    very poor reliability....workers do not show up fifty percent of the time, and are always late when they do arrive

The meals were often…read morenot what you would request ed, they lost my weekly menu form several times so I got whatever they slopped on the plate. The vegetables were so mushy. The menu said roasted Brussels Sprouts and what came you could flattened with your fork. I think they boiled everything. The food is very salty. The CNAS were so overworked. If you needed to see the Nurse (which there was only 1 for up to 18+ patients) it could take 2-3 hours. The same for the CNAS. My surgery wound got infected. Nothing was done for 10days. It would be looked at every few days and the comments were-oh this is getting redder, or this has gotten longer,-or,now its oozing. I never saw a Dr, it was all over the phone. I need to have a 4th surgery as the infection didn't respond to the antibiotic and finally the medication was increased but too late. The building was in such bad shape. To get into the shower room you had to go over a torn floor. The wheels would get caught and it was dangerous. They sold the building so they spruced up with painting the room. However, you could see where it was sloppily done. The only helpful people in the building were Kaylene (the PT) and Todd ( OT). They were awesome. Very supportive, caring and would answer all questions. However they were often overworked due to patient loads. Bottom line: DO NOT SEND YOUR LOVED ONES TO ANY AVAMERE CENTER.

The CNA's staff are really hard workers especially the new ones are very professional and the best in the west, but the experience with one of the night-shift nurses unfortunately has been continuously unprofessional and unpleasant since the beginning of time of stay without mentioning names :( The facility is somewhat basic but if having a better attitude night-shift-nurse, patients and families will be satisfied as if it was a heavenly service and consider it a five-stars-facility. We support your business and value your hard work and hope to convert this to 5 stars by the end of the stay.

She orders something and the wrong thing/meals shows up. I guess the chefs are really lazy here and I heard they steal food from the place. Maybe they can't read English. They do not have a nutritionist here so the good works with the residents diet. Overall the place is pretty clean. There's a gang of smokers out side of the place who yell at me because I wore my KC Chiefs jersey there one time. Other than that this place is not too bad really. Avamere offered the nicest outdoor experience with a walking trail, duck pond and easy access to shopping and bus lines. The interior of Avamere was also a step up! It did not have that tell tail 'older person' smell that all the other facilities had. Fresh paint and fresh carpet go a long way toward this achievement. Additionally I would like to give Cassie Delozier a shout out. She made the transition smooth from independent living for my brother to a very pleasant room that he is very happy with. She did a personal visit with my brother to make sure he would fit in, arranged to give him a personal tour of the facility and helped get us all settled in once the move happened. She made a point to introduce him to all the residents and staff on his first day and was instrumental in us choosing Avamere for my brothers forever hone. She continues to answer my periodic question about resources and general questions within the hour that they are posed, excellent customer service! Thank you Cassie! If anyone is looking into a superior senior living experience I would highly recommend Avamere at Park Place.

When he was released from the hospital he needed intense physical therapy. He was in RIO for 8 days. When he got there he couldn't walk or stand by himself. When he left after 8 days he could do both with the help of a walker. His speech also improved greatly. The staff here is amazing! They take the time to get to know their patients and their families. They work one on one with each patient. They cheer them on with every milestone. You couldn't ask for a better place to help your loved one get the care they need. We didn't think he would be able to go to RIO as it wasn't covered by the crappy insurance we had at the time. But the doctors and staff at Providence worked their magic and by the grace of God he was able to get the best care I've ever seen! They also had 2 family "update days" while he was there. The staff and my family all got together around a very large table while they told us of his progress and what to expect next. If you ever need great Physical Therapy, fight for this great place! Update: 4-24-25 My husband is doing great! He can walk without his walker but with occasional use of a cane. His speech is near normal unless he's very tired. He's driving again and baking (favorite past time) up a storm. Going back to work is still a few months off but he'll get there. Our family and I are forever grateful!

    I cannot say enough good things about the experience we had with RIO. My 89 year old father had a…read morestroke and was in ICU for nearly a week. His doctor over at Tualatin Meridian Hospital really wanted my dad to go to RIO but it was out of network. Some kind of miracle came through between the hospitals, doctors and insurance and he was approved and moved to RIO. Their reputation is the among the best with the hospital staff and doctors, so we were quite eager to have him go there. My sister is a retired PT with a doctorate and she really wanted him there too. My dad had to undergo 3 weeks of intensive therapy, from PT, OT to speech therapy. The improvements gained during his stay were nothing short of miraculous. He relearned to walk, swallow and gain use of his right hand while there, among many other small miracles. They trained him how to get out of bed and use the walker safely so he could go home instead of to the a nursing home or assisted care. He is happily home again with my mom. He has home health care and is seeing improvements every day. I think that every minute counts when it comes to strokes and the fact that he got in at the right time with the right care meant everything for his progress. My sister, who has a degree in PT and knows her stuff was super impressed with all the care and treatment. Hospital food it terrible, but we were allowed to bring in food so my dad could regain his strength, so that helped a lot. While dad was in ICU, no visitors were allowed because of Covid, but RIO allowed 3 visitors to come and go which meant my mom, sister and I, who live closest, were able to visit, which lifted his spirits. But, there's no place like home and he is home safe and sound and luckily on his way to recovery. The best part was when he got to ring a bell 3 times for graduating and the staff gathered and applauded. Still makes me very emotional. It is the most important graduation of our lives. My sister will be writing a Google review so you can read the more technical side of his experience. If you have a choice, I give RIO my highest recommendation. I think the staff are angels.

It was an overwhelming time of change. I'm especially directing this review towards the hospice team that came into my patents's home to provide hands on care. We were very fortunate to have Alyssa, Trey, and La Sarita help us out. They were excellent communicators and very caring and competent. They exhibited great teamwork. They addressed our questions without making us feel like we were wasting their time. An additional note. After my Dad passed, just 13 days after entering hospice, my Mom did not want to sleep in her bedroom alone. Gracious Care Hospice made arrangements so we could keep my Dad's rented hospital bed so I could sleep next to her and hold her hand as she grieved the loss of her husband of 68 years. That act was outside of the hospice routine, but was a key to helping my Mom pass peacefully, 17 days later. Thank you, Gracious Care Hospice.

Following a stroke, my grandmother was transferred from OHSU to West Hills, where she stayed for two months. Last week, when she was facing death, rather than going home, she chose to stay with her incredible team at West Hills. We got to know nurses, CNAs, case workers, therapists and more on a first-name basis. They all were so kind and gave such skilled and compassionate care not just to her body, but also her mind and her spirit -- she discussed her Zen Buddhism with one of her favorite nurses, talked endlessly with her physical therapists and wanted one of her CNAs to move in with her. Throughout a painful process, West Hills was a true partner and comfort to my grandmother and the entire family. We would have care conferences that lasted hours and involved upwards of six people, everyone working hard together to find the best choice. When we were down to the final hours, they brought food and water and reminded us all to eat. So many caregivers throughout the building came to say goodbye to her, came to give us hugs and cry with us. We could not ask for more from them. They came on good recommendation from a family friend, and she was so right. We are so grateful to all they have done for us (and tolerating when we all piled in her room for pizza parties). When I went in, worker after worker just walked right on be me. I ended up going to the nurse's station and they just said he hadn't arrived yet and I could wait in the lobby. No welcome or offer to show me around. After over an hour he hadn't arrived so I went back in with his stuff. They just told me his room number without showing me where I could put his stuff, so had to go back and ask. Care was terrible. Our dad said it would take them over 30 minutes to come in for help to the bathroom or anything else and would just watch him struggle to get out of bed without helping. They basically left him in bed, including to eat. He finally asked if they could help him get out of bed in the morning so he just wasn't laying there all day. Only one option for meals, which he mostly did not like. He lost over 15 pounds in 2 weeks, and left very weak, which lasted several days after discharge. He was in the hospital for one week prior, but was eating regular meals. I brought him a toothbrush, but he never once brushed his teeth. Hygiene needs were not tended to. Therapy was supposed to be everyday, 5 days a week, but lucky to get one session every other day. Their answer for his bathroom needs was to put depends on him. When he was discharged he was to have a home health nurse to go over 3 times a week. It turns out the home health agency Sapphire sent the orders was at capacity so they let Sapphire know. It's now been 1 week, with no home nurse. Nobody from Sapphire has notified us and do not return our phone calls. They seem to be there to provide the bare minimum of care.

    If I could give zero stars for how this discharge was handled, I would…read more I came to this facility after a motorcycle accident, hoping to recover safely without putting extra stress on my partner. While a few individuals on the care team truly made a difference--Damalesh, Shantay, Louie, and a handful of others were kind, attentive, and absolutely wonderful--the overall experience left me shocked and disappointed. The social services director, Sam, is the main reason this facility gets a 1-star review. He was dismissive, condescending, and ultimately responsible for the stressful, manipulative mess that surrounded my attempt to leave--saying that I was "denying care" when in reality I was taking my "care" elsewhere... Had to fight to get basic discharge paperwork (that I asked a half dozen times to get for hours on end), was pressured to sign documents without being given full information, and felt completely stonewalled when I asked for reasonable medical equipment and referrals for in-home rehab. Sam made what should have been a smooth and supportive process incredibly frustrating. As for the food? I'm vegan, and while they made an attempt to accommodate that, I can't live off bread and tofu forever. Toward the end, I could barely eat anything at all--it got that bad I understand care facilities aren't luxury resorts, but respect, communication, and basic follow-through shouldn't be optional. If it weren't for the handful of standout staff members I mentioned earlier, I would've lost my mind here. If you have a choice--keep looking. You deserve better.

    What can I say about this place? The fact that my concerns were not met when I felt completely…read moreviolated, disrespected, and shamed while having to take a shower, while a male staff member sat just inches away from where I had to take a shower. I was told that if I didn't take a shower at that moment, I would not be able to take one the next day. There were enough female staff on duty that one of them could have come to the shower instead. But yet, being humiliated. I still have trauma from that night. Only one staff member approached me about what happened. I was not apologized to by Hussain nor by anyone else. I felt absolutely outraged and degraded. Yet, no one seemed to care. The only reason I was here was to rehab from cancer treatment It was much more banal at best. A complete lack of communication between staff members. Often told "oh, I forgot, or no one told me," which is absurd when dealing with people you are there to care for. On more than one occasion, I was brought the wrong tray of food while the staff jokingly said, "oh, it happens all the time", like that's ok. Another issue.. having to put a sign on my door asking people to knock before just walking into my room. It is a common courtesy to knock on someone's door before walking in... it does not matter where you are. Yet, even with the sign, some just walked in without knocking. As far as rehab.. I did better PT in my room. A couple of old bikes and a few weights. It was a waste of my time and dignity. I stayed there two weeks too long. However, that being said.. some of the nicest people you ever wanna meet. All seem pretty stressed most of the time, definitely overworked, probably underpaid. From my personal observation along with asking the R.N's CNA's, LPN's, the rehab staff and others. Each caring for patients with different needs and specifics. The length of time spent that particular resident varies, leaving others under their care basically screwed until they can get to you personally. That's why the one star, because they do try. But ,things definitely need changing as far as respect, patient care, effective communication, the fundamentals and most valuable not treating your patient as you did your last. Each patient are different in their own right, they should be respected as so. As an individual, which we are all and not just another bed filled.
