The activities department are an extremely hardworking group who are consistently catering to the therapeutic needs of the residents and are very active in getting the residents involved and engaged. From crafts to dancing to making pizzas there is a wide variety of activities being offered to the residents for them to enjoy and partake in. The PT team is very active and encouraging with the residents recovery. They are always smiling and lending an ear while walking up and down the halls. The CNAs ensure that the patients ADL's are complete and that the patient is comfortable. I echo Laura M.'s review and want to expand on why I'm leaving a one-star rating, specifically regarding the Social Services and Activities management teams, which in our experience have been well below acceptable standards. The Social Services department--particularly the case worker assigned to my mother-in-law --has been consistently unhelpful. From the start, her approach was aggressive and lacked compassion or understanding for our situation. At one point, she even made inappropriate threats, which reflects poorly on SF Post Acute as a whole. Follow-up calls are rarely returned in a timely manner, and even in-person visits often end with staff being "unavailable." Communication has been minimal, especially concerning important matters like health insurance coverage. We were left to figure out solutions with little guidance or empathy. The Activities Director has also been inconsistent and unprofessional. Rules seem to be enforced selectively--for example, we were told only one visitor was allowed in a room, yet other rooms had multiple visitors at once. We were asked to leave certain areas while others were allowed to stay under the same conditions. When we waited in the hallway, we were repeatedly told the hallways needed to be clear, yet no alternative or direction was ever given. Her tone and approach added unnecessary emotional stress during an already difficult time. Like Laura M., we have found that basic questions are often met with vague or evasive answers. This lack of clarity shows not only poor communication but a concerning lack of compassion for both patients and their families. It also harms the reputation of the many nurses, CNAs, and therapists who do provide excellent and compassionate care. Food quality also needs improvement; meals often arrive lukewarm or cold. Laura M., thank you for speaking up--you encouraged us to share our experience as well. We truly hope SF Post Acute's upper management takes these concerns seriously and addresses the issues we and other families have raised. read more

Disappointed and Concerned Our family's experience at this rehab facility was incredibly frustrating and concerning. There is a complete lack of communication and accountability among the staff -- it was never clear who was actually in charge. Each staff member seemed to have different information, and we were constantly told conflicting things. Even basic questions were met with vague answers, and rather than receiving compassionate responses about our mothers' care and treatment, we were often confronted with "rules". One of the most troubling issues was the inconsistency and neglect in her care. Her physical therapy sessions were missed at least once and then when we asked for an explanation, they denied it was missed. Her commode was left unemptied for over 15 minutes -- completely unacceptable. On top of that, the room was too small to be shared with another patient, which it was, and it meant there was no privacy whatsoever when using the commode. It was placed right next to the head of her bed, and each patient could hear -- and smell -- everything, which was not only unpleasant but deeply undignified for both individuals. Wait times were long when my mother rang for help -- often leaving her waiting for assistance far longer than is acceptable for someone in her condition. This added unnecessary distress and discomfort to an already difficult situation. The overall cleanliness of the room was also an issue. It was our private caregiver -- not the facility staff -- who took it upon themselves to clean the room. When I visited, the improvement was immediately noticeable. That same caregiver was even doing chores that should have been handled by the facility's staff, just to maintain a basic level of cleanliness and comfort for my mother. As for the food, it was so poor that my mother wouldn't eat it. This was incredibly distressing, as she's already frail and nutrition is essential to her recovery. I brought her food a couple of times to ensure she was eating something. As family members, we constantly felt left in the dark and confused. When we asked to speak with supervisors or those in charge, staff would only take our phone numbers -- never providing us with direct contact information. Follow-up calls were rarely, if ever, returned. Permissions for certain things and needs were granted by some staff, only to be later denied by others, causing a lot of distress to our mother and us, not to mention a total waste of our time and energy. It felt chaotic, disorganized and disrespectful at every level. This facility gave us the impression that ever-changing policies were more important than people. We expected care and professionalism, but what we experienced was a system that seemed indifferent at best -- and, at worst, negligent. We hope they take serious steps to improve communication, consistency, and basic caregiving before other families go through what we did. read more

Two stars only for the workers/employees who seem to be friendly enough people. This home seems to be vastly understaffed. Not nearly enough attention being paid to each inpatient. Nobody making sure the patients are really eating. almost no one around and the call buttons don't even work. My mom's roommate kept asking me to go out of the room and find someone who could help her. The staff member did not pull the bed up close enough so that my mom could eat her food by herself without spilling on her gown. According to my brother, they are not making sure that she gets enough physical therapy. The Jewish Home on Silver street was pretty bad as well, but better than this place. I also went out into the hall when my mom needed some assistance, and nobody was anywhere to be found. read more

Interview process and call back system. I was called for an interview, arrived 15 mins prior. Was given a paper application, she had no where for me to sit so I sat behind the busy nurses station. I had my interview, and the interviewer did not listen when I answered the questions she asked then tried to offer me a position other than what I applied for. Not a safe environment to work in Was oriented for a couple minutes and thrown to work and figure things out on my own. So many medications nurses were telling me that's not important don't give. Patients families were calling in and being placed on hold for so long and when I would speak to them they would say no one ever calls me back to inform me about my family member. This facility is absolutely Amazing, the staffing here are really concerned about your well-being, the Administration, Nurses, CNA's, Physical Therapist even the Custodians, any and all of my questions were answered. The food wasn't that bad either, they have a laundry service in the building and they will wash your clothes for you and the cleanliness of this facility my room was mopped every day (with bleach) but my favorite part is the activities, my first week here I wasn't mobile it was such a Joy when someone came into my room with a cart full of games and prizes (no one loses) once I was able to go to the recreation room, we did exercises and games to help with our mobility, there were snacks and the option to eat lunch in there, but what was really Awesome was the Activities Director (Librada Guarina) made us some brownies for snack they were hot gooey and delicious. Our family had a terrible experience at SF post acute. Some of the nurses were ok but mostly place seems understaffed. The "care" team for my dad recommended his discharge at about 10 days of rehab after he suffered several rib fractures...he was still too weak and his rib fractures were not healed. My dads insurance stopped covering his stay once they said he was "ready" to leave and we ended up with a $5k bill for staying an extra week as we were scrambling to find a place for dad. What's worse is my dad fell again a few days later requiring another ER visit and rehab (thankfully found a different and much better one)! He was not ready to be discharged. I do not recommend this place if you want your loved one to get better and avoid paying thousands out of pocket. read more

A couple of days ago I wrote a bad review on this place and I'm being taken care of here now. I can't say how much a I regret doing this. These people are turning out to be pure angels. They are there to help with anything they can do before you even know you need it. I think an angry snit may sometimes lead to these poor judgements we make. Everyone from the director, Im the activity director, Eric director of Social Services, all the way down to my caretakers are wonderful. My father was transferred to this skilled nursing facility from hospital. The space is clean and spacious. Some staffs are very nice such as Lee, the activities director, Jerilee from admission and Corey from dietary services. However some CNA are pretty lazy, telling my dad to wait for another two hours before she can put him back to bed. Eric, the social worker, is terrible. He's pretty rude and can't communicate effectively with family. During discharge, he asked me to pick up my dad at 10am. I initially agreed but later found out that I couldn't make it in the morning, so I called him back to ask for another time to pick my dad up. He said I had to come 10-10:30am because that's the only time the nurse was available to go over the discharge medications with me. Mind you, I am a healthcare provider myself and I worked in a SNF before. I know it's not true...family can come pick the patients up at any reasonable time. Anyways, since he said the restricted time is due to the nurse's availability, I requested to talk to the charge nurse. He refused to transfer the call. I only requested to come in the afternoon, any time from 12-5pm which is convenient to them as well. He insisted 4 times that I had to go between 10-10:30am. Finally I asked him that what's if nobody is available in the morning. Then he finally said I could come at 2pm. I don't understand why the discharge time was so strict to only 10-10:30am....perhaps that's the time which is the most convenient to him himself!!! I wouldn't send any of my family here in the future. read more

I tried to give it 5 stars, but it isn't letting me. I was there for a couple of weeks and was in bad shape and they got me fixed up to go home. Great nursing staff, fabulous PT and OT, and quite decent food given the constraints. The facility needs upgrading but don't let that put you off. Great people make that much less important. The senior staff works diligently to make your stay highly productive. read more

My 95 year old father-in-law was admitted here after a hospital stay following a heart attack. He needs to improve to the point where he can cooperate with physical therapy. We have found the facility to be spotlessly clean and the staff highly competent and caring. We are impressed with all of the various departments communicating with us about his care and progress. Lee has done a wonderful job letting us know how visitation works to maximize our time with my husband's dad, which is important to them both. We are grateful for this highly competent and compassionate care. (Also, the meals are nutritious and well-prepared, very important to the patient.) read more

This Nursing Center is one of the best in the San Francisco area or even the state. I've never been looked after so well by any other facility. It was very hard for me to leave. Everything was great the food, the staff, the nurse, the CNAs we're all wonderful people. I would give this nursing center 0 stars if I could!!! My mom came here after being hospitalized and it was awful in many ways. First, the bathroom was never cleaned until I complained about it. Second, while in the nursing center, my mom had to use the bathroom and she was ignored. She tried to alert the staff for more than an hour each of the 3 times that she needed to use the bathroom and no one helped her. Third, she also had an open wound, and the staff did not help her change the bandaging until over 3 days later. Lastly, the staff was very inconsiderate and they were very noisy even at 11 pm, which prevented my mom from having a good night's rest. read more

To be honest I was not sure of how successful my mother was going to be at this rehab facility at first. She was placed here to recover from surgery for a broken hip. My concern went away once we had a care conference and made a plan for her stay, how it would work and how I could be involved in helping with making her recovery successful. My mother has been showing improvement every day. The steps they are taking to keep the patients, staff and visitors safe are very good. They do a really good job at keeping the rooms and the rest of the facility clean. The staff have been very nice, helpful, kind, caring, compassionate and accommodating. There are so many people who we have interreacted with that it is a very long list, but some names are Lee, Kayla, Evelyn, Mia, Abby, Bernadette, April, Esther, Daisy, Devi, Marge and many others whose names I do not know or have with me as I write this. We have to thank John and all of his rehab therapists for working so patiently and diligently with my mother. She made a connection with the therapists and is working hard with them to get better. They are very professional, caring and encouraging at the same time. She has made great progress towards getting back to her ability prior to the surgery. We were told that SF Post Acute was the best facility to go to recover from her surgery and they were right. THANK YOU to all of you who have taken such good care of her. A 0 stars!!!' MORE LIKE AN ABUSIVE CENTER! Warning all the people who have their elderly there! We had our grandma there for physical rehab after a bad fall. the nursing facility is endangering and impairing all patients recovery. The facility is understaffed, causing delays with overall care. The overall service is unacceptable! -medications are not given on time -they delay assistance for bathing and using the restroom. - the food is not based on patient's necessary diet! We asked for her food to be cut into pieces, since she cannot use her right arm at all. They forget! -her blood sugar is not checked regularly causing her highs and lows that are endangered her health. -the social worker assigned to her at this place is incompetent, not a patient advocate at all, states his own opinion and does does not offer any solutions for her stay. -the patient sharing a room with her has some virus and the lady is left unattended by the nurses, unchanged while having diarrhea, causing unsanitary space for both of them. - when they finally arrived to assist, the nurses have no skillsets on how to handle a patient, my grandmother was always hurt when being moved. - the therapies given are not at all effective - the staff is unorganized and have no sense of urgency -There was an incident where she had been waiting 40 mins to get assistance to use the bathroom, no longer able to wait she grabbed a near by bowl and she tried her best, however she couldn't balance and all the urine fell on the phone. The staff came to find the mess and the social worker went of screaming on her. - when visiting we notice the horrible care on the other patients as well! And we do not know if these poor patients have family members or not, because OMG, I cannot imagine having no one to speak up for them! - the social worker Eric Figueroa who also rated this place as "friendly" here is ABUSIVE! -lawsuit on its way! read more

After my hip replacement surgery, I was an in-patient at the San Francisco Nursing & Rehab Center for 20 days. I am writing to express my gratitude and thanks to the nurses, doctors, physical therapists, occupational therapist, admission coordinator, social worker and administrator for a job well done. They went above and beyond the call of duty to make sure that I was taken care of in every way; from addressing all of my questions and concerns to something as simple as covering me up with a warm blanket. My continued recovery and therapy was excellent! The timeframe in which my ability to walk, stand, move around and dress myself was amazing! When my husband went to a dr. appt follow up he had so many infected bedsores and a bladder infection so bad the doctor admitted him to the hospital, he's been there so far 1 month to clear all infections. SF Nursing claimed they were treating his sores & infections..with what placebos? They where suppose to be treating him for more than 1 month. (?) The hospital had them showing improvement in 7 days. They broke my husband's spirit while he was there. Bed sores, bladder infection, weight loss & loss of appetite. This place is so depressing. I don't know how they got a 5 star rating read more

My mother took a nasty fall in the beginning of January and had a surgery to relieve pressure on her brain. Her recovery was slow at 85 yrs old and she was transferred from the hospital to SF Nursing where she stayed for 6 weeks. We could not have asked for better care. The first thing you notice when you walk in is how clean the place is and bustling with activities and happy care takers. Now keep in mind, this is a place for older people so seeing that for the first time was a bit shocking, but once I was around for a few days to see how everything was run, I was so happy that families chose this place for their loved ones. My mom was treated with love and kindness and respect. There was not one thing that I think should have gone differently. It was truely a blessing to have found this place. read more

Horrible Service!!!! The nurses try and to act "nice" but really aren't. The nurses lack skill and do not give enough care. There should be at least 1 nurse in each room because the nurses don't know when people are hurt. They are not very well trained and there is no supervisor! In almost every hospital I've been to there is a supervisor unlike this nursing center. Either this place needs to be more sanitary, caring, and have more skilled nurses or else this place needs to be shut down (no joke)!!! read more
