Today we visited a loved one in the facility. We met JB who is one of the staff members caring for our Uncle. JB is respectful, attentive and caring. We feel very fortunate to have JB as a caretaker there. The staff is very polite and helpful. The environment was very bright, clean and festive. It almost feels like home. The beautiful breathtaking view outside the window is quite uplifting and gives a sense of peace. Unfortunately, my Uncle had a stroke but was fortunate, to get a month's care at Pacifica Nursing & Rehab after being released from South San Francisco Kaiser. Facility is clean but older (3 floors) with a shared room of 4 patients (Room 304) with drapes for privacy...also, when the elevator takes awhile and it's only 3 floors is telling. Staff all very helpful, friendly, professional and caring. Activities and snacks in the community room if you're lucky to be mobile. Menus and activities are always posted. No issue with his stay and care (except noise which is to be expected) BUT wish my uncle could've stayed longer to get the daily physical therapy he needed. Unfortunately, Kaiser "claimed" they didn't see improvement and released him (which seems to be counterintuitive!) even after appeals. Smh. I wish I could name all the staff members who cared for my uncle. Basically, 3rd floor staff from the nurses, CNA, interns and admins were always proactive with his health care needs and truly compassionate people. Thank you! read more

Even though Cigna confirmed they are in-network, Pacifica refused to take me as a patient because they said Cigna hasn't paid them for previous claims. I'm a patient, not a billing department. This is an older facility, but had everything I needed. I shared a room with 3 others with curtains separating us. They also have private rooms. That said, I could not have asked for more. The staff is wonderful. They were very sensitive to my pain, were very caring and kind. The CNAs asked frequently if I needed my diaper changed and did so very willingly. The nurses scheduled all my meds. Someone came to discuss my discharge and what medical devices I would need at home. I had a button to press if I needed anything and was encouraged to use it. The pt and ot care was excellent. I received it twice a day during the week and sometimes on Saturday. They had a good class on fall prevention (had hip surgery). They helped me prepare to go home. With natural healing and their help, I made excellent progress in a short period of time and exceeded my expectations. Meals: I thought the food was tasty and well prepared. They hand out a weekly menu with an alternate menu. The dietitian came to my room to discuss any dietary restrictions and my likes and dislikes. They prepared meals accordingly. I was very happy. I would recommend Pacifica Nursing and Rehab without any reservations. I also credit the hiring manager for hiring not only a competent staff, but a compassionate one. Good care from incredible people. That is here. The staff is beyond amazing- a team of new enthusiastic professionals and older seasoned staff. Every single person I have met is outstanding. Kind, respectful, thoughtful and delightful if you take the time to get to know them. The food is institutional- I have dietary restrictions and my family brings a lot from home. But the care, the PT and OT - everything that feels important in this situation is here. And it's clean. Very nicely maintained. There are occasional communication problems- but after ten days in hospital they're actually doing a better job coordinating care. GREAT care from start to discharge but you do NEED some intervention. My 96 year old mother fractured her hip in March 2025; had a partial hip replacement and was transferred to Pacifica Rehab. I did check out a few facilities but it was not up to par such as the "smell" in the facility, lack of organization and somewhat chaotic. This was not the case at Pacifica Rehab. Things will be moving very fast and staying on top of rehab is crucial. Have your "team" in place such as have your family members establish a schedule to visit and assist in the care. Here are some suggestions and our experiences: Connect with the Discharge nurse coordinator as soon as possible. Discharge coordinators meet every Wednesday. Discuss with them available resource for services after rehab such as long term care facilities and medical devices needed at home. We were able to arrange transportation from Pacifica Rehab to MD follow-up and then home upon discharge. Make a podiatrist appointment with nursing staff if needed. If this service is not covered by insurance, it was worth the $80 cash pay to the podiatrist Find Out Expected Discharge Date: Think and ask what services you might need after discharge. Purchase medical supplies you might need upon discharge Pacifica Rehab Plan: Set up a Calendar for family member times to visit and help in additional rehab - such as walking or wheel the patient into the activities room. We did a "mini spa" day; with styling hair and nails. This put a big smile on our mother's face and lifted her spirits up Day 2-5 after surgery: patient may be loopy from meds and may need more attention for safety of the patient such as fall risk. We notified staff about this and our mother was put in a room closer to the nurses' station Advise nursing staff or occupational therapist to have the patient dressed (tendency is short staff may result in putting a diaper on the patient and neglected. At times, you need to let the nurses know the diaper needs to be changed) Staffing shortage - there is a tendency to have the patients in diapers and not changed into regular clothes. Inform the nursing staff if diaper needs changing. Shortage tends to be worse on weekends. Use the activities room - have the nurses take the patient to the activities room to have lunch or participate in the day's activities such as bingo, drawing, or watch TV so the patient will get out of the hospital room. This is also a good place for the family to bring patient for a change in scenery. Physical Therapy/Occupational Therapy: 1 time a day each Monday to Friday; none on weekends. Advise them what patient was doing before rehab. Have OT help with dressing so patient is wearing regular clothes during hospital stay. Establish goals with PT/OT: such as return to home and be independent with supervision. We LOVED our PT/OT partly due to goals established and what was needed to get my mother back home. Their care was also outstanding and caring. Meals: review menu and substitute items from list that patient can eat. Have patient sit up in chair or full upright in bed to decrease chance of aspirating the food Family: get involved with the care such as walk the patient and have the patient move to increase endurance and ability to go home; otherwise the activity will be minimal! Remember: You are PART of the "team" and getting involved helps in the recovery of the patient. My mother had incredible care from Pacifica Rehab. I hope this will help guide you through the process for a smooth transition because things were moving very fast in the 10 days our mother was at Pacifica Rehab. And - we were able to get my mother back home with care from the family rather than have her go to an assisted living facility where she would not be able to thrive. read more

I spent over 20 days in the care of this facility. Truthfully, my reception there was not okay. Once settled in, however, I was extremely impressed with the level of care I received. Granted, compared to the average patient I was young. 57. I found the staff, even when understaffed to be consummately professional and accommodating. My care was minimal compared to my roommate. They were amazing with him always. The facility is very clean and warm. It does not let off a hospital vibe. More homey. The interns are sweet as well. The food can definitely use some help. My staff on the second floor made me feel cared for and my physical therapists were attentive and very helpful. I highly recommend this place. Though it's safe to say no one wants to need the services of this facility, I want to review the many elements that made this rehab facility 5 in my book. The facility was adequate but what can you say about rooms and hospital beds that is special. All 5's are given due to the care and respect given to the patient from every member of the staff. I'm conflicted to write this. Sad this is supposed to be one of the best SNFs in the area. We were glad to this place seems promising based on the reviews. We're still recovering from the shock of the reality. Beware if you send your love ones ) to this place. Be sure to know all your rights. Be attentive and check often. Read the glowing reviews but DO NOT miss the one and two star ratings. That's exactly what happened to us. Be vigilant and do not be so trusting with what they promised you. This place is lack of transparency and their staff coordination SUCKS so ASK ... actually DEMAND everything in writing and do not sign anything they asked you until you're absolutely sure. Always follow up yourself with your loved ones' needs, This place is under staffed and do not care what patient's individual need. Some of the Nurse/CNAs like Juwana and Diana are great. They worked hard and compassionate but I can't say the rest. They let your loved ones' needs unattended. The self proclaimed "guardian angel" social worker is insensitive, needed training -doesn't seem to understand patient privacy wanting to discuss paperwork in public area where there were lots of other people around, also lack of care and advocacy for patient's best. I had to keep telling her to STOP talking. Sorry to be rude I don't want to discuss in front of other people. The coordination in this place is awful. The left doesn't communicate with the right. The administration is disorganized. I had to answer the same set of questions at least five different time to five different people and they still didn't get things right. They are either very defensive or non-responsive even when you are asking basic questions or status on progress of your loved ones. Take what they promised with grain of salt. Bottom line, yes, it's good to have this SNF but always be vigilant especially if you or your loved ones are being cared here while cognitively impaired. read more

The people here make you feel like you're part of a family and not just a patient. The atmosphere is positive and cheerful and they really care about you as an individual. When I was first admitted I couldn't walk and move without a lot of help. I was discharged 4 weeks later walking out on my own power with just a cane! Imagine a hospital so good you are reluctant to leave! Ice cream and apple pie, a truly caring and highly competent staff, an equipped gym for the Physical Therapists' use to speed healing--that is how we found this facility to have earned its five-star rating. The State evaluates such facilities every year and awards the stars. The state's five-star plaque hangs on the first floor wall. Is the food good? YES! For the family, there is a lounge available for you; it's where the patients get organized recreational activities. Also for the visitors, note that there is a variety of restaurants one block away, Chinese, Mexican, MacDonald's, also a Walgreens. Some of the rooms have ocean views. The heart and soul of a Nursing and Rehap center is the staff. My wife got a in-bed shampoo within an hour of arriving, despite three fractures, she got frequent showers--everything done with a smile. After day one it all was like family. I am rarely surprised by first impressions. After my wife was transferred from the Mills-Peninsula Hospital to the Pacifica Nursing and Rehab Center (PNRC) after they repaired her broken femur, I was underwhelmed at my first sight of PNRC. Mills-Peninsula has a modern facility with the latest tools to fight disease and injury, and PNRC is housed in an older building in a residential section of Pacifica. As my wife was wheeled into PNRC, I wondered why the referral staff at Mills-Peninsula insisted that PNRC was an excellent place for a rehab facility. Then, I met the PNRC staff. Wow, you cannot judge a book by its cover. The entire staff, from the facilities maintenance group to the therapists, were top-notch. My wife was at the facility for about a month, and there was not a single day that I met someone from PNRC who was not both qualified and compassionate. As I have learned by talking to friends treated elsewhere, being competent and caring is usually mutually exclusive. The building might not have had gold fixtures, but their services were pure 24-carat gold. Incidentally, despite being an older building, the maintenance staff continually update the facility and keep it clean and safe. I wholeheartedly recommend this rehab facility if you want excellent care for your loved one while they recover from their medical problems. The occupational and physical therapy staff were fantastic. The nurses and nurse's aide were flawless. The kitchen and food delivery services were reliably good. Their social services and activity staff kept us informed and entertained during our stay. The friendly atmosphere at FNRC also promotes many friendly encounters and even bonds among the patients. Every patient has a significant physical problem; otherwise, they wouldn't be there. However, positivity is an essential factor in the rehab process. FNRC creates a positive environment for everyone. However, I have one caveat. The FNRC works best for those whose loved ones or caregivers engage and work with the FNRC staff. Some patients rarely received visits from family or caregivers. Despite this fact, the FNRC staff took good care of them. However, it was not uncommon for these "forgotten" patients to have short stays at FNRC after their family or caregivers burst into the room to complain about the facility. However, I discovered that staying at FNRC will be a productive and beneficial experience if you work with the staff. No amount of money or gold-plated toilets will help your injured loved one unless you work with the staff at this or any other rehab facility. read more

This is a serious, dedicated rehab center that gets the job done. The combination of the professional rehab attention, a positively uplifting atmosphere displayed by the staff and the serious attention given to serving excellent food and taking seriously the dietary concerns makes this an amazing nursing and rehab center. The rehab portion is handled by a dedicated staff with appropriate equipment to address every problem. Carmen (PT), Dennise (OT) and Jane (speech therapy) were all in charge of my husband. Their careful, structured therapy did wonders. They covered strengthening skills, pushing each day, setting new goals while helping my husband to become stronger, mobile, and more independent. The staff is amazing. The patients come into this facility having suffered tragic circumstances. The staff is attentive, professional, compassionate and handle each patient with care, creating a friendly "feel good" atmosphere. My 89 year old father-in-law needed a place to go to after surgery and I'm glad we chose this place. They were attentive and cared a lot for him. My FIL is not the easiest person to care for, all he wanted to do was go home and not participate in his care but the staff there worked hard to convince and help him come back home. We were also getting updates from his care team often and did not feel left out in any decisions. Whenever we had questions or concerns Susan would reach out and address them right away. horrible and unsafe care!!! I placed my mom in this unsafe environment - completely regret choosing this terrible place for her. I had heard wonderful things but it turns out they do not live up to the old reputation. based on personal experience from March 7, 2024, the care is sub standard and the state needs to make a visit. according to their own staff, "the 3rd floor has better attentive staff". my mom was unfortunately on the 2nd floor - asked to move her and this request was ignored. the nursing care was terrible; lack of communication, lack of care and medication error. the C.N.A. provide no care at all, we were afraid to leave mom alone with these people. Juanita, lacked communication skills - she just stared at you and did not respond. basically she just walked away when anything was asked of her. the charge nurse turned out to be a "clerk". no license at all. a non license person making clinical decisions on patients/residents.??? when my mom received a medication that she did not ask for since it made her confused, we were told by this clerk, "Tet", that she had asked for it through one of the CNA and it was my mom's fault. based on our investigation, the CNA never said my mom had asked for the pain medication and based on my moms neighbor, my mom needed to be called several times to wake her up and give her this medication. at this point, I had enough and took my mom out of this unsafe environment. all I have to say is- sorry mom for placing you in harm way, please forgive me and hope we do not need rehab help in the future. I will quit my job before placing you in any rehab environment and never at Pacifica rehab. read more

My wife was admitted to Pacifica Nursing & Rehabilitation Center in Pacifica for rehab. It is an excellent facility. It is very clean and well maintained by crew members Eugene, Jaylen & Sam (maintenance & housekeeping). Fatima was the nicest administrator, along with Mico, & staff, who are also very nice, admitted my wife into the faculty. Doctor Murphy is an excellent doctor in charge of my wife's health condition, and eventually determines when my wife could leave the facility after she has fully recovered. Doctor has a full staff of nurses, CNA's, LVN's, RN's, social workers & receptionists working under him. They are all professionals, very knowledgeable, caring, patience with all patients at the facility. Misfits and Psychos on staff. I went to this place two days after knee replacement surgery. It was a grave error in judgement on my part. They advertise a 5 star rating..how??? They mishandled my leg, then denied me care and medication. There were (6 typo) *5 people in my room. Many older ladies that were asking for eye drops, medication and dressing changes for hours. The response from the angry staff "nurse" I'll get to you when I can!!! The women were fearful and quietly suffering. When they told me they didn't have my medication 8hrs. after my arrival I threatened to call 911. My medication came quickly after that. But they never got all of my meds. Then I tried to eat their food which gave me severe diahrea. Nothing was fresh, mostly packaged and chemical laden, with high fructose corn syrup. They punished me by refusing to turn the light off over my bed that night. The next morning it was still a fight to get my medication, any medication or assistance! I told them I was leaving. They responded with we'll see if the doctor will release you. I said you will pack my meds and get the paperwork prepared now. I called my aid and told him to come get me. The other women and I began to ring for help in numbers. If one of us needed something, three of us would ring. That seemed to help get them coming within the hour. Make no mistake I came there to get help. I'm 64 and disabled, I just had my knee replaced and I have 21 stairs to my home. The conditions, the food and lack of qualified caring staff made me leave prematurely for my own health and safety.
