My dad has been in the memory care unit for a little over a month now, and I truly can't express how grateful and relieved my family and I are to have him here. The staff has been absolutely incredible. Every nurse who cares for my dad, along with the social worker Hannah and the rest of the team, has been compassionate, attentive, and very responsive. Any time I have a concern or question, they respond quickly and take the time to address everything thoroughly. When I call, they always answer and are kind enough to let me speak with my dad, which means so much to me. The memory care unit itself is clean, well-maintained, and always smells fresh. Most importantly, the residents appear comfortable and content, which gives me real peace of mind. I'm truly thankful for the care and support they provide every day. read more

If you are considering placing a loved one at Avantara Long Grove, I strongly encourage you to think very carefully. My mother is a quadriplegic and completely dependent on staff for virtually every aspect of daily life. She cannot transfer herself, independently get food or water, reposition herself, or simply get up and find someone when she needs help. When a facility accepts responsibility for someone this vulnerable, there should be an extraordinary level of accountability. Our experience has been anything but. There have been occasions where my mother has gone extended periods without being checked on, repositioned, or receiving basic assistance. I have had to intervene because she had not received dinner. Concerns that should be addressed proactively have instead required phone calls, follow-ups, and escalation from family. That is perhaps the most disturbing part of our experience: I cannot simply trust that things are being handled when I'm not there. Families shouldn't have to constantly monitor a nursing home to make sure their loved one is receiving basic care. We shouldn't have to repeatedly call, email, escalate, document, and advocate just to get things addressed. For a resident who physically cannot advocate for herself or meet her own basic needs, failures like these aren't minor inconveniences. There are individual nurses, CNAs, and staff members who genuinely care, and I don't want their efforts lost in this review. The problem is the lack of consistency and accountability around them. Good individual employees cannot compensate for a system where families feel they need to remain constantly vigilant. A nursing home isn't a hotel. The people living here aren't customers who can simply walk out when service is poor. Many are among the most vulnerable people in our community and are completely dependent on the people entrusted with their care. My mother deserves dignity, attention, and dependable care whether her family is standing in the room or not. Based on our experience, I cannot recommend Avantara Long Grove to families looking for a place to entrust with the care of someone they love. read more

My mom has been here three separate times. One for each side of hip prior and a rehab of mini stroke. The staff and service is top notch. The place can cook to accommodate different food pallets. The employees and staff whom I have interacted with take pride in their job and place of employment. The staff does interact appropriately with the patients and the hands on with my mom. Thousand percent helped her recovery to be faster. The rehab has a great and knowledgeable support staff that helped guide us through these times we had. Understandable and explained processes. The plan that was provided expectations and we as a whole family are blessed that we have such a great health care experience each and every time there is no other place to go to. I recommend to all whom I meet and people ask in the community for guidance. There is no question where one should go to. Avantara I will have for life a great and genuine appreciation for all that this location, staff, support, administrators has done for my mom and our family. One of the nicer facilities I've been to. It's not perfect, but there's some characteristics which make this place stand out. - Off of the main road, so it's not too noisy and it's easy to get in and out - Parking is spacious and near the entrance - There's plenty of conference rooms, outdoor areas, spacious front, and other rooms to bring a loved one to, if you do not want to spend time in the patient room. - The staff is very friendly and goes above and beyond to ensure patient comfort. The response time for requests is reasonably quick compared to other facilities I've seen. - Good selection of soft and liquid foods - Plenty of activities planned throughout the month Overall, I don't think you'll ever find a perfect facility-- but Avantara and Warren Barr are both good options in the area for rehabilitation centers. read more

My mother stayed at AVANTARA LONG GROVE Rehabilitation Center for almost two months, and we are really happy we choose this facility. AVANTARA LONG GROVE not just met all possible expectations, but it offered much more, like a single room with an excellent and personalized patient care, delicious restaurant-like food and family support. That place is very clean, and very safe. The staff is professional and friendly. My special Thanks to Tatyana, the Social Worker - she helped us with the process, answered all questions, and she made the whole rehab experience easy. Nurses and aids didn't seem to care. Patients lined up in the hallway and left to sit quite often. Physical and Occupational Therapy teams were great. Food services are fair and meals delivered cold most days. read more

My 92 year old mother was admitted to Avantara following her hospitalization for a heart attak, the staff had ll been very good and very professional. I am very pleased with the care and help that they had given to my mother. Everyone from the cleaning crew to the CNs to the nurses, the DPTs and Social Workers, they were all very upbeat and always showed that they were happy to help. The staff at this facility did the best they could. Communication is very poor. When you walk in, the entry room is very nice and smells great but just travel down the hallway and the stench of feces and old bodies permeates the air. The "Care Team" person talked to me on a Friday and told me the plans. They were changed but no one bothered to call me to let me know. I found out from a nurse, who was instrumental in my husband's recovery. I called the facility's doctor 3 time before I got a return call. They are more concerned about the insurance money than the care. The rooms are not kept very well and I had to request a better mattress for the bed. When the discharge date was finally determined, I knew from a doctor on the outside much sooner than the Care Team on the inside. The woman was asked to call me on a specific number and not my cell phone. She had every excuse why she forgot when I questioned her. After that, she continued to call my cell phone although I told her I couldn't answer it since the call would be dropped. No matter how many times she was told, she just didn't get it. She tried to tell me what the doctors' orders were. Problem was, I had already seen the written order and she argued with me that I was wrong. She found out she was wrong but never an apology. The halls have big lights but the hallways are dingy and depressing. There is nothing happy about this place. The food is not good and they don't care if you eat it or not. Some of the residents constantly yell at all times of the day or night. Makes it hard to recover when you need to rest. I would never have chosen this place. The choice was made by someone else who didn't bother to do the homework to see if this was the best place for recovery. There will never be another stay here. I witnessed workers yelling at patients, yelling at other employees and sitting with their phones instead of working. The staff is meager at best. They do the best they can but there is only so much energy you can expend in a shift. I would not recommend this Nursing home to anyone, regardless of their personal needs. read more

Speaking to my mother as she loves to tell me about her days, told me how happy she feels and is at this facility. She talks about staff and the activities she does here. She's always in a happy mood here. We are thankful as a family to have her at Avantara Long Grove. She especially speaks about Enrique how he is very kind and always goes above and beyond for her and other residents. I have met him a few times and he's a very nice man. My loved one was staying here. there was no one ever there to get help. Smelled awful and many people clearly needed help. Now with covid my loved one hasnt been moved or out of bed for days. There is no excuse for that, this is how people get sores and sick. Dont tell me theres not atleast two workers there to get her out of bed. Unacceptable. read more

I have a dear friend who has been there for 2 months recovering from two fractures. Their phone system is nonexistent! The last 2 weeks I have been unable to reach her, and her to reach me. I think it's terrible that they can't seem to repair this quickly. I feel strongly it's not a priority for them. You can not visit the patients due to Covid (I understand that) but not being able to even talk with them? Terrible. read more

This place is a great facility. Overall clean, sweet staff and great attention to loved ones that are there. My loved one is there and I feel happy known she's getting taken care of very well. She loves the attention and overall the activities she does. Our conversations are always about what she does there, the people she talks to and so much more. Knowing that makes me happy we chose this place for her. With Covid hitting she's been sad but she knows myself and our family love and care for her dearly that it's important she's safe and that the care she's receiving is well like this place is giving her. First, there is a section to the right of the reviews that references Propublica and it says this facility has zero deficiencies. I went to that site and found 26 deficiencies as of today. I decided at the last minute to go to rehab for a week following total hip replacement and the facilities recommended by my doctor either weren't in my network or were full. So I picked this facility, based on the fact that it was one of the few in my network that was closer to my home. Big mistake. I stayed from Saturday early evening until Tuesday early evening and would have left Saturday night if I could have contacted Blue Cross to make sure I wasn't doing anything that might lead to their rejecting a claim. In response to a survey they gave me, I wrote that I was not told about guest services or dietary. My family was not provided a number to call to receive patient information 24/7. I was not told about resources and services provided by Avantara. I received my admission packet the day before I was leaving. I have not been given information on home health or adult day care services. I have not been told about support groups, financial assistance or respite services. I got the second part of my welcome packet the afternoon I was leaving, a bag filled with toiletries and a fruit infusion water bottle. When I got home I found that those two items, along with some other things I had, were missing. I packed everything up in the morning and remember putting them on the cart. I went to rehab twice that day and hadn't noticed the items had been removed. I haven't contacted them about this, because quite honestly, I don't think they'd care. My first few hours told me all I needed to know about this place. No water was provided for hours and not provided until I asked for it despite the fact that they knew I just had surgery and needed to remain hydrated. No chair was in the room and nobody offered to get one for my son despite their many trips in and out of the room to get information from me. An hour after my son left I hit the call button because I needed to use the restroom. After waiting 30 minutes for someone to respond, I decided to get up and use my walker because I couldn't wait any longer. This tripped an alarm on the bed and that's when someone appeared. We walked into the bathroom and not only was it NOT prepped with a toilet riser, which you need following total hip replacement, but there was no toilet paper on the dispenser. So I stood there for another 12 - 15 minutes waiting for them to get everything needed and I wondered if the bathroom had even been cleaned. A 42 - 45 minute wait to use the restroom following surgery is inexcusable. The floor was dirty in my room. It was quite loud in that room, and they were trying to convince me to stay and offered to put me in a quieter room. The second room was quieter, and I appreciated that, but it was dirtier than the first room. There were fuzzballs and pieces of paper and things all over the floor. The tv remote was sticky, as though someone was eating jelly and channel surfing at the same time. And when the occupational therapist brought me to the *spa* to show me how to enter and exit the shower, the bathtub was dirty. They brought me a bunch of bananas along with some other things for breakfast the next day, despite my record indicating I have an allergy to bananas. They dispensed the wrong medication dosage at one point, which I caught by double checking everything. i was told by my first nurse that it was up to me to ask for my medication at the time I was due to take it - that they weren't responsible for keeping track and just dispensing. Ever hear of that before? Me either. The beds do not have the light functions enabled so you have to either get up from your bed to turn the lights on or off, or hit the call button for someone to turn them on and off for you. This is a waste of resources, since staff is running around turning lights on and off, when they could be answering calls to use the restroom in a more timely manner. I can think of no reason other than expense that these options were not enabled allowing patients to simply hit the button on the sideboard of the bed to control their own lighting. Even the staff didn't know why they weren't enabled. I can't describe the food but nothing tasted normal. Or palatable. The bright star was a nurse named Fred. Very attentive, double checks what he's doing, is great at his job and cares about patient care and comfort. HE should be running the place. ATROCIOUS!! read more

The rehab and private section may not be bad. The 1st floor was remodeled few years ago and looks clean and has no obnoxious odor. Sadly, the long-term facility in the basement is quite opposite. It is dirty, dark, and looks absolutely awful! It always smells like human excretions, and who knows what else. On the positive side, most of the staff is friendly and attentive to the family requests. read more

Not only did my younger brother die in the hands of Avantara, but they have been pursuing me, his sister, for the balance of his account. His electrical union payed them tens of thousands of dollars. The care provided was inconsistent at best, negligent at worst. Medications were left by his bed. He was unresponsive for over a day before noticed. I am the sister who cared for him and am now being harassed for a bill that I AM IN NO WAY RESPONSIBLE FOR. I am looking for a lawyer. read more

I had the chance to tour this skilled nursing and rehab facility last week and was very impressed by how bright and clean the place is and the "Preserves" wing has a spa-like feel. The therapy gym is really nice and inviting. The staff members were friendly and engaged. I will definitely consider this place for my mom's therapy. read more
