I am submitting this formal grievance regarding the care I received from Dr. David M. Ondrako in the Emergency Department at Coconut Point. I respectfully request that this complaint be investigated through Lee Health's Patient Relations, Quality Assurance, and Physician Peer Review processes. This complaint is not based on the fact that I did not receive narcotic medication. Rather, it concerns what I believe was inaccurate medical documentation, dismissive communication, and a failure to provide fair, unbiased, patient-centered care. I typically seek emergency treatment at Gulf Coast Medical Center or Lehigh Regional Medical Center. On this occasion, I chose Coconut Point because the other emergency departments were experiencing high patient volumes, and I had been suffering from severe pain that had become unbearable. Approximately one to two weeks before this visit, I slipped and fell on a marble bathroom floor. Since that incident, I had been experiencing severe spinal pain, constant shooting pain radiating into my arms and legs, numbness in my feet, difficulty walking, and pain significant enough that I had been unable to sleep or eat for approximately three days. These symptoms were superimposed on injuries from a documented head-on motor vehicle collision on December 15, 2025, on Porter Road. Since that accident, I have undergone extensive medical evaluation, including imaging studies, specialist care, pain management, physical therapy, and discussions regarding spinal surgery. My symptoms and treatment history are thoroughly documented within my medical records. Upon arrival, the nursing staff treated me professionally and respectfully. They obtained blood work, assessed my condition, and listened to my concerns. I explained my medical history, my recent fall, the severity of my symptoms, and the treatments I had already attempted without relief, including steroid injections, Toradol, and other non-opioid therapies. During my conversation with Dr. Ondrako, I explained that I had already been prescribed medications such as meloxicam and lidocaine patches and that these treatments had not provided meaningful relief. Dr. Ondrako stated that those were the medications he could prescribe. I advised him that I already had both medications at home. After that discussion, I was left with the understanding that no additional evaluation or treatment would be offered. Believing there was nothing further that would address my condition, I chose to leave the emergency department. After my departure, I discovered that my medical record documented "drug-seeking behavior." I was deeply disturbed by this documentation because it was never discussed with me during my visit and does not accurately reflect my statements or my intentions. In fact, I specifically informed Dr. Ondrako that I am in long-term recovery and that I was not requesting narcotic medication. I further offered to undergo a drug screen or any other objective testing he believed necessary to demonstrate that I was not actively using illicit substances. My purpose in seeking emergency care was to obtain an evaluation and relief for severe pain and neurological symptoms that had significantly worsened following a recent traumatic injury. The diagnosis of "drug-seeking behavior" carries substantial consequences. Such language can influence how future physicians interpret my complaints, potentially delaying or interfering with appropriate medical treatment. Because medical records are relied upon by future healthcare providers, insurers, and specialists, they must accurately reflect the patient's presentation and the physician's clinical reasoning. I respectfully request that Lee Health determine whether this documentation was supported by objective clinical findings or whether it was based upon assumptions rather than evidence. I also request that the investigation evaluate whether the care I received was consistent with Lee Health's standards of professionalism, patient rights, and ethical obligations. Specifically, I ask that the review address whether my documented neurological complaints, recent traumatic injury, inability to sleep due to pain, and ongoing specialist treatment received an appropriate emergency evaluation. I am additionally concerned that implicit bias may have influenced my care. Throughout my visit, I felt that assumptions were made about me based on my appearance, age, tattoos, or prior history rather than on my documented medical condition and my actual statements. With a shortage of MDs in Florida, you may have to book an appointment well in advance. In the interim you can schedule with a physician's assistant, likely at a different location. (Lee Health has facilities throughout town.) Dr. Fraga is competent, proactive and thorough. She is easy to work with, and easy to talk to, quick to respond to messages sent on My Chart. Lee is part of the Epic System, with records accessible to Epic providers throughout the country. ( The network facilitates continuity of care and should be mandatory everywhere. Unfortunately, it's not.) I'd also like to give a shout-out to the on-premises lab. It's efficient and well run. Walk-ins welcome, frequently with minimal wait.
