I had a frustrating experience with my mother Elena's care on Friday, May 10th (Orthodox Good…read moreFriday), at Elevate North Branch in Niles. My mom, who requires full-time skilled nursing and is fully dependent, had a crucial 9 a.m. hospital appointment. I arrived at 8:25 (as I always ride along to advocate for her), and the pickup was set for 8:30, but she was still in bed. Due to short staffing--only one inexperienced CNA for a Hoyer lift--I took action. Being fit, I lifted my mom myself, leaving at 8:50, and we arrived only 10 minutes late. I did not put anyone at risk--I'm fully capable. Yet Steve accused me of jeopardizing jobs. I'm not trying to get staff in trouble--I hold management accountable for this recurring pattern of short staffing. Hoyer lift patients require more support. I urge management to ensure proper staffing so families like mine aren't forced into this again.

Writing this solely to help someone who may be in a vulnerable spot searching for a place for their…read morefamily member as it is not easy writing this...it brings back a flood of horrible memories. No matter which nursing facility you place your family member.. if you can, please make sure to visit ..and visit often. Not one nursing facility is going to be everything you are looking for ...budget cuts/lack of staff/difficulty hiring and retaining staff unfortunately is affecting many nursing facilities. It would be naive to think one facility must be a good place due to a trusted friend or neighbor saying they had a great experience in said facility. Too many variables not mentioned that affect one's perception of care that need to be considered. Is your loved one mobile or fully dependent? Is your loved one able to communicate their needs? Is your loved one going to the facility for temporary after-care or for long-term care? Seen this facility (6840 W. Touhy Niles branch) through its transitions from when it was known as Forest Villa to Avanti Nursing Home and now Elevate Care. Do not let photos and the main floor deceive you as it does not speak of the entire facility. Offices on main floor seem nicely updated while the residents' rooms are left needing work. The first floor may seem somewhat welcoming and is mainly for their temporary residents which is where my dad first started. The rehabilitation/physical therapy room is on the first floor and if you are lucky to come here for that and go back home, great. The physical therapy staff were the highlight of my dad's days. Unfortunately, my dad's condition progressively worsened and he was moved up to the second floor which is where they place their long- term residents. This place as my dad described it was literal hell on earth. Sadly, after scouring every review and driving from one facility to the next searching for a place for him we quickly realized there is no such thing as a good nursing facility...We ruled out the nursing facilities that reeked of urine at first entrance and landed on this option as one of the "better" ones and chose to visit as often as we could. I cannot stress enough the importance of doing so. There just is not enough staff to provide the type of care you will want for your loved one...PERIOD. Add to that if your family member has a severe physical disability or is completely immobile and needs assistance feeding, brushing teeth...to even hope for any sort of consistency in that regard or think that any complaints will be rectified...serious waste of time and energy. Cannot say how painful it was to even have to wonder if your loved one has eaten or not. Trays left untouched, water cups left out of reach, call light strings left out of reach...for someone with limited mobility, 100% know that there will be neglect. As our dad declined, this place simply was not able to meet his care. His teeth went unbrushed, lack of care and being diabetic...literal teeth lost in matter of months. Whether he ate or not depended on if a kind soul paid enough attention to notice he wasn't able to reach his food or water, let alone help himself due to his limited mobility. Nearly full shifts w/ not one staff changing him out of his soiled clothes/bed until we would come and ask for it...even then call light response was horrendous. Peeking out to see if there were staff nearby to help...either no one or if there were I would often see them on their phones or talking amongst themselves while call lights were left unattended and often hearing residents yelling "help, help!"...Countless concerns from both my dad and family dismissed...Responses to rectify but no follow through...The feeling of defeat, hopelessness and frustration and knowing my dad had to live this daily...Eventually my dad ended up receiving hospice while here and even with the hospice nurse coming in weekly we did not stop our multiple days/week visits due to the neglect here. There are some kind souls working here but kindness simply does not fill the need for additional staff...with that said there are also many working here who are completely apathetic. It would be great if this facility had some time dedicated to educating their staff on elder abuse as some may not even realize that their dismissals is a form of abuse...compassionate elder care and how their small acts can really make such a huge difference. If the people running this place experienced even ONE DAY of their loved one being here or if they found themselves suddenly immobile and dependent on care, I question if that would bring about any change? Sadly the problem here is too great that I don't believe the situation here is going to get any better. My wish is this review will help someone going through the difficult situation of placing their loved one in a facility and for whoever reading this review that they do not have to go through what my dad did. May he now rest in peace.

I have since been harassed by a secretive collection agency in Wisconsin. I discovered they stole two social security accounts from my bank account that almost caused me to be evicted with a freshly broken hip and in a wheelchair. They are very secretive. The name of the company is Infinity Health Care Consulting in Milwaukee, WI--it's a collection agency! I would NEVER recommend this nursing home!! They are part of a financial scam. And the atmosphere is as depressing and confining as any other nursing home. Scammers on top of it all.

I was so touched at a recent visit to Nazarethville, I was brought to tears. But in a good way…read more This facility was founded in 1974 and sponsored by the Sisters of the Holy Family of Nazareth. It is open to all, but there are many retired nuns living here. On a side note- nuns do not receive any pension when they retire and many work as teachers and nurses, well into their 80's. It's a hard life. I believe that quality of life is the most important aspect of maintaining your health into a ripe old age. The combination of a home-like atmosphere, offering wholesome living, balanced nutritional programs with menu selection, medication supervision, social events and entertainment are what I believe makes Nazarethville so incredible. I was there on a Saturday with my seven year old niece. Her entire class was there to assist the residents in a lively session of bingo. The school requested that we bring a little gift- to be given as prizes to the winners. Apparently tissue is in big demand, because it is not provided and must be purchased by the residents. In addition to this, many people brought little stuffed animals and puzzle books and such. To see the joy and watch the faces of the residents light up when the children arrive is truly wonderful. The children enjoy themselves as well. It's really a win-win combination. Whenever the residents won at bingo, they were able to choose a gift. Although they desperately needed the Kleenex, they all chose the stuffed animals, and then the books and always gave the prize to the child that was assisting them. I can happily tell you that they organize these type of events on a monthly basis. They also have occasions where (well trained) dogs come in with their trainers for a visit. Another big hit! The facility is spotless and adorned with the most beautiful and inspiring artwork.- all painted by a resident. Sister Jolanta. Her beautiful paintings emit a magical light- similar to the work of Thomas Kinkade. She was an art teacher for many years, but I don't believe you can learn to paint like that. It's a gift from God. So remember, if you find any tissue on sale, buy a little extra and stop in Nazarethville. You'll be happy that you did : )

Residents were treated poorly. When I got there from the hospital I was put in a diaper and then placed in a wheelchair where I stayed for my entire 2-month stay. When I would soil my diaper I would have to wait over 2 hours to have it changed. The night staff was terrible I once called to go to the bathroom and the black Aid told me to go in my diaper and she will change it in the morning. The day consisted of getting up in the morning getting dressed and then having breakfast. After breakfast you would go down to therapy. The therapists were fantastic. After a therapy I would go up to my room and have lunch. I could not go to the lunchroom to eat because it was horrible. You said eating with people who did not want to speak. I asked to have my meals served in my room. As for the food it was horrible unedible and disgusting. The only decent thing to eat on the menu was the pudding. I certainly would never ever recommend this facility to anybody

The black ladies at night were horrible people. You will sit in your urine filled diapers for all…read morethey care. They have jobs but don't want to work. They all need to be fired. Do you think i wanted to be there? Food tucked. MY dr would not even go there to check on me. My insurance got me there because I don't have BlueCross. Still care should have been at least up to standard. One night the black nurse just walked out when i told her i needed my diaper changed. The day people were nice. Isabella was great but she pushed the god issue where i knew she was brainwashed. There was a Ukrainian lady that was an angel. Therapy was great. 2 guys were good. Not pushy but knew how to get me going positively. My insurance sucked. Would never extend therapy. Now i lay in bed not being able to stand up without help from my son. I try so hard. It hurts. Can not lock my knees so good. I dont want to live sometimes. Needed at least 1 more week of therapy. Insurance sucks. Should not complain because i have not paid anything even with the back surgery. But i cant walk. Needed more therapy. My crappy insurance kicked me out. Never saw anyone actually from the insurance company, just word of mouth through Alden representative. I will never trust any insurance company again.
