Quality of care is missed altogether. An emergency light can stay on for 20 - 30 minuets. CNA'z ignore their call lights, some CNA's don't come to the room all night and there is nobody to call or ask who to call. Food is only good sometimes other times the entire plate could be trashed. There are a few exceptions to the rules. Some workers try harder than others. But there appears to be nothing given. Medicine can go not given and nobody is alerted Nobody seems to care. Therapy is a joke and can be seen through Zekes actions loud and uncaring was a jerk to me in front of all to see. Zeke the jerk. BIG JERK. I tried hard but to no avail. read more

Today I was able to change the rating to 4. The facility coordinator made my surgery clearance proceed with speed to meet our short deadline. Thank you! There are some amazing things about this facility they are some amazing people Jane, Ashly, and her nickname is nounou. Kristina one of the nurses. All the head nurses are also superior. The lower rating is because it was hard to be heard at the beginning. I have a long term issue that took a while to be sure of. Once you are heard they become extremely competent. They are willing to work hard for you. I would have given a 3.8 but this was not possible with this rating system, Another bright spot is Julia and Ken in physical therapy. My brother law need a recovery facility and my sister in law found this place. The staff were very friendly (shout out to Ty and Brooke) we were greeted and welcomed. Best thing was there was no smell. It was fresh and clean. Will follow up with his care quality in a few days. read more

I was sent there by Kaiser for rehab for a total knee replacement on a late Friday afternoon. They do not have rehab on the weekends so I was confined to my bed and put in diapers, which was the most demeaning experience. If anyone has ever had total knee replacement, you are supposed to get up and walk. I would have given them fewer stars, but their staff was excellent, kind but overworked. If you needed anyone during breakfast, lunch or dinner, forget about it. The staff that was there to help you was taken away to serve meals, each consisting of two hours for each. Had two other room mates, one who was up and vociferous at midnight and played her TV all night. Obviously the facility has no rules on a silence curfew. I checked myself out and pray to god I will never have to go back. read more

The level of care my mom received was excellent. The employees were kind, compassionate, and knowledgeable. Jane, CNA, went above and beyond, always ensuring my mom had everything she needed. The PT program was outstanding. Ken went out of his way to encourage, push, and empower my mom as she got a little stronger each day. The activities staff is exceptional as well. They took the time to learn my mom's name and to really get to know her. Knee replacement surgery was much more excruciating than I expected five days in the hospital and little over two weeks at the Brookside Healthcare Center The first week was rough once I understood their system and they got to know me (a little). everything went well. My 92-yr old mom stayed for post op, the people are wonderful! Caring and loving, and most of them are well experienced! The processes can use a little help, you have to follow through, follow up, and repeat the same things over and over even if it was already discussed and agreed upon. Certified Nurses Assistants were very helpful. Physical Therapy was excellent especially Ken and Maryanne.

The grounds are beautiful. The facility is freshly painted and clean. Staff is wonderful at all levels. Medical assistants, LVNs, RNs, and OT and PT therapists listen, are caring, and helpful. The physician, Dr. Vo, was present and knowledgeable. I highly recommend Brrokside Health Care. read more

After a car accident and surgery at Loma Linda Univeristy Medical Center my mom was fortunate enough to spend several weeks at Brookside Healthcare Center - less than a mile from her home. Please do not send your loved ones to this facility. I had my father transferred here in 2023 as it was an approved VA facility. The moment he arrived the toilet began overflowing. Nurses were overwhelmed. But the worst part was, after I had left the second evening, a night CNA approached my father and asked him if he would be willing to sell him his house. At the time my father thought he was going to be going to hospice and the CNA had overheard our family speaking about next step decisions. So he waited for us to leave, then approached my father and told him he could sign his house over while IN the facility. My father called and told me and the next morning I went in to begin the process of moving him and the San Bernardino County Health department was there conducting an investigation into thefts of patients at the facility! Thankfully my father had a medical emergency a few hours later and I was able to get him into the VA where he was treated with dignity and respect. We made a complaint to the VA about the way he was treated. He was there for exactly 42 hours and it was so frightening for our family - that one of their employees attempted to take advantage of my father while at his most vulnerable. I can only imaging how many other patients have been taken advantage of. read more

I would give -3 if possible to this taken for granted place. We tried to request to move him to other facility but there were none available so we were FORCED to accept Brookside Healthcare Center. My stepdad was admitted around the beginning of November 2024 for rehab due to broken leg. The staff was ignoring the call bell and rude, the tv was not working, his table was crooked, his wheelchair is broken,and food was horrible (as my stepdad would say, "always beans"). When requested for the tv to be fixed, somebody came in but nothing was done. Requested for a better wheelchair, they gave him a smaller wheelchair and he wasn't comfortable with it due to his injury so he had to settle with the risky, broken wheelchair he has. As of January 2025, his table is still crappy and his drink still falls off. His tv still not working and he's close to being medically cleared. That's 2 months of being in bed and watching only on his phone straining his hands and neck. December 25, Christmas day, after getting home from celebration with my stepdad at the rehab, my mom received a call and my dad was asking help, crying over the phone saying no one was helping him, his calls were being ignored and he's in the bathroom. As of 01/03/24, around 7pm, the bathroom was clogged and overflowed into the room floor. I asked if there's an administrator so I could report the incidents and I was told there was none except for the nursing supervisors. I strongly suggest to stay away from Brookside Healthcare Center and choose a way better facility. I would be taking "proper procedure" so I hope the facility will be ready. read more

My mother has not always been a fan of nursing homes and when I arrived here I saw the work they're doing and said WOW! Doesn't even look like a nursing home. The care is great. My main concern was going to be my mother's pain management and they have made sure to stay on top of it. Let's not talk about the therapists because those are the people running this show! I am in Brookside Health Center following a revised knee surgery. I Have been here for one week now. Miss Jane, Miss Kimberly and Andrea, are my favorite CNA'S with a small handful of others. Is there ever enough help? NO, these workers don't get enough help from their colleagues. They work tirelessly and are compassionate and they seem to genuinely care! In the physical therapy department there is MaryAnn, Teresa and Ely that stand out the most for me.. they are the ones who will get me home sooner!! The other people who are outstanding are Justin, Alberto, Al, Lesley and Genesis, these people dispense your medication. in the office department Miss Patty, Grace and Yoshi have accommodated my needs and all the questions I have had. For all the horror stories you hear about the food, that's not the case here. The food is tasty and if there's something you want that's not on the menu ask for Eric he runs that kitchen and he will work with you to make what you want! he works with you to make you happy...he's a wonderful man and I appreciate how he treats me and cares about everyone's meals... I have had some fabulous meals, good variety and a I actually look forward to mealtime. The place is clean and I appreciate that a lot, as I worry about infection which is very important to me. All in all I think this place is pretty good. I will repeat what another reviewer said, "the squeaky wheel gets greased"so you do need to speak up! read more

We selected this place to care for my sister in law. She recently had a major stroke. I want to personally thank, Bonny, the manager, who is absolutely wonderful. My husband spent about 3 months at Brookside Healthcare Center. I would visit him just about every day. The room was immaculate and cleaned every day. The room was just renovated. The entire facility is going through a full renovation without compromising care for their patients. I live in the Redlands and needed to find a skilled nursing facility for my Mom. I know of two people who had used Brookside for their family members. Both of them spoke very highly of the Brookside staff and facility. I went to learn more about Brookside in person. I greeted by Patty and she gave us a very thorough and informative tour. After a meeting with family members we decided Brookside side was the place for Mom. Mom is still there and receiving excellent care. She is improving daily and we can't thank Brookside enough. So now this same nurse AMANDA got mad at my dad this morning and didn't give him his medication before leaving to dialysis , guess what! The dialysis tech had to call 911 because he was slurring his speech, his blood pressure was way too high because no meds were given.. my dad could have had a stroke or worse. My dad left his phone at the facility so I get a call from Kaiser emergency and it's the nurse telling me my dad was admitted No one from Brookside noticed that he wasn't back or they just didn't think to notify me. Either way I'm so angry at this facility for the lack of communication and care. Do not trust them with your family members This is ridiculous. read more

DO NOT SEND YOUR LOVED ONE HERE!!!The doctors all cleared my mom to leave but they weren't able to get her aftercare medical devices, because of the holiday per the social worker. She stated we can discharge her next Tuesday with everything that is required. Then when it came down to the day before she was to be discharged and now all of a sudden the medical devices aren't covered by her insurance, so she isn't getting anything. I said fine I will pick her up tonight instead of tomorrow, but they will NOT give her the blood thinners that she needs until tomorrow. They got to charge the insurance for another 6 days. Meanwhile I get robbed of a holiday with my mom, but these people don't care!! This is blatantly insurance fraud! read more

Currently I have been here as a patient for 5 days post-op left hip replacement surgery. No complaints at all. The facility is clean, no odors, every one of my needs are met and in a timely manner. and the food is good. All staff from housekeeping, dietary, admissions, treatment, social services departments, Doctors, PA's, Physical Therapy Dept and Therapists (loved the James Brown music to help motivate me to move), RNs, LVN's, both male and female CNA's especially Jane Lasa, her demeanor is sweet, kind, gentle, caring and professional in patient care. This is my first experience in a rehab and any expectations of the unknown has been answered here. I am not the only patient here and others are far more critical than myself but Brookside makes me feel like the priority when care is provided to me. I encourage anyone who's loved one is going here to please do your research. The staff is very nice but appears to be overextended and overworked. Also THERE IS NO DOCTOR ON STAFF. They will tell you there is one but it's thru an app. Our loved ones pain was not under control during the duration of their stay. I called several times to speak with therapists there and was rushed off the phone as they had other patients to get to. Our loved one made absolutely no progress there. It has been a huge set back for their quality of life. We had to ask 4 times- 4 time to have someone add them to a feeding list as they couldn't eat without assistance. Again please do your research before you Send a loved one here. It might be great for your family member but I suggest you Sit with them every day and be a squeaky wheel- otherwise they won't get the attention they need. read more
