My father stayed at this SNF for about 3 months, and we had a very positive experience. The facility was clean and well-maintained, and the staff were kind, attentive, and supportive throughout his stay. A special thanks to the physical therapy team, especially Rajni and her team, for their support during his recovery. If I could give this nursing home negative stars, I definitely would. In no way, shape, or form is there complete security; doors are easily opened, and residents can get out. The nurses are LAZY and racist. They leave their PTs in dirty diapers and cause bed sores. I have to call all the time to get help for my brother, who has been there almost two years now. When I or a family member calls to ask questions, they don't seem to have any answers. Their follow-through is awful, and the turnover rate is unacceptable. My developmentally disabled brother was admitted there for a month for physical therapy after a prolonged illness. The staff there took fantastic care of him, making sure he had his beloved basketball games on to watch and calling me late one night (he doesn't know how to use a phone) because he was crying and distraught after his watch went missing (it was located the next day). They labeled his clothing and kept everything clean and orderly. The rehab director Rajini Munargala worked with him to get him walking again and did a fantastic job. Ronelo Salvador, the Social Services director gave me information about services I could get for my brother after discharge that I was not aware of. The only complaint I have (minor) is that there were a few glitches in his discharge. It appears they aren't used to discharging people home with medication requiring prescriptions, but I was able to correct it. Would I send my beloved disabled brother back there? Absolutely, and I would have no qualms being a patient there myself. read more

Exceptional Care and Genuine Concern !!! ​I am writing this review to express my sincere gratitude and highly recommend this clinic facility. Overall, all departments functions like a Rolex watch (like clockwork, consistently on time, i.e. medicine, food and personal hygiene needs). Especially, the Physical Therapy team not only for the exceptional physical therapy services I received but the attitude they showed (like a concerned friend or relative). The only draw back I need to deal with (sadness) when time comes to leave. The therapists by the names of JNool and RMunagala consistently demonstrated a level of care and concern that went far beyond basic professionalism. They took the time to listen fully to my concerns, explain the "why" behind every exercise, and ensure I was comfortable with the pace of my recovery plan. One of my health issues was loosing 40-50 lbs on my weight, cardio one of the excercises, they offered and walked to the grocery store and bought what I wanted and what I missed from my regular diet. ​What truly stood out was their genuine concern for my progress and overall well-being. They were meticulous in adjusting exercises based on my feedback and always offered positive encouragement. In conclusion, all departments functioned like a synchronized Rolex watch (nurses, cna's and housekeeping). My family would give this place a zero star review if it were possible. This garbage can of a place. My grandma was brought here and had been staying here for a couple months. Initially, it went well since the people who run this place are Korean, and my family is Korean, so communication was not an issue. The problem this facility and staff had was they kept calling my family telling us that my grandma was constantly getting sick. I mean, my grandma is currently 97 and counting, so of course she's getting sick. Last week or so, the facility called my family to say that they were planning to evict my grandma from the premises. When we asked for an explanation why they intended to remove my grandma, they called her a "liability." We pressed them to elaborate, and they gave this half-assed response of, "Well, she's old, so you know. Things happen." When we asked them to elaborate what kind of things happened, they said, "You know, just stuff that we can't control." That immediately raised a red flag with my family that the convalescent home staff didn't want to outright say that the "liability" they were referring to was death. I mean, come again? You're in the business of convalescent homes. You are in charge of taking care of 70, 80, 90 year old people until they pass. We told them off that if they're concerned about people dying, then they are in the wrong business. Stay away from these people if you can. read more

My mom is 81 years old and she has a hip surgery that required a short term rehabilitation. Hector is a professional health care worker with a heart who listen to me wholeheartedly. He gave me so much of himself his knowledge to guide me that perhaps this moment of time that my mom and our life could change with long term of her stay in the future. Francis the PT worked so hard to improve my mom physical health. He update me always. Ms. Edna is so sweet . The housekeeping staff were friendly and thorough when it come to keep the facility clean. Hayward Convalescent Hospital was my brother-in-law's residence for approximately 3 years. Due to their care, he went from a recluse to a patient who roamed the halls in his wheelchair enjoying talking with the staff; he was taught how to use the Kindle they gave to him each day so he could entertain himself; and his mental state went from depression to being happy; and he considered this his "home". This was in addition to the physical care he needed due to the stroke that partially paralyzed him. My husband and I were always comfortable when we visited my brother-in-law. The staff at the home were always friendly and helpful to us also. So, thank you to the Hayward Convalescent Hospital staff for a job well done. read more

I have been here since September 27th,2021 and my stay at Hayward Convalescent Hospital has been fantastic. I love the staff, the nurses, housekeeping staff and fellow patients who have helped me during my time of need. Everything I have asked for from the nurses, they have responded in a positive way and questions I have asked from Administration have been answered in a professional manner. I have created friendships here with fellow patients, nurses, housekeeping, activity staff and therapy staff. Social service Director has been so helpful and been very straight forward with me during my stay. Hayward Convalescent Hospital is very aware of the Covid situation and I felt safe here as a patient. I would recommend this facility to anyone who needs Physical therapy. Where do I start? Almost 2 years ago we had to place my dad at Hayward convalescent, They have been nothing but the best in helping me take care of my dad. Everyone is nice, helpful, they take care of my dad very well. I have never seeing my dad so happy, singing, dancing. I feel great knowing that my dad is being taken care of with love. This place has new management team and their customer service is great. My uncle was admitted here last month for therapy followed by a knee surgery and he was discharged from the nursing home in 15 days following therapy services. The admission and discharge process was smooth. Great activity team to keep residents engaged. We visited my uncle and they strictly adhered to Covid visitation policies. My aunt stayed there for a few weeks to get rehab following a hospitalization. She said she enjoyed her time at this facility (at least as much as you can enjoy considering the reason why she had to go there in the first place). The employees were friendly, the food was good, they were well organized, and she enjoyed the activities. She said the people on the rehabilitation team was excellent. She said she will miss this place. When I picked up my aunt, she was in good spirits and I am guessing almost as strong as she was before she got hospitalized (I couldn't see her at the place she was living due to COVID). Though I obviously couldn't see much of the place because of COVID restrictions, the facility appeared to be well maintained and there was a fair amount of parking. While I hope she never has the need to go back to a nursing facility, I would have no issue having her return there. read more

I dont even know where to start.... Dad was sent to this place because his gall bladder was infected. The hospital (Alta Bates in Oakland) inserted a catether in his gallbladder to drain the infection and needed it to drain for 6weeks before removing his gallbladder. Alta Bates couldn't admit him there so they transferred him to this facility. Dad was doing fine for the first week...my sister and I visited everyday after my mom got into a car accident 5days ago. During this time, my dad has significantly deteriorated. He became very weak and lethargic, sleepying a lot, not eating, complained about sharp shoulder pains and nurses didnt do anything about it....my dad can use the bathroom with assistance and is capable of using the urinal, but they put him in a diaper, making a capable person stupid because of their negligence/laziness. When I called the ambulance and the EMT lift my dad to move him onto the gurny, his bed was soaked from his diaper not being changed. I am so furious and in shock! When I mentioned all these things to the charge nurse, they kept reasoning with how my dad was awake and smiling in the morning and they give him his medication...oh, and did i mention, my dad is diabetic, according to his nurse, he only ate 25% or his breakfast and lunch. But they gave him 3 doses of diabetes medicine. It isn't necessary if the patient isnt eating. Anyway, as I was walking out, the charge nurse mugged me. Rude, neglectful.... Just No. read more

This is place is a shithole it is unclean, personel poorly trained, it reaks of urine and feces, facility is so very old and the deterioration is noticeable, toilets backing up, plugged sinks that overflow when used, in hot days the air conditioning was not working, I had to scream to them in order to get their attention, people were umcomfortable, I felt so angry at their indiference to the situation, because the majority of the patients are elderly and can not voice their discontent, there is a prevalent culture of passing the buck, you can mention an issue and told that someone else will handle it, then that someone will tell you that someone else, will take care of it and on and on, when the nurse in turn takes a 15 minutes brake or the full hour lunch, nobody covers that period of time, while patients are in need of pain medications, insulin and other vital care, there is absolutely nobody to meet those important needs, at times patients call for any kind of service or the nurse, and nobody comes. During my longest and awful 15 days in a life in a very long time, I noticed how incompetent and negligent the staff across the board is, starting with the poor excuse for administrator that "manages" that obsolete convalescent "hospital", their "social worker" that took his vacation, that when I asked to talk to him I was told: he deserves a vacation too you know, and in the meantime of course nobody covered his absence; they don't comunicate among themselves at all which causes so much confusion. If you are looking to go into this shithole to recover after a surgery and need post-acute care, look somewhere else because, I know first hand that they don't have the skillful and specially trained personel, for this type of required care, the beds are stone age old, although when you call they will tell you, that they are great in post-acute care, which is an inmense lie, while being there I developed an infection in my left leg, and had to be strongly assertive to obtain the adequate care, to stop the spread of it to my foot, MG and the language barrier with personel in very delicate functions, like dispensing the medications, it would take me forever to understand and be understood. Their physical therapy program is a magnified joke, there are young kids heading heading that department, and everybody does exactly the same exercises and routines, what really irks me is how they report to Medicare the quantity of sessions, that I consider right down fraud, I became aware that they charged Medicare for 7 sessions of therapy, when in reality only one and a half took place (already reported). To summarize, if you want your loved ones to be safe, and well taken care of, please do not placed them there, they will be living a nightmare (that's how bad it is) is no place for those that are vulnerable and can't speak for themselves, and if you had a surgery and need a place to recover with a good regimen of physical therapy, don't go there because you will not get the appropiate care. I would not place my dear Grandma in that hell, I love her to much to put her in careless hands. Sorry for the lenghty commentary and hope you will find it helpful. read more

I made a new account just to review this facility. My experience was so much different than the yelpers before me. I'm sorry they had such a negative experience. For us, it was loads different. From the beginning of our interaction with the admissions, she was so accommodating to us. This was my first time dealing with putting my aunt into facility like this. My aunt was scared. I was scared, but the nice admissions staff answered all our questions. There were a few times I had issues, but my aunt's nurse, Leli, was so funny and compassionate. Whenever I made a call to the facility, Hector, their social worker was prepared when I needed resources for after my aunt's discharge for in home services. I would recommend this place to my friends and their loved ones. read more

This place has been great for my parents, we were in and out. It is an older neighborhood but is cozy and felt safe.

Unless you are purposely trying to legally kill a family member, I do not recommend this facility, It needs to be shut down ASAP. The help there is very pleasant to visitors, however the care they give is less than what one would give to farm animals. The staff is not trained to manage any medical issues. They leave your loved ones on the toilet for hours even when they are screaming to help them. Numerous times my loved one has been sent to the Emergency Room due to disease caused from Lack of Care and Cleanliness. The facility needs to be checked out by the Health Board of California and the Standards of Elderly Care. We are vigorously pursuing this and at the end of the day justice for our loved ones will prevail. read more

Most of the CNAs who take care of my mother are really kind and sweet. My mom can remember enough to know they are nice to her even when family is not around. I think they are genuine, for the most part. However, the care given in this place is deplorable. I can usually only visit on weekends and much too frequently my mother smells like she hasn't bathed in a week. I have to ask to bathe her when I get there. Last week I gave her a long bubble bath at my house but she's just too weak and we had a hard time lifting her out of the tub. This is why she's in a care home. She's unable to care for herself. Sadly, the staff in the home don't seem to be able to do the job either, much of the time. They say she has rights to refuse bathing but now she's getting skin infections due to lack of bathing! She was having them near her hairline when they weren't washing her hair but after complaint they began washing her hair more regularly and now her scalp is healthy. They had been giving her cream and I told them all they have to do is wash her hair! They said no the doctor said that's not the problem. Well now that her hair is washed more regularly the problem is gone. Who is the doctor at this place?? My sister has tried talking to them but they don't call back. Nobody responded to my written complaint six months ago either regarding her missing clothing and personal possessions. We decorate her room with pictures and posters if notes and encouraging words and they take them down. Why?! Don't you want happy residents?? She's dressed in rags because every time we buy her new clothes they go missing. None of her rags go missing. Just the nice new clothing. Now I just bring a bag of nice clothes when I take her out and then change her back to her rags when I leave her there. She doesn't mind. She's happy to have the nice clothes to wear in public. Last Christmas she got the fluffiest pajamas and blanket and she loved them. Pajamas were gone in a matter of weeks. Never saw them again. Her blanket lasted longer. I threw a fit and somehow the staff were able to make it appear again. But it's been gone a couple months again now. I have to choose my battles and right now her hygiene is more important. I keep complaining and it improves just a bit for a couple weeks. If you can afford something better please do. If this is the place for your loved one do not leave anything nice here. It will disappear. My mother was difficult when she arrived and quite depressed but she's better mentally now and is compliant. I think they are using the past behavior of her refusing baths to be lazy now. She doesn't refuse. I see her eager to bathe. She loved her bubble bath. but they say she does so they don't have to do it. I pray for a better situation for my mother. read more

The Social Worker of Hayward Convalescent Hospital is not reliable and lacks integrity. The Social Worker does not understand that family members of this facility are depending on him to return calls and show integrity by following up according to his word. Lack of follow-up and return phone calls creates mistrust. It appears the Social Workers' position is overwhelming for him! His reaction to his failure's when confronted, is to justify actions for his conscious. I have found it is difficult for him to admit he dropped the ball! He represents this facility. This representation is very negative for Hayward Convalescent Hospital. This attitude seems to be a triple down affect to the employees. It is difficult to contact the owner of this business! Another PT was left crawling on the floor because the nurses said she was "hard headed and needed to learn" she would try to charge her phone but the charger was broken.. she would want to call her daughter. At one point one of the RNs in cHarge got in my face because she needed help.. there were over 20 PT I was an activities aid ... I had to watch probably about 5 people trying to escape.. one man walked around scratching like crazy like he had scabies.. THIS PLACE NEEDS TO BE CLOSED DOWN.. this WAS reported but the owner did nothing i also reported to the HPD they also did nothing... the city and the nursing home both suck. read more
