    I'm honestly really disturbed after seeing the viral TikTok video going around about this place. After reading through other reviews here, it doesn't seem like this is just a one-time issue either--it feels like there's been ongoing problems for a while now. Something seriously needs to change. There should be stricter hiring practices, better background checks, and more oversight of the staff. Families trust facilities like this to care for their loved ones, and that trust is clearly being broken. The lack of accountability is frustrating, and it's even more upsetting that there hasn't been a clear response addressing what happened. The public deserves answers, and more importantly, residents deserve to be treated with dignity and respect at all times. This situation is just really disappointing and honestly unacceptable.

    upon walking in from a long drive from San Diego, this place is a total dive, and the staff there does not want to be there. The front desk doesn't even say hello to you. You have to sign in and prod where to go upon walking into my brothers room, the room was totally disheveled shorts down at the photo of the bed shirt behind the bed on the ground just hanging off the bed (which I found out we're not even his clothes), top dentures just sitting out on table, pillows on the ground to the back of the room, food all over my brother and him talking, incoherently and hallucinating and seeing people who weren't there and mumbling words that could not be understood. He was agitated and trying to pull out things trying to get up, but he couldn't. He was grabbing the food that was on the tray with his fingers and trying to put it into his mouth, but his hands were shaking, a nurse walked in and said they had to change his linens nine times and he kept pouring water over his head. Another nurse came in and they said they just got on shift and they've never seen him before so I said to help him and they said they would call the doctor, but that was taking too long, so I called 911 and got him over to Henderson Hospital. The longer my brother was in that state which I've never seen him in before, the more chances he would've been a vegetable permanently or dead. At Henderson Hospital ER they found he had a severe UTI infection, acute metabolic and encephalopathy, respiratory failure with hypoxia, and dehydration. A 5-Star shout out to nurse Kristen at TLC, who was very attentive and helpful and knowledgeable. She was the one that cared the most. Because of her dedication and care my brother was able to recover at Henderson Hospital. The other nurses were unsure what to do and just looked dumbfounded at me. I heard many remarks from people and paramedics that said, "I would not put my loved one here". This place is understaffed and horribly leaving people alone too long who desperately need care. Do not use this place!

    I really wish i could rate this place a zero. My mom just had a major back surgery. As soon as you walk in, the front of the house looks beautiful. Once you start walking down the halls, it's straight out of a horror movie. The smells, the lighting, almost as if someone is going to jump out in front of you. I asked the nurse in the hallway what room my mom was in because the lady at the front desk gave me the wrong room number, the nurse responded "Oh she's in room blah blah but she's saying she wants to leave. if she refuses to stay, we can't help her with anything. we can't even walk her outside" Not only that, once i got to my mom's room, she had explained to me that the nurse herself had told her to not expect any kind of help because "it's one of me against 53 patients " they had her walking on a shower chair & expected her to pee in the shower all over her legs. RIDICULOUS!!!! there was blood on the walls in her room, food caked into the remote to her bed, no rails whatsoever to help her. Later to find out the nurse had told her that she wouldn't be getting any kind of service until Tuesday, May 27th. Mind you, this is on Sunday, May 25th. I can't even begin to explain the disappointment I had. On top of that, they somehow lost the walker that my mom was sent with from the hospital by her physical therapist. It was disgusting. I read other reviews online that had pictures attached, bugs in the drinks, elderly people in soiled diapers for more than 24 hours to the point of infection/hallucinations to due poor care. DO NOT EVER TAKE YOUR LOVED ONES HERE!!!!!! THIS PLACE NEEDS TO BE SHUT DOWN IMMEDIATELY!! No exceptions at all. BULLSHIT

    This is absolutely horrible. They didn't even have the proper tools to sterilize and dress the PICC line!!

My beautiful lady, Mary Lou was here as a guest during her transition from being at home. Unfortunately, she's at the moment now where she needs assisted living and that extra care. It's been a long and stressful time. Rough couple of months. Finally, while this was not permanent the staff and facilities here truly were a "god send!" This is a place where your loved ones are in great hands. The rooms primarily are not private but accommodate two individuals. They are co- ed. You never know how long your neighbor in the next bed will be here. The telephone and television are on both sides of the bed, but the loudness can interfere with the patients comfort level. The doctor and nurses here are so overwhelming great the way that they handle the patients care and pain levels. The patient, even Mary Lou, my angel, does not want to be here. She wants to go home . There is no more home for her. It's assisted living. They make this transition easier for the patient. The visiting hours are longer than normal and all an individual needs to do is sign it. You're allowed to bring outside food and drinks for patients . You can decorate and adorn their space with flowers and other trinkets to make them feel a little piece of home. Love is the focus and makes the world turn. Sometimes we cannot decide where we will be. When it comes to the end we will go in and out of who and where we are ?! My Miss Marylou, my angel you are loved and will be continued to be loved as long as you are here. We will go beyond to make you happy.

    I want to start by acknowledging Brittanya, Kaylee, and Cammy. My mom genuinely loved them. They…read morewere kind, attentive, and took wonderful care of her and they deserve to be recognized. However I have serious concerns about the overall care at this facility. At one point the floor was left wet and my mother fell and hit her face. My mother is a Type 1 diabetic who recently had a stroke and should have been on a diabetic heart healthy diet. That was not even close to what she was receiving. I walked in one day to find her being served ice cream, cranberry juice, potato salad, and a sandwich on a roll so large I could not believe what I was seeing. All of this for a Type 1 diabetic recovering from a stroke. What made this even more alarming is that it was literally marked on her chart to give her a high carb diet. Someone made that decision and put it in writing. When I raised the issue they did change it, however they would still regularly serve her things like large egg salad sandwiches loaded with yolk on an enormous amount of bread. She is smart enough to know to only eat a little from the middle but she should not have to be outsmarting her own care team's meal choices. Thankfully I had been bringing her her own food in the meantime. Her blood sugar was regularly spiking to 350 and on her last day it reached 400 to 500. Staff were saying they did not know why her blood sugar was swinging, while simultaneously feeding her a high carb diet per her own chart. I was not always there and did not always have access to her Dexcom, so who knows what it was reading the rest of the time. Blood sugar at those levels in someone recovering from a stroke can cause brain damage. This is not a minor oversight, it is negligence. Additionally there was a nurse whose name I was unable to get who had a completely straight face, never smiled, barely replied when spoken to, and treated my mother as though she was an inconvenience when she needed her insulin. There was zero warmth, zero compassion and zero acknowledgment that she was caring for a vulnerable human being. Insulin is not optional for a Type 1 diabetic. It is life or death. I genuinely cannot believe someone like that is working in patient care. I also want to note that I brought my mother $100 worth of scratch tickets from North Carolina, something small to lift her spirits during a very difficult time. They were stolen. Taking anything from a vulnerable patient in your care is unconscionable. I am giving an extra star solely for the exceptional care shown by Brittanya, Kaylee, and Cammy. The systemic issues at this facility need serious and immediate attention.

The level of professionalism, knowledge, compassion and care my mom received was top notch. A couple shout out's to Dr Miranda, Nurse Makky and CNA Tasha/Juvy. They were the best!     I've been coming here for about five months now, and I honestly don't want to stop, this place has…read moreknocked at least ten years off my body. I'm one of those older guys who always thinks he can do more than he probably should, so I'm constantly pushing myself to the limit. That usually takes a toll, but these guys keep putting me back together and keeping me active. I'm physically stronger and in less pain now than I was 20 years ago, and that's because of this place. The therapists here are extremely knowledgeable and great at finding the root cause of issues instead of just treating symptoms. I'm incredibly thankful they keep me in the gym and on the road, running virtually pain-free. And yes, they now see my entire family, because they're that good. They currently treat my son for a knee issue and are amazing with him, especially since he was nervous to come in because of his autism. My wife goes there for herniated discs, and they've helped her feel stronger and so much better. Even my daughter came in after waking up one morning with severe back pain, they fixed her right up, and she didn't even want to stop coming once she hit 100% (we finally had to tell her she was good!).     I had my 2nd rotator cuff tendon surgery on November 25, 2025, after a work injury. I researched…read morein-network clinics in my area, and found Align Physical therapy clinic. I went to the clinic, and spoke with the staff. They were very friendly, and showed me around the clinic. They told me about the treatment option, and said the could fit me in right away. I gave them my referral from my Orthopedic doctor, and they scheduled my appointments for the week. The the 1st appointment after waiting about 15 minutes, my therapist said he was just doing an assessment of my range of motion. I only took about 30 minutes. He massaged my shoulder, did some slight movement of my arm, and instructed me to do some rotation and sliding exercises. He put a heating pad on my shoulder, and that was it. The 2nd appointment I had to wait 15 minutes before similar treatment. The 3rd appointment, I had to wait 20 minute before being called to a massage table. My assigned therapist was working on another patient. I was given a table just a few feet away from where he was. I was agreed to a heat pad on my shoulder while I waited for my therapist. I sat with the pad for another 20 minutes. At this point I was annoyed. Why schedule an appointment when you clearly are not available. Another therapist named JC came to my table and took the pad off my shoulder. He tried to change the position of the table, but he did not know how to operate it. He went over to another therapist to ask for assistance. That is when I realized that he was new, and obviously inexperienced! He massaged my shoulder for about 5 minutes. I was visibly in pain, and he was scared to treat me. He said it was unusual for a patient to be in that much pain even though I had surgery less than 3 weeks ago. He told me that I needed to call my doctor. I was shocked and mortified that Align clinic would assign such an experienced therapist who didn't know how to treat me. I waited 40 minutes for about 10 minutes worth of treatment. I called my doctor's office immediately after I left, and was advised not to return to Align clinic. I then called Align clinic, and told them that my doctor and I decided that I was not a good fit for this clinic and I would not be returning. In my opinion this clinic is more like a gym than a therapeutic clinic. The staff is pleasant, and the clinic is clean, however I think the clinic is not organized, as I didn't see anyone writing notes or looking at records of their patients. The clinic was very crowded and it seems the priority is on getting the most business instead of making sure each patient gets the best treatment.

    All the CNAs (Certified Nurse Assistant) in this Rehab facility are very qualified and do a good…read morejob assisting the nurses. However, there is one CNA who in my opinion is exceptional - her name is Precilla (Shiela). Shiela does her job with efficiency and always with a smile, and always asks if there is anything else needed. She brightens the day with her personality and attitude. Thanks Shiela. My wife is currently a patient at this rehab facility and has received very good care from the entire staff, which includes nurses, certified nurse assistants, physical and occupation therapist, as well as others. I have been pleasantly surprised at the variety and quality of the patient's meals. Eunice, the head chef frequently will add special touches to the food prepared, which makes the meals even better.

Lyzie etc) were so nice and cheerful when taking care of my dad. Ok now the negatives...... my dad was released from care despite having a fall two days prior to the day of release.......not good, I would have expected that they would have given some additional care to assess why he still had the propensity to fall. Also the food services for a vegetarian here are dismal and no effort was made on their part to find options. My dad lost so much weight. Not happy with this place. Proceed at your own risk and advocate for your loved ones if they are sent here
