Staff is lazy and neglectful. The call light doesn't even work and when i ring the bell for help they take hours sometimes to finally come. I had an important appointment to receive my infusions, and they forgot to set up a ride for me, my appointment was rescheduled for 3 weeks from now and now my health is at jeopardy due to their mistake. Physical therapy is a joke here. Their sessions will never rehabilitate anyone. Please don't send your loved ones here if they requiere lots of help, because they'll receive no help here. **January 2026 Update** I had high hopes when the Ensign Group purchased the facility in August 2025. Those hopes and the hopes of others have diminished over the last 6 months. The Administrator told several of us that he wouldn't be reading our emails and we were to address issues to the managers. The managers would be running their departments like their own businesses. I contacted the company Compliance line (9/25 & 1/26) as well as the California Department of Public Health over dirty silverware after repeatedly contacting both the Administrator and the Dietary Manager. In December I sent a separate email to the current Dietary Manager that the proper procedures (as explained in September by the Infection Control Nurse) were not being followed by some employees due to the condition of the what was received. Her response, "I will review with my staff. Also, we are going to rotate in new silverware" (12/26/25). Yet, I continue to receive dirty utensils as recent as 1/15/26. The residents routinely are missing things from the meal trays (butter, milk, condiments, utensils...) causing the CNAs to leave the floor and deal with hostile kitchen staff. Food preferences are often ignored (request to have small portions and food choices). I have repeatedly emailed about this as well as meals being early/late as well as menu changes with residents and staff not being notified. This is a big deal because the CNAs rely on scheduled meals to manage their time. I've come back from a shower and the food carts were in the hall 20 minutes early. Cold food on hot plates, food over cooked, mushy rice, flavorless meals.... I realize that this is institutional food, but there is a lack of attention to what goes on the plates. Milk is regularly out of inventory and residents are given cereal without milk on more than one occasion (a grocery store is next door from the facility) or chocolate milk to put on their cereal. The Administrator was copied on each email I sent to the Dietary Manager. I have been nice and I've been aggressive in my language regarding these and many others and neither approach has worked. I also have transcripts of weekly meetings with the Administrator and kitchen management where these issues and others were discussed. The Social Services Department has not processing medical referrals submitted by the facility Nurse Practitioner. One that was submitted in April and never processed. It was resubmitted in October and again January. Another was submitted in October and has not been addressed either. I manage my own appointments, but the facility must submit them to a medical facility. This is but one of many I have with the department. I decided this time I wasn't going to follow up to see how long it would take. During a November meeting with the Director of Nursing I had in November he stated that referrals were discussed daily. He directed Medical Records to see me and I requested the information showing that the October referrals has been requested and in the system. As of today the referrals haven't been processed. Housekeeping is also deficient. Most of the housekeepers do not thoroughly clean my room (others have the same issue). The showers have mold and there are often strange drippings on the walls. The lifts (standing & Hoyer) that are used for those of us who can't stand or need assistance standing were purchased that aren't adequate. These Hoyers don't get high enough to get people in their chairs or out of bed get stuck under beds. This issue was relayed to management several times yet somehow they were purchased anyway (initially they were rentals). The oldest lift is the one that most CNAs use but it has issues as well. The three Hoyer lifts I used in the last week all of them were dirty and looked like they hadn't been cleaned in some time. One had crusty finger prints and another had a layer of grime where the bar was welded. It's not unusual for the batteries not to be charged (I've had 3 batteries not be charged in a single day). Noise levels in the facility by staff at shift changes and other times are excessive. It often sounds like they are having a party while residents are trying to sleep (6:45am and 11:00pm). Management has been notified several times but that only helps for a short time. A refrigerator that was purchased to be used by residents (mandated by the State of California) sat unused for months while we waited for the rules of use to be written. Some of us have still not received the rules despite mentioning this to several managers. The screens on the windows and sliding glass doors have been broken since I came here 2 1/2 years ago. For these and many other reasons I would not feel comfortable having a family member receive care at Courtyard. read more

This is an update to my previous reviews. I've now been at Courtyard for 19 months. I have all of my faculties and very aware of the operations of the facility. A new administrator was hired in July and many employees who had been employed here have left as a result of misguided mandates and poor treatment. I witnessed this firsthand during a quarterly Care Conference. As a resident, I shouldn't realize a change in management, but it is painfully obvious when employees are no longer happy and the turnover is rapid. It's obvious that she was hired to cut everything back and save every penny to show a profit while that facility was for sale. It's obvious that necessary maintanence of the building has't been a priority. The facility is very clean except for the shower rooms. The HVAC systems are very old and it's either really cold or too warm. I use two fans in my room 24 hrs a day. Other halls are extremely hot or cold. Several times this year the systems were down and at one time 86 degree air was being pumped into my room. Mold is present in the shower rooms, tiles on the floor are missing grout, and the doors are falling apart. Last year we went 5 monthis without a Dietary Manager and even now we have a Dietary Manager who can't or won't manage his staff. As a result we constantly receive food that was not properly prepared and often overcooked to the point one can't cut through it. I've sent many text message with photos yet nothing changes. I've taken over managing my medical care after bad experiences from the Social Services Department. Luckily, I'm capable of handling my affairs, but not sure how others are adequately taken care of. They were moved to a tiny office with no room for someone to speak confidentialy. Dr. Amacher is the facility doctor and I'm glad that she actively advocates for the residents. She has provided excellent care and takes time to listen and allows me to participate in my healthcare. I'm 57 and able to advocate for myself, but there are times I wonder how others are being treated and how issues are resolved for them. Apparently the building is being sold to a company with a good reputation. My hope is that issues will not be swept under the rug under capable management. read more

I recently found myself in a situation of needing placement for my parent. I toured the facility and the staff were friendly, and mindful of my mother's needs for rehab to return to her home setting. She was there for 2months and i could not be happier with the care team and nursing knowledge. I could single out the near and dear staff, however they were ALL angels. Thank you again. read more

This facility is not the place your love one if they need Occupational Therapy. The (OT) staff are dishonest with their charting. In addition, to the facility had a serious out break of COVID during my mother stay. The administration staff were aware and even agreed with my concerns regarding the charting but nothing was done. The charted my mother was able to shower and dress herself with little or no assistance which was untrue. I spoke with the Executive Director, as well as other staff and they acknowledge my concerns. But after my mother was discharged from there facility she was back in a Rehabilitation in less then one (1) month as a result of there dishonest documentation and failure to rehabilitate her per doctor's advise and families request what was need for her to be successful at home once discharged. Please if you love your family member this is not the place for them to thrive and heal. The food is horrible and the nursing staff is nothing to write home about. However, most of the CNA's are very kind and helpful to the patients. This facility is a serious Hell Hole and DO NOT PLACE YOUR LOVE ONE HERE keeping looking there are much better places. read more

I give Courtyard 2 stars. Don'f go there if you take psych meds. I do & they screwed up with me. They only gave me half dose of my antipsychotic. They didn't give me my mood stabilizer and ER at the right time of day. I wanted to send my psychiatrist a Release of Information. Courtyard dropped the ball on that one. The staff there are very dedicated. The food is fairly good, lots of fiber. read more

I would not recommend this place to anyone wanting to rehab or house their elders. The rooms were adequate for 1 person. They were going to give me a roommate and I checked out AWA. That process was very delayed. The staff was generally nice. The facility was over staffed. I would wait 3-4 hours often to have bandages changed or urinals dumped. I couldn't get a Physical / Occupational Therapy appointment due to waits for wound care. The food was obviously canned such as refried beans, peas and sweet potatoes. The breakfast consisted of bacon (good) and powdered scrambled eggs. One staff member described the food as "Unidentifiable" I ordered delivery on more than one occasion. Sharing a rebab facility with dementia and long term care patients was not conducive to my rehab recovery and I went AWA. I have been doing much better at home. I am not aware of any off campus activities. I could not get a TV channel listing. If a relative came by to visit and I wanted to go shopping with them I was threatened with discharge. I could go on but I won't. The facilities strategy seems to be pack as many people in as possible. Ignore their complaints until they submit or die. Your rehab, long term care or mental health needs should be handled in a more humane, dignified and healthy way. read more

I liked courtyard and their staff until they abruptly discharged my mom. We had to scramble to get things set up for her return home. When I discussed this with one of the social services staff members they said "you can just set her up with some food while you're at work." Umm...no lady my mom is not a dog. She requires 24/7 care. They really don't care about patients once they decide it's time for discharge. Her final week she was supposed to continue to receive physical therapy but they found a reason not to give it to her I'm sure they still billed Medicare for the services. read more

****TRIGGER WARNING**** No stars at all. Never bring your family member here. Had a family member be here about 2 weeks. They stole/misplaced a few personal items from her & when asked they said they did not know. They do not check on patients often. They are lazy to keep patients hygiene intact & when they do it they do not use sanitized wipes, they use the same ones over again. They also misplace patients food, & feed the wrong food to them. They do not help with therapy. When patients are sick, or having an infection they do not check up on them. As I write this, my Aunt just passed away because she got worse under the care of this horrible facility!!! I do not recommend at all, my aunt was literally dying for 2 weeks with an infection caused by this place yet they had the nerve to keep her there. When asked about results on things they go weeks & say they have no idea yet they know the results.. Not once, did they check on her during the time she was having an infection to send her to the Hospital. Now she's gone, my family is so distraught. read more

I love courtyard health care. I have been coming here for a few years for different things. I've always been treated with respect and excellent care. Sometimes I have to wait for something - I'm not the only person here on thr floor who needs or wants something. The people who are taking care of me are people too. I treat my care givers with respect just as I want to be. The caregivers are very professional, compassionate, gentle, and they will do everything possible to make sure your needs are met. I was released to courtyard the 30 th of August I was supposed to be on a potassium free diet, which included no tomatoes no potatoes no orange juice pretty strict diet. I got potatoes almost every serving. It took me almost 24 hours. I have a broken shoulder. It took me almost 24 hours to get my pain meds to where they were right so the pain had got out of control so bad, it was ridiculous. I couldn't understand the gentleman that was my nurse in giving my pills. My daughter was in Washington. She was trying to advocate for me over the phone and talk to him. He said the phone just walked away. Nobody there took care of me I got up by myself and went to the bathroom. I was supposed to get somebody to help me but when I sat on the toilet for over 20 minutes of the light on nobody came in they were very, very rude. My CNA's were all wonderful. CNA's are very very nice that I have to say that but as far as all the other people involved, it was horrible. There was a lady named Sam that did it medication and she was wonderful, but there was a guy name. Wang was a miserable person. The food was incredibly horrible. My roommate and we both sent it back almost every time they idea me getting no tomatoes was to give me ravioli with no sauce on it horrible so I got sent home on the fourth Wednesday. I was supposed to get a bed. They showed me where the bed from in the works, they said I would get home before the bed and the walker got here so OK this is Saturday. I still don't have a bed. Nobody from home. Healthcare is come to CV got a broken arm. I don't get well. This place is horrible please please do not put anybody you care for love in here, it's the worst place ever I'll give it one star because I guess I have to give it a star if I could give mine I would I forgoti I to add it took them 24 hours to get my pain meds in order for me mine do I have a broken arm that they aren't setting they're letting it heal 24 hours. They left me without pain. Pills is a horrible horrible thing the worst experience ever. read more

I also stayed in this HELL HOLE a few years ago. Everything everyone said was absolutely true and correct. Call button was useless even though my room was directly across from one of the nurses stations. I've seen many a girl come into my room and realize, oh no this isn't my area and walk out. When the staff realized I wasn't going to keep my mouth shut they put a woman with dementia or something in the spare bed . I like to keep my curtain closed but she kept grabbing at my curtain to open it. One time I caught her opening my little nightstand and grubbing through it. Also one time my curtain was closed but I looked down and her head was on my side of the curtain looking up at me from the floor! A nightmare!, she also wailed on and on 27/7 "help me help me". But of course she too was ignored. I am going to have surgery soon and I will tear Kaiser apart if they even suggest! read more

Stay away from this place! my mother was there for her broken arm and in 3 1/2 weeks she received no physical therapy! Even after her follow up with an orthopedic doctor, his directions were ignored for another week. She received no clean bedding for almost two weeks. Some staff were excellent like May, Saum, and a few others but others rushed out of the room and refused to help my mother. One staff member asked my mother if she knew how many other patients were requesting assistance, after my mother had pressed the call button. Definitely won't ever go back there. There were maintenance issues that were not addressed and the food was atrocious! Making a formal complaint with the state! read more

My father was a patient at courtyard. He received amazing attentive care from all departments. The nursing staff were caring, competent, compassionate, dedicated, hardworking, and skilled!! Some nurses to note that went above and beyond on providing excellent service were Som and Liang. In addition to them, all CNAs provided exceptional care; always happy and ready to assistant with his needs. There are so many exceptional CNAs to note-Willy, Ruth, Rafaela, Sophia, Yvonne,Erin, Linda, Pendo, Ale and of course many others. They treated him with great care, made him feel welcomed and took care of his needs in a timely manner. My family is so appreciative of the quality of care my father received. The rooms were cleaned daily by detail oriented staff members and was visited daily by the Activities department. My family is forever thankful to the therapy department at courtyard. All the therapists whom worked with my dad were patient, attentive, full of energy and overall dedicated in ensuring my dad remained engaged in each therapy session. I had a family member here for therapy. They where here for a few weeks under therapy services and did so well. Whatever concerns we brought where immediately addressed by the managers. I do have to say that the social workers where really helpful when helping us plan for discharge. read more

Negligent is an understatement. My father was transferred here after surgery 02/15/20 - this morning we found him freezing cold soaked in his own urine and feces. He was never given proper medication or hooked up to any fluids and his dressing had been removed. We immediately discharged him and will personally care of him until another facility opens. His doctor was livid when he heard about the dressing and said he'll never transfer another patient here, he's had too many complaints and that the facility's lack of care is dangerous. It's difficult to find beds available but this place shouldn't be in operation. We're filing a formal complaint. read more

All of my father's things got lost here, his phone his tablet his shoes and clothes. And if you're loved one is here for physical therapy they will not receive it! This place is absolutely awful. Your loved one will not thrive here. Please take my advice DO NOT SEND YOUR LOVED ONE HERE. It's a depressing environment and I worried about my dad's care all the time when he was here and the nurses here are not caring at all besides one of the night nurses that got my dad to the hospital on time. My great grandma was a patient at Courtyard after having a stroke. She was treated very kindly by all staff. They were all so sweet to her, especially her morning nurse, who was very attentive and always made sure she was comfortable and out or pain. The receptionist was very kind and always directed my family to the right people when needing help. read more

My grandma has been in Courtyard for almost a month now. I had high hopes for this place, as it seemed to be fairly nice. First off, this place is very understaffed and rude. I've been visiting a lot during mid-day and when my grandma uses her call light, no one comes for over an hour. My grandmother says she has to yell for help AND even then, it takes a while for someone to respond when she needs to be changed. It could be an emergency. These people do not take their patients seriously. Girls at the nurses station, do not greet or ask if you need anything; you have to greet them and when you do they seem irritated. The food is terrible,repetitive and kid-like, they usually have the same bland food for dinner, for example PB&J. They don't give people much of an option. They run out of food when there's a grocery store right next door... However, Diane, Alto, Saum, and the therapist have made her feel comfortable and helped my grandma a lot and she is making recovery. read more

I would NEVER recommend this hell hole! I didn't even want to give them a 1 star but had to. Where do I start? Kaiser sent him there for rehab after his hospital stay. During the 20 loooong days he was there they sent him to the ER 3 times for falls when he was labeled a fall risk. He fell out of his wheelchair once and they told me the nurse just stepped away for a minute. He should've never been in the wheelchair by himself at 9:30pm. The other 2 falls were out of bed because he had to get up to use the restroom and it took forever for a nurse to show up and sometimes they'd tell him they need to get another nurse to assist her so he'd end up peeing himself because they didn't get him out of bed. My dad was never incontinent until being admitted to this sorry excuse for a rehab facility. The first time they sent him to the ER they didn't even contact me. They had his name and number down for the emergency contact, didn't leave a message on that phone and never looked up mine! I showed up 6 hours after they sent him just for me to just have to turn back around and drive 30 minutes again the other direction, where I had just came from! He only got worse because he was back and forth to the ER so many times (send those are around 8 hours a trip) and I'd have to remind them to give his medications even when it seemed as if the majority of the people there looked like they were given sedatives. PT is only M-F and because they didn't get him out of bed on a regular basis he just got weaker. We visited every single day he was there so we saw everything that was happening or not happening. When we visited him on Christmas Eve and Christmas Day his clothes were soaked because they let him sit in his own pee instead of helping him get up to go to the bathroom. Some of the employees weren't even wearing masks in the lobby and i even saw one walk into a patient's room without one while playing on her phone. A large majority of staff were playing on their phones instead of answering the call buttons. When I needed a nurse I'd have to go hunt them down myself because they weren't walking the halls checking on patients. On my dad's 20th day we couldn't get out of their fast enough. read more

I spent only about 2 hours in this facility after I had knee replacement surgery. When they finally decided which room to put me in, there was no provision to support my sore knee. I asked them to provide some adult diapers, they brought real diapers that were extremely big, I told them it was not what I needed. The response was that they could pull them really tight. I asked to have a nurse check the blood bandage on my arm that needed attention, nurse never came. I ended up putting pressure on it myself, once the bleeding stopped I took the bandage off, and found there was no garbage bin near my bed so I just tossed it to the floor. I pushed the call button when I had to go, no one came, so I got up on my own which I was not supposed to do. Lucky me, my room neighbor had left an adult diaper in the shared bathroom, so I used it. They brought me dinner, I did eat a little of, glad I did not eat more, the next day I was sick (food was bad). At this point I move to the doorway, and ignored some more, in a loud voice I announced that I was leaving. They were all shocked, one person told me I could not leave. My response was that I was fully grown, pay my own bills and I could certainly leave if I wanted to. At this point they wanted to know why, so I told them what I have written above. I found out that they did not have any of my medication they were supposed to give me. Being type 2 diabetic I was supposed to have insulin before dinner, and they did not have any pain medication. They had known since 9am that I was coming to their facility and no one took the time to make sure they had the medicine ready by 6pm! The male RN then asked if I wanted him to check my bloody bandage. I said no! They asked me if I would be more comfortable in a wheelchair, I said yes, he ran off to get the chair as I continued talk to one of the nurses about the issues. After what seemed like a long time, I asked one of the aids to get there remainder of my things and I headed for the door, after passing a room or two, here he came with a chair, I was more than half way to the door, I told him forget it since he took so long I was almost to the door. My girl friend was outside waiting for me. I left! I would never subject any one to this place. My actual rating of this facility is -5. read more

My auntie is here on Hospice. The nurse and CNA for my auntie were very attentive not only to my auntie, but my family's needs as well. Offering tissues and water and the CNA brought in extra chairs. Those little gestures spoke volumes. I'm a nurse too (not that it really matters) and I work in longterm care as well. I think it's hit or miss at any facility you bring you're loved one to. But I'm basing my 5 star review based on the events that happened within the 2 hour window I was there. The employees there showed compassion, I wish I knew their names. The board on the wall said Nurse: Devin and CNA: Ale but it was dated a week ago. Thank you two for showing that you cared in such a sorrowful time as this! read more

This is the shadiest place around. The piss poor care my grandma received but the nightmare afterwards was another. I am her grandson and started receiving the $980 bill in my name. I contacted courtyard and asked why I was billed when I never signed anything or had anything to do with the billing. I was informed that since I was the closest emergency contact they automatically put the bill in my name and billed me. They agreed to take my name off of the bill and correct now. Three weeks later I am now receiving bills from a collection agency regarding this and no matter how hard I try the head business center representative at courtyard will not call me back. Avoid this place like the plague! read more
