Linda has many health problems and our family would never consider having her at any other health care facility. For example: She's very susceptible to pneumonia and, at the first possible symptom, their staff immediately takes x-rays and appropriate action. In addition, every member of MCC's staff is kind and caring. Linda is always listened to with respect until the best solution for any problem is found. MCC's communication with our family is extraordinary. Their staff works closely with us to make Linda as comfortable and happy as possible. We are a large family and that enables us to devote time to the entire family since the staff at MCC takes such excellent care of Linda. On top of all that, MCC schedules a wide range of interesting and fun activities that Linda and the other residents enjoy. I've personally spent many hours in all areas of MCC and have never seen or heard of any situation even vaguely resembling that mentioned in other reviews. A bonus is that our loved one raves that the food at MCC is exceptional. Our family highly recommends Maplewood Care Center.

There were others that just wanted to collect a paycheck, they did not care for the people in their care unless they knew someone was watching them. I went to this site everyday for 2 months while my father was in hospice. They rarely cleaned the floor to the point I complained to management, management made excuses instead of addressing the issue. I witnessed a woman fall out of her chair and land on her face, they picked her up and put her at the table for dinner. I now know such a fall would be called a trauma silver and someone should of done a full head to toe inspection of her body, they did nothing while the woman cried while trying to eat. Many meals barely had any protein on the plate and most meals were cold. There were other issues.

On her first day on the floor, the nurses decided to only do spot checks on her saturation because she was always trying to take her probe out. The problem was that she was also taking her oxygen off too (and grabbing the PICC line despite the sleeve they put over it). When I asked how they would know if she was desatting - or needing less oxygen, they insisted on just doing spot checks. Keep in mind, they rarely came into her room. Another issue I had was with my mom's incontinence management. In the ER they used a purewick to help keep her dry and thus protect her skin. Once admitted, they stopped because they said it "was a waste because she was pulling it out." Instead, they wanted her to urinate in an adult diaper and then wait until they did rounds to change her. No offer of a commode or bed pan. A diaper. Meanwhile, her patient care assistant named Honey (who was amazing!!!) suggested that we use it when we get her up in the chair, but the nurses said no - because again, it was a waste. Once up in the chair, mom said she had to use the bathroom again, but the nurse started to walk out of the room anyway. I told mom that they didnt have a pure wick on but that she had a brief on, and the nurse (Katie) mumbled "that's because it would be a waste," and kept walking out. The nurse was obviously fine letting mom sit in a wet brief until her next 2 hour check. I guess I should be grateful that she didnt try to get up and go to the bathroom herself, which she kept saying she was going to do. I'm pretty sure she would have without someone in the room with her most of the day, especially when they had no clue she was pulling on her PICC line and taking her oxygen off when I told them. Like I said, they were rarely in her room. By this point, mom was afraid, restless, and wanted to go home. The plan was to discharge with the oxygen, as I wasn't comfortable with her there anymore. The doctor said that she needed to wean her off of her oxygen before discharging (or she could transport back to memory care on oxygen), but that it wasn't safe to go back without it. After RT and the doctor left, I noticed that the oxygen tubing was disconnected from the wall, so I asked her nurse, Katie, if it wasn't supposed to be on, and she said "no, it can be off." Then another nurse came in and said that it would be OK to put it back on "since she was resting, and she would be leaving with it anyway." What happened to it not being safe for her to be off of the oxygen??? Once the portable oxygen arrived, they got her dressed as quickly as possible and rushed me out the door to pull the car up before they even had her discharge papers! They brought her out, put her in the car, handed me her papers, and left. They didn't give any instructions, nor did they tell us that her prescription antibiotics were at the hospital pharmacy. This was by far one of the worst experiences in healthcare that I have ever been a part of! As a nurse of 31 years and an advocate for my mother, I can honestly say that she would not have been safe there if she didn't have family that could be with her to ask questions and make sure she got what she needed. I will not be bringing her back to St John's. I don't think anyone who has loved ones with dementia should consider bringing them to this hospital either. It's not safe for anyone who cannot speak for themselves!

She had some complicated chronic medical conditions, and there was little attempt to coordinate with providers who had an understanding of particular treatments that were important to provide. Quality of staffing was variable, with some at times seeming negligent and at other times competent. See…read morehttp://www.yelp.com/biz/high-point-surgery-center-lake-elmo#hrid:6kT-ytuaUmbPOq58jWiqSQ for the third review. It's difficult to do clinic reviews because so much of it is tied to your particular care provider (i.e. doctor, nurse, physical therapist, etc.). In my first visit to OSI, I wanted to get my knee assessed before deciding on surgery. OSI has a $30 assessment visit that is for an hour. The therapist I saw for that visit said I had some obvious weakness in my knee, but also focused in on the fact that my legs weren't aligned correctly. She recommended all kinds of corrective physical therapy before surgery as well expensive sole inserts for my shoes. Fortunately, after my surgery, I got paired with a different and much better physical therapist in Tara. UNfortunately, she's not at this location anymore. In fact, I heard she moved back to her native South Dakota. Tara was great. Very professional, very friendly, and answered all my questions and concerns with patience. I asked her about my leg alignment during one appointment and she said it was a bit off but if it's not bothering me, it's best not to mess with it. Great philosophy. I was glad that I was able to complete my physical therapy before she left and I only found out she was gone when I stopped in to say hi a few weeks later. The clinic itself isn't bad. They have some private rooms and a larger area with a bunch of different kinds of equipment for general physical therapy use. I didn't like doing my exercises in the big room with others watching, although, most of the time it was just one or two other physical therapists hanging about. I think there was only one other time where there was another patient in the room. Given that I was working on my knee, I was on their exercise bike for 5 mins before each appointment. They had fancy exercise bikes with fancy readouts which, I would assume, included a timed workout. But they would always give me a digital kitchen timer to time my biking instead. I don't know if they didn't know how to work the bike electronics, or if they just weren't working. The receptionists and assistants were always smiley and friendly and knew who I was after the first few appointments so I didn't have to always tell them my name when I checked in. It's nice to know that they're paying some attention to you week in and week out. Overall, a nice clinic and a good location. I give the facility 4 stars and my "real" personal physical therapist 5 stars. Since there are no half stars, and because of my first experience there, I have to average down.
