    I'm honestly really disturbed after seeing the viral TikTok video going around about this place. After reading through other reviews here, it doesn't seem like this is just a one-time issue either--it feels like there's been ongoing problems for a while now. Something seriously needs to change. There should be stricter hiring practices, better background checks, and more oversight of the staff. Families trust facilities like this to care for their loved ones, and that trust is clearly being broken. The lack of accountability is frustrating, and it's even more upsetting that there hasn't been a clear response addressing what happened. The public deserves answers, and more importantly, residents deserve to be treated with dignity and respect at all times. This situation is just really disappointing and honestly unacceptable.

    upon walking in from a long drive from San Diego, this place is a total dive, and the staff there does not want to be there. The front desk doesn't even say hello to you. You have to sign in and prod where to go upon walking into my brothers room, the room was totally disheveled shorts down at the photo of the bed shirt behind the bed on the ground just hanging off the bed (which I found out we're not even his clothes), top dentures just sitting out on table, pillows on the ground to the back of the room, food all over my brother and him talking, incoherently and hallucinating and seeing people who weren't there and mumbling words that could not be understood. He was agitated and trying to pull out things trying to get up, but he couldn't. He was grabbing the food that was on the tray with his fingers and trying to put it into his mouth, but his hands were shaking, a nurse walked in and said they had to change his linens nine times and he kept pouring water over his head. Another nurse came in and they said they just got on shift and they've never seen him before so I said to help him and they said they would call the doctor, but that was taking too long, so I called 911 and got him over to Henderson Hospital. The longer my brother was in that state which I've never seen him in before, the more chances he would've been a vegetable permanently or dead. At Henderson Hospital ER they found he had a severe UTI infection, acute metabolic and encephalopathy, respiratory failure with hypoxia, and dehydration. A 5-Star shout out to nurse Kristen at TLC, who was very attentive and helpful and knowledgeable. She was the one that cared the most. Because of her dedication and care my brother was able to recover at Henderson Hospital. The other nurses were unsure what to do and just looked dumbfounded at me. I heard many remarks from people and paramedics that said, "I would not put my loved one here". This place is understaffed and horribly leaving people alone too long who desperately need care. Do not use this place!

    This is absolutely horrible. They didn't even have the proper tools to sterilize and dress the PICC line!!

    I repeat do not do not do not send your family members here!!!!!!!!!! DO NOT! This place is understaffed, unsanitary, and an absolutely depressing place to be. I have no idea how this place is still in business. the nurse was overworked and had almost 30 patients to himself!!!! I felt so bad . Maria was absolutely amazing and the only person I liked there !!! She deserves to work someplace better. . She cares ! This place needs to be shut down.!!!!!!!!!! I feel terrible for the people that have no choice to stay there and have no family to see how terrible this place truly is!!!!!!!!!!!! This place didn't even have clean water jugs to put water in. A staff member told me to grab a cup that was by the sink that looked filthy dirty and told me to fill it up with water that's the only cup they had . Are u kidding me! They had ice in a dirty cooler serving to their patients . They had my family member sitting in pee all night long didn't even bother to change the bedsheets. Nothing at all. They don't give showers. They didn't even sterilize my family members PICC line correctly . They put a gauze pad in the picc line dressing which you never are supposed to do !!!!!!! management got nasty with me when I ripped my family member out of this place because they know that they neglect their patients and I was there to see it all. The night staff who was cleaning the rooms even told me that place was horrible and to get my family member out of there as soon as I can!!!!! I just couldn't believe my eyes. This place is an absolute nightmare. Whatever you do DO NOT send any family members here. !!!!!!!!!!!!!!!!

My mom has…read morebeen here for several weeks and was transferred from an acute rehab for continued nursing and physical therapy. It seems as though the care provided has been no different than had we put her in hospice... mind you, we are nowhere near that point in her life. Her weakness has only gotten worse, physical therapy has not been done as we were advised it would be, and she continues to lose weight, her muscle strength, and has been laid up in her bed for more than three weeks now with very little movement. She has become stiff and rigid in the position that she's in and is very painful to even attempt to move her into a sitting position. Communication from the staff and her case manager has been extremely poor. I was notified last week on Wednesday that Medicare was only providing one more week of treatment for her and have yet to be provided a discharge date. I have left several messages for the staff with no response and still don't know when I need to set up transfer for her to depart that facility. Based on the timeframe provided, it would appear that I have 24 hours or so from now, yet no information has been provided to our family on when she will be discharged. Her time at this facility has been a complete waste of time and only further set back any progress she had made prior to admission. While Mom says "most of the people are nice," she also said that they very rarely even check on her and never move her. Please do your research to find the best fit for your loved one. We thought we had, and while we understand that skilled nursing is a lower level of care, we feel that there is zero excuse for the lack of treatment she has been provided. It is also important to note that many times when we have been there visiting, there have been other patients crying out for help from their rooms that has gone on for more than 10 minutes before anyone responds to their request or urgency. This is uncalled for! The staff is there to care for these patients and they hardly acknowledge the calls for assistance... definitely not in a timely manner. We are extremely disappointed with our experience and how our loved one has been cared for, or lack there of.

    I want to start by acknowledging Brittanya, Kaylee, and Cammy. My mom genuinely loved them. They…read morewere kind, attentive, and took wonderful care of her and they deserve to be recognized. However I have serious concerns about the overall care at this facility. At one point the floor was left wet and my mother fell and hit her face. My mother is a Type 1 diabetic who recently had a stroke and should have been on a diabetic heart healthy diet. That was not even close to what she was receiving. I walked in one day to find her being served ice cream, cranberry juice, potato salad, and a sandwich on a roll so large I could not believe what I was seeing. All of this for a Type 1 diabetic recovering from a stroke. What made this even more alarming is that it was literally marked on her chart to give her a high carb diet. Someone made that decision and put it in writing. When I raised the issue they did change it, however they would still regularly serve her things like large egg salad sandwiches loaded with yolk on an enormous amount of bread. She is smart enough to know to only eat a little from the middle but she should not have to be outsmarting her own care team's meal choices. Thankfully I had been bringing her her own food in the meantime. Her blood sugar was regularly spiking to 350 and on her last day it reached 400 to 500. Staff were saying they did not know why her blood sugar was swinging, while simultaneously feeding her a high carb diet per her own chart. I was not always there and did not always have access to her Dexcom, so who knows what it was reading the rest of the time. Blood sugar at those levels in someone recovering from a stroke can cause brain damage. This is not a minor oversight, it is negligence. Additionally there was a nurse whose name I was unable to get who had a completely straight face, never smiled, barely replied when spoken to, and treated my mother as though she was an inconvenience when she needed her insulin. There was zero warmth, zero compassion and zero acknowledgment that she was caring for a vulnerable human being. Insulin is not optional for a Type 1 diabetic. It is life or death. I genuinely cannot believe someone like that is working in patient care. I also want to note that I brought my mother $100 worth of scratch tickets from North Carolina, something small to lift her spirits during a very difficult time. They were stolen. Taking anything from a vulnerable patient in your care is unconscionable. I am giving an extra star solely for the exceptional care shown by Brittanya, Kaylee, and Cammy. The systemic issues at this facility need serious and immediate attention.

My father's last phone call to me was of him screaming in pain and his last text I'll ever read was in the form of the word "HELP!" Neglect at its finest and I WILL know what occurred no matter what the cost... Once a brilliant intelligent resilient kind man, who as of now, has no memory of his existence and little ability to speak, think, see, move or function.
