Center Lake Ontario recently started taking patients on Sunday and what a wonderful thing. I did not have to request time off from work. I was able to schedule my appointment quickly and get in there for my procedures. I had exams that are typically "scary" but the technician-Jasmine was so patient and kind, too her time, reassuring me and giving me options if I got too anxious. From the minute I walked in on Sunday the receptionist Cindy was pleasant and accommodating to all my needs. I've used this location many times in the past but this time I felt I needed to compliment Cindy and Jasmine who were exceptional and to highlight the benefits of the weekend schedule! read more

This is my 2nd appointment canceled while in the waiting. The first time the tech was so late that I eventually had to leave. Today, I was scheduled for a 5:45 am appointment at approximately 6:30am I was informed that the tech had an emergency and would not be in, I need to reschedule. I would give negative stars if I could. They rescheduled me 3 times then just canceled my appointment. They lied and said they were waiting for pre authorization from my insurance. I don't need it, I spoke with insurance. I even 3 way called with insurance and center lake and they still wouldn't schedule me even though my insurance company told them repeatedly I didn't need preauthorization. Carmen a director has horrible customer service and needs to be fired for her attitude. Never will use this company again. Stay away and go anywhere else. They are liars, i will Never use them again. Hello. I have called over 10 times, in different timing and still no one has answered my call at all. I was calling them to just confirm my appointment and everytime I called them, I wait over 20 minutes and still no one answers at all... I'm not sure how there are workers there and no one answers phone call... very bad experience that still till now, I cannot confirm my appointment so I think just best bet if ever wanna confirm appointment, you must go to office in person and ask for that because they will never answer you nor give you a reminder of your upcoming appointment. This review is not specifically about the Ontario location. It is a breif ac ount of my last experience. I have an appt. today but I didn't to confirm purpose to driving out there. I canned the Chino location over 7 times, going through every possible option and could not speak to anyone or even leave a message. I them called 3 more locations multiple times until finally getting someone at the Ontario location. I have to say this was just ridiculous. Drove my father in for his appointment, staff was very supportive both in person and over the phone. Per usual doctor visits come with some wait time before coming in to see the doctor but everything was great. This location is very clean. All in all I'd recommend this office, I'm huge on customer service and they did amazing! read more

I was on hold for close to 30 minutes - transferred over. I wanted to make an appointment and was told that I needed to email the requisition & then call back tomorrow. I was already on-hold, and when I told her I could email the document right now. I asked for her name, she hung up on me and I went directly into their survey. Unprofessional, rude and shouldn't be representing the office on phones as she is the first BAD impression that the patient sees. Will share this with the referring doctor. I have been to the office in Ontario twice for a mammo. The first time, no one greeted me at the front desk, I felt like I was an imposition just being there. Then they kept me waiting way past my appt time to eventually tell me the mammo machine was not working. And then blamed that on Covid....4 years after Covid hit! The 2nd time was a reschedule of that fiasco, again the front desk was less than welcoming. I was asking to undress for the procedure in a tiny room that opened directly in to an office area where some dude was working. Uncomfortable! THEN during the procedure the machine stopped working! I've been to this location twice in the last year for an MRI and both times were absolutely exceptional! All the staff are super helpful, friendly and patient. They went above and beyond to ensure I was comfortable during the scans and that I had immediate access to my results. read more

I am writing this review from the waiting room of this facility. I had a CT scan scheduled for 1 p.m. and was told to arrive 15 minutes early to complete paperwork. It is now 2:25 p.m. and they still haven't even called me in. This is unacceptable, especially since I was told I needed to fast so I've had no food or water all day. All this after I was originally supposed to have an MRI which I scheduled 2 weeks prior, only for them to call me the day before and cancel it. Doctor referred my wife here in 2023 for a 3D scan (specifically 3D on the work order). Centerlake Ontario didn't have a 3D scanner so ignored the doctors order and used a standard 2d scanner. Doctor rejected the scan, re-requesting the 3d scan. Centerlake said their only 3D machine was in Anaheim but they agreed to rescan at no additional charge. My wife and I took another day off (without pay) and made the drive all the way to Anaheim. Centerlake then sent a bill! I called them, they apologized and said there would be no further charge. Over 2 years later we just received a collections notice from a debt collector! Can you believe it?! read more

I made an appointment weeks in advance and took off work only to have them leave a message the night before to say they had cancelled my appointment because they never heard back from my insurance for pre certification. 1. They had weeks to get it done, and that's literally their job 2. and most importantly, my insurance plan doesn't require pre certification. Total incompetence! read more

I arrived at 8:01AM which was before my appt and was told by the person at the front desk that they do not have X-Rays today as it is blocked out. They only take Saturday X-Rays every other Saturday. I then asked the girl at the desk, why they did not call when you all knew this prior to today and and that you guys were scheduling me over the phone... She said, "I DID NOT SCHEDULE YOU !" I replied , I said, you guys" (I knew this office visit was going South with reply reply). She would not reply on why no call was were made to me when the Appt clearly has a "X-Ray" noted on it (per Supervisor on Monday). I then asked for a note on my showing up and reference #, but she replied that we don't do that. I asked for the Supervisor or Mgr..., She replied that only Tech's on in the office. I asked for someone to come and talk to me as I will be outside. To make it short.. - No one came to talk - No one called me - Had to call back three times on Monday via the scheduling team to get a person and report this, with a follow up to me by end of day. But 4 days later, no call. - I called my Dr and Medical group and reporting this company and to take notice of the many reporting regarding this Ontario office. read more

Update- As I await finding out my biopsy results for breast cancer I blame Center lake for blowing me off and cancelling my appointments for 12 weeks. Their delays could very well have cost me time in earlier detection. This company should be shut down. read more

I've been trying to schedule an appointment for an MRI. First of all; the online appointment scheduling tool doesn't work at all! After entering all the required information and clicking "submit", nothing happens. I've tried at least six times to schedule an appointment using the phone system with no success. Each time I was on hold for at least 20 minutes without being able to connect with a representative. One time after being on hold for 26 minutes, I was transferred to a voice mail inbox and was greeted with "sorry, this mailbox is full, goodbye!". The same thing happened when I selected the leave a message option from the automated phone option menu. This is, by far, the WORST experience I've ever had trying to schedule a medical procedure! Also, I've noticed that those supposed responses from "Mike M" are just automated auto replies that mean absolutely nothing! You can see that all of the responses are worded exactly the same. I just tried again today to get through to schedule an appointment with no success. RIDICULOUS! read more

Out of your 10 southern California locations this company only has 2 locations with a Petscan!! Downey has no technician. Ontario's machine broke in July 2023 and my mom's appointment was cancelled. Rescheduled for tomorrow August 3rd. And just got a phone called from the Ontario location that there will be no technician tomorrow and they were cancelling her appointment again. This is outrageous, unsympathetic, uncaring and a dangerous company who treats cancer patients as unworthy human beings. Cancer is a serious disease that deserves immediate care for patients in need of living. What kind of company treats cancer patients as if they just have a cold!!! Where to begin if I could give a negative star I would. Scheduling an appointment is a joke. It took me a few days to finally reach someone. Phones just ring then get disconnected I was scheduling a mammogram along with a breast ultrasound and the person told me I did not need an ultrasound so she will not schedule me. Hold on since when are you my doctor? If my Dr ordered both a mammogram and ultrasound who is she to tell me I didn't need one? I then Scheduled for Ontario only to be told day before it was scheduled in chino. I have been coming here for years but the last year or so centrelake has gone downhill. I have called this place for 4 weeks to get an appointment and no one answers. It rings, puts you on hold, and rings. It doesn't matter what time of day or day of the week. I even sent a fax and have a confirmation of receipt asking they contact me, but of course, that would require someone who cares. Now I try to schedule online and it says my form can't be submitted. Great service with x-ray imaging. Unfortunately, the TV in the lobby is set to the news and my entire time in the waiting room was a political rally for one candidate. I'm sure that particular candidate pleases a lot of people but I don't feel a medical office is the right place to hear either pro or against. I have been calling for two days to schedule a MRI. On hold for 10-15 min, then sends you to voicemail which is full. Two days!!! How do you stay in business? I guess people just die waiting on a MRI. My doctor had this approved urgently which doesn't matter now if no one ever answers the phone. Will be reporting this clinic to the medical board and to Medicare since they determine who can stay in practice and be part of insurance plans. No one answers and it seems to be a constant issue. Came in for an x-ray, which is a walk in basis only. When I asked how long the wait would be, I was told there was 2 people ahead of me so it would be about 30 minute wait. I actually ended up waiting 1hr and 30 minutes. I understand that it's a walk in basis and there will be a wait but I'm upset that they did http set realistic expectations for the wait time. If you end up going here, just be aware that the wait can take 3x longer. read more

If I could give less than one star, I would! This establishment NEVER answers their phone. They also charge you when you arrive for your appointment, but do not have the MRI done. So far they have taken $100 out of my account. I've been trying to get a hold of somebody for over a month and a half... Left numerous voicemails, and call pretty much daily... It's getting very old. I would really like someone to contact me to discuss this matter before I have to take greater measures like small claims court. read more

Tried calling several times to schedule an appointment for a CT scan. I was transferred and put on hold numerous times. The representative who answered the phone greeted me in Spanish and was extremely difficult to understand, due to his heavy accent. I scheduled the appointment online and received one voicemail asking me to call their office, which forced me back into the call transfer loop that I tried to avoid with online scheduling. I have yet to schedule a date for the scan. Centerlake customer service is extremely poor. I'm going to look elsewhere for my medical needs. read more
