Supervision and care was obviously inadequate. Hospital records and time do not lie. The 21/2 yr. Old facility 2023, was a nightmare, ​ 17 lbs lighter in 2 months, died malnourished and severely dehydrated with 4 head and cracked ribs all in her bedroom, developed pneumorthorax, always found alone they would find her. Why didn't anyone assist during am waking hour? with so many head traumas putting her into a wheel char and no longer able to open her eyes. The communication was so frustrating. Our beloved passed Dec. 1, 3035. Was admitted July 22, 2025. Filed grievance and never responded to family to date.
