    My son had great care there after a severe stroke. The have him walking again and Mayo Clinic said he never would. Thanks Brooks.

    I can't even remember how we were first referred to Choice Matters.My brother was unable to…read morewalk,and a had a plethora of other medical issues including strokes.We had another company that was really awful,and no progress made.Results with Choice Matters were immediate and changed everything.No one thought he would live,let alone walk again.Natonya and Catherine-his nurses are amazing-truly committed to excellence and great teachers-they were always there for me too,answering my many questions and soothing my concerns.His physical therapist Rommell is an angel of a man-patient,kind,inspiring,and innovative.My brother went quickly from couldn't pull his blanket up to walking and almost normal activity.If you need a miracle like we did call Choice Matters.The name says it all,choose wisely,it matters a lot.

Tammie was truly amazing, patiently explaining all of the bureaucratic monstrosity that is the American health care system, from HMOs to Medicare. She was highly knowledgeable, resourceful, and extremely personable. I honestly don't know what I would have done without her; she walked me through all the steps I needed to get the home care set up and was compassionate about the many obstacles I faced due to my father's terrible HMO. She helped me with getting my dad into his primary care and assisted me with all the steps needed to get his home health referral. The rest of the staff at Choice Matters is also excellent; my dad loved his physical therapist, Allen, who helped him get back on his feet, literally. My dad is a misanthropic curmudgeon who doesn't like most people, but he looked forward to his visits with Allen and was able to walk again with his support. The name of the agency really reflects that "Choice Matters" when you choose a caregiver. I highly recommend!

    Our mother was sent her for rehab after a hospital stay. Facility was disgusting. Rooms not clean,…read moreroaches seen every day, I witnesses poop in a hallway that several staff walked past without a care. CNAs did not come into her room at all unless we called for help. For this reason we didn't leave Mom alone other than night time. We had to request her to be showered etc. Her teeth weren't cleaned, hair wasnt combed. Blood sugar not checked even though her notes stated she was diabetic. When we pushed the call button no one ever came so we had to go to nurses desk to get assistance. CNAs we're always sitting down outside patients room and on their phones, never seemed to be busy, doing the bare minimum. I would never recommend this place to anyone as the level of care was extremely poor. We had our mother transferred as soon as we could. This place should be closed down

He had a bedsore on his sacral area and ii found him sitting in the lobby screaming with his feet tied to the chair. .He never spoke a word to me after hearing his painful words that day. Days later as he lay dying in bed my mother and i were verbally assaulted by by a night shift charge nurse standing over my fathers bed shouting and lecturing us about hospice care, this was in the evening after i had been working all day, these were the worst days of my life and my parents lives. Also my mother had to hire a lawyer to get her money back when she was overcharged on his admission. Don't send your loved one here.

The directly lied about the improvement of his injury and forwarding test results request from his outside specialist doctor. Initially sold they provide a strong level of service that did not exist.

    My mother was admitted to this center to be administered an antibiotic intravenously for 21 days…read more Her overall care was consistent and was administered medications as needed and directed. Meals were delivered according to nutrition needs, physical therapy was also well received by my mother. The negative experience lies in her discharge procedures as well as lack of communication with the Director of Nursing Angie and Case Manager Jenny. Her discharge required portable oxygen as well as set up for home oxygen she was released with neither and was lied to that we simply had to call and they will deliver it. It was confirmed by the company they never received the requests. She was prolonged a procedure for several days to remove a tic and was transported twice to a hospital to have it removed without confirmed scheduling resulting in her being left once and I had to Uber her back to Life Care. The D.O.N Angie and Case Manager Jenny never returned calls and once confronted released they were telephoning a number that was never given to Them upon intake from my mother, myself or St Vincent where she was prior to being released there. She was discharged without three vital life threatening medications causing her to miss doses and resulting her being in the hospital upon arrival. If you have any other options please choose them. We have filed a formal complaint with AHCA and our family attorney has admonished us to build a case and we are.
