I have had several health issues come up while here, and it has taken anywhere from a month to three months to get anyone to address it. While they force some people to take medications, while other people can choose not to. Its definitely a feel of forced compliance, while imporant problems are ignored. I give this place one star because there are some good people working here, but it seems like the people in charge don't care. I'm sharing a room with three other patients, and the space we have for each other is like an ER room. With my clothes and TV, I can only fit in there when I'm on my bed, so I can't watch TV when I'm in my wheelchair. I can't even charge my wheelchair because there aren't enough plugs, and the numerous extension cords are a fire hazard. The air conditioner in the room doesn't work, and with the current 80+ degree weather, it feels like a sauna!" read more

AMERICA, PLEASE READ! Do not EVER under no circumstances allow any of your loved ones to go to this facility. It is a total disgrace. The staff overall is WORST than HORRIBLE. PLEASE DO NOT PUT YOUR LOVED ONE HERE. The staff constantly lie, they give the staff wrong meds, they don't have answers for anything. I have observed them sitting at the desk doing absolutely positively NOTHING while patients are screaming and hollering and are not being answered. These people do not answer the phones and are very rude and very unprofessional. The pictures that is on this site are from many many years ago. (when the facility was being ran by people with integrity. The facility stink, the furniture is damaged, very outdated and nasty. Windows are filthy, a lot of the rooms don't have remotes for their tv's. I observed a derelect come into the facility and the assistant stated "Oh he used to be here some time ago". He was looking in every room and walked through the halls of the 1st floor. The nurses have a nasty attitude. Very unprofessional. The people in administration move around to other facilities so that you won't be able to identify them. A lot of them use FALSE NAMES! They have ALLEY CAT friend DOCTORS that they call into the facility to do bogus diagnosis on your loved one. So. that they can receive monies from the state. The assistant's don't feed the patients or take their vitals per the state. They fasify their documents. Staff, What staff! They will lie and promise you the world over the phone, but, when you arrive at the facility with your loved one, the place is like a place for SQUATTERS! When you ask them to do ZOOM so that you can connect with your loved one, the lady does it very quick and pushes you off the phone. So, you never see the body of your loved one. You only see the face. The staff steal terrible from the patients. The contracted employees that fill in for them because of lack of staffing forever...has observed their behavior towards the patients. They very young women that work in this facility..... stick their head in the patients door one by one and laugh so loud walking through the hallway. None of this is funny. And, nobody sees anything. When, you get to this facility, they all pretend that they are going to assist and care for your loved one to the utmost... But, the minute you leave.... Your loved one is abused and misused. This is a very serious disturbing issue that the state truly need to look into. I was glad to see they ad violated so bad, the state issued a 40, 000 fine on the facility. This place need to be SHUT DOWN immediately! The food is unimaginable.. It's like dog food. The bathrooms for the patients are not sanitized properly. Even in the pandemic, there were no masks being worn. DO NOT ALLOW YOUR LOVED ONE TO BE CARED FOR AT ELEVATE CARE NORTH. They will be in a very horrible stinking way. Their body will be dry and cracked. If, you have to call to check on your loved one because they are disabled, phones ring off the hook. THIS PLACE IS HORRIFIC! I would not have a stray staying at this specific location. My mother was there four years and a month ago she got infection UTI moved her out of her room and supposedly gave her antibiotics for 7 days. Moved her back to her room and never told me that she also had pneumonia. Got a call from someone there, which is highly unusual because they never answer phone calls, emails or do anything. Stated they knew she still had infection but moved her back to her room without anti biotics. Left her there roughly about 1- 1/2 weeks until she had to be rushed to hospital because of sepsis. My mother was at hospital 1 week and went downhill and passed away 11/27/2023. My mother also had 400.00 stolen from her and the last few times I saw her she had no water while the staff was sitting at the 4th floor nurses station on their phones and eating desert. Had to go ask for ice. I have learned a lot from nursing homes since my mom being at one. A month ago, one of the women in my mother's room was yelling for about 40 mins to be changed. Someone finally came and said she would be right back so do so. She did not come back and instead closed the door so the nurses at the station could not hear her. So when i left I asked them that the woman needed assistance. among other situations, they lied to my mom claiming two years ago that her burial was taken care of, that they made the arrangements she asked for. Last year they moved her because of remodeling that location. didn't tell me they moved her temporarily and I found out while she was at the other in Niles that they never took care of her wishes, So i did it myself. I don't know if its all Elevate but the experience with this Chicago North location is despicable and neglectful. Don't put your loved ones or family members at this specific location. read more

This message is long but worth the read. I can't begin to describe the service at this place. The staff are absolutely HORRIBLE!!!!! I have a family member here and she has had to care for herself since day 1! She has been sick and getting worse and they do NOTHING! Constant excuses of not having enough staff or the famous "That's not my job" excuses. I thought the purpose of a call light was because the patient needs something. Instead the call light stays on for over an hour each time before someone attends to the patient. They don't clean up the room, every visit there is a mess of soiled sheets, overflow of trash, wrappers left on the floor, it goes on and on. The patient was cold one day and asked for a blanket so they gave her a folded blanket and put it at the bottom of the bed. When asked if they could cover her, they replied, "call your nurse, that's not my job"!!!! This place shows ZERO care and I highly recommend their business name be change because they show NO care and it sure isn't elevated. The patient empties her own foley bag, they never bring her medication on time and the patient has to ask them for it. I've talked to the staff there and am treated like crap for interrupting their Tik Tok time. Were treated as if a bother. The icing on the cake was when the patient begged to have an ambulance called because she was in excruciating pain and the nurse told her to just take her meds and go to bed. Patient called me and when I called to speak to the nurse I was told by this nurse Brianna that she needed the doctor to call back and authorize a transport to a hospital and stated she couldn't do anything about it. So if the doctor doesn't call back? The nurse replied, "we just wait". So let me understand something, the patient is crying in pain asking to go to a hospital and all this nurse can say was take your sleeping pill and go to bed? When I expressed my disappointment to the nurse, SHE HUNG UP ON ME! I called back and asked the front desk for the name of the nurse in charge and SHE HUNG UP ON ME ALSO!!!!! I tried calling back 6 times and they wouldn't answer my call! Is this the type of facility you want to leave your loved one in? You call to express your concerns and they hang up on you? I would NEVER EVER leave my loved one in the hands of these people. If they hate their job then perhaps they should go find another! To think that they do this with a coherent patient, imagine how they treat those patients that can't speak up for themselves!! This place is a facility where they show zero care and act as if you are a problem. If your looking for a facility where you can trust that your loved ones will be cared for... RUNNNN!!! I had a huge promotion and my father is staying at Elevate. He isnt in the best shape to come out, but he was fixed up nicely the folks at Elevate and has gotten stronger with the PT. He was accompanied by Michael Johnson who took the BEST CARE of him. Michael got him there, put him in the right place to see his daughter get promoted and Michael even took pictures. My father had a great day and was super proud to be there. Without Michael and the crew at Elevate getting it done, that celebration wouldn't be the same. Thank you to all. read more

Greetings, My name Is Dixie W. and I am compelled to share my experience with the facility known as Glen Crest Nursing and Rehab. I have been a RN for 33 years and never imagined that at age 52 I would find myself requiring subacute skilled nursing care. Unfortunately I suffered a severe fall with a head injury and secondarily a GI bleed, and was hospitalized for 11 days. During that time I also experienced 2 seizures and underwent many diagnostic procedures. I spent 5 days in ICU and was very weak and debilitated. I was unable to return to work and my prior living situation. Fortunately the Attending MD at the hospital recommended I go to Glen Crest for Nursing and Rehab Care. The very next day a representative from the facility came to meet with me to share about Glen Crest and facilitate my transition. I was so grateful to be accepted and felt hopeful! I was admitted to Glen Crest on 9/29/17 and upon admission could only transfer from my bed to a chair 2 feet away with maximum assistance. As soon as I entered the facility everything I required was provided immediately. I believe the coordination of services is the cement that binds and promotes healing. Allow me to elaborate: The Nursing staff was always courteous, professional and timely. They contacted my MD briskly if I required additional orders. The CNAs were kind and helpful, and always readily available to assist me. I also had the care and guidance from their Nurse Practioner, who was available 5-6 days a week in the facility all day. Next was the dietician who I met with 3x during my stay. She was always concerned that my menu was accurate ( had prev. gastric bypass surgery). And the food was very good. If I felt I wanted an alternative the substitute menu was very solid. I have to say it was my first experience eating Kosher, and I liked it. Upon admission I was provided a customer service booklet and it explained all I needed to know about my new dietary experience. Next was my assigned Social Worker. She was awesome! She was a good listener, compassionate, and went out of her way to help me. I certainly was emotional for a while! And last but not least, THERAPY!!! I had the most dynamic, motivated, and creative Physical and Occupational Therapists. I have been here 18 days and with the superb direction of my therapists I am close to my pre fall baseline. I receive therapy 6 days a week and it has not been easy, but ever so rewarding. Their techniques to improve my overall strength and balance are challenging. Everyday I look forward to my time in the gym, which by the way has extreme state of the art equipment. But it is not even that, my OT would take me outside regularly to do exercises that refreshed my senses, just by being outside and do something innovative. My PT would play music for me because he realized it was a great motivator for me too.I will miss them both so much. I am totally independent, doing laps outside and have recouped so much, so fast! I cannot say enough how this place has brought me back. From activities to the receptionist staff....all caring. Almost forgot to mention housekeeping and laundry. Both services are very good. My laundry was typically done and delivered back to my room, and folded the day it would go downstairs. The housekeepers on the 1st floor were standouts too. I hope I haven't left anyone out. I have so much gratitude for Glen Crest in my recovery. I am discharging the 20th and am amazed how far I have come with Glen Crest. Ever so THANKFUL!
