    I've been to several different therapy places in my life. These guys seem more interested in helping people than any place I've been.

    One on one with the PT-great attention and knowledge. Alex Delgado had an excellent routine and I felt very well cared for.

I will say that the speech therapist and physical therapist seem to be very kind and thorough, and one of the nurses/CNAs was very kind as well. They do have a large gym area for physical therapy. The facility, however, is depressing. When you enter, the lobby looks lovely with wood floors and a piano. The front desk staff range from mildly friendly to apathetic. On a recent visit a staff member was slurping soda through a straw from a can and just glared at us as we signed in without saying a word. As you move further from the lobby, however, the wood flooring is replaced with linoleum and the place looks increasingly dilapidated. The first room mom was placed in had a leaking HVAC unit with tons of towels piled under it apparently to soak up the leak. Gross. Eventually she was moved to a better room. And the food - yikes! Do they even have a dietician at this place? One dinner was a cheese enchilada - is that even a balanced meal? Last night it was a sad looking sausage sandwich with the driest and most over cooked french fries that you can possibly imagine. Anway, I guess she is getting the physical therapy which is the reason for being at TLC. But don't expect much more in terms of comforts...

My sister arrived on a Tuesday. On Thursday she took a fall and fractured her elbow. They did not inform us of her fall. When i arrived at 9am, there was a CNA sitting in her room for 1:1 with her. The CNA informed me of her fall that morning, and my sister had a horrible wound on her right elbow. I asked what protocol was, and was told, very rudely, by the LPN on duty that the Dr was notified and would be there to see her. I spoke to the charge nurse to ask why the family had not been notified of her fall. She said that we should have been, but she didn't know why we weren't. (I don't know is every single employees favorite phrase BTW) I then told her that i wanted my sister to receive xray of her arm, as her elbow wound looked very concerning. She said that she would notify the Dr of my request. My sister has lewy body/ frontal temporal dementia, and was having mass confusion and delirium. She didn't understand what was going on around her, or where she was. Imagine putting a toddler in an unsecured position where no one was watching them.. ever!! They put her in the rehab area, not the memory care unit, even though they were fully aware of what was going on and what her challenges were. We chose this facility for its location to our home, as well as it having a memory care. Many rehabs do not have a memory care. My sister did not receive xray until 9pm that night. My niece was called by the LPN on duty the next morning and told that my sister had a neck fracture!! When questioning what she just heard, the LPN then stated it was not her neck, but her collar bone. We immediately went in and spoke to the director, Tracy about the chaos and lack of care in the facility. My sister was no longer receiving 1:1 to ensure that she could not wonder, and we were unsure of her actual condition based on being told one thing and then another. When we came in, another rehab patient who was NOT memory care, informed us that when my sister had fallen, he was outside her door in the common area and heard her yelling for help. He said that she laid on the floor for 25-45 minutes screaming for help, while he told the nurses that she needed assistance. Of course they felt we should not believe him, but based on our experience so far, we could not believe THEM about anything. pretty safe bet he was the truth teller. Tracy in administration read my sister's xray report and said that it was a fracture in her radial neck. (According to her expertise lol) that was "the top of the radius where is the bone connects to the shoulder.. so it was close to the collar bone." *huge eye roll.* at this point i demanded she be transferred to the ER for further evaluation. The radial neck is the elbow btw, Tracy. Because we didn't know what else to do, we let her be transferred back to TLC after her ER visit showed no other injury. Tracy had made us feel heard and had my sister transferred to the memory care side of the facility, so we felt some relief. Little did we know, it just went from bad to worse. The memory care side of this facility is the place people go to be forgotten. My sister went from assisted walking to the bathroom, to laying in bed and pissing herself all day! Far worse confusion and Within 3 days, she began complaining of her butt hurting. Me, her daughter, her granddaughter and grandson had been taking turns sitting with her throughout every day to ensure care. On this particular day, the cna changed her wet pants while she lay on her back in the bed. I asked her to roll her, so that i could check her bottom, and my sister had developed a stage 3 bed sore! It was open and very red and painful looking. This CNA had her on her back and pulled her drenched brief straight out from between her legs, only wiping her front. Had i not been there to check, she would not have checked or cleaned the urine from her backside. When we would come, my sister would be in bed with pants drenched. With no water or fluids in sight. Often, she would have food on her tray sitting across the room, even though she was a walk/ transfer assist. Our family felt like we could not leave her and was missing work, running back and forth between home and work and did not know how to get her the appropriate care. Every time we spoke to ANYONE, the answer was "i don't know." We never saw that worthless Tracy again. We tried to speak to the in house social worker, also no help! I texted with Tracy once and asked her if the facility meets the 6-1 state ratio for memory care patients, and she informed me there was no state mandated ratio. GUESS WHAT TRACY! I spoke to Nevada elder advocates, and you are incorrect. That goes for rehab, but memory care does require a 6-1. You should be hearing from them soon! Your 3-28 and typical 2-28, based on your staffing issues, isn't legal in this state! The

They had encouraged us to discharge home, but my dad was still unable to walk, get out of bed, roll over or another kind of independent action at that time. We talked to Tammy at Sage Creek. She was so calming and reassured us that my dad would have the care he needed, they could provide transport for the office visits he needed for his heart surgery and would have the PT, OT and SLP that he needed. We were told at the rehab that a skilled nursing facility was going to just leave him in bed and he would lose any progress he had made with them. This is absolutely untrue for this facility. They got my dad up and dressed every day. He had therapy. The therapists were so patient and kind (Alexa, Vicky, Willy and all the others I'm so bad with names) Mo the nursing supervisor went above and beyond to take care of any concerns that we had. Judy, Tyler, TJ, Brandi, Lindsay, CC, Rosann, Malik and other nursing staff were all amazing. The facility doctors were wonderful with communication and the care they gave. The facility was clean and the food was good with lots of options. Danielle and Nicole in transportation were amazing taking my dad to his many doctors appointments. Paige, our case manager, worked so hard for us. Anything we need to get my dad in the VA Home she came through for me. Absolute godsend! We adopted the staff here as family and we already miss them. Their kindness and thoughtful caring was beyond any of our expectations. I 100% recommend this facility

And this last time I was beside myself. They lost her sling, they didn't answer her call light, they washed her phone. They never put her items back on her tray so they would be easy to grab. Everything was thrown into a chair the was filled sky high. The carpet is disgusting, the place looks run down. If u have another choice...take it.
