Case Manager Reymond E. has been outstanding getting everything completed in a timely manner. Every time I have a question or a concern, he has been there and has fixed it. 3. I cannot remember everyone's name on the second floor. This is about the day and night shifts every Nurse, PCT, all support staff at nurses' station and housekeeping. They are friendly, kind, will do everything that you need with a smile (even if they are not assigned to you or your floor). The staff is very professional you should be very pleased that they are working for a great company. If I ever have to go to rehab, I will defiantly come back here. 4. Dani M. has the best PT, OT and staff. I have been in quite a few rehab centers (in/outpatient) and by all means you have the best. She is doing an outstanding job. I saw the sign outside her door (come on in and chat which I did) when I was walking by with Hannah and Dani was great. 5. Edward S. definitely knows what he is doing. He evaluated me and did an outstanding job. He also listened to my concerns and came up with a plan of attack. His knowledge of the muscles and nerves helped me a lot to get where I need to be. The stuff he has given me helped me more than the OT person I had when I had my stroke. He also has the respect of everyone on the therapy floor, if they have any questions, I have seen them go the Edward and he will an something and they go back to their patient. Edward needs to be in a management position. 6. Hannah G. physical therapy knowledge is above all PT persons I have had the privilege of working with. She has a great sense of humor. I had a problem when exercising, I would hold my breath. She kept saying to me breathe, breathe, those words are stuck in my brain and will never go away and I watching you. She is the only one that made me rest in between tasks the proper amount of time required. I have seen her work with other patients, and you can tell she really cares about them and all patients. She even gave me exercise to complete at home. She is a great asset to your rehab facility you can not ever let her leave. 7. Having to stay a few extra days was what I needed thanks to Edward and Hannah I have continued the exercise that they have taught me. 8. My only negative thing I have is you TV channels, they don't match what your channel lineup has. I can only watch channels 2-34. Channels 35-52 did not work. 9. You have an outstanding facility, what would make even better is after I left inpatient care, is to come back as an outpatient to contain with PT/OT care.

Sacramento rehab sent my dad to an ER to sit for 24 hours as an out so they didn't have to care for…read morehim anymore. Because of infections that happened during my dad's stay they sent him away so they wouldn't have to be responsible for him. This facility doesn't want to care for or help patients that may require extra support and more tailored therapies. The nursing staff acts in a way that is unbecoming of the nursing profession. They have laughed at, made fun of, and rolled their eyes when asked for help with my dad. I came in multiple times to find my dad sitting in his own filth in tears that nobody would help him. Sitting in wheelchairs that were too short so his legs were jammed into his chest, left there unable to move in excruciating pain. Having nurses laugh at patients, drag their feet to offer hands on care, and leaving patients filthy and unable to move directly violates ethics standards set out by the Nursing Practice Act. Professional conduct should uphold patients dignity and show compassion not demean people who have gone through life changing health issues. The therapist for the most part are kind and caring. Though there are a few who have no business working with people in this type of environment (I'm looking at you Jeff). The rehabilitation aspect is the only thing I trust this facility with which is why my dad had a family member there nearly 24:7. My sister stayed over night three nights a week. The one star is for them and nobody else! Case managers are not communicative and unavailable the majority of the time. Perhaps other people have better reviews and experiences because they are not seeing the type of care their loved ones are getting. all I can tell you is be present and do not trust that these people are properly caring for those you love. If your loved one had a stroke and can't speak clearly, walk, or take care of themselves do not leave them alone here. A shiny new facility is a great distraction for the patient neglect that is happening when you aren't looking.

I had visited several SNFs earlier…read morebut when I took a tour of Advanced Health, I knew in a minute that this is where I wanted Jane to stay. I can't adequately express just how good this SNF is. If you want exceptional care from a team of people who go the extra step to insure the well-being, comfort, and progress of your loved one, this where they have to be. There are no exceptions to that. You must get them admitted here. The entire staff are kind, friendly, competent, professional, and, in my opinon, without peer in this area. If you have the need and they have the space, you take it. You don't think about it, you take it immediately... because that space will be gone in a Hollywood minute. The physical plant is modern, bright, airy, super-clean, and never once had that stale "SNF smell." The director of nursing, Fedilyn and the clinical nurse manager Karren will see to your loved one's every need... no exceptions. Yolanda will make admission and move-in totally stressless. When your loved one is ready for discharge, Bethany will handle everything for you. Sean and his PT staff are well-trained, patient, and totally supportive. The nursing staff is terrific as are the nurse-assistants. When you need something in the middle of the night, someone is there for you. If there is a need to be transported to a facility for a quick scan or x-ray or doctor visit, their vans are modern and their drivers are safe and personable (ask for Manny!) They have wonderful activities during the week with an ice cream social on Friday. But if your loved one wants to just stay in the room and read, that is fine too... there is no pressure to do anything... except recover. Every SNF should be as good as Advanced. None are. If you or your loved one needs rehab... this is the place.

At Sutter Roseville Medical Center I was not subjected to fluid restrictions nor at the current rehab facility I am at. That's the reason for the 1-star rating.

My mother-in-law was admitted here following an ER visit at Kaiser for recovery from a UTI, as she…read morewas experiencing instability with walking. The staff took excellent care of her--carefully monitoring her condition, administering medications, and supporting her recovery. Notably, she did not experience any falls during her stay, which was a significant relief given that she had fallen on her very first day at a previous skilled nursing facility. The reason I am giving four stars instead of five is due to a lack of communication after admission. Neither my husband nor I were contacted regarding her condition, diagnosis, care plan, goals, or discharge planning. It wasn't until I visited the facility in person four days later and requested to speak with someone that we received this information. My mother-in-law did mention that the care environment felt very constant, saying, "They never leave you alone--up at 5:30 for medications, bright lights, no privacy." However, I reassured her that the staff was being attentive and doing their job to support her recovery. By the fourth day, her infection had cleared, and she had regained independence in getting to and from the bathroom without incontinence issues. According to the discharge planner, she was also able to walk 175 feet with a walker under supervision. Special recognition goes to Sondra, the discharge planner, who was incredibly efficient. She coordinated the physician's discharge orders, medication reconciliation, and completed the necessary 602 forms for her return to assisted living--all by 2:55 PM on the same day I met her. We arranged private medical transportation, and my mother-in-law was comfortably settled back into her own bed by 6:15 PM that evening. Overall, we are grateful for the quality of care she received, with the main area for improvement being communication with family members.

My father was brought there from the hospital around 4pm one day after having a stroke. They did…read morenot tend to his pain so after many requests for a warm washcloth my cousin had to track one down to help ease the pain. They told my mother they would find rails for the bed and it ends up they never did. He would constantly try to get out of bed to use the bathroom even though he had a catheter. He was extremely confused and needed monitoring. Well at 11 pm that evening I got a call that my dad had pulled his catheter out and they were taking him to ER. I waited for my father at the hospital and once I saw him his hands were covered in dry blood and said he needed help and no one would come. He couldn't remember anything else but I assume the process of ripping the catheter out and tearing the lining of his body sent I'm into a state of shock. When I called them once I was at the hospital the man gave the phone to what I guess would be his care taker and she seemed panicked pretty much saying she didn't know what happened. That was the beginning of the downfall. Sepsis. Infection. This place is awful. My father is now dead. No stars but it wouldn't post unless I put one.

This place is now called "north pointe care center" ( it was confusing at first when looking up the…read moreplace) My father spent several months at North Pointe to complete his therapy after being in two other nursing homes that were not the right fit for his needs. What truly set North Pointe apart was their willingness to accept him when no one else would, due to behavioral challenges related to his illness. This facility gave our family hope at a time when we felt like we were running out of options. I vividly remember being at the hospital for the fourth time in just one month after more heartbreaking experiences at other nursing homes. During that stay, the nurse manager connected with North Pointe. She came back and said, "The first thing they told me was, 'Bring him in...let's make him comfortable.'" From that moment on, we felt like we had finally found the right place. They welcomed my dad with open arms, and the cycle of hospital admissions stopped. It was a huge weight lifted. From day one, the staff at North Pointe was exceptional. We were comforted to see the same nurses, CNAs, and therapists each day ...people who knew my father by name and took the time to understand both him and our family. Their consistency and compassion made all the difference. I especially want to thank the CNAs, nurses, and rehab team. They are incredibly experienced with patients living with Alzheimer's, dementia, or recovering from serious illness or injury. One thing that really stood out to us was how diverse the staff was. There were team members from many different cultural backgrounds, and many of them spoke Spanish;including nurses, CNAs, front desk staff, and even members of the cleaning crew. This made a big difference in creating a welcoming, inclusive environment where communication felt more natural and respectful, especially for patients and families who speak Spanish as a first language. The facility does a great job of helping residents stay active and engaged. The backyard area is one of the highlights...patients are able to walk outdoors safely, surrounded by large shade trees and grassy areas. There's even a big birdcage that many of the residents enjoy. Inside, there's also a piano, and music is played. It's comforting to know that patients aren't just confined to their rooms...they have opportunities to socialize and enjoy the outdoors in a secure setting. While the front porch offers a nice place to visit and provides some privacy, it gets extremely hot during the summer. Unfortunately, there's no designated indoor area for families to sit with loved ones privately, which made daily visits uncomfortable at times. The backyard is more of a shared space, and you'll often be surrounded by other residents. That felt a little intrusive in the beginning, but I quickly grew fond of many of the residents over time...they meant no harm, and it became a familiar, even comforting, part of our routine. The rooms are small, usually shared between three residents. That can be a challenge, but the staff works hard to match roommates based on compatibility. Everything is gated, which is a huge comfort if your loved one has memory issues or a tendency to wander. Safety is clearly a top priority. That said, the facility really needs renovations. The closets, dining room, and especially the bedding are outdated. We noticed torn linens and worn-out furniture more than once. It's time for ownership to invest in this space. Our seniors deserve not only excellent care...but an environment that reflects their dignity and worth. North Pointe has the heart and the team. With updated rooms, furniture, and better indoor common areas, it could become truly exceptional. Staffing was also stretched at times. While every staff member we interacted with was dedicated and hard-working, it was clear they were doing more than their fair share. They need more support from leadership. The social work team was also understaffed and could improve in communication and follow-up. I'm sharing all of this because I know how difficult it is to choose the right nursing home. We relied on honest reviews when we made our decision... and a lot of places had a lot of fake reviews....and I'm so grateful we chose North Pointe. It's not perfect, but what matters most...the staff are truly outstanding. They cared for my father with patience, skill, and compassion, and they gave our family peace during an incredibly stressful time. Thank you, North Pointe.
