Be very cautious if you ever need a second opinion or your medical history from Delaware Smile Center. After this office recommended a full mouth restoration costing $55,000-$60,000 instead of a standard whitening treatment, I requested my complete dental records to seek an independent evaluation. In explicit violation of federal HIPAA Right of Access regulations, this clinic refused to hand over my usable files. They sent only compressed, low-resolution thumbnail sheets of my X-rays and intraoral photographs that are completely unusable for diagnostic purposes by any other doctor. When pressed for my full file, the office explicitly stated in writing: "At this time, no treatment plans are available, and I cannot release the doctor's clinical notes." Under federal law, a dental office cannot legally refuse to release your clinical notes or withhold diagnostic-quality records. Because of their written refusal to provide my legal health history, I have formally filed regulatory complaints against Delaware Smile Center with both the U.S. Department of Health and Human Services (HHS) for HIPAA violations and the Delaware Board of Dentistry and Dental Hygiene. Please protect your patient rights and go elsewhere. UPDATE (July 16, 2026): After I notified the office that I filed formal state and federal complaints, they finally emailed my files--but they deliberately degraded the medical evidence. They intentionally converted all of my original, digital clinical intraoral photographs into black-and-white files. This is a direct violation of HIPAA Privacy Rule 45 CFR § 164.524, which legally requires providers to supply records in the exact form and format requested if readily producible. Grayscale photos strip away critical diagnostic data, making it impossible for a new dentist to properly evaluate soft tissue and gum health. This is my opinion. Reviews shared here and all the stars support my allegations regarding Delaware Smile Center (DSC). Seeing all these happy patients convince me patients must be treated differently dependent upon profitability or stereotype. In the beginning (years ago) ...the practice was phenomenal. As they grew my opinion slowly changed. More specifically I noticed a change in the behavior exhibited by Dr. Sophie following her return from maternity leave. The quality of service from the office managers declined in parallel. I personally am blessed with good insurance and have always been treated fairly by those in the Medical and Dental field which is why I can tell the difference. That being said, some years back I became the Legal Guardian of a sweet boy who is on Medicaid. He needed braces badly and although difficult, Dr. Sophie worked to get this approved. She provided excellent care and did so with kindness. Unfortunately this changed. The following are just a "few" of the incidents that support my feelings: *Apparently there were issues with processing claims with Medicaid. Without prior discussion, Dr Sophie came out to the reception area where 2 other patients stood and told her receptionist not to schedule any more appointments for us because our insurance co wasn't paying. Embarrassed, I responded (as she proceeded to turn her back on me) that I wasn't aware and would immediately call to rectify the situation. We walked away with no appointment. When I called Medicaid they told me DSC was not submitting the paperwork correctly. They were provided the updated codes to use but they didn't bother to correct and resubmit any of the claims. This happened several, several times throughout the years until I caught on to the fact this was routinely their practice. Every Dr. and Dentist I know simply corrects their error and automatically resubmits claims seamlessly. You don't wait for your patients to tell you to do it, unless their Medicaid patients I suppose. At his next appointment I took Dr Sophie aside behind closed doors and explained to her a phone call or private chat is appropriate and discussing financials in front of children and other patients is not. Guessing that was the first strike. *On another occasion a Hygienist thought it helpful to inform this teenage boy that with his type of "insurance" he would have to pay for his own visits out of pocket when he turns 18. *Most recently they again ignored Medicaid's instructions to correct a billing code (which they provided) and resubmit. This time it was for his broken retainer and time was of the essence. We waited weeks for an open appointment (called 10/1 and received an appointment for 10/29). On 10/29 they submitted a claim for the retainer and after a month passed I got a copy of a letter sent to DSC instructing them to resubmit the claim with the correct code. Due to their track record I called to make sure this would be done. Another month passes, I call and Medicaid advises me they never received a correction. Now we are going on 3 months without a retainer. I call DSC and they ensure me it's resubmitted now. I was understandably upset mostly because they didn't even offer an explanation or apology. Knowing it will take another month for approval, I advise the Office Manager that this boy will have been without a retainer to keep his teeth aligned for 4 months by the time the approval comes in. To decrease the additional wait time I wanted to schedule his next app instead of waiting 30 more days for an approval from Medicaid. After all this has been going on since 10/1. It's just an appointment and could be rescheduled easily. They refused to even make the appointment until they received the approval in hand. I voiced my concerns and added that the situation is directly due to their mistake. The least they could do is make an appointment to eliminate another 30 dy delay and the risk of his teeth shifting. After going round and round they finally agreed to make the appointment if I personally paid them directly for the retainer right there and then over the phone. I did so and I let them know I was holding them responsible for correcting the alignment of his teeth if applicable. I also let the Office Manager know that I felt they don't treat Medicaid patients the way they do other patients. Thinking this would concern them especially with all that's been happening in our society today regarding equality but this angered them instead. After a few days I received an email containing his last X-rays with no other explanation. I called to inquire why I received them and I was told it was sent because they are dismissing him as a patient due to my feelings in regards to how they were handling Medicaid patients. All in all, it helped me to understand what is meant by the word PRIVILEGED. Up till now I have taken it for granted. Witnessing the way someone less fortunate on Medicaid is treated was eye opening. read more

I like the dentists and staff but was misled on several occasions about billing. New to the area, I received a USPS flyer for cleaning and a full exam for $99. My son and I went and were charged $200+ After many calls, the issue was resolved to: I paid $99 and my son paid $69 - same insurance. I went back for my second cleaning to see if, somehow, the billing was really an error as they had said. Cost of cleaning I was told is $100. Turns out it is $105, that's okay, but every time they clean your teeth the doctor visits and charges $62. So the real cost is $167 - this was not disclosed to me despite telling staff about the initial problem and that I would be carefully monitoring the bill. I found the office manager to be rude, she interrupted me several times when we spoke about this second problem, so I am moving to a different dentist. Very disappointed. read more

Dr. US mani was my daughters dentist for 3 years... in September 2014 she started with pain in her lower tooth... we went in more than 10 times over the course of 4 months with my 13 year olds continued complaint of tooth pain, Dr. US mani told me he had completed a root canal in that tooth and kept telling us she needed a crown to stop the pain... but that required her to be 14 for the insurance to cover it... we waited for her birthday the office promised to have everything in order for it to be done immediately after her birthday... her birthday came and the pain increased to the point of her not being able to eat... I called them for help... the response was that... the insurance had not approved the crown.. but they would do it for cash with a 700 deposit up front. I called the insurance to find out the delay... the insurance informed me that no claim had been filed by the doctor since September... where that claim was approved.. but for a stainless steel crown!!! I had not been informed.. because the doctor wanted to wait to do a porcelain crown which makes him more money!!! He decided to keep my daughter in pain... to make more money!!! Or take my cash!!!! I took her to another dentist the following day... and found out that Dr. Usmani had never done a root canal... but he did a filling touching her root!!! No wonder she was in pain! The new dentist did a root canal immediately!! Astounding! The new dentist told me the crown would have never stopped her pain.. I plan to file a claim with the dental society. Unethical selfish people should be reported when they make decisions based in money but not to be in business to help others.We are all here to help each other... I would want to know ahead of time if I was taking my child to a good dentist... that is the real reason for this posting. Be careful if you plan to make this man your dentist. Update recently my daughter had to have 3 of her fillings removed because this dentist filled the tooth too closely to the root requiring now root canals. I just had a dental implant in Canada and was visiting family in Philadelphia when disaster struck! A slice of pizza BROKE my temporary tooth off leaving the implant screw exposed. I was horrified and anxious that I had ruined the implant. My sister-in-law suggested I visit Dr. Sohaib Usmani. I was very nervous about seeing a different dentist, but she reassured me that he was the best. From the moment I walked into Delaware Smile Center I felt comfortable. The receptionists and dental assistant were welcoming and professional. After meeting Dr. Usmani all my worries and stress disappeared and I trusted his professional ability immediately. He identified several treatment plans and discussed the pros and cons of each. Dr. Usmani and the dental assistant (Ajah?) helped me feel so comfortable in a terrible predicament and being under their care felt like home. THANK YOU 100x over for your care!!! It allowed me to enjoy the rest of my visit in Philadelphia and to not stress about my dental implant. Plus I had a tooth, which helped my family enjoy my smile! Delaware Smile Center was the perfect place to bring my 8 year old, who had 3 cavities. Dr. Usmani informed us well before the procedure so I truly felt confident in his abilities. Started going to DSC this past fall on a recommendation from a friend. I had made the decision to switch from my previous Doctor, as they were just mediocre and I would dread going. From my first visit of X-rays and consultation to my first trays of Invisalign, I couldn't be happier with the switch. I literally look forward to my next visits. This dentist office is literally the only one I have been able to go to without getting hives. Everyone is beyond nice and I'm confused by some of the negative reviews. I had a consultation before having work done, it was to ensure I understood all the fees and what work was going to be done. I normally don't go out of my way to make a review unless it's a really bad experience, HOWEVER I was absolutely blown away with the quality of care that I received at Delaware Smile Center I needed to make it known. Every single staff I came across was absolutely amazing. Each person was extremely kind, helpful and inviting. When it came to the exam and procedure the dental assistant made a point to tell the doctor I was nervous and he went above and beyond, he explained everything he was doing and allowed me to calm down while he sat there and explained in detail a treatment plan. He was extremely patient and gentle. And to top it off they have been so flexible with scheduling and once I was evaluated got me in quickly. I've had such horrible experiences in the past. I was terrified to go to the dentist. I am amazed at the service I received from them. 5 STARS! If I could give more I would. Myself and my family will continue to go here! I was visiting from Florida last week and my wife had a really bad tooth ache. I called delaware smile center upon my friends recommendation who lives there and is a patient of Dr. Usmani for some time. The office was so accommodating, I don't think that any other doctor or office would be this caring to their patients needs. The office staff is all very professional. She ended up needing a root canal and a crown! And they were able to schedule that while we were in Delaware. Thank you Dr Usmani ! Back in October, I broke my tooth in half. I called around for an emergency treatment, but since it was late on a Saturday, the earliest anyone could see me was Monday and it was going to cost extra. I made an appointment with Delaware Smile Center in Middletown. By Sunday, it was no longer bothering me, so I decided to cancel the appointment. Fast forward 5 months, it's still broken, but not bothering me. Delaware Smile Center calls me back in March and asks me if I ever got it taken care of. I say no, they offer to schedule me in and get it fixed, with no emergency upcharge this time. I don't normally go there, but since they reached out, sure why not. This place is 20 miles from me. My dentist is 2 miles. I get there this morning, 30 minutes before my time, because I expect to have to fill out a bunch of paperwork. They tell me it will be $261.00 and that my insurance requires that I pay up front and they reimburse me. No big deal. I ask her what is being done today because that amount seems awfully cheap to repair a broken tooth. She tells me that they are doing an x-ray and letting me meet with a treatment consultant. ARE YOU KIDDING ME??? I don't need a consultant to tell me that my tooth is BROKEN. You COLD CALLED me to set up an appointment to FIX MY TOOTH and then you try to sucker an extra appointment out of me? Give me a damn break. These companies that only give a crap about making money, especially when a customer's health and well being are concerned, make me sick. I will NEVER give this place a dime and will be sure to tell everyone I know that Delaware Smile Center - Middletown only cares about the bottom line. What a joke. I even argued with them to try to get them to do the work today and they said that they can't do that. They have to do the consult first and schedule it another day. Every dentist I've ever been to in my LIFE has done an x-ray and then the work on the same day. I guess this place just isn't good enough for that or like I already suspect, only care about money. I only wish they allowed reviews on their Facebook page so I could give them a nice big 1-star. I went to the Delaware Smile Center once in March so I could get a cleaning and a recommendation for my wisdom teeth to be removed. When I got there they did xrays and told me that I had five cavities and needed extensive gum cleaning that would take four treatments and it would cost me $5,000 after my insurance and that they would not give me a regular cleaning until I had those taken care of. I got my wisdom teeth removed about a month later and I recently went to another dentist and they told me that I didn't have any cavities what so ever and my gums were fine and I just needed a regular cleaning that they scheduled me to have that day. Nonetheless, The Delaware Smile Center is obviously a scam to get unsuspecting people to spend way more money on fake problems. I recommend never going there unless you want to waste your money and have procedures done that you don't need. Awful place. read more

They told me they took my insurance. Then they give me some crazy bill and say it was not covered. No, you checked before I ever came in and I doubled checked when I got here. Finally some lady told me we take all insurance but we are not an approved for Aetna so your fees are higher. They knew exactly what I meant when I asked do you accept my insurance. I was uneasy in their office. When you ask how much something is you get a crazy response about a treatment overall cost and they won't separate each item. WHAT? What an odd policy. I didn't even make it to seeing a dentist or hygienist at my first visit due to poor office management. I called and made an appointment two weeks earlier and provided my insurance information which they assured me they accepted. The appt was for Monday morning and on the Friday afternoon before they called asking about my insurance. Well I was traveling for work and didn't get the message till a couple of hours later and the office was closed. So I came in a half hour earlier than normal the Monday of the appt and they said I was not responsive to requests and they cancelled my appt. What? That wasn't part of the message.... They then said I could still be seen. Okay... So I fill out the paperwork and take a seat. They called me up to say I had to pay up front and my insurance would send me a check later. According to them, that's how my insurance worked. I've had this insurance for years and never paid upfront anywhere else. And besides, they said they took my insurance on my initial call. Whatever, I pay and sit back down. They then call me up again and say they spoke with my insurance and I don't need X-rays. I know this, I had them last year, they hadn't asked me yet. They then say I can't see a dentist till they review them. However my old dentists office isn't open for a while today so I'll need to reschedule. At this point I've had enough. They apparently charged me for everything before even seeing if I needed it. They didn't itemize so I thought it was just for the cleaning. I then have to speak to the office manager to get a refund and her priority is rescheduling me. I decline because they should have checked these things for a new patient prior to my appt. Her excuse was that they're busy. I've never had any problems with any medical office, this one just doesn't know the basics apparently. Leaving the office I called and made an appt with a different dentist who immediately asked about my prior dentist so they could get my records. BEWARE OF THIS BUSINESS!!!!!!! Initially they lure you in with modern office technology, beautiful atmosphere, pleasant conversation and then they zapp you with lies and deceit once your in the door and seated in the chair. Both Dr. Sohaib and Saqib are smooth talkers who distract you with their incompetent quick talking office staff. They lie about procedures they've never performed and insult you once you catch it on the back end. They make sure to confuse you about office policy and out-of-pocket expenses by DEMANDING PAYMENTS UP FRONT FOR FUTURE APPOINTMENTS AND THEN NOT PERFORMING THE PROCEDURE. My 6 yr old son was scheduled to have (2) cavities filled recently. I paid in advance for this appt and found later they took my money, applied it to a previous appt w/o informing me b/c they weren't happy with my insurance reimbursement rates. So instead of telling me they didn't perform his fillings - they gave my son Novocain and left him alone in the exam room for over 30-35 mins without supervision while I was in another room discussing the mistakes of a previous visit w/ Dr. Sohaib They also didn't honor their promotional $100 coupon. You can't win with this group so I highly recommend you stay away. I've been in the healthcare business for over 25 years and I've never felt so taken advantage of.
