My family had a loved one brought here for care. It was THE WORST place I have ever been to in my life! It was filthy, with dirt and messes everywhere. Our loved one was plopped into an old metal bed that didn't even hold him. He was falling off the side of the bed. They had to search and Try and find a pillow to prop his arm up. He lay there waiting to be attended to for hours! We observed "nurses"?? using the same pair of gloves for everything they did. I would not leave my dog there!! My family member was in and out of their Waukegan facility for several months. The staff were attentive, compassionate and went above and beyond to provide support for our loved one and family. You are warmly greeted by the front desk manager, Andre, when you enter the facility. Several of the nurses said they work there because they love taking care of people, especially older people. Their intake coordinator and social worker were always available to answer questions. We spoke with the head of nursing, after hours, on more than one occasion. The nutritionists and cook did their best to accommodate special requests. The physical therapist would not take no for an answer which was necessary. I am a friend of a resident who has been staying at the facility for a few years. I've worked with many social workers, nurses, and case managers throughout my time, however, Jamie has been wonderful. She is newer to the facility but has answered all my questions, made me feel secure, and has helped me more than ever. I'm very thankful for her. I was not able to review this facility when my relative was there. My family feared backlash so badly that we tiptoed around trying not to make waves, and even when we did have a complaint, it was completely ignored. Let me be clear in that our concerns were not petty, by any stretch of the imagination. The staff seemed to be in La-La Land. I asked about pain control, one staff member told me that my relative was not receiving any pain meds, another said he was being given liquid pain meds like Motrin, yet I found Fentanyl patches on my relative. Upon visiting one day, I noted a very strong smell of feces, and informed staff that a change was in order. The floor nurse sent an NA into the room to do the change, as I waited in the hallway. When done, I went back into the room, and inspected my relative, and found that there was dried feces all over him, and the watery stool had seeped under a bandage on his tailbone that was placed for a large bedsore, and the bandage was not changed. Neither were sheets that had stool on them, etc. My relative was not bathed, nor groomed...I could touch his arm, and dead skin cells would come off his arm in my hands in little rolls. His hair was full of dead skin cells, his face was oily, he smelled very bad every time I visited. Upon arrival, my relative was not able to communicate. After a time, he was able to, however, and communicated to me that although the staff did not 'treat him bad', they were very neglectful in that they did not attend to his cleaning, feeding, etc. in a timely manner. Another patient was placed in my relative's room, and began calling my family when issues arose with my relative. When staff found out, they made threats to move the other patient. Staff threatened to take my cell phone because I was taking photos. They claimed it was for the privacy of other patients, but I was in my relative's area, with curtains drawn so that no one else would be in the photos. When I refused to give up the phone, or shut it off and not take photos, stating that it was my right as next of kin to do so, they told me that they would have security remove me from the building. I did manage to take many, many photos of my relative, and the care he received at this facility. Word to the wise: Either don't leave your loved one there, or be with them around the clock to monitor their care. read more

I put my mother and her life partner in this facility in Feb with the impression that it had new owners and better staff. I was proven wrong on so many levels. The first floor looks amazing and is just a cover as all the other floors are in the worst condition. There are holes in the wall, paint peeling and things that just do not work. There is 1 nurse that somewhat knows what she is doing and communicates in English well but lacks the skills needed to stay on top of things. The director of nursing does what she can but it is hard when every other staff members do not do their tasks. It took months to explain what a power of attorney entitles and that I needed to be aware of all medical issues, procedures and med changes but I think we finally have it down. The change of staff constantly makes it hard to keep track of every one but no one lasts here. Then they have other staff step in and makes matters worse. My mom and her partners biggest complaint that even before covid there is NO activities bus or even bus to take them to the stores. Now with covid in effect and whole 3rd floor changed into the quaratine floor activities have halted making all residents feel trapped and a prisoner to their rooms. I have seen plenty of other facilities still doing activities with residents including bingo but with more space between so definitely doable. Also the transport company the use Elite Ambulance is a mess. I went to 2 different appts and first one they were so unorganized that i could not even sit in the office with my mom as they had my mobile mother strapped down to a gurney to the point even the dr questioned why as she could walk. The second appt I traveled all the way from Elgin, IL to Evanston, IL to find out they didnt even arrive at the nursing home yet so had to reschedule the appt again. Go the following week and the office staff could not even get ahold of Elite to pick my mother up so I ended up transporting her in my own vehicle back as they would of left her stranded. Worst company ever and no idea what corporate is doing as none of these issues are getting better and covid restrictions halt me from taking my mom to appts which is ridiculous as i sit in the room with her regardless so what is the issue of her riding in my car to get there? Totally unacceptable and no one knows what next nurse is doing honestly. They do not use the EMR like they should as I dropped off after visit summary on the 16th with orders that I reiterated to the nursing supervisor that needed to be scheduled and now 8 days later still not scheduled nor can find the paperwork but ADON called to assure someone faxed in a request on 18th and she has personally taken the matter into her hands to get it scheduled. We will see as I am pretty sure I will be the one scheduling. read more

Sorry to say my dear Father passed away at this horrible nursing home after only 4 days!!!! What a deplorable place Dr Lee from North Chicago who had been my Dad's primary care doctor for probably 30 years sent my dad to recover from Deep Vein Thrombosis and Blood clots in his lungs to this hell hole!! He was sent because they said he needed physical therapy that the hospital could not provide but in turn sent him to a place that the nurses station gas so far from his room he had no chance of surviving and even after one day When I was visiting he stated that they didn't answer the call button. I noticed that the toilet had overflowed two times and asked to send a mop because I would clean it to prevent my father from falling so they finally sent housekeeping who in turn told me there was no water until I showed them it and then they mopped it up. I had to take disinfectant wipes to clean the dirt and grime that was left from previous patients. The bed was dismantled from the wall in my Dads room.When They called to let me know my Dad Passed away they had a patient in the other bed that said he was sorry that my Dad passed away. That patient was not in the room the night before when I visited my Dad they moved him in there possibly that night and then left him in the room after my Dad passed away. Why would you do that??? Also When we arrived to pay pay last respects and call the funeral home we were reminded that we had 7 hours to get the body out of there.
