Back pain is horrendous & I was in excruciating pain for months while "under care" of the doctor. Once excessive tests (many within their own practice) were done & it was determined my case was too extreme for Derman, I was once again referred only to an internal doctor that first available months out. It is absolutely inhumane to think so narrow-mindedly that a patient is neglected. I begged for help, called multiple times a week for help only to be forced to the ER & find a doctor that cared more about my pain than keeping every dollar within their 'network'. Now months later, major surgery now behind me with a true gem of a neurosurgeon & human being, I am still getting bills from Texas Back Institute that I cannot even pay via the link nor QR code nor phone number on the bill. The account # has insufficient digits & the phone # refers to me a different phone # that is outbound calls only. How on earth does anyone trust their future & spine with an office so disconnected with patient care & follow-up? I have had many doctors through the years & many large organizations, this is by far the most disappointing experience, every step of the way.

I made it a point to drive from Central Texas just to see this clinic over the past three years. Their plan of care recommended a revision surgery, but in order to increase the likelihood for success, they suggested I lose weight to get to a BMI that the surgeon was comfortable proceeding with. I was scheduled for a one year follow-up, and additional imaging was repeated that showed moderate progression of spinal issues. Again, the challenge with surgery was weight, and the plan of care was to suggest pain management as an option in the interim, until we reached a weight that was satisfactory to proceed with surgery. This week, I called in to schedule a one year follow-up with one of their PA's. Acknowledging that it felt like a waste of time to see the surgeon because I had not reached their suggested BMI, I expressed my desire to schedule with a mid-level provider. This is where things went awry. I was informed that because my BMI was out of range, they would not be able to schedule an appointment, and instead I would receive a call from their office. I took to MyChart and sent a message to the PA I had seen before, requesting help with scheduling. A nurse responded quite tersely, saying that I am out of range to be seen based on my BMI, and an office visit would not be offered to me. After some pushback, the nurse responded that I would be offered a phone visit with a different PA to see how my pain management injections went. This didn't make any sense, since both providers work in the same system and have access to each other's clinical notes. Eventually, the PA who I originally wanted to see, messaged that a visit would not be needed since we wouldn't be discussing surgery at this time. They were willing to schedule, but again reiterated it wouldn't be necessary, since we wouldn't be discussing surgery. Pointing out my BMI as a reason for not proceeding with surgery is not the reason for this review. These are clinical guidelines they've decided to place as metrics for success. I understand that. What did disappoint me greatly, was using BMI as a method to gatekeep ongoing care and consultations on a lifelong illness like scoliosis. I have had both scoliosis and weight issues since childbirth. In fact, I am undergoing genetic sequencing to identify issues related to weight loss, and bone and joint issues, and wanted to relay these updates to this team in hopes of it somehow furthering their practice and understanding of the progression of this illness. To have a provider and their staff abruptly place BMI as a prerequisite to continue receiving conservative and preventative orthopedic treatment and evaluations, lacks the care and attention that Baylor Scott & White promotes to their patients. To be cut off in this manner leads one to believe that the providers are assuming the patient has not tried to make meaningful progress for weight loss in order to qualify for surgery, or, that the patient simply isn't serious about it. This is not true, and these are details that could've been unpacked, if there was interest in hearing the patient out and forming a plan together. There was not. When a clinic or provider establishes a barrier to access care without seeking to track the patient's progress, understand obstacles to success, and notate changes in their disease progression - that really speaks volumes to the lack of interest in actually treating patients. It changes the dynamic of the provider from a basis in care and wellness, to "surgery as a service" and the patient as just another person to operate on. This approach lacks humanity, respect for patients, and is shameful for an organization which prides itself on doing better. Update: I received a call from the office manager extending their apologies while they implement this BMI protocol. I hope this feedback improves care/pt outcomes.

He suggested fibromyalgia, which was blowing me off, because he clearly didn't look at my chart. Because of the lack of scanning, the PT caused a tendon tear. I may have to have surgery. Office staff were rude and not easy to work with. They wanted us to drop everything the day of the appointment for information or were going to reschedule, when the appointment had been in place for months and we both work. They were hostile. If you aren't a pro athlete, they don't care.
