Having few choices, I called for a consult. Policy is to pay 35-50% of surgery costs up front. Outside imaging is not accepted; I was told to bring cash payment of $500.00 for a CT. "We will reimburse you if your insurance pays us." If you have that kind of cash on hand, go for it, but be prepared to endure his bedside manner. Rude & unempathetic is putting it lightly. I told Forfar I was allergic to and did not want Versed for my procedure. Apparently, he thought it would be funny to go ahead and give me Versed even though I told him not to. I had a reaction to the medication and almost wound up hospitalized with Police getting involved. Additionally, I had a recurring infection as a result of his work (poor quality workmanship). I followed up with him for 3 weeks while this infection was literally draining puss into my mouth - he told me it was fibrin drainage. I went to a different dentist who, in 15 minutes, identified that it was infection draining from a perforation in my sinus where Forfar had done the work. TERRIBLE quality of care, literally gave me medication i told him not to give me, then didn't even fix the issue caused (perforation in sinus) by his poor workmanship. Stay away. My experience with Dr. Forfar was not good. I went in for a Consultation on a cracked and infected tooth from a failed root canal that had to be extracted. He barely gave me the time of day. The estimated fee for the removal was outrageous including $760 for an unnecessary Debridement treatment that was not covered by insurance. I promptly went to another Oral Surgeon for a 2nd opinion and was told that the Debridement (infection removal) was part of the extraction and they do not charge for it. I had been referred to this location for a wisdom tooth extraction. I have anxiety already about the procedure but I came the week before so they could do a consultation and take a panoramic x ray of my teeth. First off, my mom and I waited nearly almost an hour to be seen. I was told a week prior to making the appointment that there was an extraction going on that day so it might take a while... yikes, didn't know we'd be waiting that long. When we finally were seen, everything went smoothly from there. We went through insurance and made the appointment for the extraction which SHOULD'VE been on Friday. The woman at the front desk by the name of Debra, said she calls to confirm the day before and if no one answers, she'll leave a voicemail. Well no one called me Friday so I decided to call THEM an hour before they closed their Wheaton location. I gave my name and appointment time where they said it was confirmed. About 7:30 I get a call from a number and receive a voicemail from Dr. Forfar. He says how he "bought" his business back from Elite after having back surgery. He also said how he wasn't told that I would be having my teeth extracted. So because of this I would not be having an appointment. This is outrageous!! I had my appointment set a week in advance!! He even claimed that Elite KNEW that he was going to take his business back. So who allowed this to happen? I even took 3 DAYS OFF OF WORK SO I CAN GET THIS DONE. This is so unprofessional and so deluded that I would stay far away from both businesses, whether Elite or Forfar. read more

OMG! I went to see this rude unprofessional horrid doctor as I was referred. What a mistake that was folks! This guy is greedy and only cares about money. He could care less about his patients and it shows. I had gone for an extraction of a dental implant and was sedated. Prior to being sedated I had attempted to make small talk with the doctor. No talking here. Not much to say. It's like poking a cadaver. I'm serious. After having oral surgery there were some bone fragments surfacing so I had to have it removed. Against my better judgement I decided to give this guy a call to make it easy on me. After he had performed the oral surgery his staff threw me out! I wasn't given the chance to wake up! My poor husband had to escort me and help me through the parking lot as I almost lost my balance as I wasn't able to awaken first. Yet according to the snotty gal who answered the phone thru always give patients time to wake up. When I tried to rebook an appointment to see him again I was promptly informed they had raised their prices and I'd be charged again for another consultation even though he had already seen me and was paid in full. $300 consultation fee and $925 just for sedation alone! Not including the extraction!!! Unbelievable! It's all about the money. It's not about the patient or the people or growing your business through establishing patient:client relationships. Nope. If you think I am lying about his lack of bedside manners or his wonderful personality then check his reviews on Google and Yelp. Several other people told the truth about his egregious behavior and his lack of empathy. I loved his attitude towards everything - straight and to the point - he tells it like it is and technically he is the best. He did a fantastic job and I would definitely recommend him. My husband had a tooth extracted probably 6 months ago. Since then Dr For far has sold his practice to elite oral Syrians and he been weaned out. However in the process of this my husbands claim was never submitted to insurance. They keep sending us a bill stating insurance has paid with no record on the bill that it has. I contacted my insurance and they never received a claim. I contact the office and leave a message only to never get a call back but simply another bill threatening to send us to collections.....very disappointed. read more

So this is my second review for this establishment (I wrote one a couple of years ago that was 5 stars in 6/2017). This review is solely based on TRYING to get an appointment! My hubby needs his wisdom teeth pulled so I called F&A on 1/8/20 since it was a fantastic experience all around the first time. This time, the info in the internet was incorrect, the Wheaton location phone number I found was to F&A's "concierge service". I was on the phone with the rep for 20 minutes TRYING to get an appointment (she WAS VERY PLEASANT) however, I mentioned which location, gave her important info, name, insurance info, etc...at the end of this call she said she would pass this info along to the Schaumburg office. I told her repeatedly throughout the conversation that I wanted to go to the Wheaton office. Then she says someone will call me back. Been 24 hours and nothing. TOO MUCH trouble to get an appointment. Not sure if Dr. F tried to expand too quickly, but the customer service has DEFINITELY suffered...lack of correct info on the internet, concierge service is the pits, and apparently they are doing so well that they don't need to call patients back! read more

If i could give negative stars i would. first they made us prepay $580.00 for services on 12/10/2018 even tho we are blue cross ppo members. then we had to hassle them to file a claim with blue cross which they finally did in june of 2019. blue cross paid them in full except for $180.00 which means we are owed $400.00 dollars by them, because they have been double paid. after literally 100 calls to their office by both me and blue cross on my behalf we still do not have our money. they do not return phone calls and the office staff is continually changing and rude. i now have no other option, but to take them to small claims court to get my money back. this is a scam !! read more

Avoid!!! I went in September, needed an extraction. Paid $81 for a consultation. Dr. No Personality said he'd contact my medical doctors as I use an anti-platlet medication. Waited about 2-3 weeks never got a call. Emailed and got a call from nurse/hygenist/receptionist a few days later. "We didn't forget about you", I was told. Waited another month and contacted my medical doctor about the medication. Got an answer in 2 hours. Now it's January. Either he doesn't have the expertise to do my extraction or figured he couldn't make enough money off me. Even when there was an insurance question I said I would pay out of pocket. After being diagnosed with Temporomandibular Joint Dysfunction (TMJ), my Dentist recommended I see Dr. Forfar for treatment and to be fitted for a night guard. After waiting 30+ minutes after my scheduled appointment time to be seen, I was finally placed in a room. The first strike for me wasn't even the long wait time, it was the fact that Dr. Forfar examined my jaw and placed his fingers inside my mouth with NO GLOVES on. Absolutely disgusting. I was recommended by my regular dentist to Dr. Forfar to have my four wisdom teeth removed. I definitely don't regret it, he was super patient and pleasant the entire time. I don't know where the negative reviews are coming from, he has been doing this a long time and judging by the volume of other people coming in to get their wisdom teeth removed, he does it well. I've heard horror stories of people having wisdom teeth removed and being really bruised up afterwords but that wasn't the case here. I had a great experience with Dr. Forfar. I was recommended to him by my dentist to get my wisdom teeth extracted. I was extremely nervous due to the fact I have never had to go to an Oral Surgeon before and I had heard awful stories about how painful a procedure it was. Much to my surprise the procedure was virtually pain free and went very smoothly. Dr. Forfar made me feel comfortable and confident in his abilities for a seamless procedure. I would highly recommend him! Side note: As far as the bad reviews regarding how expensive he is, I would disagree. Having worked in the Healthcare Industry for 18+ years many patients are just not aware of how expensive procedures can be and what their insurance will & will not cover. Avoid this place at all costs! The service was okay, I had a couple of wisdom teeth pulled over a year ago and the procedure went fine, however the billing has been a complete nightmare. During the time of the procedure, I paid the portion FORFAR AND ASSOCIATES RECOMMENDED, as they said they'd expect insurance to cover the remaining balance. Lo and behold, I still ended up receiving a very large bill in the mail. I spoke with my insurance company, they said they believe the dentist misread their statement, so they joined me on a 3-way call with the dentist office, and it seemed like everything got sorted out. Then a couple of months later I received another bill for that same large amount and we did the whole thing over. It's been over a year since my procedure and I've been going through the same exercise over and over. I have been on at least 20 calls, and have been told "it would be taken care", "there was a change in ownership and the billing was wrong", that they've "been receiving many calls with similar issues", etc. and ultimately this thing comes back full circle and starts over like a horror version of Groundhog's Day. Unfortunately, I cannot recommend Alan Forfar, he is not very professional, starting when he make me wait for more than an hour and a half, they told me he was in a conference call, he thinks that his time is more valuable than our time as patient, he was very impolite all the time and he was taking care of 3 different patients at the same time. About his work, I think he choose the wrong career, he should be butcher not oral surgeon, his work was one of the worst I had in my life, this is not the first time a wisdom tooth was removed for me. Pass. Went here for a simple procedure for my daughter and gladly walked out paying just the $50 consult fee. He was 'in network' yet his 'deposit' was the exact same price for an out of network provider's total out of pocket cost for my family, for the same procedure. And his fee was 4x the deposit! Their service was poor and it began with the first call. The receptionist was vague on the dr's fee on that first call. Probably not her fault, maybe the business model. The assistant was condescending when I challenged the cost vs my out of network option. Mind you we waited an hour AFTER our appointment time. No empathy. Avoid at all costs. Unscrupulous billing practices. They changed the name from "Forfar and Associates". They charge upfront for more than your "patient pays" portion is going to be after insurance, but once they receive the insurance payment they won't give you the refund for the difference. We've been fighting with them for 5 months to get the refund we're due. How did they respond to our requests? They took our son's zero balance account from years ago and turned it over to a collection agency for thousands of dollars of charges they invented out of thin air. The account is 0 balance, and he hasn't set foot in the office for nearly three years, and now all of a sudden they want to punish him because we're asking them to send our refund? You know it's illegal to send an account to collections without a "notice of collection" letter. "Hmm? Illegal? Unethical? Pfft." We just wanted a simple wisdom tooth extraction: we'll pay a deposit, they'll bill the insurance company, they receive the insurance payment and send us a refund. It could be so simple. It doesn't have to be angry, confrontational, escalating, and vindictive. It just doesn't have to be this way. We could have been friendly neighbors with good will toward each other. When I made the phone call to make the appointment I told them specifically we didnt want/need teeth xrays (because it had nothing to do with his teeth). When we got there we filled out forms, sat down and when they called his name the nurse waited until they got to the door to look back at me and say "ok we are taking xrays" and she shut the door on me. So that made the price of this CONSULT visit go up from $81 to $210! When we finally got back to the office the doctor looked in his mouth for .5 seconds and said "did you really come in here for this?" Like... yes you arent supposed to have a bump on the inside of your mouth sir. He was extremely condescending when we asked any questions and treated both of us like 5 year olds. I have had a great experience with Dr. Forfar and his office and before I go into my story I feel compelled to comment on the 9/29/14 review. Dr. Forfar's office did check to see what my insurance covered and told me exactly what would be submitted to medical and dental. I find it hard to believe this person was told that if he checked with his insurance, Dr. Forfar's office could not treat him. In my experience his staff have been nothing but professional every time I've called or gone in for a follow up appointment. Anyway, I had a large infection in my mouth which had eaten away the bone. After months of trying to save my teeth, my endodontist recommended Dr. Forfar because I had no other options remaining except to extract the teeth. I called Dr. Forfar's office who got me in within a week for a consultation. After the consult, it was confirmed that two teeth would need to be extracted and a bone graft would also be needed. Dr. was concerned with the amount of bone loss I had and was able to schedule the surgery in one week. The day of the surgery went as well as could be expected. I had no swelling at all and only needed the pain meds for the first day. I had a follow up appointment 3 days later and have had 3 additional follow-up appointments since. My gums and bone are healing great and I have had no complications since my surgery 3 months ago. I think that Dr. Forfar is a no-nonsense, tell-it-like-it-is dr. but I appreciated that about him. He is direct and answered all my questions without rushing me. Hopefully I don't need to get any more teeth extracted but I would go back to him in a heartbeat if I did. read more

I have had a great experience with Dr. Forfar and his office. I called beforehand to verify insurance coverage and his office staff was very helpful over the phone to verify my coverage and go over what out of pocket expense I would have and how it would vary after I had my consultation depending on how many teeth I would have to get removed. Wisdom teeth removal isn't a cheap procedure and out of pocket cost will vary depending on your insurance. I was referred to Dr. Forfar from my dentist, but was also give 4 other names. His office staff was the most friendly and patient on the phone and had availability within a week to schedule an appointment. I had my consultation and procedure done on the same day. The day of surgery went extremely well. I had all 4 teeth removed due to infection and I had no swelling at all. The front office, as well as Dr. Forfar, were extremely thorough in their explanation to my dad, who was my person to wait and take care of me. Dr. Forfar was concerned about possible infection afterwards due to all 4 of my teeth being infected prior. He gave me a pain medication, an antibiotic, an oral antibiotic rinse, as well as a syringe to clean out the areas throughout the day, especially after meals to prevent any bacterial or food getting caught. I felt completely aware and prepared of what I was going to need to do for the next week. They also gave a follow up call the day after to check in, make sure everything was ok and further explain next steps. I thought that was a great touch. I had a hard time eating for about a week but that's to be expected with getting 4 teeth removed. There was no swelling, I only had to ice the first day and then used a heating pad for the remainder of the week for when I had pain (mainly after eating). Again, I had no swelling, no bruising, no dry-sockets, no further infections, and everything is healing nicely! I think that Dr. Forfar is a straight-shooter and I appreciated that about him. Him and his staff provided excellent customer service and made sure I was knowledgeable about the entire process. read more

Warning: This office overcharges and is difficult to work with! If you want a smooth and easy process, do not go to this place. While the Dr. Forfar does great work, I would never go here again and would never recommend them. EVER. I was told that I needed a biopsy for two cysts in my mouth. They told me it was dental issue, and that they would go through the dental insurance. However, they send everything to a medical lab. When I got my $500 bill, I questioned them and my insurance. My dental insurance could not pay it because it was a medical lab. Dr. Forfar's office told me that I needed a referral. They should have told me that. Then they told me to go talk to my primary doctor to ask for a referral for the work that was done with them. Did I mention this was all happening THREE MONTHS later?? I paid them a good amount of money for my portion. Later on, they sent me a bill for the rest of it. I talked to my insurance. While my insurance was late on paying them, Dr. Forfar was asking for $3,700 to remove the cysts. My insurance said no that $1,600 was what was allowed. That goes to show how overpriced his rates are. My insurance says that I should have paid half of that. However, I paid more than that. I called to question them. They said that I need to pick up the money on the anesthesia. Needless to say, I need to call my insurance to truly question this. If you want to spend your time back and forth with this office, your insurance, medical labs, etc., then this is the right fit for you. Personally, I don't have time for this. This is a horrible experience. Considering I was worried about a possible cancer, this was not right. Even in regards to that, I had to keep asking and calling to see if they got my results and how they came up. I was told, "Nothing there. You will live." I think that is an odd response from a surgeon to a nervous patient. read more

Dr Forfar has a terrible bedside manner. Never have I had a doctor treat me so disrespectfully in my life. I had my wisdom teeth out December 27th 2019 by a different doctor who was really nice. When it was time to make my follow up appointment, they were extremely hard to get ahold of. I ended up calling my dentist who got me in right away and looked at my teeth. He said it was an infection and put me back on an antibiotic but he still wanted me to see the oral surgeon who did the extraction and he would try to get ahold of them since they were not getting back to me. Even my dentist said he had a hard time reaching them. I ended up calling my dentist again cause I was having bad throbbing pain and he ended up finally getting ahold of them and told me to come in ASAP. When I got there, I found out the original doctor was not there but a doctor named Dr. Forfar would take a look. When Dr. Forfar looked at my extraction, he was very rude. He rudely told me I need to open my mouth all the way if I want him to look at it (it's painful for me to open wide as I have TMJ) and basically forced it open while I was in horrible pain. He raised his voice at me for not keeping my chin down as well and for flinching in pain saying I need to "sit still." He wanted to numb the extraction site and clean it out, but I left before he could do that. So rude and not a doctor to see if you want to be reassured and treated gently. When I was leaving he also told me he is going to call my dentist and tell him I am refusing medical treatment. Jokes on him cause my dentist said he won't send anyone there anymore. I was in extreme pain accompanied by a killer earache that would not let up no matter what I did. I received no apology for the looong wait & the office was freezing. When he finally came into the room, he was very arrogant, cold and unfriendly. After rushing thru everything, he sent his assistant in to go ovr what I wld owe. They first charged me $50 for a consultation that was nvr mentioned ovr the phone, ok , but then they tried to charge me $110 for an xray that I brought in with me! my husband also brought in his xray to get his wisdom tooth pulled and was charged $50 for what our dentist had already told him! ( that he needed the tooth out) he didn't even do anything except glance in his mouth for 2 sec. tops..bam! $50!...long story short, it was all about money, I was so frustrated by the whole ordeal and the way I was treated, I told the asst. That I was leaving, bc it appears Dr. Forfar kept walking by making rude comments, like I was wasting his time..what abt the nearly 2 hrs of my time he wasted!? Despite my pain, I was treated so horribly, I told my husband I was leaving...my husband then convinced me to stay, bc he knew how much pain I was in... so I went bk and told the asst I wld move forward with the procedure..she then told me instead of the $310 dwn..it wld be $410 bc the doc "may" nd to send out for a biopsy?. . wow! so on my way to inform my husband of the $100 possible increase, Dr Forfar stops me and ask, "ARE WE GOING TO DO THIS OR WHAT!? Really?? I was already in pain & instead of being comforting & putting me at ease..he made this a nightmare of an experience! Bottom line, this "man" does not know how to treat or appreciate his patients, true we all have bad days, but without patients. ..you dont have a practice! Fellow Yelp readers, the best way to deal with ppl like this is to hit them in their pockets...write reviews and spread the word! find a oral surgeon who cares and knows how to treat their patients. Do not go here if you can help it. I went here because my insurance reported that he was one of the Oral Surgeons in my network. This was to be a preliminary consultation and and xray for wisdom tooth extraction. $50 consult and $110 panoramic. Waiting area was packed. We waited about an hour past the appointment time to see Dr. Forfar. When I finally got into an exam room, I see that my xray is displayed on a monitor. After a few minutes, the Dr. struts in, grabs what is likely the x ray or some documents and smugly asks me why I am there. It caught me off guard because I explained the whole scenario when considering this location for my procedure pre appointment.Nonetheless, I stammer out that I was told by my dentist that I have wisdom teeth that need removal and was told we needed an Oral Surgeon to do it so I guess I came in to find out how it would be done and how much it would cost. He snaps off that there are (4) wisdom teeth to come out, and (2) are impacted. He tells me that it'll require the patient to be temporarily knocked out. After hearing his recommendations, I respectfully ask why the patient would need to be asleep, when I personally had all four of my impacted wisdoms removed with Novocaine but a different dentist about 20 years ago... Well Dr. "Full-o-pride" gets all agitated, that I would dare question his expertise, and approaches the monitor and starts snapping off that these teeth are special and are gonna be more of a challenge. Ok, I am speechless and he knows it. This appears to be exactly what he was hoping for because afterwards he proceeds to star me down. His tone and mannerism was completely unprofessional and unnecessary. He then switches gears and tells my kid to open his mouth so he can have a look with his special Oral Surgeon popsicle-stick tool. A 10-second peek and into the sink he throws it. He then says that it should be taken care of as soon as possible. Go to the front office and the girls will let us know the next available opening. He, Dr. Forfar will not disclose his prices directly, you must go to reception and they'll break it down. So, (4) wisdom teeth extracted under anesthesia gas (about $500 just for this) would cost $2100., with 25% (or $525) due after the procedure. Total procedure 45 mins. My insurance would cover 50% of the balance, so my total out of pocket would be $1100. Honestly, considering his pompous attitude, I already knew that there was no way I was about to let this conceited jerk take one more dime of our hard earned money. She could have told me $1.00 and I still would have declined the offer. I was furious. The receptionist told me they had an opening a week away and could get the appointment scheduled. I don't think they are accustomed to refusals because she seemed offended that I said I was only going to pay the bill for today's services and then I'd discuss our options with my husband. I paid the bill and got out of that place. Fast forward to the end of the story, being that the teeth had (2) impactions, and I was concerned about infection, I spent the rest of that evening (Fri) and the next morning (a Saturday) combing through Oral Surgeons around DuPage County. I had already opted to consider Dr's out of network because I was so desperate. I was about to give up. Seriously! I honestly said to my husband that I may have no other choice but to make that appointment with Forfar. Then, to my astonishment, instead of getting an answering machine, the very next call was answered by a person but not just any person - it was the Dr. himself. He patiently listened to my situation and asked how soon can you come?. Within 30 minutes, my son was seated in Dr. David Lictenwalter's dental chair. Yes, he told me, they would be tricky but not impossible. NO Gas, just the regular numbing shots. About (2) hours later, all (4) were out. PAIN -free. Sure, the next few days, my son was swollen and sore but Dr. L's aftercare instructions were on point all the way. The only prescription for healing was ice packs and Ibuprofen. Healed perfectly!!! I swear, this Dr. was God sent. I honestly have never felt in such good care. Oh and the cost? Under $1000.00 total. Moral of my story? If you have to see a specialist, never settle for the one that thinks they are the thing so special. So glad I listened to my inner voice! read more

I would give this place 0 stars if I could! Horrible experience! I started with Dr. Forfar in October 2018 and had a tooth extraction. Several months later, I had an implant. I returned for follow-up and Dr. Forfar was gone. The new doctor (Dr. A with Elite Oral Surgery) found that my implant failed and promised to fix it. I had another bone graft and was scheduled for a new implant next week (3/9/20). Dr. A is now gone and Dr. Forfar is back. There is no record of my appointment for 3/9/20. On top of the sketchy office changes/upheaval, I also had a ridiculous number of problems with billing! They sent my bill to collections even though I was making payments. The payments were not being applied to my account! It has all been a huge nightmare that took HOURS to sort out. It's 1 1/2 years later and I still don't have an implant/crown. I would highly caution against using this practice! read more
