I have been a resident at this facility since 1/16/26-7/2/26 The building is of old converted construction and occasionally, there are problems with the heat and AC. The building does need a complete plumbing and AC update. The maintenance staff does work very hard to keeps things operating properly. There are a few young ladies who work here making appointments and doing other paperwork for the patients. I will not mention any names but we all know who they are and I have to say Thank You to to them for their help and pleasant attitude. Sometimes I drive them crazy but they are very understanding and accommodating. The facilities social worker is very pleasant and helpful. There are minor (and sometimes) major operational problems with the facilities three Hoya Lifts. My room, the hallways and common areas are cleaned everyday of the week (not that they need It). In other reviews, some stated, upon entering the building, there was a strong odor of urine. I have been here for some time and I have never experienced that. There are everyday activities to keep the residents occupied and stimulated. The food is not 5 stars but it is fairly good for nursing home standards with a few outstanding dishes. There is a problem with some of the kitchen staff not being able to read and understand what you ordered due to the language barrier (or they just don't care). The kitchen director is constantly trying to correct the problem. However, as of this writing, the problem continues almost every day, three meals a day. I hope that he fixes it because at times, it can be very, very frustrating. Going on six months and NOTHING has been corrected. You would think that after six months, the problems would be corrected but not this place. Some will think that I'm a racist and I don't really care because everybody that knows me, knows differently. Everybody is entitled to work, however you should be able to communicate in the language of the country where you reside and are employed, no exceptions. Dock them an hour's wage for each mistake and in a few days, they'll be speaking perfect English:) As far as the nurses and nurses aides are concerned, its the same no matter where you go. Some are pleasant and like doing their job and others are very lazy and just plain miserable and unfriendly. There are some that would expend more energy trying to get out of doing their job properly than doing it right in the first place. I am not going to mention any names but I think that those not so nice and unfriendly people know who they are. However, to the pleasant and helpful staff (which there are quite a few and I won't mention their names), I have to say Thank You for making my stay a little more bearable, you were very kind to me. What drives me crazy is how some of the nurses are not fully prepared when it comes to providing such items as wound dressings, colostomy supplies, etc..... When one nurse gathers supplies for a patient, the others act like squirrels stealing the stored supplies for their patient's use. Example: I needed a wound to be dressed which should normally take no more than ten minutes. An hour later, the nurse returned because she had to search two other floors for the proper dressing which is extremely inefficient, time consuming and not very helpful for the patient. As I leave, I am giving this facility a 2.5 star rating and I hope that the owners and staff read it and make the necessary corrections and improvements because when you're recovering from an illness, the patient does not want to tolerate all of these petty, easily fixed problems. Do not put your loved one at Care None! The aides have zero compassion! We deeply regretted placing our loved one here and hated to leave her when it was time for us to go home. if you don't want to help the elderly then do not become an aide!!!! Aides were always on their cellphones, barely spoke to any family members, slammed doors & drawers!! Our loved one was afraid and that is just not right. Very pleased with the level of care provided at this Care One facility. Our family member spent 6 weeks in rehab at this facility and we were very pleased with the care received. The place was clean and bright and the staff attentive and pleasant. We were kept up on his progress and medication management . Everyone from the front desk reception to the nursing staff was friendly and kind Discharge was quick and smooth . The only down side was dietary and the quality of food. Quality , selection and accuracy of the food ordered was awful . But if you need to lose weight during your stay this is the place for you . read more

The Front Desk is the worst, Devon specifically . Unable to communicate with staff @ facility with the exception of Michelle. If you care for your loved one never send them to Careone @ Madison Avenue. They do nothing to help the family except give them an ulcer & high blood pressure with their ignorance and lack of care. No one seems to be there at night so the phone just rings & rings. No supervision to oversee staff at night. I wondered if I should call the police for a welfare check being that no one answers and not sure if the staff is there. Spoke to Emma from first floor who just transfers to no one on second floor. DON'T TRUST THIS PLACE I honestly believe they keep patients there just for the money. read more

This place was awful, do not put your loved ones here. Our parents were in hospice and the memory care area. On the outside the building is lovely but the place is a huge disappointment. Our parent was given a room right next to the desk area downstairs and when we arrived they were storing boxes and other things in the room even though I asked every day to get the boxes out of there, it was never removed for the entire 2.5 weeks. He was constantly wet and hardly changed and always so thirsty, and very dehydrated. They just forgot about him because he could not speak any longer. The aides were shameful and the only nurse was a day nurse who was caring was Sheryl. I was setting him up to come back home and let him spend the rest of his time at home and he passed the next day. He died just after 4 am and I was given called by an aide and it was, as a matter of fact, so cold. I called the funeral home at 5 am and by 7am I had an annoyed nurse's aide asking when he was being picked up. I advised her that I called and they would be there between 8-9 am. They called me back at 845 to say no one had picked him up, I called the funeral home and they arrived within minutes they were actually at the back of the building, they were so horrendous and bizarre. So many disappointments in the end, the only thing this place wanted was his documents so they could bill Medicare since it was hospice. This is the worst regret I have to have selected this place, they presented themselves as caring and dependable, and I could not have been more wrong. What a regrettable experience. All the staff members at CareOne at Madison are superb! Always fully staffed and the aides/therapists are all caring and responsive. I feel that they all go above and beyond to help others and are always willing to accommodate all needs. The facility itself is beautiful and there's always plenty of parking by the fountain or in the garage below. Semi-private rooms are spacious and are always clean! Horrible care at this place. No one took care of father in law, or his room. Had 2 urinary infections while there because his Depends were RARELY changed, ended up back in hospital because of it. Moved him out of there before discharge because we were NOT leaving him there. Administration was not hearing out concerns. A family member visited EVERY day for over a month, you would think he would be getting the best care because we were clearly watching. Imagine if we were there every day to SEE!?! read more

Beautiful place - TERRIBLE CARE! This facility looks beautiful and I wish I could say the same about the quality of care. The day staff was attentive, but the weekends and evenings were another story. Consults by specialists were promised for WEEKS and never done. I watched my loved one deteriorate medically, emotionally and cognitively while in their care. I called the on call doctor 3x regarding a medication mistake and never received a call back . They are terrible communicating with family, even though the patient was cognitively unable to advocate for himself. They should be ashamed of the poor care they provide! I spent 5 weeks at Care One Madison ave. I had been in a Wheel chair for 7 months. The physical therapy and occupational therapy department is superior to many that I have had in the past. They had me walking by the end of my stay. the staff, from the young lady at the front desk, the woman that cleaned my room everyday, the kitchen staff, my nurse that took care of me everyday, they were all so kind, caring and compassionate. I was required to spend three months here after a hospital stay. Here are the highlights...excellent physical therapists who give their all; cleanliness of the facility; clean linens and bath products provided; whellchairs and walkers top quality; nurses' aides in general are hard working and polite and helpful; social activities are many and varied. Free nail salon, staff hairdresser is competent and very patient. Dining facilities are lovely when eating in DR- in room service leaves much to be desired.Doctors assigned to me were prompt, caring and I have retained all of them since discharge. The negatives: some staff are rude and heartless. When a complaint was lodged to director of nursing, little to no resolution was reached, just lip service. Security is non-existent. Night nurses are either 100%effective or go off to other floors to meet colleagues. Some of the residents are not of honest character so valuables and money are not to be housed here. Everyone seems to want a title here but do little to earn it. Administrators are rarely seen and have no contact with residents. read more

My grandma was in this facility for several weeks. While many of the employees were kind, one woman employee took it upon herself to harass and harm my grandma. This employee hid my grandma's shoes and laundry, improperly removed her IV which wasn't even a part of her duties, turned on lights and spoke at maximum volume before sunrise, and came into her room even when she was banned. This kind of behavior is shocking and it's truly appalling that they would allow this to go on for so long. None of these facilities has a good rating and the reviews are hair-raising. read more

Due to Covid 19 I couldn't be personally there so I trusted this facility to care for my brother who recently had a leg amputation and other injuries, and was completely dependent on nurses. In 10 weeks that he spent there, he wasn't taught anything in occupational and physical therapy. Nita, the director of rehab, was sarcastic and almost mocked my brother's disability. Niel, director of nursing, told me to "chill" once when they kept screwing up my brother's transport schedule and pre-op medication. He was always short with me on the phone. I was shocked at the unprofessionalism of senior staff. Briana, social worker, simply ignores your phone calls or text messages. Food was disgusting. My brother hasn't had a shower in 10 weeks there. Do you know what it does to a young person that just suffered a loss of a limb and can't take care of himself? It crushes them mentally. I was glad that I brought my brother home earlier then rehab suggested we keep him there. They only charge your insurance, and don't provide the care that they do beautifully advertise!!! I don't know how people trust to leave their loved ones for long term care at that horrible facility. It's my honest, from the bottom of my heart opinion. read more

Sad is an understatement! Twice now during the nighttime someone has come into my Grandmother's room and taken things from her. We mailed her a huge box of treats and snacks. The overnight staff took what they chose, even the wooden crate is gone. Last night, a gift basket was rumagged through. She hadn't even opened the basket, yet it is now open with items missing. If this is the standard with a patient who is completely aware, how are they treating someone with less capacity. read more

My grandfather had a TBI and stayed here for months, and it was such a terrible experience. Nobody took care of him, we did not feel comfortable going more than a day without visiting him because we had to advocate for his basic needs to be met. Social workers, administration, and head nurses alike all gave him the bare minimum of care and were rude and ignored our concerns. One therapist, Courtney, was amazing and caring, but we were incredibly upset with just about every other staff member. Their lack of care landed my grandfather back in the hospital with multiple UTIs, just before he was set to move into an assisted living facility with his wife, and caused alot more complications for him and my family. They didn't have to be perfect, they just had to care for my sweet, friendly grandfather who did not deserve to be treated the way he was. I don't recommend ANYONE take a family member here, especially if they have any loss of cognitive functioning- they will not care for you! read more

I had a good experience when my father was at this facility. The good: the social worker, Brianna, was wonderful and would always keep me up to date on what was happening with my dad. She really spent so much time making sure i felt heard and ensured my dad was taken care of. The nursing staff was very kind to my dad. The bad: My father was very bored when he wasn't in rehab because there is nothing for him to do. I was told there is an activities department but i do not know if they actually have anyone employed in that department. I was very disappointed with that. read more

NEVER EVER allow your loved ones to stay at this facility. Care One nearly killed my very elderly father. This facility is badly understaffed and the quality of care is very low. The staff ignored his calls for help. (He fell and hit his head when no one responded to his call for help getting to the bathroom and he ended up back at the emergency room.) They dropped a tray of food on his side table and didn't care whether he ate or not. When we asked why no one helped him, the response was, He eats it or he doesn't. They simply didn't care. The staff administered medication he was allergic to, blaming it on confusion among the physicians. Dad developed hallucinations and hospitalitis and we thought he wouldn't survive the experience. As an aside, the upholstery on the chairs in his room was filthy and looked like it hadn't been cleaned or updated in years. We signed him out and after a few weeks of excellent care at his assisted living facility, he's doing well. Just DON'T. Care One is an embarrassment to the nursing and rehab industry. I think the facility should be shut down and I've written a letter to the state attorney general's office to let them know. read more

My Husband was admitted to Care One in Morristown after one month in ICU/Hospital and Acute Rehabilitation facilities. I was scared and distrustful after bad experiences at both previous places. I was in for a wonderful surprise. I was met by a team of caring, well qualified Nurses, Aides, Managers, etc. I actually had people LISTEN to me and HELP my husband. He went in there barely able to walk or feed himself. He was not able to get dressed, write or even carry on a normal conversation. Within three weeks, he was able to all those things and more. He was on the Lower Level (which is the Dementia area) and the staff there were amazing. They cared! Joan, Frederick and Amine are a few of the kindest people I have ever met. The food is absolutely horrendous and the kitchen staff NEVER puts what is ordered on the tray. I feel very sorry for anyone who can't be their own advocate in this place. The worst place you could ever leave your loved ones!!! Don't, just Don't, you'll live to regret your decision!!! They sell you like a used car lot with nothing but lies!!! They are completely incompetent, putting the patient last. They're extremely understaffed, having a direct impact on the overall well being of their patients. The facility is no where near cleanliness standards. When you reach out to them in regards to your loved one, they don't get back to you for days, sometimes a week!!! No one goes the extra mile in caring for the patients, they do the bare minimum and no more!!! I will forever regret not pulling my mom from their care. They would rather have the poor souls mess a diaper, taking away their dignity, then walk them to the bathroom. They never just pay them a casual visit to say hello, ask how they're doing, so they don't feel so alone, especially during these trying times of covid. My mom would use the call button and would have to wait great lengths of time before anyone would respond. She couldn't get out of bed on her own, yet they would move her tray far away from her bed leaving her unable to get her water, phone, tissue and other items for long periods of time. I like others call it Care None!!! They don't deserve to take care of any of our family members. Please Heed My Warning!!! I Don't want anyone to live with what I have to live with. Do your do diligence when finding a rehab center for your loved one!!! Ask questions , lots of them, all kinds, relating to the care your loved one will need. Set a goal for the length of time they need, then get them out. If for any reason your not satisfied with how things are going, go with your gut, its telling you something, there are no second chances, take them out find a better solution. I lost my mom, and i truly believe it was directly related to their negligence. My life is forever changed, I am broken hearted. I hope some day in the not to far future, I hear they've been shut down!!!! No human deserves what they have to offer!!! read more

Do your lived ones a favor and just say no to this place. It was suggested to me as a good facility for my mom while undergoing palliative radiation. The most hanous of actions and what causes me to write this review is the fact that moms condition was changing..they did call their doctor who ordered IV fluids, but ended up transferring to a hospital. You may ask why I consider this to be the last straw and this is why .. Even though I spoke to the nurses at the desk for 2 days in a row several times a day because mom wasn't answering her phone or calling us. Back ( we live an hour and a half away and I had taken a few days to get ready for the new school year) I was told she was just tired from radiation...they never called me when they suspected a problem and did not call me until an hour after they sent her to the hospital. When I asked why mom's nurse said he was busy. When asked why I wasn't called when she took a turn his response was this is the first time I've taken care of this lady. When asked why they took her to a different hospital when they took her every day to another hospital for treatment his response was.. If no one says differently we take them to the nearest hospital. Exasperated I said if you had called I would have told you. I asked to speak to nursing supervisor and after waiting for some time the nurse returned to the phone and said the supervisor was busynd would call me back. It is now almost 3 am and I have yet to receive that call... Last Thursday they requested a meeting for discharge planning..we held a conference call..During that time the OT said mom was in pain so after one week there with pain meds ordered, no one was giving them to mom prior to transfer to radiation or PT.. I have been a nurse for 34 years... Pain management with a Percocet is nursing 101... They also couldn't tell me if mom was receiving insulin or not... I told them several times radiation was closed on Labor Day. So what happened on Labor Day??? Yes they transferred her to radiation...Give me a break..Don't buy into the hype that they have an Oncology Unit... I only agreed to let her stay for the 2 weeks of her treatment and now she's in a serious situation due to their negligence . Keep your loved one home and hire a Nurses Aide to help... This place is pitiful. read more
