell clean. My father landed in the rehab portion after being in the hospital. My mother was so upset because the staff would not listen to her about his medication. I know she is emotional so I had to see for myself. I went to see my father. My father looked like he was dead and when I asked the staff they seemed to think this was normal because of what a case manager from the hospital observed for one night. I asked why my father looked like this and what they gave him. His medication was all messed up and wrong. It was shocking. After I had to calmly explain that this is not normal for my father to look comatose they asked me if he should go back to the hospital. I told them yes. If I had not come there he would probably be dead right now because they did not listen to my mother. My father has been moved to a hospital and has organs shutting down. I can't believe these trained people could not see something was not right. read more

I gave one star for that's the only way I could write a review. This place is disgusting! Overworked Cna's with 8-10 residents a day . They put most in depends for they are to lazy to toilet them, showers aren't given for the same reason. It's dirty , the staff use the residents toilets , medication isn't available at all times ( no one plans ahead ) residents are neglected, food is terrible, administration and corporate are aware and all this has been brought to their attention many times and nothing is done. If you want your loved one to maintain dignity, this is not the place. If it were up to me and many others that I have spoken with that visit their loved ones - this place would be closed tomorrow! read more

This facility is shameful with the exception of Donna Lee. My 91 year old mother was sent there after a complete hip replacement and several complications. Whomever they contract with for transport wanted to pick her up from the hospital at midnight! Imagine after a serious surgery thinking that was okay? When she arrived the next day, nobody thought to contact the family to check in. Because of Covid they have many restrictions, which I appreciate, but when we tried to drop off some of her belongings we were told they had to be inventoried and laundered. She wouldn't get her things for 2-3 days. My mother sank into a deep depression (which nobody mentioned for days). Every time we called nobody answered the phone at the nurses' station or a very sour nursing supervisor did and was incredibly curt. My mother was there for four days and it took that long to get her discharged. Once we got through to someone they obliged. When we saw our mother, I cried.......she had not had a shower, nor any physical therapy which is why she was there. She is now getting PT at home, healthy food and lots of rest in her own bed. She is happy again and gaining strength. Don't go near this hell hole. When she was discharged the aide handed us about 10 pages of new prescriptions and some notes. Nobody even had the decency to go over any instructions with us. I could continue but I think you get the message! Steer clear of this disorganized chaotic place. read more

I visited a family member who is a patient at Boulevard Rehab. I brought lunch since she told me most of the food was poor quality and tasted awful. Also the kitchen staff did not pay attention to her dislikes and continued to serve them although they were clearly written on her meal ticket. I arrived at noon and was upset to see that she was just getting dressed and out of bed. Upon questioning the staff I was told each CNA had 10 patients wash and get ready. I asked my relative about the service and she said that after pressing the call light for help she usually has to wait from thirty to forty-five minutes for a CNA. Most of the residents are kept in diapers all day and night and are not changed often enough. Also showers are not given frequently. Her clothing sent to the laundry is lost, but they recently started to put name labels on items, so that may improve. Sometimes certain medications are missing and patient is told that they are on order at the pharmacy. The diaper supply was empty for days and I had to buy diapers for her. Patients with obvious dementia are allowed to go through the corridors all day screaming. It's obvious that they need proper organization and adequate staffing. read more
