Took an elevator to the second floor since the stairs looked exhausting. The front staff was very friendly and provided me the all too familiar registration packet to fill out. The waiting room was small but only a few people waiting. I was called into an examination room within ten minutes. Dr Pollak is a joy to speak with and is super knowledgeable in her profession.

The problem is that this policy is not clearly disclosed before the appointment, and the practice cannot explain what qualifies as a "high deductible." On May 6, I attended an appointment with Dr. Jackeline Castillo. Before the appointment, I completed all required paperwork and provided my insurance information. At no point during scheduling, registration, or in any forms was I informed that I would be required to pay the full cost of the visit because of my deductible. No one explained what deductible amount triggers this policy or what my financial responsibility would be before I arrived. When I arrived, I was told I had to pay $203.30. I was already there, I was sick, and I needed to see a specialist, so I paid $203.30 and proceeded with the visit. The medical care was excellent. My complaint is not about Dr. Castillo or the quality of care. It is about the office's billing practices. After receiving my UnitedHealthcare Explanation of Benefits (EOB), I learned that my actual responsibility for the visit was only $14.92. I was charged $188.38 more than my insurance determined I owed. On June 19, 2026, I contacted the office and asked where this policy is disclosed and what deductible amount is considered "high." The representative could not direct me to any website, form, or written policy and simply stated that it was the office's internal policy. I am requesting reimbursement of the $188.38 overcharge and that this practice be required to clearly disclose these billing policies to patients before appointments are scheduled.

I made the mistake of being honest with women who makes his appointments and told her i was a very nervous person and i didn't want to have any procedures done in office unless the Dr Garner would ease my concerns and explain the procedure and if they were painful. The woman put me on hold and came back and said that Dr Garner didn't want to waste his valuable time and told me she wouldn't keep the appointment she made for me on 2-11-25. I told her that was unethical and maybe illegal and she put Dr Garner on the line. He reinstated the same comment his time was too valuable and it would be a waste of time. I told him maybe if he would comfort me by explaining what was going to happen then that might calm me down. He insisted that all the procedures were painful and simply refused my need to see a urologist. I even said given the choice to have pain or do nothing at all then i would bite the bullet and go through with the procedure but he said flat out NO! This guy was totally riding on his i am god almighty and dont bother to even begin to ask me any questions in person. Just because hes been doing this for years doesn't make it right.     eric chenven, md is simply outstanding . trained by america's top doctor, & internationally…read morerenowned urologist, the " father of ureteroscopy, demitri bagley, md, of the thomas jefferson medical school, divison of urolgy in philadelphia -- the oldest urological program in the country ‼ dr. chenven, spends time with you. you do not feel hurried. he is kind, & compassionate, & most accommodating ‼ furthermore , he is highly trained in endo-urology, minimally invasive urological surgery. robotic surgery. he is the only urologist in the entire state of fl who did additional fellowship training at the top urological program in the country ‼ i can only tell you, as an operating room rn x35 years, & a kidney stone patient x 42 years, i fell truly blessed knowing dr. chenven is only 45 minutes away from boca. not all urologists know how to laser or basket a kidney stone. he managed me beautifully & pro-actively as i had asked to avoid major obstruction & hospitalization, for which i could never thank him enough. awesome person & urologist . you will not be sorry. i can not speak for the rest of this group, only dr. chenven. thank god he is in south fl.

As far as what needed to be done: I was told I would have a procedure done based off of an ultrasound. I was given a Rx for an antibiotic. I let them know that there was a risk of tendon damage with the prescribed antibiotic. They sent a script for a new antibiotic. I was in contact with the office 2 days before the procedure. I expected to have the procedure done when I went in. I'm not too keen on taking antibiotics due to the damage it does to good and bad bacteria in your gut. It's not healthy to take antibiotics unless necessary. It would have been necessary if I received the procedure. I took it for a day and a half, and got to the office to find out that it was supposedly not the best route to treat me to do that procedure after paying for (2 antibiotics) and taking one of them. The procedure is something Dr Sofer said she would have told a family member (of hers) to get and it's difficult to get certain images and diagnoses confused when it's a close family member and a patient has the same thing, so with that said, it seems as if the more invasive procedure is just being pushed for other reasons. Idk for sure, but I'm confused and quite honestly afraid to go through with the other procedure being recommended. I was given a physicians order for a certain type of MRI that this place is charging $95 for in addition to what my insurance covers.

Dr Ead had nothing to with the surgeries, but such a tremendous help in the recovery process. Without getting too graphic, part of my healing process was the need for a catheter, first time ever -- in my "male organ" by a hospital emergency room medical staff. The pain and agony I had gone through. The following day I went to see, through a great referral from a surgeon I know, -- Dr Ead at his office. Dr Ead has a confident and assuring demeanor, with a good sense of humor, as well, and simply the magic touch in placing a new catheter in me, and several times thereafter. What a difference! In both cases my experience has been more than positive, for the doctor is straightforward, caring, competent and professional. For a while, the office environment was not as stellar; however, when this was brought to the doctor's attention, things quickly changed for the better. Also, the wait time tends to be on the long side. I recommend Doctor Ead without hesitation to anyone who may need a capable, experienced and knowledgeable urologist and surgeon.
