    Plenty of staff but most seem to be unconcerned for the residents. The best aide are the ones that come from an agency.

My mom was a diabetic who was staying there because she had a fall and broke her hip, she was only 46. My mom would need help changing soiled pants and she would be left there for hours at a time while nurses would sit there and do nothing at the nurses' station. There would be times I would go to the nurse's station, and I would even ask them if they could come to my mom's room. I was even told one time "It's gonna be awhile" and she just sat there on the computer. They have no sense of urgency. They are completely unprepared to have diabetic patients. I walked in to visit my mom to find her in a state of Hypoglycemia, moaning, groaning, and completely out of it. I had to call in a nurse who claimed "She wasn't gone for that long" my mother was a sensitive diabetic and they knew that but left her unattended after my mom had told them her sugar was getting too low. For about an hour they were running around trying to find something to bring her sugar up and they COULD NOT find anything because no one knew where the needed items were, they were frantically running around telling me my mom would be fine, and they handled the situation unprofessionally after they were the ones that caused the situation, they ended up having to call 911. This was only one of 3 or 4 times this has happened. Two of them I was there for. In any bad place there are always good people though, the people in the cafeteria were always so kind to me and my family and the front desk people as well, just be aware that the cons weigh more than the pros with this place.

My family has been sitting in a dark room daily because the ceiling light needs to be replaced. Their response is to say they don't have maintenance to fix it. They "forgot" to order their powerful pain patch, and they are in excruciating pain, probably going through withdrawal. They keep saying their pharmacy is in Indianapolis instead of making arrangements to get the medication locally. The meals are tremendously lacking. One meal was a tuna salad sandwich with nothing to drink. They obviously don't have a dietitian on staff to ensure the patients receive proper nutrition. The call button goes unanswered for a long time. When I walked to the nurses' station, two staff members were sitting at their computers. People were obviously available to respond. It is sad to have a beautiful facility that refuses to provide the most basic requirements.

I asked for his food to be cut up, and it never happened. They did not follow doctors' instructions for care of the ankle .He saw his surgeon on Feb 22 and sent instructions to have the wound cleaned every two days then on Feb 23 in the evening, the aid could not get his walking boot on I said I don't think yr suppose to force it on but she did. Then, on Feb 27th I got a call they were taking my husband to the ER in Michigan city because his wound opened and the hardware was exposed. I called to complain, and they said the doctor sent him back that was I was there it was fine . Then she said on Feb 23 that it was open but waited till the 27 th to call me as a result he developed 3 infection s and is on antibiotics IV 3 times a day and required a second surgery . One shower in 20 days, the most uncaring people I have ever encountered, the nursing director and social worker, were as rude as I have ever encountered. So if u have someone there or know someone, check on them ask questions .

They present well.....Starbucks, Grand Piano, bar, nice dining room etc. They tour well. But watch out. Once you commit, then the real Symphony "kicks in." My Dad went in for PT and came out worse than when submitted. I don't know if he will ever recover. He is now in skilled nursing in a new Sr. Home. Symphony's systems are horrible at best. Health care is based on inconsistent schedules and one shift doesn't know from the other. Administration does not know what is going on for sure. The two young social workers who are suppose to be the conduit of information are worthless. I was to get a report/update on my Dad every Tues or Thurs. I got one call in three weeks. I had to track them down. The transportation missed an appointment that we had schedule for 10 WEEKS!!!. There is very little social engagement on the C side (Therapy). And God forbid the patient pushes the button for assistance. It would sometimes take 30 minutes for someone to acknowledge. SO DON'T JUDGE THIS BOOK BY THE COVER. BECAUSE IT IS A HEALTH CARE NIGHTMARE. DON'T SIGN UP FOR THIS NIGHTMARE. P.S.: Interesting to note that once I got my Dad out of Symphony and told other medical professionals of my move....I was told that "they had heard nothing good about Symphony."

I've never felt so much at ease than I did with them. Their bedside care are impecable. For the first time in 20 years I felt seen and heard. My PCP is also part of Northwest Health and I can't say enough good things about him. But these three doctors were on top of everything. Always communicating. Making me feel better. I suffer from terrible CPTSD and they went above and beyond to make sure I was doing well and better. In the past I never liked this hospital due to previous experiences but now, thanks to Drs. Choudry, Reddy and Singla I will go back again and again. If I find their practice or specialty I will go to them without any hesitation. Hospital impecable clean. Nurses empathetic and techs always asking if there's anything I needed. Best experience of my life.

I called my insurance, and they told me the second authorization request was never submitted. Bet! The doctor's assistant went to bat for me, and once the paperwork was submitted it was approved. Meanwhile, the prior authorization manager LIED and said she personally asked the doctor to do a peer review with insurance and hit it approved for me. The doctor and the insurance company both said that didn't happen, and peer reviews only occur if they are appealing a denial, Their intake paperwork states that patients are responsible for obtaining all prior authorizations, never mind that we lack the resources to submit paperwork to do so, the chief operating officer dismissively told me it doesn't really mean that, but apparently it does. Patient beware!

The actual rehab was great got me back up on my feet and able to function and I was able to come home and be able to take care of myself with no problem. The food at this facility is pretty good I don't have any complaints there might have been one meal I didn't like but everybody has their own taste. Things are never going to be perfect at a rehab facility. Most of the time they're spread quite thin they got a lot to take care of and you have to be more understanding as a patient. If your needs are more than what they can handle then you might consider going to a different facility where you get more attention. The job of these people is to get you back on your feet and make you self-sufficient. I would highly recommend it my mother was in there for a period of time also and they were able to do wonders with her and she was 85 years old and get her up and walking too.

We were charmed by the looks of the facility and the assurances provided by the person who gave us the tour. The supposed services they provided both to the patient and the patient's family. It was not only a total disappointment, but it was actually dangerous in my opinion. My sister had intense pain and horrible swelling in the leg that had surgery. They did nothing about it until my nephew continued to ask what was happening. They checked for a blood clot and did an xray but took no further action. There was supposedly a doctor that saw her but we never heard anything regarding his visits. One of things we tried to do was get her to a follow up visit with the surgeon. Not only would they not help in facilitating that, they insisted that they could take an xray and send it to the surgeon. They took the xray but the surgeon has never received it. They told her to pee herself in the night because she was too hard to get out of bed and they would change her in the morning. She was moved to a different facility for insurance reasons. Within the first 24 hours the new facility did a skin assessment due to the swelling in her legs. They have an orthopedic doctor who will look at her leg. They will arrange a podiatrist to see her feet which was something we requested multiple times at Avalon and it never happened. Add to all of this, when we knew she was being discharged from Avalon we requested a call with her discharge instructions and the plan of care they were forwarding to the new facility. Despite promising that they would, they never provided that. The morning she was moved, the new facility didn't' know she was coming. Avalon didn't provide the pain meds or let the new facility know that they needed to get a script from their doctor. They never provided the information on the physical therapy she had while at Avalon. The only good thing I can say is the Avalon nurse that morning gave the most comprehensive care and attention that I saw anyone give in the entire time my sister was there. They consistently didn't comply with the most basic of promises. Such as there was supposed to be a weekly call to go over her progress. We had one call the first week she was there. We never had another call. We could never find anyone to ask about her care. If we did manage to get in touch with someone the general answer was that person didn't have access to that information. They would have someone else call back. We are still waiting. My nephew found the head nurse one evening. She was totally uninterested, provided no information and made it quite clear that she was completely irritated that he had asked her questions. Don't be fooled by the pretty building. This place is horrible.

I arrived on my first appointment with a prescription from my doctor for medicare-covered machine-therapy (iontophoresis and ultrasound) plus PT. After a 1-hour evaluation with a therapist, only THEN was I informed they do not offer the medicare-covered machine therapy prescribed by my doctor. Three different people eventually told me that my prescribed medicare-authorized treatment was not supported by medical research and they recommended laser therapy that was NOT covered by medicare and would cost me hundreds of dollars for a series of treatments. I was SHOCKED! I asked ... you mean a medicare-authorized treatment is NOT supported by medical research?! And you cannot provide me exactly what I came here to get?! And you waited until AFTER a 1-hour evaluation before you told me all that?! I was in total disbelief and felt terrible because of all the subtle but polite pressure that was put on me when all I wanted was what my doctor had ordered. Why did they need to make me feel so terrible? I just couldn't believe it. After I asked my questions, the "manager" was consulted several times and I was eventually informed that they COULD do most of what was prescribed by my doctor and it would all be covered by medicare. I felt trapped since I had already been there one hour so proceeded with the treatment. This is where the 5-star rating starts. All hands-on people were OUTSTANDING and the full course of PT fixed my issue 100%. My advice is to be very cautious if you are recommended treatments NOT ordered by your doctor. Otherwise, an excellent place.
